Temporal trends in the relative prevalence of dysphagia etiologies from 1999-2009

AIM:To examine the relative prevalence and temporal variation of dysphagia etiologies in patients undergoing upper endoscopy(EGD) over the past ***:EGDs with the indication of dysphagia at an urban,university medical center in 1999,2004 and 2009 were retrospectively identified from the electronic medical *** entire patient chart,including EGD,pathology,manometry,radiographic and clinician reports,was reviewed for demographic and clinical data and to determine the etiology of *** number of EGDs in which an esophageal biopsy was performed was also *** reflux disease(GERD) as a cause of dysphagia independent of peptic stricture was defined by symptoms with erosive esophagitis or symptom response to proton pump inhibition(PPI).Cases of eosinophilic esophagitis(EoE) were defined by an appropriate clinical history and histological criteria of ≥ 15 eosinophils per high powered ***-responsive esophageal eosinophilia was not routinely reported prior to *** analysis was performed using one-way analysis of variance to analyze for trends between 1999,2004 and 2009 and a post-hoc Tukey analysis was performed following a significant main ***:A total of 1371 cases(mean age 54 years,43% male) met pre-specified inclusion criteria with 191,504 and 675 cases in 1999,2004 and 2009,*** were older in 2004 compared to 2009(mean ± SD,54.0 ± 15.7 years vs 52.3 ± 16.8 years,P = 0.02) and there were more males in 1999 compared to 2004(57.5% vs 40.8%,P = 0.005).Overall,GERD(27.6%) and EoE(7.7%) were the most common identifiable causes of *** unspecified diagnosis accounted for 21% of overall *** were no significant differences in the relative prevalence of achalasia or other motility disorders,peptic stricture,Schatzki's ring,esophageal cancer or unspecified diagnoses over the 10-year time *** was,however,a decrease in the relative prevalence of GERD(39.3% vs 24.1%,P < 0.001

Interaction of haptoglobin with hemoglobin octamers based on the mutation <i>α</i>Asn78Cys or <i>β</i>Gly83Cys

Octameric hemoglobins have been developed by the introduction of surface cysteines in either the alpha or beta chain. Originally designed as a blood substitute, we report here the structure and ligand binding function;in addition the interaction with haptoglobin was studied. The recombinant Hbs (rHbs) with mutations alpha Asn78Cys or beta Gly83Cys spontaneously form octamers under conditions where the cysteines are oxidized. Oxygen binding curves and CO kinetic studies indicate a correct allosteric transition of the tetramers within the octamer. Crystallographic studies of the two rHbs show two disulfide bonds per octamer. Reducing agents may provoke dissociation to tetramers, but the octamers are stable when mixed with fresh human plasma, indicating that the reduction by plasma is slower than the oxidation by the dissolved oxygen, consistent with an enhanced stability. The octameric rHbs were also mixed with a solution of haptoglobin (Hp), which binds the dimers of Hb: there was little interaction for incubation times of 15 min;however, on longer timescales a complex was formed. Dynamic light scattering was used to follow the interaction of Hp with the alpha Asn78Cys octamer during 24 hours;a transition from a simple complex of 15 nm to a final size of 60 nm was observed. The results indicate a specific orientation of the αβ dimers may be of importance for the binding to haptoglobin.

Cancer is one of the two leading human fatal *** development for cancer intervention has progressed well in past decades yet existing drugs face many limitations in applications and effectiveness and are often associated with serious of side effects,which can further deteriorate the patients’quality of *** development of natural product based and therapeutically sound anti-cancer agents have gained popularity in the field of functional foods,in which a few have demonstrated efficacy and minimal toxicity toward the prevention and treatment of *** multiple active molecular components,citrus peels and derived extracts have demonstrated potent efficacious properties against various cancers due in large part to the rich content of flavonoids present in citrus *** review summarizes the results of currently available data regarding the in vivo anti-cancer activity of citrus peel flavonoids,and identifies opportunities for subsequent human clinical trials to assess preventive and therapeutic effects in the near future.

An equation of state(EOS) is developed for salt-water systems in the high temperature *** an example of the applications,this EOS is parameterized for the calculation of density, immiscibility,and the compositions of coexisting phases in the CaCl-ho and MgCl-ho systems from 523 to 973 K and from saturation pressure to 1500 *** available volumetric and phase equilibrium measurements of these binaries are well represented by this *** EOS is based on a Helmholtz free energy representation constructed from a reference system containing hard-sphere and polar contributions plus an empirical *** the temperature and pressure range in this study,the electrolyte solutes are assumed to be *** water molecules are modeled as hard spheres with point dipoles and the solute molecules,MgCl and CaCl,as hard spheres with point *** free energy of the reference system is calculated from an analytical representation of the Helmholtz free energy of the hard-sphere contributions and perturbative estimates of the electrostatic *** empirical correction used to account for deviations of the reference system predictions from measured data is based on a virial *** formalism allows generalization to aqueous systems containing insoluble gases (CO,CH),alkali chlorides(NaCl,KCl),and alkaline earth chlorides(CaCl,MgCl).The program of this model is available as an electronic annex(see EA1 and EA2) and can also be downloaded at:http://***/***.

An equation of state(EOS)based on thermodynamic perturbation theory is presented for the NaCl-ho-CH *** equation consistently reproduces PvTX properties and phase equilibria with an accuracy close to that of data in the temperature,pressure and concentration ranges from 648 K to 873 K,0 to 2500 bar and up to 2.37 mol%*** agreement with recent ternary immiscibility data from 673K to 873 K suggests that the EOS may provide accurate predictions for NaCl concentrations as high as 40 mol %.We could not find any experimental data above 873 K that can be used to validate the predictions of the EOS inside the ***,parameters for the mixed ternary system were established from parameters evaluated for pure and binary systems and accurate combination ***,predictions in the ternary should be reliable to the high temperatures and pressures where the EOS for the lower order systems are valid (about 1300 K and 5000 bar).Using the same combining approach,an EOS for the quaternary NaCl-ho-CO -CH-4 is constructed on the basis of parameters from our earlier model for the NaCl-ho-CO system and the present NaCl-ho-CH *** suggests that predictions of the quaternary EOS are reliable also to about 1300 K and 5000 bar.
