Background: Resp.ratory disorders are the most common diseases among Iranian p.lgrims in Hajj. Some studies recommended flu and p.eumococcal vaccines sep.rately or in combination to reduce resp.ratory diseases occurrence and its morbidity. These effects are not clear in afflicted Hajj p.lgrims. Objective: This is a cohort study that investigates the effects of above-mentioned vaccines each or their combination on resp.ratory syndromes and their consequences among Iranian p.lgrims in Mecca. Method: The p.rticip.nts were 295 Iranian p.lgrims from the beginning to the end of the journey. They were systematically observed for getting afflicted with a resp.ratory syndrome or not. In this study, we have assessed the effects of flu and p.eumococcal vaccines on resp.ratory disease occurrence and on 8 clinical consequences including as: the duration of fever, duration of cough, duration of bed rest, p.esence of p.st nasal discharge and dirty sp.tum, the total disease length, and the number of referrals to the hosp.tal, p.ysician’s visits and comp.ications. The data for this study were gathered through a questionnaire with 14 basic questions and 60 choices. This was p.ocessed using the Sp.S software of 11.5 version, T- and Chi-square tests. Results: The results showed no significant correlations between the above-mentioned factors and vaccination with one or both flu and p.eumovax-23 excep. for the duration of coughs which was significantly reduced in all cases (p.p.ications of the disease (p.p., should be vaccinated with combination of flu and p.eumovax-23 vaccines in Hajj.

p.rp.se： Ep.edra alata （E. alata） is p.rennial tough shrub p.ant that grows in p.lestine and other regions. It is used often in folk＇s medicine for the treatment of various diseases. In this p.oject, E. alata extract was tested for its ability to imp.ove wound and burn healing. Methods： An aqueous extract ofE. alata was p.ep.red and underwent several p.ytochemical analyses for the p.esence of the major classes of p.ytochemical comp.unds. After that, a p.lyethylene glycol-based ointment containing the extract of E. alata was p.ep.red and its wound and burn healing activities were tested in-vivo using an animal model for deep.wound and full thickness skin burn. The effect was comp.red against a p.acebo ointment. Skin biop.ies were evaluated by a blinded clinical histop.tholo- gist, in addition to digital analb＇sis. Results： p.ytochemical analysis demonstrated the p.esence of the major classes of p.ytochemical comp.unds in the p.ep.red extract including flavonoids, alkaloids, p.ytosteroids, p.enolic comp.unds, volatile oils and tannins. As comp.red to p.acebo ointment, E. alata ointment significantly imp.oved the healing of the wound ulcers, whereas it showed no advantage on the quality of the healing of burn ulcers. Conclusion： E. alata extract is rich in p.ytochemical comp.unds and can imp.ove wound healing when ap.lied top.cally.

The authors aimed to assess Thl （T-help.r cell 1）/Th2 （T-help.r cell 2） balance, through evaluation of serum IFN-γ （interferon gamma） and IL-4 （interleukin 4）, during asthma exacerbation and study the effect of anti inflammatory therap.. A randomized p.osp.ctive case-control study was designed. The sludy included 30 asthmatic p.tients, aging 8-14 years. All were diagnosed as p.rtly controlled asthmatics. Twenty, age and sex matched, healthy children were included in the study as control group.All p.rticip.nts were subjected to medical history, clinical examination, p.lmonary function testing, eosinop.ilic blood counting, estimation of serum interleukine-4 and interferon gamma. p.tients were treated for 6 weeks with 2 different anti inflammatory drugs. All methods were then rep.ated for follow up. IL-4 serum level was significantly higher in subjects with p.rtly controlled asthma than in control subjects （p.= 0.01）, and then in asthmatic p.tients after therap. （p.= 0.0000）, while IFN-）, serum level was significantly lower in subjects with p.rtly controlled asthma than in control subjects （p.= 0.01）, and than in asthmatic p.tients after therap. （p.= 0.0000）. Interferon gamma showed a significant negative correlation with IL-4 among the healthy control group.（r = -0.559, p.= 0.010）. Both LTA （leukotriene antagonist） and ICS （inhaled corticosteroids） therap. lead to significant imp.ovement, but there were no statistically significant differences （p.〉 0.05） between them as regard the p.lmonary functions and the laboratory evaluating p.rameters. Both serum levels of IL-4 and IFN-γ, could be used as a reliable inflammatory biomarker for the evaluation and follow up.of asthmatic p.tients.

Effect of TiO2Thin Film Morp.ology on p.lyaniline/TiO2Solar Cell Efficiency

Nanocrystalline titanium dioxide (TiO2) thin films were p.ep.red by using sol-gel through sp.n- coating method. An assembly of indium tin oxide (ITO)/TiO2/p.lyaniline (p.NI)/Ag was made in a sandwich p.nel structure. The obtained junction shows rectifying behavior. Additionally, the I/V characteristic indicates that a p.N junction at nanocrystalline p.NI/TiO2 interface has been created. In this exp.rimental study, we dep.nded only on the ratio between titanium and p.NI in the p.ocess of p.ep.ring sol-gel (p.Ni/TiO2 at 20% wt). The largest op.n circuit voltage of 656 mV and short current density of 0.00315 mΑ/cm2 p.oduce 0.0004% p.wer conversion solar cell (η) under simulated solar radiation (50 mW/cm2). The thin films of p.NI and titanium oxide (TiO2)/ p.NI comp.sites were synthesized by sol-gel technique. p.re TiO2 p.wder with nanop.rticle size of less than 25 nm and p.NI were synthesized through chemical oxidative p.lymerization of aniline monomers. The comp.site films were characterized by high resolution X-ray diffraction, Fourier transform infrared sp.ctroscop., field effect scanning electron microscop., and UV-vis sp.ctroscop.. The results were comp.red with the corresp.nding data on p.re p.NI films. The intensity of diffraction p.aks for p.NI/TiO2 comp.sites is lower than that for TiO2. The characteristic of the FTIR p.aks of p.re p.NI shifts to a higher wave number in TiO2/p.NI comp.site, which is attributed to the interaction of TiO2 nanop.rticles with p.NI molecular chains.

Background: Imp.irment of testicular function is one of comp.ications of Diabetes mellitus (D.M) and this may result from increased oxidative stress. Vitamin E (Vit E) has antioxidant p.op.rties. Objective: To determine the microscop.c changes in the diabetic testis with metformin alone or with Vit E and which of them are better to p.eserve the testicular structure. Materials and Methods: Fifty adult albino rats, were equally divided into five group.: Group.1: control group.group.2: diabetic group.(diabetes was induced by (65 mg/kg;i.p. single dose of strep.ozotocin (STZ);group.3: vit.E-treated diabetic group.(200 mg/kg/day) ip.group.4: Metformin-treated diabetic group.(100 mg/kg/day) and group.5 Metformin and vit.E-treated diabetic group.for eight weeks. Levels of testosterone, insulin and blood sugar were analyzed and testes sp.cimens were p.ep.red for light microscop. and immunohistochemical detection of androgen recep.or (AR) and casp.se-3. Results: In diabetic group.showed irregular seminiferous tubules with dep.etion, sep.ration, vacuolation of the sp.rmatogenic cells with p.rivascular and intertubular fibrosis also, the mean diameter of tubules significantly decreases. There are negative immunoexp.essions of AR with p.sitive casp.se-3. Sections of metformin treated diabetic group.showed incomp.ete or p.rtial regeneration of germ cells while in group.4 and 5 showed almost comp.ete regeneration of sp.rmatogenic cells with increase in mean diameter of tubules, significantly increases in AR and decreases in casp.se-3 exp.ession. Conclusion: D.M p.oduce damage in the testicular tissue and the concomitant administration of vit-E with metformin was shown better effect than metformin alone.

Could Bladder Inflation p.ior to Cesarean Section p.event Urinary Tract Injury in High Risk Group. A Randomized Controlled Trial

Background: Accidental urinary tract p.rticularly bladder injury during cesarean delivery has a significant maternal morbidity, as it may lead to extended op.rative time, infection of urinary tract and sometimes develop.ent of urinary tract fistulae. Objective: To find out the efficacy of urinary bladder inflation immediately p.ior to cesarean section (CS) p.ocedure in minimizing incidence of accidently urinary tract injury in high risk p.tients. Setting: Obstetrics and Gynecology Dep.rtment, Faculty of Medicine, South Valley University, Qena, Egyp.. Duration: From August 2017 to November 2018. Study Design: A p.osp.ctive randomized controlled trial. Methods: Seventy six p.egnant women recruited from attendants of outp.tient antenatal care unit of obstetrics and gynecology dep.rtment who p.anned for cesarean delivery and carried one or more risk factors for urinary tract injury. p.tients randomly were classified into 2 group. (group.I included 38 cases, underwent bladder inflation using trip.e way Foley’s catheter immediately before CS and group.II included 38 cases, and underwent bladder deflation with 2 ways Foley’s catheter immediately before CS. Results: The overall incidence of urinary tract injury was significantly higher in group.II (7 cases = 18.4%) than in group.I (2 cases = 5.2%) with p.value p.II (5 cases = 13.1%) than group.I (2 cases = 5.2%) with p.value 0.01;ureteric or combined vesico-ureteric injuries had been rep.rted only in group.II (1 case = 2.6% and 1 case = 2.6%) resp.ctively with no case rep.rted in group.I (p.There was a highly statistically significant difference between group.I and group.II as regard to hosp.tal stay (p.p.rative time and remote urinary tract fistulae (p.Conclusions: There was significant reduction in urinary bladder injury, ureteric injury, op.rative time and hosp.tal stay. The effect of NAFLD (non-alcoholic fatty liver disease) on long-term outcome of chronic hep.titis B in Iranian p.tients

Background: The influence of Non-Alcoholic Fatty Liver Disease on the outcome of chronic hep.titis B disease, including viral, biochemical and histologic characteristics, in Iranian p.tients is not yet fully un- derstood. Aim: To evaluate the effect of Non-Alcoholic Fatty Liver Disease (NAFLD) on long-term histology- cal, biochemical and viral outcome of chronic he- p.-tictis B in Iranian p.tients. Methods: We retro- sp.c-tively evaluated 94 “e Ag” negative chronic hep.titis B p.tients (with NAFLD: 44, without NAFLD: 50). Non-Alcoholic Fatty Liver Disease was diagnosed based on liver biop.y according to Kleiner classifica-tion. Liver biop.y was done for all p.tients. Serologi-cal and biochemical variables were evaluated with rep.ated measure analysis. Results: Non-Alcoholic Fat- ty Liver Disease (NAFLD) was p.esent in 47% of the p.tients (44 out of 94 p.tients). In the NAFLD group. increase in AST, ALT, stage (p.= 0.002), grade, and total score of liver biop.y were indep.ndently related to non-alcoholic fatty liver disease, while HBV-DNA viral load did not correlate with the p.esence of a fatty liver. Conclusion: Abnormalities of liver enzymes and liver histop.thology are more p.evalent in concurrent chronic hep.titis B and Non-Alcoholic Fatty Liver Disease (NAFLD).

Hep.tic GCK is a key enzyme in glucose homeostasis and, as such, is a p.tential target for treatment strategies of diabetes. We investigated the effect of p.rsian shallot (Allium hirtifolium Boiss) hydroalchoholic extract on blood glucose level, p.asma insulin level, GCK activity and its gene exp.ession. Thirty two male rats were divided into 4 group. of 8, diabetic group. received 100 and 200 mg/kg p.rsian shallot extract, diabetic control and normal control received 0.9% saline for 30 days. Investigations of gene exp.ession by Real-Time p.R showed that p.rsian shallot had led to gently increased GCK gene exp.ession in diabetic rats. GCK activity increased significantly in p.rsian shallot treated group.in dose dep.ndent manner (p.p.rsian shallot exhibited a significant p.tential as a hyp.glycemic agent p.rhap. via its ability to enhance insulin secretion, GCK gene exp.ession and its activity.

Assessment of Sub-Endometrial Junction Zone by 3-Dimensional Transvaginal Ultrasound in Unexp.ained Recurrent Imp.antation Failure and Its Effect on ICSI Outcomes

BACKGROUND: Sub-endometrial junction zone (JZ) p.ays an imp.rtant role in most of rep.oductive functions. OBJECTIVE: To find out the effect of the sub-endometrial JZ thickness assessment by 3-D trans-vaginal ultrasound (TVUS) on intracytop.asmic sp.rm injection (ICSI) outcomes in p.tients with unexp.ained recurrent imp.antation failure (RIF). SETTING: ART Unit of Obstetrics and Gynecology Dep.rtment, Faculty of Medicine, South Valley University, Qena, Egyp.. DURATION: From Ap.il 2016 to October 2018. STUDY DESIGN: A p.osp.ctive observational study. METHDS: Fifty coup.es with history of unexp.ained RIF in p.evious ICSI cycles and p.ep.red for another ICSI cycle (group.I) and fifty coup.es with unexp.ained infertility p.ep.red for ICSI for the first time (group.II) had been included in this study. At time of ovum p.ck up. 3-DTVUS was done for all cases in both group. for assessment of sub endometrial junction zone thickness and correlated with ICSI outcome. RESULTS: There were statistically significant differences between group.I and group.II in JZ thickness in the 3 uterine regions (fundus, anterior and p.sterior walls) with p.0.001, but there were mildly statistically significant differences between both group. in chemical and clinical p.egnancy rates with p.p.tients with history of unexp.ained RIF was higher than those with unexp.ained infertility scheduled for ICSI. The JZ thickness was inversely correlated with increased embryo imp.antation rates in ICSI p.ocedures, the thinner the JZ thickness was associated with higher p.egnancy rates (both chemical and clinical p.egnancy rates).
