Prevalence and Factors Associated with Virologic Failure among People Living with HIV (PLHIV) monitored in a Decentralized Health Care Facility

Introduction: In Senegal, the decentralization of Health Care Centers has contributed significantly to the intensification of antiretroviral treatment. However, Care providers are still facing the treatment optimizing challenge. Objectives: To determine the prevalence of virologic failures of PLHIV monitored in a decentralized Health care center and to determine associated factors. Patients and methods: This is a cross-sectional descriptive and analytical study of PLHIV, aged 18 years and over, on first-line treatment, monitored onsite from February 1st to December 31st, 2018. A data collection form was completed from medical records (clinical, immuno-virologic and evolutionary). Any VL > 1000 cp/ml after 6 months of antiretroviral therapy (ART) was considered as virologic failure. Data were captured and analysed using the EPI INFO 2002software. Chi-square test and the Fisher test were used to compare the proportions;a value of p ≤ 0.05 was considered significant. Results: 331 patients were treated with HIV-1 profile in 89% of cases. A proportion of 55% was married and 97% came from rural areas. 80% were either not or poorly educated. The median of age was 44 ± 11 years with a F/m ratio of 3.4. At baseline, 56% were symptomatic at stage 3 or 4 of WHO. They had severe immunosuppression with a median CD4 count of 217 ± 187 cells/mm3, the viral load was detectable in half of the patients with a median VL of 97,000 cp/ml ± 70,569. The antiretroviral regimen combined 2 NRTIs with 1 NNRTI in 88% of cases. The median of follow-up was estimated at 60 ± 43 months. The prevalence of virologic failure was 19%. This prevalence was associated with age less than 25 years (p = 0.04), late diagnosis (CD4 at baseline less than 200 cel/mm3 (p = 0.002), stage 3 or 4 WHO (p = 0.04) High viral load greater than 10,000 (p = 0.04) at baseline. Conclusion: These results suggest making the new therapeutic classes accessible for first-line treatment.

Prevalence of Hypertension and Associated Factors in Patients Living with HIV Followed at the Ambulatory Treatment Center (CTA) of Fann National University Hospital in Dakar

Introduction: The efficacy of antiretroviral therapy in people living with HIV (PLHIV) has been associated with an important increase in metabolic disorders, such as hypertension (HTA). This work allowed us to estimate the prevalence of hypertension in PLHIV and to describe associated factors. methods: A retrospective, descriptive and analytical study was carried out based on the records of people living with HIV followed at the Ambulatory Treatment Center (CTA), from January 1st 1998 to 31st, December 2014. The WHO criteria were used to diagnose hypertension. Data entry was performed using ESOPE software and data analysis was done using Epi Info software version 3.5.3. A multiple logistic regression was used to identify the risk factors associated with hypertension. Results: During the study period, 3624 patients followed at CTA were included of which 1184 patients (32.7%) had hypertension. The average age was 47. 3 years ±10.5 years, with a sex ratio of 0.7. The most common opportunistic infections at diagnosis were tuberculosis (14.9%) and oral candidiasis (15.3%). The HIV infection was advanced (stage 3 or 4 of WHO classification) for 39% of cases. Overweight and obesity was found in 19.1%. Only 17.6% were treated by protease inhibitors. Higher average age (OR:1,05;IC [1.04 - 1.05], p = 0.000001), higher average BmI (OR:5,3;IC [3.3 - 8.5], p = 0.00001), WHO clinical stage I-II (OR:1,4;IC [1.2 - 1.6], p = 0.00003), and ARV treatment (OR:2,5;IC [1.7 - 3.7], p = 0.000001) are associated with the occurrence of hypertension. Conclusion: The prevalence of hypertension was high among PLHIV and associated factors were: advanced age, high BmI, WHO clinical stages I and II and antiretroviral therapy. Hence, the interest of a systematic screening of hypertension and others cardiovascular risk factors particularly in patients under ARV antiretroviral therapy.

Background: Optimizing antiretroviral therapy is an essential step to reach the 90 - 90 - 90 targets. Despite tremendous progress made for antiretroviral treatment (ART) to be accessible in countries with limited resources, health care providers continue to face challenges due to the under-optimization of ART due to therapeutic failures and poor retention. Objectives: To determine the prevalence of adverse therapeutic outcomes in a decentralized health care center and to determine associated factors. Patients and methods: This is a cross-sectional descriptive and analytical study targeting PLHIV, aged 18 years and over, on first line antiretroviral treatment (ART), monitored onsite from February 1st to December 31st, 2018. A data collection form was completed from medical records (clinical, immuno-virological, therapeutic and evolutionary). Data were also collected from interviews with patients for additional socio-demographic information including the level of HIV knowledge. Data were captured and analyzed using EPI 2002 and R software. Proportions were compared using the chi -square and Fisher tests and logistic regression. A value of p married and 98% came from the rural area. 80% were either not or poorly educated. The median of age was 44 ± 11 years with a F/m ratio of 3.5. 30% that had not shared their HIV status, and more than half had a low knowledge of HIV transmission. At baseline, 56% were symptomatic at WHO stage 3 or 4. They had severe immunosuppression with a median CD4 count of 217 ± 187 cells/mm3;the viral load was detectable in half of the patients with a median viral load (VL) of 97,000 ± 70,569 cp/ml. The antiretroviral regimens combined 2 nucleoside reverse transcriptase inhibitor (NRTI) with 1 no nucleoside reverse transcriptase inhibitor (NNRTI) in 88% of the cases. Family Planning Knowledge and Practices among Reproductive Age Women in Widou Thiengoly, Louga Region, Senegal

Family planning is an effective intervention to improve the health of the mother, newborn and child. It was introduced in Senegal in the 1960s. Significant disparities were noted between urban and rural areas. This research aimed at measuring family planning methods knowledge and practices among reproductive age women (RAW) in the village of Widou Thiengoly, Louga region, Senegal. A descriptive and analytical cross-sectional study was carried out from 12 to 20 August 2016 and involved 150 RAW with an average age of 29.34 years. They were mostly married (98%), with a fertility rate of 3.22 children/woman. 67.3% of RAW knew at least one FP method and the contraceptive prevalence was 40.7%. Birth spacing is the main reason or utilization for more than half of users, while for non-users, more than half were ignorant of FP. Knowledge and practices of contraception were not related to the age of WCA, or the number of children. Fears or experiences of side effects were barriers to initiation and continuous use of FP methods, hence the importance of focusing awareness campaigns on alleviating fears about side effects.

Epidemiological and Clinical Aspects of Domestic Violence in Senegal

Introduction: Despite being under-reported, domestic violence remains a significant challenge in Senegal. The aim of this study is to provide a descriptive analysis of the epidemiological and clinical factors characterizing domestic violence in Senegal. methodology: A descriptive and retrospectively observational study was conducted. The data was collected from the court records of female victims of physical and/or sexual violence registered from 2006 to 2015. Female victims of physical and/or sexual violence at the hands of their husbands who had a court record at one of Senegal’s high courts during this period were also included. All records that met the inclusion criteria were selected for the study, and the data was analyzed using Epi Info 3.3.2. Results: According to the court records of 148 female victims of domestic violence, the average age of the victims was 30.6 ± 10.1 years. more than 3/4 (76.4%) of the victims were housekeepers, 82.4% of whom were uneducated. The average age of the perpetrators was 40.4 ± 11.4 years, and they were self-employed in the informal sector in 47.3% of the cases. Additionally, more than 3/4 (78.4%) of the perpetrators were uneducated. Eleven women (8% of the victims), were abused while pregnant. Physical violence was predominant (95.3%), while those associated with sexual assault accounted for 4.7% of cases. Of the 7 recorded cases of sexual violence, 3 were cases of unwanted sexual touching, and all cases of physical violence were cases of assault and battery. The violence took place at the home of the perpetrators in 81.8% of cases. In 84.7% of the cases, victims received treatment and care within 24 hours or less. Among the victims, 73% showed clinical lesions. Contusions, hematomas and penetrating wounds were most frequent, representing 23.1%;19.4% and 13.9% of cases respectively. Introduction: Abscesses of the liver are due to an infection of bacterial, parasitic or fungal origin. Through amoebiological imaging and serology, their diagnosis was facilitated. The aim of this study was to determine the epidemiological, clinical, diagnostic and evolutionary aspects of these liver abscesses at the Department of Infectious and Tropical Diseases in Fann Teaching University Hospital. Patients and methods: This was a retrospective and descriptive study, based on records of patients hospitalized for abscess of the liver over a period of 8 years, from January 1, 2008 to December 31, 2015. Results: We collected 20 cases of abscesses including 12 cases of confirmed amoebic abscesses and 5 cases of pyogenic abscesses. The mean age of the patients was 43.15 ± 15.12 years. There were 18 men and 2 women. The average hospital stay was 32 days. The most common clinical signs were abdominal pain (100%), fever (75%), hepatomegaly (80%). A leukocytosis greater than 12,000 Gb/mm3 was found in 13 patients. The amoebic serology was positive in 71% of cases and negative in 29% of cases. No germ was isolated from the blood culture. On hepatic ultrasound the abscess was unique in 90%. Treatment was exclusively medical in 11 patients (55%). The trend was favorable in 85% of cases. Conclusion: The incidence of liver abscesses has significantly decreased. It is important to develop diagnostic tools better in the case of pyogenic abscesses.

Case-Control Study on the Vulnerability of Women to HIV, Kaolack, 2019

Introduction: Despite a low prevalence that hides disparities between regions and genders, HIV infection in Senegal is progressing. Women are more vulnerable. The objective was to study the determinants of women’s vulnerability to HIV/AIDS infection. methodology: This was a case-control study carried out at the Kasnack Health Center, Kaolack in 2019. And 80 HIV-positive cases and 240 HIV-negative controls participated in the study. A questionnaire was administered to the selected women after their consent. Data were entered and analyzed using Epi Info and Stata software. Results: the mean age of the cases was 32 ± 9.7 years and that of the controls was 29.5 ± 8.5 years. Women were often housewives or traders;half of them had no schooling. Care was accessible for 74.4% of women. The level of knowledge of means of HIV prevention was medium in 71.3% of cases and 0.8% of controls. That of the modes of transmission was medium in 47.5% of cases and 0.8% of controls. The majority of controls (99.2%) had a low level of knowledge of means of prevention and modes of HIV transmission. Risky practices were 40% and 7.9% among cases and controls, respectively, and vulnerability was 40% and 7.9%, respectively. Conclusion: The level of HIV knowledge is low among most women. It is better in HIV-positive women who are more exposed to risky practices that increase their vulnerability. Reducing this vulnerability requires improving their knowledge and their empowerment.

A Cross-Sectional Survey on Non-Communicable Diseases and Risk Factors in the Senegalese Army

Background: The non-communicable diseases (NCDs) have become a public health priority. The objectives of this study were to measure the prevalence and to assess the risk factors of NCDs among the Senegalese military population to initiate an intervention program. methods: A cross-sectional survey was conducted in 2014. Two-level stratification was used to sample participants. Data were collected following the protocol recommended by the World Health Organization. Data were managed using Epi-Info 6 software and analyzed using R software. Results: A total of 1224 participants were recruited, of whom 96.9% were men. Their ages ranged from 25 to 60 years with a mean of 39.7 ± 9.2 years. Of participants, 17.2% were active smokers. Average duration of active smoking was 19.9 ± 9 years. The prevalence of current alcohol consumption was 11.5%, with an average of 4 ± 2.7 glasses a day. 97.17% of participants consumed fewer than 5 servings of fruits and vegetables a day. 18.63% had insufficient physical activity. The overall prevalence of high blood pressure was 26.9%. The proportion of those who were overweight was 27.2%;3.3% were obese. 3.0% of participants had diabetes and 44.1% had hypercholesterolemia. After adjusting for waist circumference, the risk of high cholesterol was 2.42 in the 35 - 44 age group and 2.86 in the 45 - 60 age group in comparison with the 25 - 34 age group. 32% were classified as having stage 2 chronic kidney. Conclusions: The findings of this study indicate the importance of risk factors for NCDs in the Senegalese military population. Intervention based on prevention and health promotion is needed.

Study on the Perceptions and Social Representations of Husbands on Family Planning in the Health District of Pout/Senegal

Contraception is a set of procedures that aim to achieve temporary infertility in a woman without her ability to conceive being engaged in the future. It plays an essential role in the health of women and children, the psychological well-being of husbands and allows, among other things, savings for the family. The objective was to study the perceptions and social representations of husbands on family planning. A mixed method was used with a qualitative dominance, carried out in the health district of Pout. The result showed that the perceptions and representations of husbands influence this practice family planning. Even if they have positive attitudes and a good level of knowledge about contraceptive methods, their involvement in FP practices remains low with gaps to be filled. Direct or intermediate variables such as the social, cultural, economic and religious aspects are pitfalls. Indeed, the commitment of men and the consideration of their social attributes are variables to be integrated into the process of information and awareness on family planning;this is crucial in achieving Sustainable Development Goal (SDG) 3 (Good Health and Well-being), including its target 7 focused on sexual and proactive health: a second best move towards the demographic dividend.

Low Socio-Economic Level: A major Obstacle to Therapeutic Observation (Survey of 125 diabetic Patients)

Introduction: diabetic patients adhering to their treatment are less likely to face complication from their disease. For this reason, we studied the compliance of diabetics with their treatment. materials and method: This was a comprehensive cross-sectional study of adherence in diabetic patients. All patients meeting the inclusion criteria were enrolled in the study. This study took place at a private pharmacy called “pharmacy Elhadj Seydou Nourou Tall” located in the city of Pikine during the period from April 1 to September 30, 2019. Results: In total, the study population was composed of 125 diabetics;frequency of 8.47%. The average age was 62.5 years and there were more women (58.4%). Of the patients, 87% had a low socioeconomic level and 72% a low educational level. Only 65.6% of the patients knew their diabetes type. Even though 97.6% of the patients were confident in the treatment but due to financial reasons 20.8% did not adhere to the prescribed treatment and 16.8% to their diets. moreover, 15.2% of the patients had changed the dosage of the prescribed medication, 44.4% had forgotten to take their medication regularly. 78.4% of them thought they were taking too much medication. No factors were found to be statistically associated with poor adherence, but the proportion of poor adherence patients was higher in patients over 60 years of age, with low socio-economic status and low education. Conclusion: Low socio-economic status is the main barrier to good adherence. Recommendations have been formulated to improve this adherence.
