Darier-Ferrand dermatofibrosarcoma (DFS) is a cutaneous mesenchymal tumor of intermediate malignancy. It is a rare but not exceptional tumor, accounting for 0.1% of malignant skin tumors. Histological examination is essential for diagnosis. Wide surgical excision is the standard treatment. DFS is a tumor whose prognosis and evolutionary risk are mainly linked to the delay in diagnosis and the quality of the first excision. Late diagnosis makes excision and reconstruction surgery difficult. The chances of recovery in the case of well-performed primary surgery are significantly greater than in the case of salvage surgery. To improve prognosis, early, codified, multidisciplinary management is essential. In our African context, and especially in the case of patients living in rural areas, errant diagnoses are often found hence the importance of raising awareness and providing information to healthcare personnel. We report a case of an enlarged left supraclavicular Darier-Ferrand dermatofibrosarcoma. The patient had come for a late consultation at a stage when the tumour was large. The CT scan was a great help in the pre-operative phase. The patient underwent complete surgical excision, the postoperative course was straightforward and the histology of the surgical specimen confirmed the diagnosis. The resection margins were healthy. The patient is alive at one year with no recurrence or metastasis.

Background: Resistance to anti-thyroid drugs (ATDs) is a rare entity recently described. We report two African observations in the treatment of Graves’ disease. Case 1: A 19-year-old Senegalese woman presented on admission with thyrotoxicosis syndrome associated with diffuse goitre and Grave’s orbitopathy. TSH levels were low (0.005 mIU/ml;N = 0.27 - 4.20) and fT4 elevated (60 pmol/L;N = 12 - 22]. Combination therapy with propranolol (40 mg/day) and carbimazole (starting dose of 45 mg/day and increased to 60 mg/day) was initiated. In view of the persistence of symptoms despite good therapeutic compliance, carbimazole was replaced by methimazole with an initial starting dose of 40 mg/day, followed by 60 mg/day. Despite the change in therapy, clinical symptoms of thyrotoxicosis persisted, and fT4 levels remained elevated. The patient was diagnosed with resistance to ATDs in Graves’ disease. Total thyroidectomy following 10 days of preoperative preparation with 1% Lugol’s solution was performed successfully. As part of the training Master2in Business Administration, Operations Research work is carried out. The research is presented as a memory validated and defended in front of a panel of professors. It is in this perspective that we reflect on the topic "Customer Relationship Management as a Tool of Satisfaction of Customer and Company:The case of SONATEL Dakar." This topic is very interest because telecom sector is spearheading Senegal. In recent years, mobile telephony, internet, fixed merge simultaneously growth dynamism and innovation due to increased competition in this market segment. Customer Relationship Management （CRM） is a branch of marketing. It is more relevant in the field of telecommunications as it is essential for any business. CRM allows the company to build a single customer relationship as the company and the customer gain from the exchange, providing both parties benefit in the long term relationship. The purpose of this research is to analyze the CRM and also evaluate customer satisfaction of SONATEL Dakar. To approach this understanding, the thesis begins with three questions on which the first studies were published. Among these previous studies were collected and through them a questionnaire was created and submitted to102persons, taken as sample, within the customers of SONATEL Dakar. Then the data were collected and were analyzed by using SPSS software. The findings show that there has a relationship between a successful CRM and customer satisfaction and company. The study shows that CRM is a pillar of customer satisfaction. Indeed, if a customer is satisfied it is faithful and profitable for the company. It was also found that the relationship between SONATEL and his client should be improved. Most customers are satisfied because of the quality and safety of the products as the relationship with the company. The recommendations, in summary, are that the company should more invest in the relationship with their clients and also

This work aims to look for a simplifying surface that can represent the effect of the dual wheels on the variation of the stress and deformation state prevailing during the passage of traffic loads. This was facilitated by the results of thiam (2016) [4] obtained on the distribution of the vertical contact stress in the space described by the dual wheels. The analysis of the results of this study, on all the loading circles considered, shows that the radius loading circle equal to 0.181 m makes it possible to most probably represent the effect of the dual wheels. With this new surface, the effect of the dual wheels can be determined in 2D. The choice of this load is confirmed by a study in case of overload. Thus, the single axle with dual wheels is represented by a simplified diagram equipped on each side by a disk of radius 0.181 m. These results are obtained using a numerical simulation under Cast3M with a gravelly lateritic pavement.

Taking into Account the Effect of Dual Wheels on the Behavior of Lateritic Gravelly Pavements

The representation in the three-dimensional space of the physical model symbolized by the application of the pairing of the wheels described by 2 disks with a radius of 0.125 m and a 0.375 m spacing has the advantage of more accurately simulating the loading. In most cases, the assumption of a uniform distribution of loading on the circular surface is considered. In order to determine the effect of dual wheels, this research proposes a new approach to the distribution of vertical contact stress in three-dimensional space with loading on the circular surface. This allowed evaluating the maximum deformation on the space described by the dual wheels. The results showed that vertical deformation is maximum at the center of each circular surface. The distribution of the vertical contact stress is non-uniform on the circular footprint and has a significant influence on the deformation of the pavement at the level of the bituminous layer. The impact assessment of the twinning reveals that the dual wheels load causes less damage than that of a single wheel. These results are obtained by means of a numerical simulation under Cast3M with a gravitational lateritic pavement.

Prevalence and Clinical Characteristics of Chronic Constipation in Senegalese Children

Introduction: Constipation is the infrequent emission of hard stools. It is common, most often of functional origin, with a prevalence ranging from 0.7% to 29.6%. The aim of our study was to investigate the prevalence of constipation in children undergoing paediatric gastroenterology. Methodology: This was a bi-centric retrospective study carried out at the Albert Royer Children’s Hospital and the Dalal Jamm Hospital Pediatrics Department. Children followed in the pediatric gastroenterology consultation during the period from January 1, 2013 to February 29, 2020 were included. Results: A total of 337 children (205 boys;60.83%) were included, representing a prevalence of 25%. The median age was 3 years 6 months. The 13 - 24 months age group was the most frequent (27.6%). Rare stools were the main reason for consultation (92.8%). Rare stools were noted in 92.88% of cases. Bristol types 1 and 2 accounted for 6.53% and 54.60% respectively. The main associated signs were abdominal pain (29.4%), followed by abdominal bloating (20.96%). Abdominal pain was functional in 73.58% of cases. Under treatment, 90.19% of children had a favorable outcome. Conclusion: Constipation is common in Senegalese children with a prevalence close to that reported in Western countries. In most case, it is functional with good prognostic.

The study of the deflection due to the passage of an axle on a pavement structure has the advantage to make possible to pronounce on the portance, the rigidity and the homogeneity of this one. In the case where the allowed axle load is not respected, surface deflection leads to premature deterioration of the roadway. In order to study the evolution of these deformations, deflection measurements were made by using the Benkelman method in the Fatick-Kaolack road in Senegal by varying the axle load with the following values: 10, 13 and 16 tons on three different zones. The results show a linear evolution of the deflection as a function of the axle load. Also, the impact of truck overloading was studied by considering different values of the axle load with comparison to the allowed axle load by using frequent types of vehicle. A numerical simulation of the Cast3M pavement was done first, in the case of an isolated single wheel with a radius of 12.5 cm, then in the case of an equivalent dual wheel with a radius of 18.1 cm for loads of 10 to 19 tons. In the same way, the pavement design software (ALIZE) is used to analyse the variation of the axle load. The results show a linear evolution of the deflection, which corroborates the measurements made in the field and those obtained by Samb (2014) with a slope coefficient equal to 1.7. Nevertheless, the case of an isolated single wheel of 12.5 cm radius is closest to the measurements with a straight line of slope coefficient equal to 1.9.

Cardiac Echography in Pediatrics at the Regional Hospital of Diourbel: Indication and Result

Childhood heart disease is a real public health problem. In our contexts, care remains a major challenge. Doppler echocardiography remains the essential examination for diagnosis. The objective of our study was to describe the different clinical indications for cardiac ultrasound in children and the main pediatric heart diseases at the Diourbel Heinrich Lübke Regional Hospital. This was a descriptive and analytical retrospective study spanning from 2020 to 2022;covering a series of 140 cases. The study was carried out using patient clinical observation sheets and consultation sheets. We identified 140 ultrasounds. The average age of patients was 35.96 months with extremes of 0.03 months and 192 months. The female sex was predominant, with an M/F sex ratio of 0.72. Ultrasound was systematically requested in 67.14% of the study population. The indications found were mainly: heart murmur, polymalformative syndrome, respiratory failure, bronchiolitis, cardiomegaly, suspicion of heart disease, cardiomegaly, pre-therapeutic and pre-operative assessment. A total of 71 cardiac abnormalities were found in 65 patients, i.e. a prevalence of 46.42%. Congenital heart disease was more frequent (found in 49 patients, a prevalence of 35%) and was dominated by the CIA. There was a female predominance in almost all heart diseases except in the case of endocarditis where codominance is found. Our study has made it possible, through echocardiography, to evaluate the indications for echocardiography in children, to determine the prevalence of heart disease in children and also to assess the relevance of requests for echocardiography.
