Pancreatic paraganglioma with multiple lymph node metastases found by spectral computed tomography: A case report and review of the literature

BACKGROUND Primary pancreatic paraganglioma is exceedingly *** patients with pancreatic paraganglioma lack a typical clinical presentation,and the tumor is difficult to accurately differentiate from other pancreatic neuroendocrine tumors,making the misdiagnosis rate extremely *** excision is the primary treatment modality but is considered high *** of its rich vascularity,the tumor easily bleeds during surgery,especially malignant paragangliomas invading large blood ***,a thorough preoperative evaluation of the tumor is ***,we report a primary malignant pancreatic paraganglioma,the second such case in a young patient that was successfully resected *** SUMMARY A 26-year-old female patient was admitted to the hospital with unexplained abdominal ***-layer spectral-detector computed tomography(DLCT)revealed a mixed density mass in the pancreatic body and *** patient was transferred to our hospital after previous failed surgical resection at other *** patient and her family strongly desired *** a thorough preoperative evaluation and adequate preparation,a large mass with the greatest dimension of 8.0 cm was successfully *** final pathological diagnosis was malignant *** patient was discharged in good condition 2 wk *** The rare malignant pancreatic paraganglioma reported here was difficult to diagnose *** filling of the draining vein may be a crucial diagnostic imaging *** can provide more precise information for surgical resection through dual-energy imaging.

Factors Associated with the Size of HIV DNA Reservoir

Objective： To review the recent literatures related to the factors associated with the size of the HIV reservoir and their clinical significance. Data Sources： Literatures related to the size of HIV DNA was collected from PubMed published from 1999 to June 2016. Study Selection： All relevant articles on the HIV DNA and reservoir were collected and reviewed, with no limitation of study design. Results： The composition and development of the HIV- 1 DNA reservoir in either treated or untreated patients is determined by integrated mechanism comprising viral characteristics, immune system, and treatment strategies. The HIV DNA reservoir is a combination of latency and activity. The residual viremia from the stochastic activation of the reservoir acts as the fuse, continuing to stimulate the immune system to maintain the activated microenvironment for the rebound of competent virus once treatment with antiretroviral therapy is discontinued. Conclusion： The size of the HIV-1 DNA pool and its composition has great significance in clinical treatment and disease progression.

High-throughput and simultaneous inertial separation of tumor cells and clusters from malignant effusions using spiral-contraction-expansion channels

Tumor cell clusters are regarded as critical factors in cancer pathophysiology,and increasing evidence of their higher treatment resistance and metastasis compared to single tumor cells has been ***,existing cell separation methods that are designed for single tumor cells cannot be used to simultaneously purify tumor cell *** address this problem,we demonstrated a microfluidic approach for the high-throughput,continuous-flow ternary separation of single tumor cells,tumor cell clusters,and WBCs from clinical pleural or abdominal effusions by coupling slanted spiral channels and periodic contraction-expansion *** first systematically explored the influence of particle size and flow rate on particle *** separation performance indicated that 94.0%of WBCs were removed and more than 97%of MDA-MB-231 tumor cells were recovered at a high flow rate of 3500µL/***,more than 90%of tumor cell clusters were effectively preserved after ***,we successfully applied our device for the ternary separation of single tumor cells,tumor cell clusters,and WBCs from different malignant effusions collected from patients with metastatic ***,our spiral-contraction-expansion device has potential as a sample pretreatment tool for the cytological diagnosis of malignant effusions.

Carbon-coated hybrid crystals with fast electrochemical reaction kinetics for ultra-stable and high-load sodium-ion batteries

Owing to its high theoretical capacity and low cost,Sn has attracted significant attention in sodium-ion ***,the slow kinetics of electrochemical reactions and the rapid decay of capacity resulting from drastic changes in the volume of Sn,as well as persistent side reactions between Sn and the organic electrolyte during the(de)sodium process,have limited its *** improve the electrochemical performance of Sn-based materials,the bottom-up method is normally used to prepare carbon-coated ***,its complex preparation processes and harsh conditions make it unsuitable for practical ***,a carbon-coated hybrid crystal composite(Sn/SnO_(x)@C)was prepared using an up-bottom method with commercial Sn/SnO *** effects accelerate the electrochemical kinetics and inhibit the coarsening of Sn *** Sn/SnO_(x)@C composite electrode exhibited capacity retention of 80.7%even after approximately 1000 cycles(360.4 mAh·g^(−1)) at a current density of 1 A·g^(−1).The high-load Na_(3)V_(2)(PO4)3||Sn/SnO_(x)@C full cell presents a capacity retention rate of 91.7%after 150 cycles at the current density of 0.5 A·g^(−1).This work may significantly accelerate the commercialization of the Sn/SnO_(x)@C composite in sodium-ion batteries with high energy density.

A Historical Cohort Study on the Efficacy of Glucocorticoids and Riboflavin Among Patients with Late-onset Multiple AcyI-CoA Dehydrogenase Deficiency

Background： Late-onset multiple acyl-CoA dehydrogenase deficiency （MADD） is the most common type of lipid storage myopathies in China. Most patients with late-onset MADD are well responsive to riboflavin. Up to now, these patients are often treated with glucocorticoids as the first-line drug because they are misdiagnosed as polymyositis without muscle biopsy or gene analysis. Although glucocorticoids seem to improve the fatty acid metabolism of late-onset MADD, the objective evaluation of their rationalization on this disorder and comparison with riboflavin treatment are unknown. Methods： We performed a historical cohort study on the efficacy of the two drugs among 45 patients with late-onset MADD, who were divided into glucocorticoids group and riboflavin group. Detailed clinical information of baseline and 1-month follow-up were collected. Results： After 1-month treatment, a dramatic improvement of muscle strength was found in riboflavin group （P 〈 0.05）. There was no significant difference in muscle enzymes between the two groups. Significantly, the number of patients with full recovery in glucocorticoids group was less than the number in riboflavin group （P 〈 0.05）. On the other hand, almost half of the patients in riboflavin group still presented high-level muscle enzymes and weak muscle strength after 1-month riboflavin treatment, meaning that l-month treatment duration maybe insufficient and patients should keep on riboflavin supplement for a longer time. Conclusions： Our results provide credible evidences that the overall efficacy of riboflavin is superior to glucocorticoids, and a longer duration of riboflavin treatment is necessary for patients with late-onset MADD.

Resonances Characteristics of Parallel Plate Waveguide Cavities

The influence of air gaps on the response of transmission for a transverse-electric mode parallel-plate waveguide（TE-PPWG） with a single cavity and double cavities has been studied experimentally. As the air gap is larger than the resonant wavelength of high order cavity mode in the single deep grooved waveguide, only the fundamental cavity mode can be excited and single resonance can be observed in the transmission spectrum. Based on above observations, a tunable multiband terahertz（THz）notch filter has been proposed and the variation of air gap has turned out to be an effective method to select the band number. Experimental data and simulated results verify this band number tunability. This mechanical control mechanism for electromagnetic induced transparency（EIT） will open a door to design the tunable THz devices.

Clean air actions （CAAs） in China have been linked to considerable benefits in public ***,whether the beneficial effects of CAAs are equally distributed geographically is *** high-resolution maps of the distributions of major air pollutants (fine particulate matter[PM2.5]and ozone[O3]) and population,we aimed to track spatiotemporal changes in health impacts from,and geographic inequality embedded in,the reduced exposures to PM2.5and O3from 2013 to *** used a method established by the Global Burden of Diseases *** analyzing the changes in loss of life expectancy（LLE） attributable to PM2.5and O3,we calculated the gain of life expectancy （GLE） to quantify the health benefits of the air-quality ***,we assessed the geographic inequality embedded in the GLE using the Gini index （GI）.Based on risk assessments of PM2.5and O3,during the first stage of CAAs（2013 to 2017）,the mean GLE was 1.87 *** of the sum of the GLE was disproportionally distributed in about one quarter of the population exposed （GI 0.44）.During the second stage of CAAs（2017 to 2020）,the mean GLE increased to 3.94 months and geographic inequality decreased （GI 0.18）.According to our assessments,CAAs were enhanced,from the first to second stages,in terms of not only preventing premature mortality but also ameliorating health *** enhancements were related to increased sensitivity to the health effects of air pollution and synergic control of PM2.5and *** findings will contribute to optimizing future CAAs.

Survival Analysis of Patients Undergoing Intraoperative Contrast-enhanced Ultrasound in the Surgical Treatment of Malignant Glioma

Objective:Complete resection of malignant gliomas is often *** previous study indicated that intraoperative contrast-enhanced ultrasound(ICEUS)could aid in the detection of residual tumor remnants and the total removal of brain *** study aimed to investigate the survival rates of patients undergoing resection with or without the use of ICEUS and to assess the impact of ICEUS on the prognosis of patients with malignant ***:A total of 64 patients diagnosed with malignant glioma(WHO grade HI and IV)who underwent surgery between 2012 and 2018 were *** them,29 patients received *** effects of ICEUS on overall survival(OS)and progression-free survival(PFS)of patients were evaluated.A quantitative analysis was performed to compare ICEUS parameters between gliomas and the surrounding ***:The ICEUS group showed better survival rates both in OS and PFS than the control *** univariate analysis revealed that age,pathology and ICEUS were significant prognostic factors for PFS,with only age being a significant prognostic factor for *** multivariate analysis,age and ICEUS were significant prognostic factors for both OS and *** quantitative analysis showed that the intensity and transit time of microbubbles reaching the tumors were significantly different from those of microbubbles reaching the surrounding ***:ICEUS facilitates the identification of residual *** and ICEUS are prognostic factors for malignant glioma surgery,and use of ICEUS offers a better prognosis for patients with malignant glioma.
