Chronic pancreatitis is a long-standing inflammation of the pancreas,characterized by progressive inflammatory and fibrotic changes,resulting in permanent structural damage of pancreatic parenchyma[1].Pancreatitis(chronic or acute)is the primary risk factor for pseudo-splenic artery aneurysms,along with pancreatic pseudocysts and *** the disease progresses,patients with chronic pancreatitis may develop complications due to exocrine and endocrine pancreatic functional loss,such as fat malabsorption with steatorrhea,glucose intolerance,and ultimately diabetes mellitus[1,2].Furthermore,severe pancreatic inflammation can cause weakening of vessel walls,with subsequent formation of arterial *** artery is the most commonly affected visceral artery[2,3].Liver cirrhosis,portal hypertension,liver transplantation,atherosclerosis,hypertension,pregnancy and multiparity are main risk factors for true splenic artery aneurysms[2].Rupture and intraperitoneal bleeding is the cause of death in 30%−50%of ***,prompt diagnosis and appropriate treatment are of great clinical importance[4,5].Herein,we present a case of a giant splenic artery pseudoaneurysm within a walled of necrosis involving pancreatic parenchyma,as a complication of long standing chronic pancreatitis.

The paper aims to defme the lawfulness （model）, by which one can reliably estimate the distribution of motor units＇ force from the moment when leg extensor muscles start to generate the force until the moment when the maximal level is achieved. The study included 110 participants. To assess the contractile characteristics of leg extensors, standardized equipment and standardized isometric test in sitting position with the angle of the knee joint of 125° were used. The participants were instructed to exert their maximal force as quickly as possible. Using descriptive statistics, cluster analysis and fitting the exponential model of the distribution of force of leg extensor motor units in the whole range of force generation is defined. The model has the following form： y = 0.2051e3.3855x, where y is the motor units force expressed in daN （decanewtons）, x is the time expressed in s （seconds）. It provides an understanding of the control of multivariate motor unit recruitment and distribution of their force during sports movements as well as training programming for the adoption of forms for conlrolling force distribution of motor units, the development of their maximum force and their involvement speed.

Specific cell subpopulations identified as cancer stem cells(CSCs)can be found in basal cell carcinoma(BCC).Generally,CSCs have a marked trans-differentiation potential that could potentially be used in differentiation ***,there are no studies regarding BCC CSCs *** aim of the study was to analyze the characteristic of CSCs of BCC with emphasis on their differentiation potential upon specific *** staining and cell morphology were used for differentiation confirmation,along with the expression analysis of osteogenic(ALP,BSP,Runx2,OCN,BMP2),chondrogenic(COL1 and COL2A1),adipogenic(PPAR-γ)and neurogenic(Nestin and MAP2)*** CSCs differentiated into osteogenic and chondrogenic lineages,as judged by staining and high expression of specific markers(from 2-to 92-fold higher upon induction).Concomitantly with differentiation,the levels of cancer stem cell markers decreased in the ***-differentiation and neuro-differentiation were *** conclusion,BCC CSCs exhibit the capacity to trans-differentiate,a characteristic that may potentially be useful in the development of new strategies for the treatment of aggressive BCCs.

Introduction： Demographic structure in Belgrade is regressive biological type with 16.4% of the Elderly in whole population. Addressing this fact Public Health Institute of City of Belgrade in 2008 established the EHO （Elderly Health Office）. Aim： This paper emphasizes the importance of Public Health response to ageing population challenges. Method： This public health study is retrospective review based on work of EHO from 2008 to 2017 year. Results： Numerous health promotion activities were performed by EHO： development guidelines for elderly care services coordination as well as for effective communication within health care system; development of age-friendly web portal; cooperation with the mass media; building partnership with representatives of the state- and private-owned facilities and NGOs （non-governmental organizations）; support to strengthening inter-generation cooperation; promotion of elderly age-friendly phone line; organizing and participation in public health events, meetings and conferences. EHO participated in evaluation capacities of healthcare in Belgrade, regarding human resources, equipment and facilities, and monitoring elderly health status. Conclusions： The role of the EHO oflPHB is：（1） support to active, healthy, productive ageing of the citizens of Belgrade; （2） strategic planning of the Elderly health care; （3） strengthening of intersectional cooperation. Measures suggested： Public Health approach of EHO is response to challenges of ageing population and potential model for age oriented program management.

Problems in screening colorectal cancer in the elderly

AIM:To explore the problems in the screening of colorecta carcinoma in the elderly. METHODS:Three models of colorectal cancer prevention were examined:standard screening,active check-up of suspected cases and summons to have endoscopic check- up for previously diagnosed colorectal *** study was performed among three groups of elderly individuals: Group 1 (167 cases),hospitalized asymptomatic individuals without symptoms in large *** 2 (612 cases): old individuals at home for the aged,out of which 32 showed symptoms of colon disorders;Group 3 (44 cases):elderly people with diagnosed *** a result of 1788 rectosigmoidoscopies,we identified 61 individuals with polyps,out of which 44 patients were over 65 years old. However,only 9 of these 44 individuals agreed to have the endoscopy performed again. RESULTS:One cancer and 13 polyps were detected in Group 1,and two polyps in Group ***,it should be noted that only eleven individuals from Group 2 agreed to have the *** Group 3,there were no relapses of the polyps among the nine individuals who came back for the endoscopy. CONCLUSION:Poor understanding of the screening procedures is one of the greatest problems in early detection of the cancer in the ***,the cooperation is better with hospitalized patients,than with 'successfully old' persons.

Environmental Monitoring Systems: Review and Future Development

Monitoring and recording large time series of data and making them available for studying—are the key roles of environmental monitoring systems. This study produce review of three different measurement monitoring systems (NSUNET, WAHASTRAT and MERIEXWA) with same design, which were placed during different time frame in the Northern Serbia (Vojvodina Province). Each of three monitoring systems has different demands and requirements which were addressed accordingly in their design. Power supply conditions for two systems are primary cells and solar panels, while NSUNET has constant power supply only during nighttime. Data is transmitted in MERIEXWA using binary protocol, WAHASTRAT using http, while NSUNET sends data over ftp. Same topology is used in all systems—each node sends data directly to the central location (in case of NSUNET two locations are provided for backup purposes). NSUNET system sends data using specific structure and stores them as plain text files. It also has different approach for time synchronization and monitoring issues. The main result of this study is to present how to create system that provides good quality and unchanged data from monitoring sensor to the end user while maintaining whole data structure transmission costs low. Furthermore, data collected from NSUNET were used in order to assess the influence of urbanization on regional climate modification, which leads to the creation of urban climate. Propositions for new system development combining best from all three systems are discussed.

Splenectomy with chemotherapy vs surgery alone as initial treatment for splenic marginal zone lymphoma

AIM： To evaluate the clinical characteristics of splenic marginal-zone lymphoma （SMZL） following antigen expression and the influence of therapeutic approaches on clinical outcome and overall survival （OS）.METHODS： A total of 30 patients with typical histological and immunohistochemical SMZL patterns were examined. Splenectomy plus chemotherapy was applied in 20 patients, while splenectomy as a single treatment-option was performed in 10 patients. Prognostic factor and overall survival rate were analyzed.RESULTS： Complete remission （CR） was achieved in 20 （66.7%）, partial remission （PR） in seven （23.3%）, and lethal outcome due to disease progression occurred in three （10.0%） patients. Median survival of patients with a splenectomy was 93.0 mo and for patients with splenectomy plus chemotherapy it was 207.5 mo （Log rank = 0.056, P 〉 0.05）. Time from onset of first symptoms to the beginning of the treatment （mean 9.4 too） was influenced by spleen dimensions, as measured by computerized tomography and ultra-sound （t = 2.558, P = 0.018）. Strong positivity （＋＋＋） of CD20 antigen expression in splenic tissue had a positive influence on OS （Log rank = 5.244, P 〈 0.05）. The analysis of factors interfering with survival （by the Kaplan-Meier method） revealed that gender, general symptoms, clinical stage, and spleen infiltration type （nodular vs diffuse） had no significant （P 〉 0.05） effects on the OS. The expression of other antigens （immunohistochemistry） also had no effect on survival-rate, as measured by a χ^2 test （P 〉 0.05）.CONCLUSION： Initial splenectomy combined with chemotherapy has been shown to be beneficial due to its advanced remission rate/duration; however, a larger controlled clinical study is required to confirm our findings.
