Aim： To evaluate the effectiveness of a community-based intervention program for NCD （non-communicable disease） risk factors prevention. Materials and methods： The authors conducted a quasi experimental study with a control group over five years between 2009 and 2014 in the region of Sousse, Tunisia. In each group, the sample size required was 1,000 participants. For this, the authors randomly selected 500 households from two areas and they included all adults aged 18 years and older in these households. They evaluated the habits of participants from both intervention and control groups at baseline in 2009-2010 and after three years of intervention in 2013-2014. They focused on the main lifestyle risk factors represented by poor diet, physical inactivity, and tobacco use. Results： Tobacco use decreased significantly among men by 7.3% （/9 = 0.03） in the intervention group, versus an increase of 3.5% in the control group （p = 0.4）. Physical activity and fruits and vegetables intake increased significantly respectively by 25% （p 〈 0.001） and 19% （p 〈 0.001） in intervention group. Conclusion： The results suggest that a combined community-based lifestyle intervention in a developing country, can significantly improve some NCD risk factors. However, a supportive environment or a gender approach is required to maximize the effectiveness and maintain the sustainability of the health intervention.

Intracranial complications of sinusitis are rare. However, they have an important morbidity and mortality rate, and can be a source of disabling neurological sequelae in the absence of a rapid diagnosis and adequate treatment. We carry a retrospective study of 23 patients having sinusitis with intracranial complications, treated between 1996 and 2011. All patients underwent complete ENT and neurological examination, biological investigations and sinonasal and cerebral CT. An intraveinous large-spectrum antibiotherapy was administered to all patients. Twenty patients underwent surgery. It included evacuation of the intracranial collection, sinus drainage, with or without cranialization of the frontal sinus. Evolution was assessed on clinical biological and radiological criteria. Mean age was 25 years and sex-ratio was 3.6. Neurological signs were the most frequent symptoms. Rhinological signs were essentially purulent rhinorrhea (14 cases) and nasal obstruction (12 cases). Nasal endoscopy showed pus in the middle meatus in 10 cases. On CT, intracranial complications included subdural empyema (11 cases), extradural empyema (7 cases) and brain abscess (5 cases). Associated cerebral thrombophlebitis was noted in 4 cases. Three patients with extradural empyema had had an exclusive medical treatment. All other patients (20 cases) were operated. Clinical and radiological evolution was favorable after initial treatment in 14 cases (60.8%). Six patients required secondary surgery. Two patients have died despite intensive care. The intracranial complications of sinusitis are serious and source of important morbidity and mortality. Management should rapid and adequate, combining effective antibiotic therapy and eventually neurosurgical treatment.

syndrome （BCS） and identify the aetiologies of thisdisease in ***： Patients with BCS, hospitalised in our unitfrom January 2004 until June 2010 were included and theaetiological factors were assessed. Patients presentinga BCS in the setting of advanced-stage cirrhosis or aliver transplantation were excluded from the study. Thediagnosis was established when an obstruction of hepaticvenous outflow （thrombosis, stenosis or compression）was demonstrated. We diagnosed myeloproliferativedisease （MPD） by bone marrow biopsy and V617FJAK2 mutation. Anti-phospholipid syndrome （APLS） wasdetected by the presence of anticardiolipin antibodies,anti-β2 glycoprotein antibodies and Lupus *** also detected paroxysmal nocturnal haemoglobinuria（PNH） by flow cytometry. Celiac disease and Beh？etdisease were systematically investigated in our *** anticoagulant protein deficiencies were alsoassessed. We tested our patients for the G20210Amutation at Beaujon Hospital. Imaging procedures wereperformed to determine a local cause of BCS, such as ahydatid cyst or a liver ***： One hundred and fifteen patients wereincluded. Mean follow up： 32.12 mo. Mean age： 34.41years, M/F = 0.64. Chronic presentation was frequent：63.5%. The revealing symptoms for the BCS wereascites （74.8%） and abdominal pain （42.6%）. Themost common site of thrombosis was the hepatic veins（72.2%）. Involvement of the inferior vena cava alonewas observed in 3 patients. According to the radiologicalinvestigations, BCS was primary in 94.7% of the cases（n = 109） and secondary in 5.2% （n = 6）. An aetiologywas identified in 77.4% of the patients （n = 89）; itwas multifactorial in 27% （n = 31）. The predominantaetiology of BCS in our patients was a myeloproliferativedisease, observed in 34.6% of cases. APLS was foundin 21.7% and celiac disease in 11.4%. Nasopharyngeal carcinoma: Long term follow-up of 83 patients

AIM: To analyzes the clinical, paraclinical, therapeutical and evolutive features of nasopharyngeal carcinoma(NPC).METHODS: All the patients treated for a nasopharyngeal carcinoma between 2000 and 2013 in the Ear-NoseThroat Department of the Military Hospital of Tunis, Tunisia were collected in this study. Eighty-three patients signed a written consent prior to study enrollment. The gender ratio(M/W) was 6.5 with a median age of 49 years(range 16-85). The median follow up time was 37 mo(18 to 108 mo). The evolution, during and after the end of therapy, was assessed on clinical biological and radiological exams. Different parameters were analyzed and compared to other series: complications of chemo and radiotherapy, recurrence of the disease, metastasis and overall survival ***: Of the 83 patients of our study, 15% had T1 tumors, 20% had T2, 23% had T3 and 41% had T4 disease. At the time of diagnosis, 14% of the patients had a cranial nerve deficit. Only 12 patients had exclusive radiotherapy and the remaining of our patients had concomitant radio chemotherapy. Iatrogenic complications were diagnosed in 53% of the cases: radioepithelitis(28%), radiodermatitis(9%), xerostomia(17%), osteoradionecrosis(3%), cerebral radionecrosis(1%) and a pancytopenia(17%). The follow-up period varied from 18 to 108 mo(average: 37 mo). During the first six months after treatment, a persistence of the disease was found in 11% of patients, while a recurrence of the cancer was diagnosed in 6% and distant metastasis developed in 14% of the patients. Fifteen patients needed remedial chemotherapy for a relapse or metastasis and five had palliative chemotherapy for very advanced cases. We report 3 cases of death during the follow-up. CONCLUSION: Despite its excellent radio-chemotherapy response, and general good prognosis, a careful follow-up of patients with NPC is necessary to detect and manage any iatrogenic complication, locoregional recurrence or metastasis of the disease.

Multiple primary cancers are becoming an increasingly frequent situation and are often the source of many diagnostic and therapeutic difficulties. We report the case of two patients diagnosed with head and neck synchronous carcinomas. The first case is a 33-yearold man with a history of a keratinizing squamous cell carcinoma of the eye lid and who was operated 4 mo later from a mucoepidermoid carcinoma of thesubmandibular salivary gland. The second case is a 71-year-old woman who underwent a total parotidectomy for a mucoepidermod carcinoma of the left parotid gland and who consulted 2 mo later for epistaxis. The explorations concluded to a squamous cell carcinoma of the nasopharynx. The patient had a complementary radiotherapy. No local neither distant recurrence of the two tumors has been detected after a follow-up of 36 mo in the first case and 24 mo in the second one.
