To investigate the relationship between 25-hydroxyvitamin D (25-OHD) deficiency and hepatic encephalopathy (HE) in patients with chronic liver disease (CLD). METHODSA retrospective analysis of the results of 392 adult patients with chronic liver disease who were assessed for liver transplantation between 2006 and 2010 was undertaken. HE, severity of CLD, nutritional status and 25-OHD were analysed in patients assessed for liver transplantation between 2006 and 2010. patients who presented with acute, fulminant or subacute disease, with a primary diagnosis of liver cancer, were assessed for re-transplantation or who did not have a 25-OHD measurement were excluded from the analysis. RESULTSOne hundred and sixty-five patients were included in this analysis. The mean age of all patients was 53 ± 8 years. Moderate to severe 25-OHD deficiency was identified in 49 patients of whom 36 had grade 2-3 HE compared with 13 patients who were not encephalopathic (p ≤ 0.0001). Mild 25-OHD deficiency was not associated with HE. There was a significant correlation between the severity of 25-OHD deficiency and the severity of liver disease (r = 0.39, p ≤ 0.0001) and disease severity and the presence of HE (p ≤ 0.0001). Importantly, individuals with 25-OHD deficiency were more likely to have a diagnosis of overt HE (OHE) at a significantly lower model for end stage liver disease (MELD) score than individuals without OHE (p ≤ 0.0001). This significant difference was observed with MELD scores from 10 to 38. CONCLUSION25-OHD deficiency was observed in the majority of patients with CLD and for the first time was found to be significantly worse in patients with OHE.

Droop Control Methods for pV-Based Mini Grids with Different Line Resistances and Impedances

Duchenne muscular dystrophy(DMD)causes severe damage and atrophy in skeletal muscle and a dilated cardiomyopathy in the *** disease is caused by absence of the cytoskeletal protein,*** mdx mouse also lacks dystrophin and is a model of *** with the absence of dystrophin,the disease is characterized by reduced levels of all the dystrophin-associated proteins includingβ-dystroglycan and nNOS(neuronal nitric oxide synthase)and increased levels of CAV3(caveolin 3,a scaffolding protein for muscle caveolae),TRpC1 (a candidate protein for the stretch-activated channel found in muscle),src kinase(a ubiquitous tyrosine kinase) and various sub-units of NADpH oxidase(a superoxide producing enzyme present in muscle).All of the above proteins are found in the *** studies of the mdx mouse found that a stretch-activated channel(SAC) was activated and we showed that blocking this channel(with Gd or streptomycin or GsMTx-4)lowered stretchinduced intracellular Caand reduced muscle damage(Yeung et al.2005).Subsequently we have found that the activation of SACs and the resulting Caentry can be caused by reactive oxygen species(ROS)(Gervasio et al., 2008).Furthermore,scavenging ROS can reduce the rise of intracellular Cain isolated mdx muscles(Gervasio et al.,2008)and the ROS scavenger,N-acetylcysteine,reduced the skeletal and cardiac muscle damage in intact mdx mice(Whitehead et al.,2008,Williams and Allen,2007).ROS also activated src kinase and blocked src kinase with pp2 reduced ROS-induced Caentry in a cell line expressing TRpC1 and CAV3 and reduced stretch-induced muscle damage in isolated mdx muscles(Gervasio et al.,2008).Our current working hypothesis is that abnormalities in the caveolae and associated proteins lead to increased ROS production which triggers a range of damage pathways in mdx muscle.A clearer understanding of the damage pathways may lead to therapeutic interventions that could reduce the severity of muscle and cardiac damage in

Different droop control methods for pV-based communal grid networks (minigrids and microgrids) with different line resistances (R) and impedances (X) are modelled and simulated in MATLAB to determine the most efficient control method for a given network. Results show that active power-frequency (p-f) droop control method is the most efficient for low voltage transmission networks with low X/R ratios while reactive power-voltage (Q-V) droop control method is the most efficient for systems with high X/R ratios. For systems with complex line resistances and impedances, i.e. near unity X/R ratios, p-f or Q-V droop methods cannot individually efficiently regulate line voltage and frequency. For such systems, p-Q-f droop control method, where both active and reactive power could be used to control pCC voltage via shunt-connected inverters, is determined to be the most efficient control method. Results also show that shunt-connection of inverters leads to improved power flow control of interconnected communal grids by allowing feeder voltage regulation, load reactive power support, reactive power management between feeders, and improved overall system performance against dynamic disturbances.

AIM： To establish the efficacy and safety of bimatoprost 0.03% monotherapy in glaucoma and ocular hypertension （OHT） patients with inadequate intraocular pressure （lOp） on current therapy. METHODS： pre- and post-switch lOps were analyzed for 59 consecutive patients who were switched from current therapy to bimatoprost monotherapy between 2011-2015. Demographic information, diagnosis, and any adverse events were recorded. Change in lOp post-pre switch was analyzed using a 2-sided Student＇s paired t-test at the 5% significance level. RESULTS： There was a statistically significant mean reduction in lOp at the first follow up visit, which was maintained at subsequent follow up visits for patients regardless of diagnosis, or pre-switch treatment （p〈0.001）. Subgroup analysis also demonstrated a statistically significant mean reduction in lOp when looking at OHT patients only, as well as patients with any diagnosis switched from latanoprost monotherapy to bimatoprost monotherapy （p〈0.001）. CONCLUSION： This is the largest independent data set which supports switching glaucoma patients with poor response to current treatment onto bimatoprost monotherapy before considering other adjuvant medical or more invasive therapy.
