Nitric oxide activation of a potassium channel (BK_(Ca)) in feline lower esophageal sphincter

AIM:To assess the effect of nitric oxide (NO) on the large conductance potassium channel (BKCa) in isolated circular (CM) and sling (SM) muscle cells and muscle strips from the cat lower esophageal sphincter (LeS) to determine its regulation of resting tone and ***:Freshly enzymatically-digested and isolated circular smooth muscle cells were prepared from each LeS *** study outward K + currents,the perforated patch clamp technique was *** assess LeS resting tone and relaxation,muscle strips were mounted in perfused organ ***:(1) electrophysiological recordings from isolated cells:(a) CM was more depolarized than SM (-39.7 ± 0.8mV vs-48.1 ± 1.6 mV,P ent was similar (27.1 ± 1.5 pA/pF vs 25.7 ± 2.0 pA/pF,P > 0.05);(b) The NO donor sodium nitroprusside (SNP) increased outward currents only in CM (25.9 ± 1.9 to 46.7 ± 4.2 pA/pF,P 0.05);(c) SNP added in the presence of the BK Ca antagonist iberiotoxin (IbTX) produced no increase in the outward current in CM (17.0 ± 2.8 vs 13.7 ± 2.2,P > 0.05);and (d) L-NNA caused a small insignificant inhibition of outward K + currents in both muscles;and (2) Muscle strip studies:(a) Blockade of the nerves with tetrodotoxin (TTX),or BK Ca with IbTX had no significant effect on resting tone of either muscle;and (b) SNP reduced tone in both muscles,and was unaffected by the presence of TTX or ***:exogenous NO activates BK Ca only in CM of the ***,as opposed to other species,exogenous NO-induced relaxation is predominantly by a non-BK Ca mechanism,and endogenous NO has minimal effect on resting tone.

Colonic lesion characterization in inflammatory bowel disease:A systematic review and meta-analysis

AIM To perform a systematic review and meta-analysis for the diagnostic accuracy of in vivo lesion characterization in colonic inflammatory bowel disease(IBD), using optical imaging techniques, including virtual chromoendoscopy(VCe), dye-based chromoendoscopy(DBC), magnification endoscopy and confocal laser endomicroscopy(CLe). MeTHODS We searched Medline, embase and the Cochrane library. We performed a bivariate meta-analysis to calculate the pooled estimate sensitivities, specificities, positive and negative likelihood ratios(+LHR,-LHR), diagnostic odds ratios(DOR), and area under the SROC curve(AUSROC) for each technology group. A subgroup analysis was performed to investigate differences in real-time nonmagnified Kudo pit patterns(with VCe and DBC) and real-time *** We included 22 studies [1491 patients; 4674 polyps, of which 539(11.5%) were neoplastic]. Real-time CLe had a pooled sensitivity of 91%(95%CI: 66%-98%), specificity of 97%(95%CI: 94%-98%), and an AUSROC of 0.98(95%CI: 0.97-0.99). Magnification endoscopy had a pooled sensitivity of 90%(95%CI: 77%-96%)and specificity of 87%(95%CI: 81%-91%). VCe had a pooled sensitivity of 86%(95%CI: 62%-95%) and specificity of 87%(95%CI: 72%-95%). DBC had a pooled sensitivity of 67%(95%CI: 44%-84%) and specificity of 86%(95%CI: 72%-94%). CONCLUSION Real-time CLe is a highly accurate technology for differentiating neoplastic from non-neoplastic lesions in patients with colonic IBD. However, most CLe studies were performed by single expert users within tertiary centres, potentially confounding these results.

We sought to better understand how autonomous vehicle(AV)communication strategies impact human road users’*** specifically,we explored the impact of different external human-machine interface(eHMI)designs on task load,comfort,trust,and *** accomplish this,we created virtual reality(VR)scenarios where human participants interacted with AVs in biking,driving,and pedestrian *** were brought back after initial testing to explore acclimation and learning *** designs included a text-based grille eHMI,a text-based roof eHMI,a text-based driver-side door eHMI,anon-textual LeD windshield strip eHMI,and a non-textual side mirror arrow *** presence of an eHMI was the strongest positive predictor of comfort,trust,and acceptance outcomes in the statistical models when controlling for all other *** was a clear divide between text-based eHMIs and non-text eHMIs with text-based eHMIs experiencing better perception *** LeD Windshield experienced the worst perception *** were perception acclimation effects detected which were most notable for task load(which decreased over time)and comfort(which increased over time).Perception scores for the different eHMI designs tended to cluster over ***,the acclimation effects had less of an impact than the presence of an *** outcomes had weaker relationships with participant characteristics than with AV *** suggest that eHMI presence and design,AV behavior,and acclimation are most impactful in terms of perceptions.

Disease spectrum and use of cholecystolithotomy in gallstone ileus

BACKGROUND: Gallstone ileus is a heterogeneous and highly morbid condition that suffers from a lack of consensus regarding the timing and approach to management of the biliary tree and associated fistula. MeTHODS: We report three cases that demonstrate the spectrum of gallstone ileus with classical examples of both Barnard’s and Bouveret’s syndromes. Clinical presentation diagnostic imaging, surgical technique and outcome are discussed. ReSULTS: One patient with Barnard’s syndrome presented with recurrent gallstone ileus. To minimize the risks of complex, definitive biliary surgery and avoid further recurrent episodes, a cholecystolithotomy was performed with effect Two cases of Bouveret’s syndrome were successfully managed with enterolithotomy/cholecystectomy and multivisceral resection respectively, thus highlighting the diverse nature of this disease and management options. CONCLUSIONS: Following enterolithotomy, potentially morbid definitive one-stage surgery in typically compromised, elderly patients needs to be weighed against the risk of recurrence and ongoing biliary pathology. We suggest the use of open cholecystolithotomy for the removal of residual gallstones when the patient is not suitable for definitive biliary surgery.
