Obesity is termed as the pathology defined by excess body weight due to the increase in adipocytes. It can be visceral, close to the omentum, mesentery, and subcutaneous, accumulated in the hypodermis. It is a disease that entails metabolic, respiratory, locomotor, and cardiovascular problems, type II diabetes, cancer, and even mental problems. Established by the Body Mass Index (BMI) and defined from the relationship between weight (kg) and height (m) of individuals. In numerical terms, a person is considered obese when the BMI is equal to or greater than 30 kg/m. This retrospective study took into account the more than 30 years that the coordinator of this work has devoted to morbidly obese people and all his 47 years of experience in medicine. The first obese person of his career was 41 years old and weighed 210 kg. As soon as she arrived at the hospital, she said: “Doctor, I am here, because I need to enter a ‘workshop’, to repair my body”. From that moment on, the name “workshop” became part of a fantastic project with obese people called “Obese Workshop”. Over time, the number of patients with this comorbidity has only increased, reaching, once, the point of speaking to 325 interested parties in a single day and in the same hall. In order to achieve satisfactory results, we have established a number of tests, by which we could diagnose several reasons that cause obesity. We selected randomly 20 patients of both sexes, morbidly obese, operated in 2013. We do not take into account age, race, creed, or social status.

Despite the growing needs from hospitals for activities such as risk control, cost management, quality control and digital communication, most clinical engineers are not aware or do not risk to be responsible for these tasks. Unfortunately, the majority of CEs do not defy the comfortable state but keep claiming the lack of support from administration. Up to now, the Body of Practices as well as the Body of Knowledge for Clinical Engineering is not a world consensus. Hence, Clinical Engineering Societies must start to discuss how to change this situation; how to improve the interaction not only among clinical engineers but with the health staff and discuss how to help clinical engineers to present themselves as problem solvers regarding medical equipment technologies. Ultimately, the Clinical Engineering Societies must start a regional movement to define what is the Clinical Engineering model that best suits the need of the health system and communicate the academy about their findings. Here it is discussed some initiatives to be adopted by regional clinical engineering societies to promote the profession.

This paper lbcuses on the main characteristics of a tree-shaped timber structural system or simply tree-shaped. One of them is the complexity of its steel connections, responsible for the joint of bar elements, compounding a complex structural system, which requires the application of the CYPECAD software version 2007 to solve the calculations problems. Its efficiency was confirmed by a variety of laboratory tests carried out with the whole structure. The tree-shaped is a timber structure that can be used in a large number of destinations, including residences, malls, sheds, hangars, etc.. Originally, it was conceived based on the well-known masterpiece ＂Sagrada Familia＂, whose author was Architect Antoni Gaudi I Cornet （1852-1926） in Barcelona/Spain. It was designed at the end of 19th century and its construction is still not finished. Gaudi inverted the logical order of the gothic concepts, i.e., light weights below and heavy weights above. Based on this concept, he always had in mind the figure of a tree in nature. The tree-shaped follows the same idea, using timber pieces connected by steel plates. Theoretical and numerical analyses have shown its efficiency and lightness for use in timber structures.

Performance of alpha-defensin lateral flow test after synovial fluid centrifugation for diagnosis of periprosthetic knee infection

BACKGROUND The quantitative alpha-defensin enzyme-linked immunosorbent assay(ELISA)demands a prior synovial fluid centrifugation,whereas this processing is not routinely required prior to the alpha-defensin lateral flow *** To evaluate whether a prior synovial fluid centrifugation could lead the lateral flow performance to achieve comparable results to ELISA during periprosthetic joint infection(PJI)*** Fifty-three cases were included in this study:22 classified as PJI and 31 classified as aseptic cases,according to Musculoskeletal Infection Society 2013 *** fluid samples were submitted to centrifugation,and the supernatant was evaluated by ELISA and lateral flow *** sensitivity(SE),specificity(SP)and accuracy of each method were calculated as well as the agreement between those two *** In all of the 31 samples from aseptic patients,alpha-defensin ELISA and lateral flow tests showed negative results for *** the 22 infected patients,the lateral flow test was positive in 19 cases(86.4%)and the ELISA was positive in 21(95.5%).Sensibility,SP and accuracy were,respectively,86.4%(95%CI:65.1%-97.1%),100%(95%CI:88.8%-100%)and 93.2%(95%CI:82.8%-98.3%)for the lateral flow test and 95.5%(95%CI:77.2%-99.9%),100%(95%CI:88.8%-100%)and 98.1%(95%CI:89.9%-100%)for *** agreement of 96.2%between those methods were *** statistical difference was found between them(P=0.48).CONCLUSION Alpha-defensin lateral flow test showed high SE,SP and accuracy after a prior synovial fluid centrifugation,achieving comparable results to *** the lower complexity of the lateral flow and its equivalent performance obtained in this condition,a prior centrifugation might be added as a valuable step to enhance the PJI diagnosis.

BACKGROUND Alpha-defensin has been widely studied for the diagnosis of periprosthetic joint infection(PJI).However,there is a lack of detailed information regarding the proper laboratory technique of the enzyme-linked immunosorbent assay(ELISA)method,such as sample *** To assess the influence of dilution in the synovial fluid during ELISA for the diagnosis of knee PJI;and determine which dilution presents a better *** Forty samples of synovial fluid from arthroplasty knees were included,17 in the infected group and 23 in the aseptic group,according to Musculoskeletal Infection Society ***,five synovial fluid samples from each group were assessed for quantitative analysis of alpha-defensin using *** dilution ratios(1:10,1:100,1:500,1:1000 and 1:5000)were tested based on the predetermined cutoff value of 5.2 mg/*** dilutions that performed better were used to compare the results of all *** For infected cases,a gradual increase in the dilution of synovial fluid samples led to an equivalent increase in alpha-defensin *** same was not observed in the aseptic *** 1:1000 and 1:5000 dilutions presented satisfactory results to differentiate infected and aseptic *** analyses were performed using 1:1000 and 1:5000 for all 40 *** 1:1000 dilution resulted in a sensitivity of 88.2%(95%CI,66%-98%)and specificity of 95.7%(95%CI,79%-99%),whereas the 1:5000 dilution presented a sensitivity of 94.1%(95%CI,73%-99%)and a specificity of 100%(95%CI,86%-100%).CONCLUSION The synovial fluid dilution had an important influence on the alpha-defensin ELISA *** of 1:5000 showed the best performance for the diagnosis of knee *** results of this study set the basis for a more reliable and reproducible alpha-defensin ELISA during the investigation of PJI,contributing to the expansion of this technique in different treatment centers worldwide.

To assess the prevalence, the antimicrobials resistance and to identify risk factors of nasopharyngeal colonization by Streptococcus pneumoniae in children with sickle cell disease (SCD) using prophylactic penicillin and immunized with 7-valent pneumococcal conjugate vaccine. A total of 424 swabs were collected from the nasopharynx in 216 children with SCD and 109 samples from the control group, both from 2 to 60 months age range. Isolation and identification of pneumococci followed standard procedures. Minimum inhibitory concentration (MIC) for penicillin was determined by the E-test method. Prevalence of nasopharyngeal colonization by pneumococci in children with SCD was 17%, and 11% in the control group. The risk factors for increased colonization in children with SCD were the presence of more than five people at home, daycare/school attendance and low prophylaxis compliance. The prevalence of strains with penicillin resistance was 57.5% in patients with SCD and 25% in the control group. Conclusions: Prevalence of pneumococci nasopharyngeal colonization was similar among the study groups. Prophylactic use of penicillin may have increased the prevalence of resistant strains. The vaccine did not decrease the colonization with penicillin resistant strains.

The bipartite begomoviruses (Geminiviridae family), which are DNA viruses that replicate in the nucleus of infected cells, encode the nuclear shuttle protein (NSP) to facilitate the translocation of viral DNA from the nucleus to the cytoplasm via nuclear pores. This intracellular trafficking of NSP-DNA complexes is accessorized by the NSP-interacting guanosine triphosphatase (NIG) at the cytosolic side. Here, we report the nuclear redistribution of NIG by AtWWPI, a WW domain-containing protein that forms immune nuclear bodies (NBs) against begomoviruses. We demonstrated that AtWWPI relocates NIG from the cytoplasm to the nucleus where it is confined to AtWWP1-NBs, suggesting that the NIG-AtWWP1 interaction may interfere with the NIG pro-viral function associated with its cytosolic localization. Consistent with this assumpti on joss AtWWPI functi on cuased plants more susceptible to begomovirus infection, whereas overexpression of AWWP1 enhaneed plant resistance to begomovirus. Furthermore, we found that a mutant versi on of AtWWPI defective for NB formatio n was noIonger capable of interacti ng with and relocating NIG to the nucleus and lost its immune function against begomovirus. The antiviral function of AtWWP1-NBs, however, could be antagonized by viral infection that induced either the disruption or a decrease in the number of AtWWP1-NBs. Collectively, these results led us to propose that AtWWPI organizes nuclear structures into nuclear foci, which provide intrinsic immunity against begomovirus infection.

Healthcare Professionals’ Adherence to Contact Precautions at a Maternal and Child Teaching Hospital

Background: Healthcare-associated infections affect hundreds of millions of patients worldwide. Children have greater susceptibility to healthcare-associated infections due to the immaturity of their immune system. Contact precautions aim to promote safety, protection and prevention of contamination. Thus, the objective of this study was to verify adherence to contact precaution measures, as well as compliance to the use of personal protective equipment. Methods: This cross-sectional observational study was carried out from July to October 2019 using a checklist to evaluate health professionals’ adherence to hand hygiene procedures and the use of gloves and surgical gowns when assisting children on contact precautions. Results: A total of 941 observations were carried out in a total of 300.532 hours. Hand hygiene was performed before and after contact with the patient in 58.84% and 75.09% of the cases, respectively and a surgical gown was used in 86.40% of the cases. The use of gloves was the variable most adhered to by professionals (87.57%). Intensive care unit professionals were the care workers who most complied with the regulation regarding hand hygiene after contact with the patient (p = 0.009) and the use of the surgical gown (p < 0.001). The correct hand hygiene technique was the recommendation with least adherence. Non-compliance to the hand hygiene technique was statistically significant among intensive care unit professionals (p = 0.002). Conclusions: Adherence to hand hygiene before contact with the patient and compliance with the hand hygiene technique were neglected by most professionals. However, there was good adherence to the use of surgical gloves and gowns, as well as high compliance to the techniques of removing these items. .

The adoption by health professionals to the practice of patient isolation is a decision that reduces patients’ risks of occupational exposure to potentially contaminated biological material and prevents nosocomial infections. Verify the compliance of health professionals to the practices of contact precautions in patients colonized by microorganisms in a teaching hospital. This was a prospective observational cohort study in a special-sized hospital located in southeastern Brazil. The observation of health professionals in delivery of health care to patients with contact precaution was performed regarding hand hygiene, use of overcoat, glove, and mask. We carried out 1502 observations involving the following professional categories: nursing technicians and/or nursing assistants (n = 1028;68.4%), nurses (n = 200;13.3%), physical therapists (n = 185;12.3%), and physicians (n = 89;5.9%), totaling 971.8 hours. Regarding the level of compliance of professionals to contact precaution practices with hand hygiene after the procedures, the use of overcoats, gloves, and masks, all were statistically significant (p Klebsiella pneumoniae was the most positive pathogen identified in laboratory tests. This study provided the mapping of the compliance of health professionals to the practices of contact precautions in order to support a safer management of patient care reducing the risks of Healthcare-Associated Infections (HAI). This study also showed that health professionals are using practice management to deal with their behavior to protect their health.
