Timing of tracheostomy in mechanically ventilated COVID-19 patients

According to the World Health Organization as of September 16,2021,there have been over 226 million documented cases of coronavirus disease 2019(COVID-19),which has resulted in more than 4.6 million deaths and approximately 14%develop a more severe disease that requires respiratory assistance such as *** tracheostomy is recommended for patients that are expected to be on prolonged mechanical ventilation;however,supporting data has not yet been provided for early tracheostomies in COVID-19 *** aim of this study was to explore established guidelines for performing tracheostomies in patients diagnosed with *** considered were patient outcomes such as mortality,ventilator-associated pneumonia,intensive care unit length of stay,complications associated with procedures,and risks to healthcare providers that performed *** observational studies,meta-analyses,and systematic reviews were collected through a PubMed Database *** sources were found through *** search was refined to publications in English and between the years of 2003 and *** keywords used were“Coronavirus”and/or“guidelines”and/or“tracheostomy”and/or“intensive care”.Twenty-three studies were *** to the complex presentation of the respiratory virus COVID-19,previously established guidelines for tracheostomies had to be reevaluated to determine if these guidelines were still applicable to these critically ill ventilated *** specifically,medical guidelines state benefits to early tracheostomies in critically ill ventilated non-COVID-19 ***,after having conducted this review,the assumptions about the benefits of early tracheostomies in critically ill ventilated patients may not be appropriate for COVID-19 patients.

Comparative studies of elemental composition in leaves and flowers of Catharanthus roseus growing in Bangladesh

Objective:To investigate the elemental composition of the leaves and flowers of Catharanthus roseus(***) due to the plant's wide application in the indigenous medicinal system and its chemical constituents' ***:The atomic absorption spectrophotometer was used for quantitative analysis of various ***:Total 13 important elements were analyzed in leaves and flowers of *** indicated the presence of Na,K,Ca,Mg,Cr,Fe,Zn,Al,Cu,Ni,Pb,Cd and Mn in both leaves and *** most important finding of the work was that,leaves of *** showed high concentration of all elements except K and Zn while flowers of *** showed higher concentration of K and ***:The elemental composition in both leaves and flowers of *** were found to be ***,different parts of this medicinal plant are enriched in some micro and macro nutrients like Fe,Ca,Na,K,Zn,which are very important for biological metabolic system as well as human health.

葡萄苗木的活力和成活率是影响葡萄园品质的关键因素,苗木的越冬期是其植物生长发育的重要阶段之一。目前还没有对葡萄休眠期苗木的生命力和存活率进行准确评估的研究。本研究的目的是探讨如何利用分子信息评估苗木生存力和存活率,并利用该方法比较埋土与田间放置的葡萄苗越冬的效果,以及不同省份出产的苗木的抗寒能力。实验材料从河北,江苏和山东获得。为比较埋土与田间越冬巨峰葡萄苗木的活力差异,通过(Sq.)RT-PCR和qRT-PCR的方法检测裸根处理1-5天4个芽中6个看家基因的表达水平。结果表明,看家基因的表达与苗木活力和存活率呈正相关关系,较高的表达与较高的活力和存活率密切相关,较低的表达与较低的活力相关,最低的表达与最低的活力和存活率显著相关。而且,从DNA和RNA的质量就可以直接判断苗木质量的好坏。山东莒县、山东莱西、河北怀来、辽宁绥中、河北昌黎、山东冠县、江苏张家港等地育苗成活率分别为100.00%、100.00%、100.00%、100.00%、87.77%、93.33%。埋土越冬的苗木成活率高于田间放置的苗木。此外,所有省份的埋土葡萄苗均呈现较高成活率,而田间放置的葡萄苗则表现出不同的抗寒能力,河北、江苏和山东田间越冬苗成活率分别为87.77%、81.11%和77.77%。综上所述,分子技术是一种利用基因信息评估休眠葡萄苗木活力和存活率的潜在方法。冬季低温显著降低了南京地区葡萄休眠苗的成活率。因此,巨峰葡萄苗木采用埋土法能有效提高越冬成活率。

Computational Fluid Dynamics Analysis of Multi-Bladed Horizontal Axis Wind Turbine Rotor

The principal objective of this work was to investigate the 3D flow field around a multi-bladed horizontal axis wind turbine (HAWT) rotor and to investigate its performance characteristics. The aerodynamic performance of this novel rotor design was evaluated by means of a Computational Fluid Dynamics commercial package. The Reynolds Averaged Navier-Stokes (RANS) equations were selected to model the physics of the incompressible Newtonian fluid around the blades. The Shear Stress Transport (SST) k-ω turbulence model was chosen for the assessment of the 3D flow behavior as it had widely used in other HAWT studies. The pressure-based simulation was done on a model representing one-ninth of the rotor using a 40-degree periodicity in a single moving reference frame system. Analyzing the wake flow behavior over a wide range of wind speeds provided a clear vision of this novel rotor configuration. From the analysis, it was determined that the flow becomes accelerated in outer wake region downstream of the rotor and by placing a multi-bladed rotor with a larger diameter behind the forward rotor resulted in an acceleration of this wake flow which resulted in an increase the overall power output of the wind machine.

Group psychological intervention for maternal depression: A nested qualitative study from Karachi, Pakistan

AIM To understand the experience of maternal depression, the factors implicated in accessing health, and the acceptability of the psychosocial intervention. METHODS The participants were recruited from the paediatrics outpatient department of Civil Hospital Karachi, Pakistan. The study started in December 2009 and completed in December 2010. Women with maternal depression, aged 18-44 years with children aged 0-30 mo who had received nutritional supplements, and participated in the intervention programme [called Learning through Play(LTP) plus] were included in the study. Qualitative interviews were conducted with 8 participants before the intervention and 7 participants after the intervention. A semi structured topic guide was used to conduct the *** Framework analysis procedures were used to analyse the qualitative data. Four themes emerged:(1) the women's contextual environment: Interpersonal conflicts, lack of social support and financial issues being the major barriers in assessing healthcare;(2) women's isolation and powerlessness within the environment: Sense of loneliness was identified as a restricting factor to access healthcare;(3) the impact of the intervention(LTP-Plus): Women felt "listened to" and seemed empowered; and(4) empowered transformed women within the same contextual environment: The facilitator provided a "gardening role" in nurturing the women resulting in a positive transformation within the same environment. The women's homes seemed to be more happy homes and there was a positive change in their behaviour towards their *** Findings informed the further development and testing of culturally-appropriate psychosocial intervention(LTP^+) for addressing maternal depression.

Dependence of Laser Ablation Produced Gold Nanoparticles Characteristics on the Fluence of Laser Pulse

Effect of laser fluence on the characteristics of gold nanoparticles (NPs) produced by laser ablation method is investigated experimentally. Five samples of gold NPs were synthesized by nanosecond pulsed laser ablation of a high purity gold plate in distilled water at five different fluences in the range of 1-3 J/cm2. Results show that in this experimental condition gold NPs are produced by nucleation of Au atoms in the plasma, generated on the target surface during the ablation process. There is threshold fluence at which absorption of laser energy by water rises significantly. Absorption of the laser energy by water leads to increase oxygen atoms in the ablation medium and formation of gold oxide NPs. By increasing the laser fluence below the threshold, size of produced NPs decreases while by increasing the laser fluence above the threshold size of produced NPs increases.

Metacognition is defined as the knowledge and ability to monitor and control cognitive conditions in the learning *** is the most important skill of the four language skills in both learning and teaching,and plays an important role in our daily *** nowadays there is a deeper perception of listening,it needs more attention and ***,the present study investigates the relationship between the Metacognitive awareness and listening performance of female Dentistry students in English for General Purposes(EGP)course in Urmia University of Medical Sciences(UMSU).To do so,50 Iranian female dentistry students were participated in this *** completed Metacognitive Awareness Listing Questionnaire(MALQ)and listening section of the final ***,the listening section of final exam was applied to the participants at classrooms by the *** after the administration of the examination,the MALQ were *** analysis exposed a weak positive relationship between learners’Metacognitive awareness scores and listening *** correlation was not high,still there is more correlation in problem solving,directed attention and planning evaluation than others.

Non-invasive diagnosis of H pylori infection: Evaluation of serological tests with and without current infection marker CIM

AIM:To evaluate the performance of commercially available immunochromatographic (ICT) and immunoblot tests covering the current infection marker CIM and conventional ELISA for the diagnosis of H pylori infection in adult dyspeptic patients. METHODS:Consecutive non-treated dyspeptic patients undergoing diagnostic endoscopy were tested for H pylori infection by culture, rapid urease test, and histology of gastric biopsy specimens. Serum from 61 H pylori infected and 21 non-infected patients were tested for anti-H pylori IgG antibodies by commercial ELISA (AccuBindTM ELISA, Monobind, USA), ICT (Assure H pylori Rapid Test, Genelabs Diagnostics, Singapore), and immunoblot (Helico Blot 2.1, Genelabs Diagnostics, Singapore) assays. ICT and immunoblot kits cover CIM among other parameters and their performance with and without CIM was evaluated separately. RESULTS:Sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and accuracy of ELISA were 96.7%, 42.8%, 83.1%, 81.8%, and 82.9%, of ICT were 90.1%, 80.9%, 93.2%, 73.9%, and 87.8%, of ICT with CIM were 88.5%, 90.4%, 96.4%, 73.0%, and 89.0%, of immunoblot were 98.3%, 80.9%, 93.7%, 94.4%, and 93.9%, and of immunoblot with CIM were 98.3%, 90.4%, 96.7%, 95.0%, and 96.3%, respectively. CONCLUSION:Immunoblot with CIM had the best performance. ICT with CIM was found to be more specific and accurate than the conventional ELISA and may be useful for non-invasive diagnosis of H pylori infection.

Occurrence of Manganese Ore Deposits and Their Mineralogy in Vizianagaram-Visakhapatnam Manganese Ores Belt (Andhra Pradesh) India

The Manganese deposits of Andhra Pradesh are associated with a thick sequence of Precambrian rocks, belonging to Khondalite and Charnockite groups of Dharwar Supergroup that forms part of 2500 to 3000 m.y. old Eastern Ghat complex of India. The study area is the Manganese deposits of Vizianagram-Visakhapatnam Manganese Belt of Andhra Pradesh. The study area lies about 150 km NE of Visakhapatnam between 18°12′N - 18°30′N and 83°20′E - 83°45′E. The mineralization of Manganese ores is confined to different rock types, belonging to both Khondalite and Charnockite groups, where they are dispersed throughout the country rocks as small lenses, pockets, veins and irregular bodies of varying dimensions. Quartz, garnet, clay, limonite and apatite are the common gangue minerals in the Manganese ores. The presence of quartz, garnet and apatite brings down the grade of the ore. Ferruginous laterite and ochre generally work as capping of the Manganese deposits. The various Manganese ore minerals present in these deposits are indentified as 1) Primary minerals-braunite, bixbyite, vredenburgite, jacobsite and hausmannite, 2) secondary minerals-psilomelane, cryptomelane, hollandite, pyrolusite and wad. The primary ore minerals are considered to be syngenetic and regionally metamorphosed while, the associated secondary ore minerals are formed due to alteration of the primary ores.

Petrographic Studies of Pb-Zn Ore Deposits of Rajpura-Dariba-Bethumni Belt in District Udaipur (Rajasthan) India

The Proterozoic Aravalli-Delhi orogenic complex hosts a large number of economically important stratabound base metal sulphide deposits. In the present work, rock samples taken from Outcrop and Underground Mine of Sindeskar Kalan, Vedanta Group, Rajpura-Dariba-Bethumni Belt which are located at a distance of 76 Kms from Udaipur District (Rajasthan) have been studied. The chief litho units of the group are identified which contain sulfide-bearing calc-silicate and graphite mica schist, dolomite marble, calc-biotite schist and quartzite. Importance to the ore minerals like galena and sphalerite which have been reported in association with the buffer minerals like pyrite and pyrrhotite occurr in the host rocks of the study area. Ore petrography denotes two types of sphalerites, 1) highly deformed and dark brown colour, 2) less deformed and with light yellow colour. Galena shows high reflectance and a network of triangular pits. Pyrite shows high reflectance and anomalous anisotropism because of high brittleness.
