Biomedical image processing is a hot research topic which helps to majorly assist the disease diagnostic *** the same time,breast cancer becomes the deadliest disease among women and can be detected by the use of different imaging *** mammograms can be used for the earlier identification and diagnostic of breast cancer to minimize the death *** the proper identification of breast cancer has mainly relied on the mammography findings and results to increased false *** resolving the issues of false positives of breast cancer diagnosis,this paper presents an automated deep learning based breast cancer diagnosis(ADL-BCD)model using digital *** goal of the ADL-BCD technique is to properly detect the existence of breast lesions using digital *** proposed model involves Gaussian filter based pre-processing and Tsallis entropy based image *** addition,Deep Convolutional Neural Network based Residual Network(ResNet 34)is applied for feature extraction ***,a hyper parameter tuning process using chimp optimization algorithm(COA)is applied to tune the parameters involved in ResNet 34 *** wavelet neural network(WNN)is used for the classification of digital mammograms for the detection of breast *** ADL-BCD method is evaluated using a benchmark dataset and the results are analyzed under several performance *** simulation outcome indicated that the ADL-BCD model outperforms the state of art methods in terms of different measures.

The Effects of Pre-Surgical Education on Patient Expectations in Total Knee Arthroplasties

As patients prepare for total-knee arthroplasty surgery, they have many expectations related to their long-term recovery and function. This research examined whether the use of a pre-surgical patient education class with an additional long-term expectation module addressing recovery during the first 12 months after surgery was more effective in modifying participant’s pre-surgical expectations than participants receiving the standard pre-surgical education class alone. Prior to the class each participant completed one disease-specific instrument, a general-health survey, and a total-knee replacement expectation survey. After the class, each participant once again completed the total-knee replacement survey. Included in the study were 42 participants who were enrolled in a pre-surgical education course that was randomized. The participants in the control group received the standard pre-surgical education addressing pre-surgical topics. The participants in the intervention group received the standard pre-surgical education plus an additional module that specifically addressed long-term recovery and function up to 12 months post surgery. The primary outcome of the data revealed that participants’ who received the standard pre-surgical education with the additional module and who had an educational level higher than highschool, had expectations that were able to be modified to coincide with the surgeons’ expectations.

Suitable energy platform significantly improves charge separation of g-C3N4 for CO2 reduction and pollutant oxidation under visible-light

The photocatalytic activities of g-C3N4can be significantly improved by increasing life time of the photogenerated charges. Here, in this work we introduced TiO2as proper energy platform to accept the photogenerated electrons from g-C3N4during photocatalysis. The nanophotocatalysts formed from the combination of a suitable amount of TiO2nanoparticles and g-C3N4nanosheets showed 8.75 and 4.22% enhancement in photocatalytic activities for CO2reduction and 2-chlorophenol(2-CP) degradation under visible light illumination as compared to bare g-C3N4. Based on the surface photovoltage spectra, photoluminescence spectra and examination of formed hydroxyl radicals, it was confirmed that these enhanced photoactivities were attributed to the much-improved charge separation via the electron transfer from g-C3N4to TiO2. From trapping experiments,it was found that hydroxyl radicals were the major species involved in the photocatalytic degradation of *** study is helpful to synthesize efficient photocatalysts to cope with energy and environmental issues.

Assessment scales for delirium: A review

Over the years many scales have been designed for screening, diagnosis and assessing the severity of delirium. In this paper we review the various instruments available to screen the patients for delirium, instruments available to diagnose delirium, assess the severity, cognitive functions, motoric subtypes, etiology and associated distress. Among the various screening instruments, NEECHAM confusion scale and delirium observation scale appear to be most suitable screening instrument for patients' in general medical and surgical wards, depending on the type of rater(physician or nurse). In general, the instruments which are used for diagnosis [i.e., confusion assessment method(CAM), CAM for intensive care unit(CAM-ICU), Delirium Rating Scale-revised version(DRS-R-98), memorial selirium assessment scale, etc. ] are based on various Diagnostic and Statistical Manual criteria and have good to excellent reliability and fair to good validity. Among the various diagnostic instruments, CAM is considered to be most useful instrument because of its accuracy, brevity, and ease of use by clinicians and lay interviewers. In contrast, DRS-R-98 appears to be a comprehensive instrument useful for diagnosis, severity rating and is sensitive to change and hence can be used for monitoring patients over a period. In the ICU setting, evidence suggests that CAM-ICU and Nursing Delirium Screening Scale had comparable sensitivities, but CAM-ICU has higher specificity. With regard to assessment of delirium in pediatric age group, certain instruments like Pediatric Anesthesia Emergence Delirium scale and pediatric CAM-ICU has been designed and have been found to be useful.

Nephrotic Sydrome Can Be a Marker for Prostatic Carcinoma

Paraneoplastic syndromes (PS) represent a large spectrum of symptoms, associated with malignant diseases. PS can be diagnosed in asymptomatic patients with occult carcinoma, clinically active cancer, and during clinical remission, suggesting a recurrence of the neoplasm. The underlying mechanisms of PS are not completely understood but several authors have suggested that the increased production of biologically active immune factors and cytokines from the neoplastic cells may underlie the etiology of PS. Although rare, the renal involvement of patients with prostatic carcinoma has been reported. The most common paraneoplastic-associated glomerulopathy in prostatic cancer is the membranoproliferative glomerulonephritis with nephrotic syndrome (NS). In this review, we aimed to discuss the incidence of nephrotic syndrome secondary to prostatic carcinoma, its challenging diagnosis, clinical manifestation, and treatment.

A comparison of survival and pathologic features of non-alcoholic steatohepatitis and hepatitis C virus patients with hepatocellular carcinoma

AIM： To compare the clinical outcome and pathologic features of non-alcoholic steatohepatitis （NASH） patients with hepatocellular carcinoma （HCC） and hepatitic C virus （HCV） patients with HCC （another group in which HCC is commonly seen） undergoing liver transplantation. METHODS： Patients transplanted for HCV and NASH at our institution from January 2000 to April 2011 were analyzed. All explanted liver histology and pre-trans- plant liver biopsies were examined by two specialist liver histopathologists. Patient demographics, disease free survival, explant liver characteristics and HCC features （tumour number, cumulative tumour size, vascular invasion and differentiation） were compared between HCV and NASH liver transplant recipients. RESULTS： A total of 102 patients with NA^SH and 283 patients with HCV were transplanted. The incidence of HCC in NASH transplant recipients was 16.7% （17/102）. The incidence of HCC in HCV transplant recipients was 22.6% （64/283）. Patients with NASH-HCC were statisti- cally older than HCV-HCC patients （P 〈 0.001）. A signif- icantly higher proportion of HCV-HCC patients had vas- cular invasion （23.4% vs 6.4%, P = 0.002） and poorly differentiated HCC （4.7% vs 0%, P 〈 0.001） compared to the NASH-HCC group. A trend of poorer recurrence free survival at 5 years was seen in HCV-HCC patients compared to NASH-HCC who underwent a Liver trans- plantation （P = 0.11）. CONCLUSION： Patients transplanted for NASH-HCC appear to have less aggressive turnout features com- pared to those with HCV-HCC, which likely in part ac- counts for their improved recurrence free survival.

Objective: To review current literature and experience with Gore-Tex? implant extrusion following medialization thyroplasty, as well as to report the unique case of Gore-Tex? implant extrusion following revision medialization ***: Review of existing literature and description of personal experience with unique case of Gore-Tex? implant extrusion following revision medialization ***: Review of existing literature found no prior reported cases of Gore-Tex? implant extrusion following revision medialization thyroplasty. Risk factors for implant extrusion include the pressure of the implant on insertion and the inability to secure the implant. Cases of implant extrusion can be managed operatively via an endoscopic or via an external open ***: This is the first reported case of Gore-Tex? implant extrusion following revision medialization thyroplasty. Careful consideration should be given in revision medialization thyroplasty as additional implant material may cause increased pressure, a risk factor for implant extrusion.

Computer-aided diagnosis(CAD)models exploit artificial intelligence(AI)for chest X-ray(CXR)examination to identify the presence of tuberculosis(TB)and can improve the feasibility and performance of CXR for TB screening and *** the same time,CXR interpretation is a time-consuming and subjective ***,high resemblance among the radiological patterns of TB and other lung diseases can result in ***,computer-aided diagnosis(CAD)models using machine learning(ML)and deep learning(DL)can be designed for screening TB *** this motivation,this article develops a Water Strider Optimization with Deep Transfer Learning Enabled Tuberculosis Classification(WSODTL-TBC)model on Chest X-rays(CXR).The presented WSODTL-TBC model aims to detect and classify TB on CXR ***,the WSODTL-TBC model undergoes image filtering techniques to discard the noise content and U-Net-based image ***,a pre-trained residual network with a two-dimensional convolutional neural network(2D-CNN)model is applied to extract feature *** addition,the WSO algorithm with long short-term memory(LSTM)model was employed for identifying and classifying TB,where the WSO algorithm is applied as a hyperparameter optimizer of the LSTM methodology,showing the novelty of the *** performance validation of the presented WSODTL-TBC model is carried out on the benchmark dataset,and the outcomes were investigated in many *** experimental development pointed out the betterment of the WSODTL-TBC model over existing algorithms.
