Applying Item Response Theory Methods to Improve the Measurement of Fatigue in a Clinical Trial of Rheumatoid Arthritis Patients Treated with Secukinumab

Background: Many clinical trials include multiple patient-reported outcomes (PROs) to measure fatigue as secondary or exploratory endpoints of treatment effectiveness. Often, these instruments have overlapping content. The objective of this study was to compare the combined measurement properties of two fatigue scales, the Functional Assessment of Chronic Illness Therapy-Fatigue (FACIT-Fatigue) and SF-36 vitality (VT) scale using item response theory (IRT). Methods: The FACIT-Fatigue and SF-36v2 were administered at baseline and weeks 2, 4, 7, 12, and 16 to rheumatoid arthritis (RA) patients (n = 237) enrolled in a 52-week multicenter, randomized, double-blind, placebo-controlled, parallel-group, dose finding study to evaluate the efficacy and safety of subcutaneous secukinumab administered to pa- tients with active RA. Confirmatory factor analysis (CFA) was used to investigate unidimensionality among FACIT- Fatigue and VT items. A generalized partial credit IRT model was used to cross-calibrate the FACIT-Fatigue and VT items and weighted maximum-likelihood estimation was used to score a composite fatigue index. Analysis of variance was used to compare the composite fatigue index with the original scales in responding to ACR improvement and treatment effects. Results: CFA found less than adequate fit to a unidimensional model. However, specifications of alternative multidimensional models were insufficient in explaining the common variance among items. An IRT model was successfully fitted and the composite fatigue index score was found to be more responsive than the original scales to ACR improvement and treatment effects. Effect sizes and significance tests for changes in scores on the composite index were generally larger than those observed with the original scales. Conclusion: IRT methods offer a promising approach to combining items from different scales measuring the same concept that could improve the detection of treatment effects in clinical studies of RA.

The invasion process involves overcoming certain barriers with negative effects on the biodiversity and ecosystem function. This is the case of the African dung beetle Digitonthophagus gazella, which was introduced in the United States and other countries in Central and South America to help in dung removal activity. The distribution of D. gazella has been documented in the Americas, but in Venezuela there is little information. Trying to understand the process of invasion of the Colombian-Venezuelan savannas and measure their potential impact, the information stored in collections and literature was collected. Five new state records were reported for Venezuela, as well as four scenarios of the process of introduction and establishment of this species in the region. although the amount of records has increased, the number of individuals sampled is still incipient. The possible areas of invasion and the potential negative effects that the presence of this species exerts in these regions are also discussed. In addition, potential areas of invasion and negative effects of this species are discussed. Results emphasize the importance of generating models of the invasive process in order to design programs that monitor the effects incurred by the presence of D. gazella on native species and propose control and mitigation mechanisms.

Periodontal bacteria in different sampling sites of pericoronitis patients

The study investigated bacteria sampled from the mandibular third molar pericoronitis with the hypothesis that the same species also can be detected in several sites of the oral cavity. Subgingival plaque samples were taken from 41 patients (mean age 23.2 ± 4.2 years), from the third molar pericoronal pocket and from 3 adjacent gingival pockets, from the tongue and saliva. Aggregatibacter actinomycetemcomitans (A.a.), Porphyromonas gingivalis (P.g.), Tan- ne-rella forsythia (T.f.), Prevotella intermedia (P.i.), Pre-votella nigrescens (P.n.), and Treponema denticola (T.d.) were analyzed by qualitative PCR method. all the species analyzed were found in the pericoronitis samples in the following frequency: T.f. 73%, T.d. 68%, P.i. + P.n. 35%, A.a. 5%, and P.g. 5%. Saliva and tongue samples were also highly positive with T.f. (both 76%) and T.d. (saliva 57%, tongue 76%) and better sources for A.a. (saliva 8%, tongue 14%), and P.g. (saliva 14%, tongue 8%) than the pericoronitis pocket. The gingival pocket samples were less frequently positive the further the site in question was from the pericoronitis site. Contrary to earlier observations T.f. was frequently defected in pericoronitis sites. The results emphasize the importance of pericoronitis as a focus of infection.

Intraoperative management of liver transplant in a patient with an undiagnosed ventricular septal defect:A case report

BACKGROUnD The intraoperative management of patients undergoing orthotopic liver transplantation(OLT)frequently encounters hemodynamic instability after reperfusion of the new liver *** resulting post-reperfusion syndrome is characterized by an increase in pulmonary vascular resistance and decrease in systemic vascular *** the presence of a left to right intracardiac shunt,this hemodynamic perturbance can lead to shunt reversal followed by hypoxemia and embolization of air and debris into the systemic circulatory *** SUMMARY A 43 years-old male with end-stage liver disease due to primary sclerosing cholangitis complicated by portal hypertension and hepatocellular carcinoma presented for an OLT.A bedside transthoracic echocardiography(TTE)was performed immediately before the procedure and unexpectedly identified a ventricular septal defect(VSD).The patient and the surgical team agreed to proceed with the surgery as it was a time critical donation after circulatory organ *** developed an intraoperative plan to optimize pulmonary and systemic pressures using vasoactive support,optimized mechanical ventilation,and used transesophageal echocardiography(TEE)for intraoperative *** reperfusion,considerable turbulent flows with air were noted in the right ventricle,but no air was visualized in the left *** flow Doppler showed no reversal flow in the *** the end of the procedure,the patient was extubated in the operating room without complication and was transferred to the transplant unit for *** Our case highlights the importance of echocardiography in the perioperative assessment of patients undergoing liver *** TTE findings obtained immediately before the procedure and the real-time use of intraoperative TEE to modify our management during the critical phases of the transplant resulted in continuity of care and a good surgical outcome for this patient.

A single-end adaptive-optics(AO)module is experimentally demonstrated to mitigate the emulated atmospheric turbulence efects in a bi-directional quantum communication link,which employs orbital angular momentum(OAM)for data encoding.A classical Gaussian beam is used as a probe to detect the turbulence-induced wavefront distortion in the forward direction of the *** on the detected wavefront distortion,an AO system located on one end of the link is used to simultaneously compensate for the forward and backward ***,with emulated turbulence and when the probe is turned on,the mode purity of photons carrying OAMℓ=1 is improved by∼21%with AO *** also measured the performance when encoding data using OAM{ℓ=−1,+2}and{ℓ=−2,+1}in the forward and backward channels,respectively,at 10 Mbit/s per channel with one photon per pulse on *** this case,we found that the AO system could reduce the turbulence efects increased quantum-symbolerror-rate(QSER)by∼76%and∼74%,for both channels in the uni-directional and bi-directional cases,*** QSER improvement is observed for the opposite direction channels in the bi-directional case.

Background: The ExcelsiusGPS?(Globus Medical, Inc., Audubon, PA) is a next-generation spine surgery robotic system recently approved for use in the United States. The objective of the current study is to assess pedicle screw accuracy and clinical outcomes among two of the first operative cases utilizing the ExcelsiusGPS?robotic system and describe a novel metric to quantify screw deviation. Methods: Two patients who underwent lumbar fusion at a single institution with the ExcelsiusGPS?surgical robot were included. Pre-operative trajectory planning was performed from an intra-operative CT scan using the O-arm (Medtronic, Inc., Minneapolis, Mn). After robotic-assisted screw implantation, a post-operative CT scan was obtained to confirm ideal screw placement and accuracy with the planned trajectory. A novel pedicle screw accuracy algorithm was devised to measure screw tip/tail deviation distance and angular offset on axial and sagittal planes. Screw accuracy was concurrently determined by a blinded neuroradiologist using the traditional Gertzbein-Robbins method. Clinical variables such as symptomatology, operative data, and post-operative follow-up were also collected. Results: Eight pedicle screws were placed in two L4-L5 fusion cases. Mean screw tip deviation was 2.1 mm (range 0.8-5.2 mm), mean tail deviation was 3.2 mm (range 0.9-5.4 mm), and mean angular offset was 2.4 degrees (range 0.7-3.8 degrees). all eight screws were accurately placed based on the Gertzbein-Robbins scale (88% Grade A and 12% Grade B). There were no cases of screw revision or new post-operative deficit. Both patients experienced improvement in Frankel grade and Karnofsky Performance Status (KPS) score by 6 weeks post-op. Conclusion: The ExcelsiusGPS? robot allows for precise execution of an intended pre-planned trajectory and accurate screw placement in the first patients to undergo robotic-assisted fusion with this technology.
