Background. Vulval Keratoacanthoma (ka) is very rare and its differential diagnosis from the commonly occurring Vulval Squamous Cell Carcinoma (SCC) is important to avoid overtreatment. Case. A case of Vulval ka in a 79- year- old lady is reported. She presented with a 9- mm firm raised lesion on the antero- lateral left side of the clitoris of a few months’ duration with no associated symptoms. The lesion was resected with clear margins and the excision site was well healed at her appointment 4 weeks later. Conclusion. We share our experience reporting the fifth case of a vulval ka in the world literature. Consideration to the occurrence of vulval ka is important to avoid both an erroneous diagnosis of vulval SCC and the associated consequences of radical surgery.

8头体重32.9±4.3kg(平均值土标准差)的健康长白×约克夏杂种猪,随机交叉设计试验,按10mg/kg静注或50mg/kg内服吡喹酮,给药间隔时间为2周。以乙醚萃取法提取血浆中的药物,反相高效液相色谱法测定血浆吡喹酮的浓度。非线性最小二乘法计算机程序拟合静注及内服的药时数据,分别适合二室开放模型及一级吸收一室开放模型。静注给药的动力学参数是:t1/2α0.31±0.08h,t1/2β1.50±0.57h,Vd(area)3.09±1.19 I/kg,Cl_B24.57±8.57ml/kg/min,AUC 7.48±2.36μg/ml·h。内服给药的动力学参数是:C_(max)0.27±0.21μg/ml,t_(max)0.97±0.50h,t1/2ka 0.53±0.31h,t1/2 Ke 1.07±0.38h,tlag 0.07±0.08h,AUC 0.91±1.14μg/ml·h,F 3.20±5.7％。猪内服吡喹酮后,生物利用度很低的原因可能是吡喹酮在肝内有极强的首过效应。

Paleomagnetic records from globally distributed locations are essential for fully understanding geomagnetic field variations,particularly non-dipole field fluctuations[1].Paleomagnetic studies on historical time scales have demonstrated the existence of persistent geomagnetic flux patches,such as the Canadian(north America)and Siberian(East Asia)flux lobes,which may result from an organizing structure imposed on the geomagnetic field by lower mantle heterogeneity[2].Holocene paleomagnetic secular variations,reconstructed from the Alaskan margin of the Subarctic Pacific Ocean(SPO)[3]and revealed by dynamo modeling[4],further suggest that time-varying flux expulsions on the core-mantle boundary recurrently occur in high-latitude locations(***).

The objective of this guideline is to outline the role of magnetic resonance imaging(MrI) in clinical decision making and outcome prediction in patients with traumatic spinal cord injury(SCI).Methods A systematic review of the literature was conducted to address key questions related to the use of MrI in patients with traumatic *** review focused on longitudinal studies that controlled for baseline neurologic status.A multidisciplinary Guideline Development Group(GDG) used this information,their clinical expertise,and patient input to develop recommendations on the use of MrI for SCI *** on GrADE(Grading of recommendation,Assessment,Development and Evaluation),a strong recommendation is worded as " we recommend," whereas a weaker recommendation is indicated by "we suggest." results Based on the limited available evidence and the clinical expertise of the GDG,our recommendations were:(1) "We suggest that MrI be performed in adult patients with acute SCI prior to surgical intervention,when feasible,to facilitate improved clinical decision-making"(quality of evidence,very low) and(2) "We suggest that MrI should be performed in adult patients in the acute period following SCI,before or after surgical intervention,to improve prediction of neurologic outcome "(quality of evidence,low).Conclusions These guidelines should be implemented into clinical practice to improve outcomes and prognostication for patients with SCI.
