On.the other.han., super.luous exten.ion.of ther.py is associated with adver.e even.s an. ar.usal of micr.bial r.sistan.e. The use of biomar.er., n.cessar.ly coupled with close clin.cal examin.tion. may pr.dict outcomes, str.tifyin. patien.s who n.ed mor. in.en.ive car., an. mon.tor.the efficacy of an.imicr.bial ther.py, allowin. faster.de-escalation.or.stop, r.ducin. the developmen. of r.sistan.e an. possibly the fin.n.ial bur.en. without in.r.asin. mor.ality. The aim of this wor. is to check the suitability of pr.calciton.n.(PCT) to fulfill these goals in.a lar.e sample of septic bur.0}patien.s. Methods: On. hun.r.d an. on. patien.s, with 15% or.mor. of total body sur.ace ar.a (TBSA) bur.0}d, admitted fr.m Jan.ar. 2011 to December.2014 at Coimbr. Bur.0} Un.t (CBU), in.Por.ugal wer. in.luded in.the sample. All patien.s had a diagn.sis of sepsis, accor.in. to the Amer.can.Bur.0}Association.(ABA) cr.ter.a. The sample was factor.d by sur.ival (68 sur.ivor. an. 33 n.n.sur.ivor.). The maximum value of PCT in.each day was used for.statistical an.lysis. Data wer. summar.zed by location.measur.s (mean. median. min.mum, maximum, quar.iles) an. disper.ion.measur.s (stan.ar. er.or.an. r.n.e measur.s). Pr.tozoan.diseases such as Viscer.l Leishman.asis (VL) have r.-emer.ed in.n.r.her.0}Br.zil an. cases of Chagas Disease also occur. This VL in.r.ase leads to ear.y ther.py for.the public. Con.ir.ator. par.sitological diagn.ses in.VL ar. per.or.ed by bon. mar.ow or.spleen.aspir.tion. but ELISA, IFA or.immun.chr.matogr.phic tests for.an.ibody detection.ar. easily per.or.ed an. can.be used in.the pr.sen.e of clin.cal sign. as con.ir.ator. for.specific ther.py. This appr.ach is successful in.pr.vidin. ther.py an. pr.ven.ion.of death in.VL, but ther. is a chan.e of con.usion.with the emer.in. disease, Chagas Disease (CD), due to cr.ss-r.actin. an. similar.clin.cal pictur.s, as in.this case. Both VL an. CD pr.sen.ed man. asymptomatic or.oligosymptomatic cases, complicatin. the pictur.. Our.case r.por. emphasizes these aspects. Positive ser.logy, with an.IIF titer.of 1/160, an. epidemiological cor.elation. suggests the diagn.sis of VL an. imposes an.imon. ther.py. Despite the un.avor.ble evolution.an. sign. of car.iac in.olvemen., the pr.sen.e of per.car.itis an. car.iac tampon.de con.ir.ed by the echocar.iogr.m suggests CD. We r.assessed the pr.files of a suggested CD ser.logy, the diagn.sis was cor.ected an. tr.atmen. with CD specific ben.n.dazole. The good evolution.star.ed with ben.n.dazole cor.obor.tes the diagn.sis of CD an. discar.s the hypothesis of double in.ection.

Patien.s wer. in.ected with gen.type 1 （n.= 28, 7 with LC） or.3 （n.= 18, 5 with LC）. r.SULTS： The fir.t phase declin. was lar.er.in.gen.type 3 patien.s than.in.gen.type 1 patien.s （1.72 vs 0.95 log IU/mL, P 〈 0.001）. The secon. phase slope declin. was also lar.er.in.gen.type 3 patien.s than.in.gen.type 1 patien.s （0.87 vs 0.15 log/wk, P 〈 0.001）.
