Background There were limited data comparing the major clinical outcomes between first-generation （1G）-drug eluting stentts （DES） and second-generation （2G）-DES in patients with acute myocardial infarction （AMI） after percutaneous coronary intervention （PCI） during very long follow-up periods. We thought to investigate the comparative efficacy and safety of 2G-DES compared with 1G-DES in AMI patients during 5-year follow-up periods. Method A total of 1016 eligible AMI patients who underwent PCI with 1G-DES [paclitaxel-, sirolimus-, 1G-zotarolimus-eluting stent （endeavor~ or endeavor sprintS）, n = 554] or 2G-DES [2G-zotarolimus （endeavor resolute~）- or everolimus-eluting stent, n = 462] were enrolled. The primary endpoint was the occurrence of major adverse cardiac events （MACE） defined as total death, non-fatal myocardial infarction （MI）, target lesion revascularization （TLR）, target vessel revascularization （TVR）, non-target vessel revascularization （Non-TVR） and the secondary endpoint was stem thrombosis （ST） at 5 years. Results Two propensity score-ma- tched （PSM） groups （232 pairs, n = 464, C-statistic = 0.802） were generated. During the 5-year follow-up period, the cumulative incidence of TLR [hazard ratio （HR）： 3.133; 95% confidence interval （CI）： 1.539-6.376; P = 0.002], TVR （HR： 3.144; 95% CI： 1.59645.192; P = 0.001） and total revascularization rate （FIR： 1.874; 95% CI： 1.086-3.140; P = 0.023） were significantly higher in 1G-DES compared with 2G-DES after PSM. However, the incidence of total death, non-fatal MI and ST were similar between the two groups. Conclusion In this single-center and all-comers registry, 2G-DES＇s superiorities for TLR, TVP, and total revascularization in AMI patients suggested during 5-year clinical follow-up periods.

AIM:To determine long-term outcomes of surgical treatments for patients with constipation and features of colonic ***:Consecutive 42 patients who underwent surgery for chronic constipation within the last 13 years were prospectively *** identified a subgroup with colonic pseudo-obstruction(CPO) features,with dilatation of the colon proximal to the narrowed transitional zone,in contrast to typical slowtransit constipation(STC),without any dilated colonic *** outcomes of surgical treatments for chronic constipation with features of CPO were analyzed and compared with outcomes for ***:Of the 42 patients who underwent surgery for constipation,33 patients had CPO with dilatation of the colon proximal to the narrowed transitional *** were 16 males and 17 females with a mean age of 51.2 ± 16.1 *** had symptoms of chronic intestinal obstruction,including abdominal distension,pain,nausea,or vomiting,and the mean duration of symptoms was 67 mo(range:6-252 mo).Preoperative defecation frequency was 1.5 ± 0.6 times/wk(range:1-2 times/wk).Thirty-two patients underwent total colectomy,and one patient underwent diverting transverse *** was no surgery-related *** histologic examination showed hypoganglionosis or agangliosis in 23 patients and hypoganglionosis combined with visceral neuropathy or myopathy in 10 *** contrast,histology of STC group revealed intestinal neuronal dysplasia type B(n = 6) and visceral myopathy(n = 3).Early postoperative complications developed in six patients with CPO;wound infection(n = 3),paralytic ileus(n = 2),and intraabdominal abscess(n = 1).Defecation frequencies 3 mo after surgery improved to 4.2 ± 3.2 times/d(range:1-15 times/d).Long-term follow-up(median:39.7 mo) was available in 32 patients;all patients had improvements in constipation symptoms,but two patients needed intermittent medication for management of *** 32 patients had distinct

采用响应面分析法优化金银花凉茶的浸提工艺条件,以金银花、菊花、夏枯草和鱼腥草为原料,绿原酸得率为指标.在单因素试验基础上,应用Box-Benhnken中心组合设计试验,对浸提工艺影响较大的料液比、浸提时间、浸提温度3个工艺参数进行优化.结果表明:金银花等原料的绿原酸水提工艺的最佳条件为不粉碎原料、固定金银花1g,菊花、夏枯草和鱼腥草质量比为2∶2∶1共1.5g,料液比为1∶32,浸提时间为71min,浸提温度为78℃,浸提2次,在该条件下验证绿原酸得率达到3.01%,接近于预测值3.04%.研究表明,优化得到的回归模型具有良好的预测能力.

Although chaotic signals are considered to have great potential applications in radar and communication engineering, their broadband spectrum makes it difficult to design an applicable amplifier or an attenuator for amplitude conditioning. Moreover, the transformation between a unipolar signal and a bipolar signal is often required. In this paper, a more intelligent hardware implementation based on field programmable analog array （FPAA） is constructed for chaotic systems with complete amplitude control. Firstly, two chaotic systems with complete amplitude control are introduced, one of which has the property of offset boosting with total amplitude control, while the other has offset boosting and a parameter for partial control. Both cases can achieve complete amplitude control including amplitude rescaling and offset boosting. Secondly, linear synchronization is established based on the special structure of chaotic system. Finally, experimental circuits are constructed on an FPAA where the predicted amplitude control is realized through only two independent configurable analog module （CAM） gain values.

Spontaneous or traumatic intramural bleeding of the esophagus, which is often associated with overlying mucosal dissection, constitutes a rare spectrum of esophageal injury called dissecting intramural hematoma of the esophagus (DIHE). Chest pain, swallowing diffi culty, and minor hematemesis are common, which resolve spontaneously in most cases. This case report describes a patient with spontaneous DIHE with recurrent massive bleeding which required critical management and highlights a potential role for therapeutic angiography as an alternative to surgery.

Solitary pancreatic involvement of tuberculosis is rare, especially in an immunocompetent individual, and it may be misdiagnosed as pancreatic cystic neoplasms. Pancreatic cystic neoplasms are being identified in increasing numbers, probably because of the frequent use of radiology and advances in endoscopic techniques. However, they are composed of a variety of neoplasms with a wide range of malignant potential, and it is often difficult to differentiate pancreatic tuberculosis mimicking cystic neoplasms from benign or malignant pancreatic cystic neoplasms. Non-surgical diagnosis of pancreatic tuberculosis is inconclusive and continues to be a challenge in many cases. If so, then laparotomy should be employed to establish the diagnosis. Therefore, pancreatic tuberculosis should be kept in mind during the differential diagnosis of solitary cystic masses in the pancreas. We report a patient who had solitary pancreatic tuberculosis masquerading as pancreatic serous cystadenoma.

A case of intussuscepted Meckel’s diverticulum

We report colonoscopic features of an intussuscepted Meckel's diverticulum, presenting with hematochezia. A 35-year-old woman presented to the emergency room with acute onset, transient, sharp, severe epi-gastric pain that began 6 h earlier. Colonoscopy re-vealed a reddish, soft, fi st-sized polypoid lesion in the terminal ileum. The lesion was misinterpreted as a hematoma by an inexperienced endoscopist. The pa-tient began to complain of intermittent, severe peri-umbilical pain following the colonoscopic examination. Subsequent computed tomography showed an enteric intussusception. An exploratory laparotomy revealed an intussuscepted Meckel's diverticulum, with trans-mural infarction. Colonoscopy was of little use in as-sessing the intussusception. However, colonoscopic examination may be performed initially, especially in an intussuscepted Meckel's diverticulum present-ing with hematochezia. Endoscopists should note the endoscopic features of an intussuscepted Meckel's diverticulum.

采用响应面分析法优化山楂中总黄酮超声辅助提取的工艺条件.以总黄酮(以金丝桃苷计)得率为指标,在单因素试验的基础上,应用了响应面中的Box-Benhnken模型设计试验,对提取工艺影响较大的山楂粉碎粒度、超声时间、料液比、超声功率4个工艺参数进行优化.结合实际操作,最佳的工艺条件为粉碎粒度40目,超声时间33min,料液比1∶33(g/mL),超声功率160W,在此条件下,总黄酮得率为5.21%,与预测值5.26%基本相符.
