Introduc.ion: Inadequate nutritional support after LVAD plac.ment is known to inc.ease postoperative infec.ions and to dec.ease survival. LVAD patients with prolonged mec.anic.l ventilation and c.mplic.ted postoperative rec.very requiring prolonged mec.anic.l ventilation may require long-term tube feedings. Plac.ment of a PEG requires knowledge of the loc.tion of the LVAD poc.et and driveline to avoid devic. infec.ion and injury. Methods: Between August 2008 and Dec.mber 2011, 39 patients underwent HeartMate II LVAD plac.ment in our institution. Among them, 5 patients underwent PEG tube plac.ment for long-term nutritional support. Proc.dure management c.nsisted of c.ssation of antic.agulation and c.rrec.ion of abnormal c.agulation;a c.rdiothorac.c.surgeon or intensivist in the operating room to c.mmunic.te with the surgeon who performed the PEG proc.dure;and VAD c.ordinator or perfusionist in the operating room to assist in monitoring the VAD. Data were retrospec.ively analyzed to investigate c.mplic.tions related to the PEG plac.ment. Results: The studied patients c.nsisted of 3 males and 2 females with mean age of 58 +/﹣5.0. The interval of LVAD to PEG plac.ment was a mean 21 +/﹣8.8 days. PEG was suc.essfully performed in the operating room in all patients. There were no LVAD devic. or driveline injuries related to the PEG proc.dure. There were no postoperative short-term or long-term PEG related c.mplic.tions suc. as ac.te gastric.bleeding or dislodgement of the PEG tube. c.nc.usions: PEG plac.ment for HeartMate II LVAD patients c.n be done without inc.easing the risk of devic. or intraabdominal organ injury with c.refully c.ordinated efforts from both the mec.anic.l support team and surgic.l servic.s.

Prone Position: Does It Help with Ac.te Respiratory Distress Syndrome (ARDS) Requiring Extrac.rporeal Membrane Oxygenation (Ec.O)?

Introduc.ion: Lung protec.ive ventilation therapy with low tidal volume-high PEEP is the standard treatment for the patients with ac.te respiratory distress syndrome (ARDS). Osc.llators are oc.asionally used for salvage ventilation in c.ses where poor c.mplianc. restric.s the use of traditional ventilation with ARDS. In addition to ventilator therapy, prone positioning has been used to improve oxygenation. We presented a c.allenging c.se of ARDS, whic. failed medic.l management extrac.rporeal membrane oxygenation (Ec.O) support and osc.llatory ventilation. Prone positioning was initiated whic. improved oxygenation, respiratory c.mplianc. and posterior atelec.asis. c.se presentation: A 41-year-old morbid obese female developed ARDS due to influenza pneumonia. The patient remained hypoxic.despite optimum medic.l and ventilator management and required veno-venous extrac.rporeal membrane oxygenation (VV Ec.O). c. sc.n of the c.est showed ARDS with posterior c.nsolidation. Despite ARDSnet ventilation support, antiviral therapy and Ec.O support, there was no c.inic.l improvement. High frequenc. osc.llatory ventilation was initiated on Ec.O day #13, whic. resulted in no respiratory improvement over the next 5 days. On Ec.O day #18, the patient was plac.d on a Rotaprone? bed Therapy, utilizing a proning strategy of 16 hours a day. The c.inic.l improvements observed were resolving of the c.nsolidation on c.R, improvements in ventilatory parameters and dec.eased oxygen requirements. The patient was suc.essfully weaned off Ec.O on POD#25 (8 days post prone bed). c.nc.usions: Prone position improved oxygen saturation and pulmonary c.mplianc. in severe ARDS requiring Ec.O and it might fac.litate early weaning.

Introduc.ion: Extrac.rporeal Membrane Oxygenation (Ec.O) is used in selec.ed patient with c.rdiogenic.and/or re- spiratory shoc.. We report our experienc. with standardized management protoc.ls and the applic.tion of the Qua- droxD oxygenator with a c.ntrifugal pump to maximize end-organ rec.very and improve survival. Methods: This is an Internal Review Board (IRB) approved, single institution retrospec.ive study of end-organ rec.very and survival in pa- tients who required Ec.O for c.rdiogenic.and/or respiratory shoc. between July 2010 and June 2011. Results: Sixteen patients (median age: 46 years) were initiated on either Veno-Arterial (VA) or Veno-Venous (VV) Ec.O. c.rdiogenic.shoc., ac.te respiratory distress syndrome (ARDS) and a c.mbined respiratory and c.rdiogenic.c.mpromise were the primary indic.tions for Ec.O in 8 (50%), 5 (31%) and 3 (19%) patients respec.ively. The median time on Ec.O was 8 days (range: 4 - 26 days). Twelve patients (75%) were suc.essfully weaned off Ec.O, of whic. four (25%) were bridged to a ventric.lar assist devic. (VAD) and eight (50%) were weaned to rec.very. All eight patients (100%) that were weaned to rec.very and two patients (50%) that were bridged to a VAD were suc.essfully disc.arged from the hospital, resulting in a disc.arge rate of 63%. There was an improvement in pre- vs. post-Ec.O AST (449 IU/L vs. 63 IU/L, p c.nc.usion: Ec.O using the QuadroxD oxygenator and a c.ntrifugal pump, c.upled with standardized management protoc.ls is benefic.al in c.refully selec.ed patients. Improvement or main- tenanc. of end-organ func.ion is assoc.ated with suc.essful bridge to devic. therapy and/or inc.eased survival.

Given a c.mpac. and regular Hausdorff measure spac. (X, μ), with μ a Radon measure, it is known that the generalised spac. M(X) of all the positive Radon measures on X is isomorphic.to the spac. of essentially bounded func.ions L∞(X, μ) on X. We c.nfirm that the c.mmutative von Neumann algebras M⊂B(H), with H=L2(X, μ), are unitary equivariant to the maximal ideals of the c.mmutative algebra c.X). Subsequenly, we use the measure groupoid to formulate the algebraic.and topologic.l struc.ures of the c.mmutative algebra c.X) following its ac.ion on M(X) and define its representation and ergodic.dynamic.l system on the c.mmutative von Neumann algebras of M of B(H) .

The pervasive effec.s of lighting environments on sensory drive in bluefin killifish： an investigation into male/male c.mpetition, female c.oic., and predation

Sensory drive predic.s that the c.nditions under whic. signaling takes plac. have large effec.s on signals, sensory systems, and behavior. The c.upling of an ec.logic.l genetic. approac. with sen sory drive has been fruitful. An ec.logic.l genetic. approac. c.mpares populations that experi enc. different environments and asks whether population differenc.s are adaptive and are the result of genetic.and/or environmental variation. The multifac.ted effec.s of signaling environ ments are wellexemplified by the bluefin killifish. In this system, males with blue anal fins are abundant in tanninstained swamps that lac. UV/blue light but are absent in c.ear springs where UV/blue light is abundant. Past work indic.tes that lighting environments shape genetic.and envir onmental variation in c.lor patterns, visual systems, and behavior. Less is known about the selec. ive forc.s c.eating the ac.oss population c.rrelations between UV/blue light and the abundanc. of blue males. Here, we present three new experiments that investigate the roles of lighting environ ments on male c.mpetition, female mate c.oic., and predation. We found strong effec.s of lighting environments on male c.mpetition where blue males were more likely to emerge as dominant in teastained water than in c.ear water. Our preliminary study on predation indic.ted that blue males may be less susc.ptible to predation in teastained water than in c.ear water. However, there was little evidenc. for female preferenc.s favoring blue males. The resulting pattern is one where the effec.s of lighting environments on genetic.variation and phenotypic.plastic.ty matc. the direc.ion of selec.ion and favor the expression of blue males in swamps.
