Objective:Functional endoscopic sinus surgery is a commonly performed otolaryngologic procedure that often uses the microdebrider device for tissue *** the ubiquitous nature of the instrument,we sought to better define the patterns of device failure using the postmarket surveillance openFDA ***:The openFDA database was queried for all microdebrider‐related adverse events from January 1,2000 to november 1,*** information on the nature of device failure and any associated patient injury was *** not directly related to device failure were excluded from the ***:A total of 641 events were included in the *** most common device failure was overheating(n=348,54.3%),followed by material separation(n=173,27%),and inconsistent device activation(n=52,8.1%).Of the reported events,the vast majority did not result in patient harm(n=579,90.3%).On review of the remaining cases,only 24 events(3.7%)resulted in true harm to the patient,defined as a temporary or permanent injury or>30 min of additional anesthesia *** these cases,the need to reschedule surgical cases(n=5,0.8%),retained foreign body(n=5,0.8%),and thermal tissue injury(n=3,0.5%)were the most *** patients suffered an injury due to surgeon error unrelated to device malfunction(n=5,0.8%).Conclusions:Microdebrider device failures are extremely *** they do occur,less than 10%result in patient *** cases of patient harm related to microdebrider failure,preoperative testing of the device before use could prevent many of the reported malfunctions.

水污染是指将能源或物质不当的排放到地球资源,包括空气、水和土地中,对生态系统造成有害且往往是持久性的破坏。这种污染严重影响了生活的质量,造成负面后果,而且影响到生态系统的活生物体和非活组成部分,破坏了自然环境和其正常功能。制造业的增长引起了人们对环境污染的严重担忧,纺织、造纸和印刷等行业释放的大量有机染料尤其令人担忧。这些彩色染料会阻止阳光的射入,危害水生生物。健康问题涉及各种疾病的快速和性价比高的诊断。在这些诊断中,使用简单、经济高效的设备检测肌酸酐（生物分子）对研究人员来说意义重大。肌酸酐是哺乳动物的肌肉代谢废物,分子量为113.12。它在肌肉中产生,通过血液运输,并在尿液中通过肾脏过滤。长期以来,它一直被认为是肾脏和肌肉疾病的生物标志物,因为它在血清中的较高水平表明肾脏功能障碍,而较低水平表明肌肉问题。因此,本研究主要致力于对上述问题做出贡献,具体如下。1.具有大比表面积和多孔结构的材料有益于提高吸附和分离性能。在这项工作中,使用ZIF-67作为牺牲模板,通过一步溶剂热法制备了Co-ni LDH中空结构,并用于去除水中的有机污染物（甲基橙染料MO）和肌酸酐（生物）传感。所制备的Co-ni LDH样品在一般条件下90分钟内对MO分子表现出优异的去除性能（92.6%）,这是由于吸附和光催化过程的协同作用。双功能行为是由于其表面带正电荷、比表面积大、孔径优化以及中空的结构,而且材料中的Co/Co和ni/ni离子为吸附和光催化提供了丰富的位点。此外,Co-ni LDH中空结构在多次循环后保持了高的去除能力和结构完整性。此外,首次在便携式电化学设备中,使用丝网印刷碳电极上的循环伏安法,研究了Co-ni LDH中空结构作为肌酸酐（生物）传感的传感器。在这些传感器中,Co-ni LDH中空结构在500-1000 n M的线性检测范围内表现出高灵敏度（1.23μA/cm.n M）、低检测限（243.5 p M）和低定量限（812 p M）。因此,制备的样品Co-ni LDH在处理废水污染和临床分析方面显示出巨大的潜力。2.在这项工作中,使用涂覆有聚苯乙烯泡沫（即包装材料废料）和锐钛矿相V-Ti O2微针的复合受体的一次性丝网印刷碳电极,设计了一种小型、经济高效且一次性的电化学肌酸酐传感器,采用溶胶-凝胶化学方法合成了钒参杂的Ti O微针,并用FTIR、PXRD和SEM对其进行了表征。电化学传感器能够在7.4 p H和室温下监测标准氧化还原溶液中的1–25μM肌酸酐。电化学测量是使用安卓智能手机中的便携式恒电位仪进行的。测量循环伏安法和微分脉冲伏安法所示的性能来监测肌酸酐的浓度。该传感器显示出30.95±1.73μA/cmμM的出色肌酸酐灵敏度和184 n M肌酸酐的极低阈值检测限。因此,这项工作可能会为肾功能衰竭和肌营养不良的常规监测和临床诊断提供一种负担得起的POC（检测）方案。

HER-2/neu Status in Gastric Carcinomas in a Series of Egyptian Patients and Its Relation to Ki-67 Expression

Objective: HER-2/neu status in gastric carcinomas (GCs) has not been studied before in the Egyptian population. Materials and Methods: Seventy-three GCs were evaluated for the expression of HER-2/neu and Ki-67 using immunohistochemistry (IHC). Fluorescence in situ Hybridization (FISH) was done for HER2/neu IHC score 2+ cases. Results: Out of the 73 gastric carcinomas, 23 (31.5%) were score 0, 17 (23.3%) were score 1+, 23 (31.5%) were score 2+, and 10 (13.7%) were score 3+. FISH analysis revealed that the HER-2/neu gene was amplified in 11 out of the 23 cases (47.8%) scored 2+ by IHC. Therefore, the overall HER-2/neu positivity rate was 28.8% and it was significantly associated with higher T-stage and lymphovascular invasion (LVI). The Ki-67 expression rate ranged between 10% and 100%, with 84.9% of the cases (n = 62) featuring high Ki-67 scores. High Ki-67 score was significantly associated with male sex, tumor grade, and number of positive nodes. HER-2/neu protein expression correlated significantly with Ki-67 score. Twenty tumors showed combined HER-2/neu positivity and high Ki-67 score and were significantly associated with higher T-stage and occurrence of LVI, implying a more aggressive behavior. Conclusions: The rate of HER-2/neu positive GCs in our series simulates universal rates, thereby mandating routine evaluation of HER-2/neu status in all GCs submitted to our laboratory to benefit from trastuzumab therapy. Further studies on a larger number of GCs are required to prove that the concurrent HER-2/neu positivity and high Ki-67 score are markers of worse prognosis.

Coronavirus disease 2019(COVID-19)is a global pandemic where several comorbidities have been shown to have a significant effect on *** with diabetes mellitus(DM)have a higher mortality rate than non-DM patients if they get *** studies have indicated that patients with a history of diabetes can increase the risk of severe acute respiratory syndrome coronavirus 2 ***,patients without any history of diabetes can acquire newonset DM when infected with ***,there is a need to explore the bidirectional link between these two conditions,confirming the vicious loop between“DM/COVID-19”.This narrative review presents(1)the bidirectional association between the DM and COVID-19,(2)the manifestations of the DM/COVID-19 loop leading to cardiovascular disease,(3)an understanding of primary and secondary factors that influence mortality due to the DM/COVID-19 loop,(4)the role of vitamin-D in DM patients during COVID-19,and finally,(5)the monitoring tools for tracking atherosclerosis burden in DM patients during COVID-19 and“COVID-triggered DM”*** conclude that the bidirectional nature of DM/COVID-19 causes acceleration towards cardiovascular *** to this alarming condition,early monitoring of atherosclerotic burden is required in“Diabetes patients during COVID-19”or“new-onset Diabetes triggered by COVID-19 in non-Diabetes patients”.

【目的】系统测定云南常见野生牛肝菌中总Hg含量,分析不同产地、种类及不同部位牛肝菌对Hg的富集规律和积累特征,评价野生牛肝菌的食用安全性,为野生食用菌产业的发展和食用安全评估提供数据依据。【方法】采用MA-2000直接测Hg仪—冷原子吸收光谱法测定牛肝菌菌盖、菌柄中总Hg含量,计算同一牛肝菌菌盖与菌柄总Hg含量比(QC/S),分析不同种类、部位牛肝菌中总Hg含量的差异和富集规律。根据中国《食品安全国家标准食品中污染物限量》(GB 2762—2012)规定的食用菌中Hg限量标准及联合国粮农组织和世界卫生组织(FAO/WHO)规定的每周Hg允许摄入量(provisional tolerable weekly intake,PTWI)现行标准,评价云南野生牛肝菌的食用安全性。【结果】同一子实体菌盖与菌柄的比值(QC/S)在0.3~8.8之间,95%牛肝菌样品菌盖、菌柄总Hg含量比(QC/S)≥1,表明多数牛肝菌菌盖对Hg的富集能力强于菌柄。本研究中多数野生牛肝菌菌盖、菌柄总Hg含量超过GB 2762—2012规定的食用菌中Hg含量限量标准(≤0.1 mg/kg);根据PTWI标准,假设成年人(60 kg)每周食用300 g新鲜牛肝菌,则多数样品Hg摄入量低于允许摄入量,无安全风险,而2013年采自玉溪大营街的美味牛肝菌,2012年采自楚雄永仁县的铜色牛肝菌及2014年采自玉溪新平的绒柄牛肝菌等少数样品Hg摄入量高于该标准。【结论】不同产地、种类牛肝菌菌盖、菌柄中总Hg含量存在明显差异,有少数牛肝菌总Hg含量超过相关标准,食用具有一定的潜在风险。

Changes in Serum Lipid Profile among Patients Suffering from Chronic Liver Disease Secondary to Hepatitis C

Objective: To find out the changes in lipid metabolism among patients suffering from chronic liver disease secondary to hepatitis C. Study Design: Hospital based observational study. Setting: Medical Unit-I, Ward–5, Jinnah Postgraduate Medical Centre, Karachi. Duration: July 2013 to December 2013. Patients and Methods: About 110 patients admitted in Medical Unit-I with a diagnosis of chronic liver disease were included in the study. Patients suffering from DM, HTn, CKD were excluded from the study. Fasting lipid profile was done in all cases. Results and Observations: There were 44 (40%) male and 66 (60%) female patients. Mean age of the patients was 50.18 (±11.7) years. Total cholesterol was decreased in 76 (69.09%) patients. normal range was present in 34 (30.91%) patients. none of the patient had hypercholesterolemia. Serum triglyceride levels were low in 14 (12.72%) patients, normal in 82 (74.54%), borderline high in 7 (6.36%) and hypertriglyceridemia was seen in 7 (6.36%). HDL-c was below normal in 26 (23.63%) cases, normal in 78 (70.91%), and high in 6 (5.45%). LDL was near optimal/above optimal in only 5 (4.5%) patients. Mean TC/HDL ratio was 2.53 (±1.02). Mean LDL/HDL ratio was 1.23 (±0.73). Mean TC of HCV +ve patients was 130.5 mg/dl as compared to that of HCV –ve patients which was 82.85 mg/dl (P-value: 0.011). Mean TGs of HCV +ve group was 151.5 mg/dl while that of HCV –ve was 79.9 mg/dl (P-value: 0.025). Mean HDL & LDL levels were 43.67 mg/dl and 39.78 mg/dl in HCV group while 34.83 mg/dl & 64.67 mg/dl in the other group with P-value of 0.026 and 0.081 respectively. Conclusion: When it comes to its relationship with lipid metabolism, HCV is a remarkable virus. Its interaction with lipoproteins and its ability to induce massive steatosis are quite unique and idiosyncratic. 汞（Hg）是危害人类健康的主要重金属元素之一，多数食用菌对Hg有很强的富集能力，测定食用菌中Hg含量，并对其进行食用安全性评价具有重要意义。采用冷原子吸收直接测汞仪系统测定85份云南常见牛肝菌属真菌菌盖、菌柄中总Hg含量；以同一牛肝菌子实体菌盖与菌柄总Hg含量比（Q（C/S））分析牛肝菌属真菌对Hg的富集特征；根据联合国粮农组织和世界卫生组织（FAO／WHO）现行每周Hg允许摄入量（provisional tolerable week—lyintake，PTWI）标准和中国GB2762—2012规定的食用菌中Hg限量标准评价样品的食用安全性。结果表明，菌盖和菌柄中总Hg含量分别在0．13—22．00、0．20～8．40mg·kg^-1DW，不同产地、种类及不同采集年份的样品中总Hg含量存在明显差异；同一牛肝菌菌盖、菌柄总Hg含量比（Q（C/S））在0．28～4．08，92％的样品Q（（C/S））〉1，表明多数样品中菌盖对Hg的富集能力强于菌柄。根据GB2762-2012规定的食用菌及其制品中总Hg含量限量标准（≤0．1mg·kg^-1），发现所有测试样品的总Hg含量均超标；根据PTWI标准，假设成年人（60kg）每周食用300g新鲜牛肝菌，则多数样品Hg摄入量低于允许摄入量，无安全风险，而少数样品Hg摄入量高于允许摄入量，食用有一定的潜在风险。同时，牛肝菌总Hg含量与种类、产地、采集时间等因素密切相关，采食及安全评价需综合考虑这些因素。

Rapid,accurate and high-throughput sizing and quantification of particulate matter(PM)in air is crucial for monitoring and improving air *** fact,particles in air with a diameter of≤2.5μm have been classified as carcinogenic by the World Health *** we present a field-portable cost-effective platform for high-throughput quantification of particulate matter using computational lens-free microscopy and *** platform,termed c-Air,is also integrated with a smartphone application for device control and display of *** mobile device rapidly screens 6.5 L of air in 30 s and generates microscopic images of the aerosols in *** provides statistics of the particle size and density distribution with a sizing accuracy of~93%.We tested this mobile platform by measuring the air quality at different indoor and outdoor environments and measurement times,and compared our results to those of an Environmental Protection Agency–approved device based on beta-attenuation monitoring,which showed strong correlation to c-Air ***,we used c-Air to map the air quality around Los Angeles International Airport(LAX)over 24 h to confirm that the impact of LAX on increased PM concentration was present even at 47 km away from the airport,especially along the direction of landing *** its machinelearning-based computational microscopy interface,c-Air can be adaptively tailored to detect specific particles in air,for example,various types of pollen and mold and provide a cost-effective mobile solution for highly accurate and distributed sensing of air quality.

Frequency of neurological Disorders and Related EEG Finding in a Tertiary Care Hospital of Karachi

Background: Electroencephalogram (EEG) is defined as a method of detecting brain waves signifying the electrical conductivity of the brain. Globally, EEG is used to further classify neuropsychiatric disorders. Objective: To evaluate the prevalence of abnormal EEGs and related neurological disorders and it’s correlation with age and gender. Methodology: A descriptive cross sectional study was conducted in neurology department, Ziauddin hospital, Karachi, Pakistan from June 2018 to May 2019. A total of 440 individuals based on inclusion criteria were selected through non-probability consecutive sampling. Informed consent was obtained from study participants. A self-designed structured questionnaire and EEG record were used for data collection. Data were entered and analyzed using SPSS v 20. Mean and standard deviation was calculated for numerical variable and frequency and percentages were calculated for categorical variable. Independent t-test and chi square was used to check association of abnormal EEG with age and gender. P value of ≤0.05 was considered statistically significant. Results: The mean age of study participants was 52.98 ± 22.68 years. There were 54.5% (n = 240) males. Approximately 45.2% (n = 199) participants had predisposing condition which can lead to abnormal EEG. EEG records of 39.8% (n = 175) of the patients was normal while 60.2% (n = 265) had abnormal records. Diffuse neuronal dysfunction consistent with encephalopathy was found in 45.2%. Mild neuronal dysfunction consistent with encephalopathy accounted for 33.5% of cases. Significant difference was found in Abnormal EEG among different age group (p = 0.01). Chi square shows an association between abnormal EEG and male gender (p = 0.025). Conclusion: EEG plays a vital role in the diagnosis of neuropsychiatric conditions in developing countries. EEGs can further help to determine the relationship of different neuropsychiatric conditions and can help in early diagnosis and better prognosis.
