Biliary fistula after treatment for hydatid disease of the liver:When to intervene

AIM:To determine the outcome of patients with biliary fistula(BF)after treatment for hydatid disease of the liver. METHODS:Between January 2000 and December 2010,out of 301 patients with a diagnosis of hydatid cyst of the liver,282 patients who underwent treatment [either surgery or puncture,aspiration,injection and reaspiration(PAIR)procedure]were *** were grouped according to the presence or absence of postoperative biliary fistula(PBF)(PBF vs no-PBF groups,respectively).Preoperative clinical,radiological and laboratory characteristics,operative characteristics including type of surgery,peroperative detection of BF,postoperative drain output,morbidity,mortality and length of hospital stays of patients were compared amongst *** analysis was performed to detect factors predictive of *** operative characteristics(ROC)curve analysis were used to determine ideal cutoff values for those variables found to be significant.A comparison was also made between patients whose fistula closed spontaneously(CS)and those with intervention in order to find predictive fac-tors associated with spontaneous closure. RESULTS:Among 282 patients[median(range)age, 23(16-78)years;77.0%male];210(74.5%)were treated with conservative surgery,33(11.7%)radical surgery and 39(13.8%)underwent percutaneous drainage with PAIR procedure A PBF developed in 46(16.3%) patients,all within 5 d after *** maximum cyst diameter and preoperative alkaline phosphatase levels(U/L)were significantly higher in the PBF group than in the no-PBF group[10.5±3.7 U/L vs 8.4±3.5 U/L(P40 U/L)and a peroperative attempt for fistula closure were significant predictors of PBF development (P=0.02,95%CI:-0.03-0.5 and P=0.001,95%CI:0.1-

HEMS-enabled transactive flexibility in real-time operation of three-phase unbalanced distribution systems

This paper proposes a coordinated two-stage real-time market mechanism in an unbalanced distribution system which can utilize flexibility service from home energy management system(HEMS)to alleviate line congestion,voltage violation,and substation-level power *** the grid level,the distribution system operator(DSO)computes the distribution locational marginal prices(DLMPs)and its energy,loss,congestion,and voltage violation components through comprehensive sensitivity *** using the DLMP components in a firststage optimization problem,the DSO generates two price signals and sends them to HEMS to seek flexibility *** response to the request of DSO,each home-level HEMS computes a flexibility range by incorporating the prices of DSO in its own optimization *** to future uncertainties,the HEMS optimization problem is modeled as an adaptive dynamic programming(ADP)to minimize the total expected cost and discomfort of the household over a forward-looking *** flexibility range of each HEMS is then used by the DSO in a second-stage optimization problem to determine new optimal dispatch points which ensure the efficient,reliable,and congestionfree operation of the distribution ***,the second-stage dispatch points are used by each HEMS to constrain its maximum consumption level in a final ADP to assign consumption level of major appliances such as energy storage,heating,ventilation and air-conditioning,and water *** proposed method is validated on an IEEE 69-bus system with a large number of regular and HEMS-equipped homes in each phase.

Synthesis and structure of Ti complex having a pair of chelating aminotropone[O-N]ligand have been *** of density functional theory（DFT） studies suggest that bis（aminotropone） Ti complex,when activated with methylaluminoxane （MAO）,have high potential for the polymerization of olefinic *** theoretical studies also show that the active species derived from bis（aminotropone） Ti catalyst normally possess higher electrophilicity nature compared with those produced using bis（phenoxyimine） Ti complexes（Ti-FI catalysts） which are known as high performance olefin polymerization catalysts. Bis（aminotropone） Ti catalyst generates a catalytically active species that has higher electrophilicity than a Ti-FI catalysts.

The time‑varying effects of oil prices on oil-gas stock returns of the fragile five countries

This study analyzes oil price exposure of the oil–gas sector stock returns for the fragile five countries based on a multi-factor asset pricing model using daily data from 29 May 1996 to 27 January *** endogenous structural break test suggests the presence of serious parameter instabilities due to fluctuations in the oil and stock markets over the period under ***,the time-varying estimates indicate that the oil–gas sectors of these countries are riskier than the overall stock *** results further suggest that,except for Indonesia,oil prices have a positive impact on the sectoral returns of all markets,whereas the impact of the exchange rates on the oil–gas sector returns varies across time and countries.

背景:前列腺癌是全球最常被诊断的癌症之一,并且是美国男性因癌症死亡的第二大病因。与白人男子相比,非洲男子更容易出现前列腺癌的早期病变,且较容易被诊断出来,而且死亡率更高,且随年龄增长前列腺癌的发病率增加。2018年全球登记的新发前列腺癌约占男性所有新发癌症病例的7.1%。尽管前列腺癌经过早期诊断和有效治疗(如手术和化疗)可提高患者生存率,但前列腺癌仍然是全球面临的主要健康问题。因此,探索前列腺癌致病的分子机制对于开发新颖、有效的前列腺癌治疗非常重要。Sirtuin家族由SIRT1-SIRT7组成,在细胞凋亡、线粒体生物合成、脂质代谢、脂肪酸氧化、细胞应激反应、胰岛素分泌和衰老等不同的细胞过程中发挥重要作用。有研究表明SIRT4可通过抑制谷氨酰胺过表达来抑制和抑制诸如结肠直肠和胃肿瘤等肿瘤细胞的生长、增殖和转化,但目前尚无研究对其功能在前列腺癌发生发展中的作用。目的:明确SIRT4在前列腺癌细胞系中的表达情况,并研究其在前列腺癌细胞中增殖和侵袭的作用。方法:应用实时定量逆转录聚合酶链反应(RT-qPCR)技术检测正常细胞与LNCaP细胞中SIRT4基因表达情况。通过细胞转染技术将SIRT4质粒插入LNCaP细胞。应用CCK-8法检测不同组LNCaP细胞增殖能力变化。运用Transwell技术检测细胞侵袭能力的改变。应用Western blot实验和RT-qPCR技术检测KLK3、P53、GLUD-1、mTOR表达情况。结果:RT-qPCR结果提示较正常细胞相比,前列腺癌细胞中SIRT4表达下调。CCK-8实验结果提示过表达SIRT4基因抑制LNCaP前列腺癌细胞的增殖能力。Transwell实验结果提示过表达SIRT4基因抑制LNCaP前列腺癌细胞侵袭进程。WESTERN BLOT和RT-qPCR结果提示,SIRT4相关基因KLK3、GLUD-1于SIRT4过表达组表达下调,P53、mTOR基因于SIRT4过表达组表达上调,差异有统计学意义(P<0.05)。结论:在这项研究中,我们发现SIRT4在前列腺肿瘤细胞系LNCaP低表达,且过表达SIRT4抑制前列腺恶性细胞的增殖和侵袭,对SIRT4相关基因KLK3、GLUD-1、MTOR、P53的影响可能是其潜在机制。

Predicting the Two-Phase Liquid-Solid Drag Model Using the Calculus of Variation

The simplified momentum equations of the two-phase flow have been adopted as the basic assumptions in this study. For vessels of small diameter, the shear stress becomes important and the friction pressure drop proposed by Ergun considers this effect by involving the wall effect. By replacing the Ergun pressure drop and the first order velocity term for particles drag model in the momentum equations, the relation for the drag coefficient versus the volume fraction is obtained. The calculus of variations is used with certain restriction for extremization of this drag coefficient. An analytical correlation for the drag coefficient is obtained depending on the volume fraction of “fluid particles”. The drag function obtained in previous studies does not match with the empirical data in the bed volume fraction range of [0.45 to 0.59]. Therefore, the function is modified and the results are better adjusted with the empirical data.

Effect of Knitted Structures and Yarn Count on the Properties of Weft Knitted Fabrics

This paper is focused on change in shrinkage, GSM, Wales per Inch and Course per Inch due to structure and count variation and state variation. This work was carried out with 12 samples of single jersey, single lacoste and weft lock knit fabrics of different counts which were scoured with NaOH. After pretreatment process the samples were tested for widthwise shrinkage, width, GSM, Wales per Inch and Course per Inch and compared between different states like grey state and finished state. The result obtained in this research indicated that physical properties like structures with tuck stitch possess higher value of widthwise shrinkage, width, GSM and Wales per Inch and Course per Inch value lowers prominently in those structures having float or miss stitches. In both cases i.e. grey state and RFD state values of those properties are high in coarser yarn and low in finer yarn. During project knitted fabrics specification along with machine specification and major factors which are necessary to calculate different types of variation and variables on which the whole fabric construction depends were managed to watch carefully. Main efforts were to develop a dependable way so that the resulting fabric specifications with required configuration can be easily visualized and be forecasted. A great emphasization was provided to the adjustable points on which fabric GSM, stitch length, fabric width and shrinkage percentage directly or indirectly depends.

Retrorectal(also known as presacral) tumor(RT) is a rare disease of retrorectal space. They can be classified as congenital, inflammatory, neurogenic, osseous, or miscellaneous. The most common presentation is an asymptomatic mass discovered on routine rectal examination, but certain nonspecific symptoms can be elicited by careful history and physical examination. The primary and only satisfactory treatment is surgery for RTs. Three approaches commonly used for resection are abdominal, transsacral, or a combined abdominosacral approach. Prognosis is directly related primary local control, which is often difficult to achieve for malignantlesions.
