Due to the drastic increase in the number of critical infrastructures like nuclear plants,industrial control systems(ICS),transportation,it becomes highly vulnerable to several *** become the major targets of cyberattacks due to the increase in number of interconnections with other *** research works have focused on the design of intrusion detection systems(IDS)using machine learning(ML)and deep learning(DL)*** the same time,Blockchain(BC)technology can be applied to improve the security *** order to resolve the security issues that exist in the critical infrastructures and ICS,this study designs a novel BC with deep learning empowered cyber-attack detection(BDLE-CAD)in critical infrastructures and *** proposed BDLE-CAD technique aims to identify the existence of intrusions in the *** addition,the presented enhanced chimp optimization based feature selection(ECOA-FS)technique is applied for the selection of optimal subset of ***,the optimal deep neural network(DNN)with search and rescue(SAR)optimizer is applied for the detection and classification of ***,a BC enabled integrity checking scheme(BEICS)has been presented to defend against the misrouting *** experimental result analysis of the BDLE-CAD technique takes place and the results are inspected under varying *** simulation analysis pointed out the supremacy of the BDLE-CAD technique over the recent state of art techniques with the accuy of 92.63%.

The Role of Endoscopic Retrograde Cholangiopancreatography (ERCP) in the Management of Intra-Biliary Rupture of Liver Hydatid Cysts (IBRH): Follow-Up of 12 Cases

Background: Intra-biliary rupture of hydatid cyst [IBRH] is not a rare complication of hydatid disease of the liver and surgery until recently is the only definitive treatment. With the introduction of ERCP preoperatively or postoperatively there was a great reduction in the operative complications. There were reports in which the ERCP was the sole treatment of this condition. Objective: To evaluate role of ERCP in the management of cases of IBRH. Design: A retrospective study of 7 cases of IBRH and prospective study of 5 cases of IBRH managed by ERCP. Setting: The ERCP Unit of Kurditan Center for Gastroenterology & Hepatology, Asulaimaneyah-Iraqi, Kurdistan, Iraq. Main Outcome Measurements: Improvement in the symptoms, obstructive liver functions pattern and ultrasonic findings in these patients following ERCP management. Methods: This is a study of 12 cases of IBRH managed by ERCP in the Kurdistan Center of Gastroenterology (KCGE) in Asulaimaneyah-Iraq, from 2007-2010. Seven cases of these were studied retrospectively from the available information on the center computers and on follow-up of these patients, while the other five patients were studied prospectively during the study period. We collected the available information: laboratory investigations, trans-abdominal ultrasonography (TAUS), computed tomography (CT) or magnetic resonance images (when available), before and after ERCP. We depended on the liver functions, trans-abdominal ultrasound and the clinical presentation before and after the ERCP;some patients were contacted via mobile phone for follow-up. Results: Cases were more males than females (58% vs 42%);most were from the ages between 30 - 50 years. The clinical presentation was fever, jaundice, itching and right hypochondrial pain in most patients. The liver function tests were obstructive pattern with elevated direct bilirubin and alkhaline phosphates in most patients. It is becoming clear that Climate Change is getting severe. It was reported that the improved ocean heat measurements have a rate of warming upwards from 4 to 5 Hiroshima bombs liberated heat per second or 388,800 per day. It was reported also that our climate has accumulated the equivalent of a total of more than 2.8 billion Hiroshima bombs’ worth of heat since 1998. Despite this global concern about the effect of global change on environment, it is believed that the problem is much more severe and with greater impact on all facets of life. The effect of Climate Change, especially with a 1℃increase in global temperature (from 14℃to 15℃) is equivalent to the heat liberated from explosion of about 300 million Hiroshima of atomic bomb. This is why this CoP 26 becomes more serious and had targeted year 2060 as a year of zero CO2 emission. This paper discusses the impact of climate change on ten sectors;water, water desalination, energy, renewable energy supply, health, society, agriculture, economy, industry, and built environment.

The accuracy of the statistical learning model depends on the learning technique used which in turn depends on the dataset’s *** most research studies,the existence of missing values(MVs)is a vital *** addition,any dataset with MVs cannot be used for further analysis or with any data driven tool especially when the percentage of MVs are *** this paper,the authors propose a novel algorithm for dealing with MVs depending on the feature selec-tion(FS)of similarity classiﬁer with fuzzy entropy *** proposed algo-rithm imputes MVs in cumulative *** candidate feature to be manipulated is selected using similarity classiﬁer with Parkash’s fuzzy entropy *** predictive model to predict MVs within the candidate feature is the Bayesian Ridge Regression(BRR)***,any imputed features will be incorporated within the BRR equation to impute the MVs in the next chosen incomplete *** proposed algorithm was compared against some practical state-of-the-art imputation methods by conducting an experiment on four medical datasets which were gathered from several databases repository with MVs gener-ated from the three missingness *** evaluation metrics of mean abso-lute error(MAE),root mean square error(RMSE)and coefﬁcient of determination(R2 score)were used to measure the *** results exhibited that perfor-mance vary depending on the size of the dataset,amount of MVs and the missing-ness mechanism ***,compared to other methods,the results showed that the proposed method gives better accuracy and less error in most cases.

Could Bladder Inflation Prior to Cesarean Section Prevent Urinary Tract Injury in High Risk Group? A Randomized Controlled Trial

Background: Accidental urinary tract particularly bladder injury during cesarean delivery has a significant maternal morbidity, as it may lead to extended operative time, infection of urinary tract and sometimes development of urinary tract fistulae. Objective: To find out the efficacy of urinary bladder inflation immediately prior to cesarean section (CS) procedure in minimizing incidence of accidently urinary tract injury in high risk patients. Setting: Obstetrics and Gynecology Department, Faculty of Medicine, South Valley University, Qena, Egypt. Duration: From August 2017 to November 2018. Study Design: A prospective randomized controlled trial. Methods: Seventy six pregnant women recruited from attendants of outpatient antenatal care unit of obstetrics and gynecology department who planned for cesarean delivery and carried one or more risk factors for urinary tract injury. Patients randomly were classified into 2 groups (group I included 38 cases, underwent bladder inflation using triple way Foley’s catheter immediately before CS and group II included 38 cases, and underwent bladder deflation with 2 ways Foley’s catheter immediately before CS. Results: The overall incidence of urinary tract injury was significantly higher in group II (7 cases = 18.4%) than in group I (2 cases = 5.2%) with p value alue 0.01;ureteric or combined vesico-ureteric injuries had been reported only in group II (1 case = 2.6% and 1 case = 2.6%) respectively with no case reported in group I (p There was a highly statistically significant difference between group I and group II as regard to hospital stay (p al stay. The effect of NAFLD (non-alcoholic fatty liver disease) on long-term outcome of chronic hepatitis B in Iranian patients

Background: The influence of Non-alcoholic Fatty Liver Disease on the outcome of chronic hepatitis B disease, including viral, biochemical and histologic characteristics, in Iranian patients is not yet fully un- derstood. Aim: To evaluate the effect of Non-alcoholic Fatty Liver Disease (NAFLD) on long-term histology- cal, biochemical and viral outcome of chronic he- pa-tictis B in Iranian patients. Methods: We retro- spec-tively evaluated 94 “e Ag” negative chronic hepatitis B patients (with NAFLD: 44, without NAFLD: 50). Non-alcoholic Fatty Liver Disease was diagnosed based on liver biopsy according to Kleiner classifica-tion. Liver biopsy was done for all patients. Serologi-cal and biochemical variables were evaluated with repeated measure analysis. Results: Non-alcoholic Fat- ty Liver Disease (NAFLD) was present in 47% of the patients (44 out of 94 patients). In the NAFLD group, increase in AST, alT, stage (P = 0.002), grade, and total score of liver biopsy were independently related to non-alcoholic fatty liver disease, while HBV-DNA viral load did not correlate with the presence of a fatty liver. Conclusion: Abnormalities of liver enzymes and liver histopathology are more prevalent in concurrent chronic hepatitis B and Non-alcoholic Fatty Liver Disease (NAFLD).

Effect of Brine Discharge From al-Dur RO Desalination Plant on the Infauna Species Composition in the East Coast of Bahrain

The GCC countries have been experiencing an accelerated socio-economic development process since the 1970s,resulting in rapid demographic and urbanization growth and associated with rapidly increasing municipal water *** meet these demands’quantity and quality,the GCC countries resorted to ***,the GCC countries collectively possess the largest desalination capacity in the world(~45%),and based on the current urbanization trends,it is expected that current rates in desalination capacity growth will continue in the ***,desalination has a number of environmental externalities on the marine,and their severity will depend on various factors(i.e.,site-specific).The objective of the present study is to investigate the impacts of the brine water discharged by al-Dur RO desalination plant on the coastal infauna benthic species composition in the east coast of *** samples were collected from 10 locations selected at different distances from the discharge *** species composition of infauna was investigated using univariate analysis and multivariate *** results showed an extreme elevation in temperature(>38℃)and hypersaline waters(>55‰)at locations nearby the discharge outlet,and also at bottom waters of depths more than 3 m during the high and low tide cycles in both seasons with exceptional levels in *** main groups of benthic infauna identified are represented by Polychaeta(12 taxa),Bivalves(4 taxa),Gastropod(2 taxa),and Amphipod(3 taxa)including 256 *** univariate analysis indicated spatial variations in infauna species composition where the lower diversity indices were found at locations close to the discharge outlet and at Station 5 where noticeable vertical differences were observed indicating exceptional elevation of hypersaline waters with lesser extent of temperature at bottom ***,the highest species diversity indices characterized the most offshore *** infauna speci

Assessment of Sub-Endometrial Junction Zone by 3-Dimensional Transvaginal Ultrasound in Unexplained Recurrent Implantation Failure and Its Effect on ICSI Outcomes

BACKGROUND: Sub-endometrial junction zone (JZ) plays an important role in most of reproductive functions. OBJECTIVE: To find out the effect of the sub-endometrial JZ thickness assessment by 3-D trans-vaginal ultrasound (TVUS) on intracytoplasmic sperm injection (ICSI) outcomes in patients with unexplained recurrent implantation failure (RIF). SETTING: ART Unit of Obstetrics and Gynecology Department, Faculty of Medicine, South Valley University, Qena, Egypt. DURATION: From April 2016 to October 2018. STUDY DESIGN: A prospective observational study. METHDS: Fifty couples with history of unexplained RIF in previous ICSI cycles and prepared for another ICSI cycle (group I) and fifty couples with unexplained infertility prepared for ICSI for the first time (group II) had been included in this study. At time of ovum pick up, 3-DTVUS was done for all cases in both groups for assessment of sub endometrial junction zone thickness and correlated with ICSI outcome. RESULTS: There were statistically significant differences between group I and group II in JZ thickness in the 3 uterine regions (fundus, anterior and posterior walls) with p 0.001, but there were mildly statistically significant differences between both groups in chemical and clinical pregnancy rates with p al and clinical pregnancy rates).

Effect of Vitamin D Supplementation on Intracytoplasmic Sperm Injection Outcomes: A Randomized Double-Blinded Placebo-Controlled Trial

OBJECTIVE: To assess the effect of vitamin D supplementation on enhancement of embryos implantation rates in intracytoplasmic sperm injection procedures. STUDY DESIGN: A randomized double-blinded controlled trial. SETTING: Assisted Reproduction Unit of Obstetrics and Gynecology Department, Faculty of Medicine, South Valley University, Egypt. DURATION: From December 1st 2017 to the end of October 2018. SUBJECTS and METHDS: Eighty infertile couples that underwent ICSI procedure were entrapped in this study. Patients were divided into two groups;Group I (Vitamin D group) = 40 patients (received vitamin D supplementation in the form of Vidrop oral drops 600 IU/day starting after ovum pickup) and Group II (placebo group) = 40 patients (received a placebo “normal saline oral drop”). RESULTS: There was a significantly increased embryo implantation rates (53.2% in vitamin D group versus 46.7% in placebo group), there was a mild statistically significant difference in the clinical pregnancy rates (45.9% in vitamin D group versus 39.4% in placebo group) with p-value ally significant difference in first trimester miscarriage rates (12.5% in vitamin D group versus 17.5% in placebo group) with p value CONCLUSIONS: Vitamin D supplementation could improve embryos implantation rates and rate of ongoing pregnancy by decline in the first trimester miscarriage rate.
