Human metapneumovirus (hMPV), a newly discovered paramyxovirus, has been associated with acute respiratory tract infections (ARI) ranging from upper ARI to severe bronchiolitis and pneumonia. Important questions remain on the contribution of hMPV to ARI, and it’s impact on public health. During the 2001-2002 season, we conducted a collaborative study with four provincial public health laboratories to study the prevalence of this new virus in the Canadian population. A total of 445 specimens were collected from patients of all age groups suffering from ARI and tested for the presence of hMPV by RT-PER. Of these, 66(14.8%) tested positive for hMPV Positive(?)

Human metapneumovirus (hMPV) has been associated with respiratory illnesses like those caused by human respiratory syncytial virus (HRSV) infection. Similar to other pneumoviruses, genetic diversity has been reported for hMPV. Little information is currently available on the genetic variability of the G glycoprotein (G), which is the most variable gene in RSV and avian pneumovirus. The complete nucleotide sequences of the G open reading frame (ORF) of 24 Canadian hMPV isolates were determined. Phylogenetic analysis showed the existence of two major groups or clusters (1 and 2). All but one of the hMPV isolates that we examined belonged to cluster 1. Additional genetic variability was observed in cluster 1, which separated into two genetic subclusters. Within cluster 1 the nucleotide sequence identity for the G ORF was 74.2 to 100%.

AIM： To examine the relationships between pre-diag- nostic biomarkers and colorectal cancer risk and assess their relevance in predictive ***： A nested case-control study was designed to include all first primary incident colorectal cancer cases diagnosed between inclusion in the SUpplemen- tation en VItamines et Min^raux AntioXydants cohort in 1994 and the end of follow-up in 2007. Cases （n = 50） were matched with two randomly selected con- trols （n = 100）. Conditional logistic regression models were used to investigate the associations between pre- diagnostic levels of hs-CRP, adiponectin, leptin, soluble vascular cell adhesion molecule-1 （sVCAM-1）, soluble intercellular adhesion molecule-I, E-selectin, monocyte chemoattractant protein-1 and colorectal cancer risk. Area under the receiver operating curves （AUC） and relative integrated discrimination improvement （RIDI） statistics were used to assess the discriminatory poten- tial of the models. RESULTS： Plasma adiponectin level was associated with decreased colorectal cancer risk （P for linear trend -- 0.03）. Quartiles of sVCAM-1 were associated with increased colorectal cancer risk （P for linear trend = 0.02）. No association was observed with any of the other biomarkers. Compared to standard models with known risk factors, those including both adiponectin and sVCAM-1 had substantially improved performance for colorectal cancer risk prediction （P for AUC improve- ment = 0.01, RIDI = 26.5%）. CONCLUSION： These results suggest that pre-diag- nostic plasma adiponectin and sVCAM-1 levels are as- sociated with decreased and increased colorectal cancer risk, respectively. These relationships must be confirmed in large validation studies.

New Multilevel Mixed Topology Development to Improve Inverter Robustness for Domestic Photovoltaic Installations

Multilevel inverters are well used in grid connected domestic photovoltaic applications because of their ability to generate a very good quality of waveforms, reducing switching frequency, and their low voltage stress across the power devices. However, this kind of inverter has to be modified to both limit common-mode currents and improve the robustness of the system. This paper presents a new mixed 5-level inverter that meets these challenges. The operating principle of the converter is proposed. Several experimental measurements are described to validate this new concept. The output voltage and current and the THD of the output voltage are particularly discussed.

越来越多的自身免疫性脑炎(Autoimmune encephalitis,AE)患者的资料显示,其临床表现和结局的特异性依赖于患者脑脊液、血清中特定抗原的抗体。这些特异性包括了癫痫相关的临床表现及对抗癫痫药物的反应。虽然学者们对这一类疾病的研究热情不断增加,且发现了新的抗体和相关的临床综合征,但仍有一些问题需要进一步解答。首先,鉴于每一种自身免疫性抗体介导的综合征的严重程度、患者特点、治疗时间不尽相同,治疗需要个体化;其次,缺乏随机对照试验是形成适当的免疫治疗策略的重大障碍。文章就一些已阐明的AE患者的癫痫诊断和治疗方面的新进展和挑战作一综述,并阐述在这一新兴领域中合理应用精确药物的原则。
