Inflammatory response related scoring systems in assessing the prognosis of patients with pancreatic ductal adenocarcinoma:a systematic review

BACKGROUND： Various scoring systems based on assessment of the systemic inflammatory response help assessing the prognosis of patients with pancreatic ductal *** the present systematic review we evaluated the validity of four pre-intervention scoring systems： Glasgow prognostic score（GPS） and its modified version（mGPS）, platelet lymphocyte ratio（PLR）, neutrophil lymphocyte ratio（NLR）, and prognostic nutrition index（PNI）.DATA SOURCES： MOOSE guidelines were followed and EMBASE and MEDLINE databases were searched for all published studies until September 2013 using comprehensive text word and MeSH terms. All identified studies were analyzed, and relevant studies were included in the systematic ***： Six studies were identified for GPS/mGPS with3 reporting statistical significance for GPS/mGPS on both univariate analysis（UVA） and multivariate analysis（MVA）.Two studies suggested prognostic significance on UVA but not MVA, and in the final study UVA failed to show *** studies evaluated the prognostic value of NLR. Six of them reported prognostic significance for NLR on UVA that persisted at MVA in 4 studies, and in the remaining 2 studies NLR was the only significant factor on UVA. In the remaining5 studies, all in patients undergoing resection, there was no significance on UVA. Seven studies evaluated PLR, with only one study demonstrated its prognostic significance on both UVAand MVA, the rest did not show the significance on UVA. Of the two studies identified for PNI, one demonstrated a statistically significant difference in survival on both UVA and MVA, and the other reported no significance for PNI on ***： Both GPS/mGPS and NLR may be useful but further better-designed studies are required to confirm their value. PLR might be little useful, and there are at present inadequate data to assess the prognostic value of PNI. Incidental findings at MRI-enterography in patients with suspected or known Crohn's disease

AIM:To determine the frequency and clinical impact of incidental findings detected with magnetic resonance imaging(MRI)-enterography in patients with suspected or known Crohn's disease(CD). METHODS:Incidental findings were defined as unexpected lesions outside the small intestine,not previously known or suspected at the time of referral, and not related to inflammatory bowel *** a systematic review of medical charts we analyzed the clinical impact of incidental findings,and compared the MRI findings with subsequent diagnostic procedures. RESULTS:A total of 283 patients were included in the analysis,and MRI detected active CD in 31%,fistula in 1.4%and abscess in 0.7%.Extra-intestinal findings notrelated to CD were recorded in 72 patients(25%),of which 58 patients(20%)had 74 previously unknown *** or incompletely characterized findings were detected in 17 patients(6.0%).Incidental findings led to 12 further interventions in 9 patients(3.2%) revealing previously unknown pathological conditions in 5(1.8%).One patient(0.4%)underwent surgery and one patient was diagnosed with a malignant disease. MRI detected incidental colonic lesions in 16 patients of which additional work-up in 4 revealed normal anatomy. Two patients(0.7%)benefitted from the additional examinations,whereas incidental findings led to unnecessary examinations in 9(3.2%). CONCLUSION:In a minority of patients with suspected or known CD,important incidental findings are diagnosed at ***,a substantial number of patients experience unnecessary morbidity because of additional examinations of benign or normal conditions.

Individual automatic detection and identification of big cats with the combination of different body parts

The development of facial recognition technology has become an increasingly powerful tool in wild animal indi-vidual *** this paper,we develop an automatic detection and recognition method with the combinations of body features of big cats based on the deep convolutional neural network(CNN).We collected dataset including 12244 images from 47 individual Amur tigers(Panthera tigris altaica)at the Siberian Tiger Park by mobile phones and digital camera and 1940 images and videos of 12 individual wild Amur leopard(Panthera pardus orientalis)by infrared ***,the single shot multibox detector algorithm is used to perform the automatic detection process of feature regions in each *** the different feature regions of the image,like face stripe or spots,CNNs and multi-layer perceptron models were applied to automatically identify tiger and leopard individuals,*** results show that the identiﬁcation accuracy of Amur tiger can reach up to 93.27%for face front,93.33%for right body stripe,and 93.46%for left body ***,the combination of right face,left body stripe,and right body stripe achieves the highest accuracy rate,up to 95.55%.Consequently,the combination of different body parts can improve the individual identiﬁcation ***,it is not the higher the number of body parts,the higher the accuracy *** combination model with 3 body parts has the highest *** identiﬁcation accuracy of Amur leopard can reach up to 86.90%for face front,89.13%for left body spots,and 88.33%for right body *** accuracy of different body parts combination is lower than the independent *** wild Amur leopard,the combination of face with body spot part is not helpful for the improvement of identiﬁcation *** most effective identiﬁcation part is still the independent left or right body spot *** can be applied in long-term monitoring of big cats,including big data analysis for animal behavior,and be helpful

Osteoarthritis is clinically associated with escalating chronic pain, physical disability, and decreased quality of life. Early diagnosis of mild osteoarthritis in relatively young patients presents an opportunity for primary health care providers to manage pain, increase quality of life, and decrease risk of disability. CONCLUSION Primary health care providers can implement these recommendations in their own practices to provide care to patients with knee osteoarthritis based on current best evidence.

Capacity of Old Trees to Respond to Environmental Change

Atmospheric carbon dioxide [CO2] has increased dramatically within the current life spans of long-lived trees and old forests. Consider that a 500-year-old tree in the early twenty-first century has spent 70% of its life growing under preindustrial levels of [CO2], which were 30% lower than current levels. Here we address the question of whether old trees have already responded to the rapid rise in [CO2] occurring over the past 150 years. In spite of limited data, aging trees have been shown to possess a substantial capacity for increased net growth after a period of post-maturity growth decline. Observations of renewed growth and physiological function in old trees have, in some instances, coincided with Industrial Age increases in key environmental resources, including [CO2], suggesting the potential for continued growth in old trees as a function of continued global climate change.

Recursion by herbivores is the repeated use of the same site or plants. Recursion by wild animals is rarely investigated but may be ubiquitous. Optimal foraging theory predicts site recursion as a function of the quality of the site, extent of its last use, and time since its last use because these influence site resource status and recovery. We used GPS collars, behaviour and site sampling to investigate recursion to foraging sites for two elephant Elephas maximus borneensis herds in the Lower Kinabatangan Wildlife Sanctuary, Borneo, over a 12 month period. Recursion occurred to 48 out of 87 foraging sites and was most common within 48 hours or between 151-250 days, indicating two different types ofrecursion. Recursion was more likely to occur if the site had previously been occupied for longer. Moreover, the time spent at a site at recursion was the same as the time spent at the site on the first occasion. The number of days that had passed between the first visit and recursion was also positively correlated with how much time was spent at the site at recursion. Habitat type also influenced the intensity of site-use, with more time spent at recursion within riverine/open grass areas along forest margins compared to other habitat types. Recursion is a common behaviour used by the elephants and its pattern suggests it may be a foraging strategy for revisiting areas of greater value. The qualities of recursion sites might usefully be incorporated into landscape management strategies for elephant conservation in the area [Current Zoology 60 （4）： 551-559, 2014].

Differential effects of two dominant plant species on community structure and invasibility in an old-field ecosystem

Aims In this study,we examined the effects of Solidago altissima(hereafter Solidago)and two species in the genus Verbesina,Verbesina virginica and Verbesina occidentalis(hereafter Verbesina),on the structure of an old-field plant community and establishment by an invasive plant species,Lespedeza cuneata(hereafter Lespedeza).Methods We removed Solidago,Verbesina and both Solidago and Verbesina from 4m^(-2)plots in an intact old-field community during two growing *** then quantified the effects of these removals on richness,evenness,diversity and composition of the subdominant plant *** also measured the total aboveground biomass and the aboveground biomass of the subdominant *** assess how these removals affected establishment by Lespedeza,we planted 20 seeds in each plot and tracked seedling emergence and survival for one growing *** Findings Subdominant community evenness and Shannon diversity were higher in plots from which Solidago and Verbesina were removed relative to control ***,there were no effects of dominant species removal on species richness or composition of the subdominant *** aboveground biomass was not affected by dominant species removal,suggesting that the community of subdominant species exhibited *** fact,subdominant community biomass was greater when Solidago,but not Verbesina,was *** availability was also greater in plots where Solidago was removed relative to control plots throughout the growing *** addition,removal of dominant species,in particular Solidago,indirectly reduced the emergence,but not survival,of Lespedeza seedlings by directly promoting subdominant community *** together,our results suggest that dominant old-field plant species affect subdominant community structure and indirectly promote establishment by Lespedeza.

PIV measurements of turbulent jets issuing from triangular and circular orifice plates

The present study experimentally investigated the near-field flow mixing characteristics of two turbulent jets issuing from equilateral triangular and circular orifice plates into effectively unbounded surroundings,*** particle image velocimetry(PIV) was applied to measure the velocity field at the same Reynolds number of Re=50,000,where Re = UeDe /with Ue being the exit bulk velocity and the kinematic viscosity of fluid,D e the equivalent *** instantaneous velocity,mean velocity,Reynolds stresses were *** the mean velocity field,the centreline velocity decay rate and half-velocity width were *** the mixing characteristics of the two jets,it is found that the triangular jet has a faster mixing rate than the circular *** triangular jet entrainments with the ambient fluid at a higher rate in the near *** is evidenced by a shorter unmixed core,faster Reynolds stress and centreline turbulence intensity *** primary coherent structures in the near field are found to break down more rapidly in the triangular jet as compared to the circular *** the entire measurement region,the triangular jet maintained a higher rate of decay and ***,all components of Reynolds stress of the triangular jet appear to reach their peaks earlier,and then decay more rapidly than those of the circular *** addition,the axis-switching phenomenon is observed in the triangular jet.
