Unfused Crossed Renal Ectopia with Nephrolithiasis Diagnosed Incidentally in Emergency Room

Crossed renal ectopia is the second most common fusion anomaly of the kidney after horseshoe kidney. The incidence of both fused and unfused cases is 1 in 7000 in autopsies. The unfused crossed renal ectopia is much rare with an incidence of 1/75000 in autopsies. Ninety percent ofectopic kidneys are fused to their ipsilateral mate and the ureter of the ectopic kidney inserts into its normal position in the bladder at the contralateral side. But demonstration of the anomaly may provide to search for accompanying congenital anomalies and to plan a follow-up program for possible future complications in most of the cases. However it may remain asymptomatic for years as in the patients. Here the authors are presenting a 64-year-old male patient, with a right sided unfused crossed renal ectopia anomaly without any complaints until this age.

Background： We examined the impact of adjuvant modalities on resected pancreatic and periampullary adenocarcinoma（PAC）.Methods： A total of 563 patients who were curatively resected for PAC were retrospectively analyzed between 2003 and ***： Of 563 patients, 472 received adjuvant chemotherapy（CT） alone, chemoradiotherapy（CRT） alone, and chemoradiotherapy plus chemotherapy（CRT-CT） were analyzed. Of the 472 patients, 231 were given CRT-CT, 26 were given CRT, and 215 were given CT. The median recurrence-free survival（RFS） and overall survival（OS） were 12 and 19 months, respectively. When CT and CRT-CT groups were compared, there was no significant difference with respect to both RFS and OS, and also there was no difference in RFS and OS among CRT-CT, CT and CRT groups. To further investigate the impact of radiation on subgroups, patients were stratified according to lymph node status and resection margins. In node-positive patients, both RFS and OS were significantly longer in CRT-CT than CT. In contrast, there was no significant differencebetween groups when patients with node-negative disease or patients with or without positive surgical margins were ***： Addition of radiation to CT has a survival benefit in patients with node-positive disease following pancreatic resection.

Background: In oral and maxillofacial surgery, synthetic bone grafts are most widely used as bone substitutes, due to the limited sources of autologous bone. The aim of this study was to examine the influence of three different synthetic bone grafts (Cerasorb, Fortoss and Perioglass) on sisters chromatide exchanges (SCEs) in peripheral lymphocytes. Materials and Methods: Peripheral blood samples taken from 68 patients (45 females and 23 males), who underwent oral surgery procedures, such as apical resection, cyst enucleation or periodontal curretage, were obtained for SCE a day before and two months after the surgeries. A control group included 30 patients, while the study group was made of the patients who underwent bone grafting with Cerasorb? (11 patients), Fortoss? VITAL (10 patients) or Perioglass? (17 patients). Results: Comparing with the results of the study group before and after the treatment, it was concluded that the results were statistically significant (p = 0.001). In the Perioglass? subgroup, a greater statistical significance (p = 0.003) was noted, than that in either the Cerasorb? (p = 0.620) or Fortoss? (p = 0.210) subgroups, in which there was no statistical significance. Conclusions: Although further investigations may be necessary, our results suggest that the synthetic bone grafts might have an influence on SCE in peripheral lymphocytes.

Functioning Adrenocortical Oncocytic Carsinoma: A Case Report

Introduction: Oncocytic neoplasms of the adrenal cortex are extremely rare and are usually nonfunctioning. Only a little more than fifty cases have been described in the literature. Case Presentation: We report the case of an adrenocortical oncocytic neoplasm with malignant potential in a 21-year-old female with co-secrete testosterone and cortisol. The patient had been operated of adrenal mass. Fallowing operation, the Cushing’s syndrome and high testosterone levels resolved. Conclusion: In our knowledge this is the first case in the l?terature which co-secret cortizol and testosterone in malign oncystoma.

Alstrom Syndrome is an autosomal recessive genetic disorder affecting multiple systems. The characteristic features of this syndrome are obesity, type 2 diabetes mellitus, rod-cone dystrophy, hearing loss. Developmental delay, nistagmus, dilated cardiomyopathy, hypertension, recurrent pulmonary infections, short stature, hepatic and renal failure endocrine abnormalities are other clinical features of this syndrome. Here we report on a case with Alstrom Syndrome at the age of thirteen. He was referred to medical genetics clinic from endocrinology where he has been watched because of obesity, type 2 diaibetes mellitus and hypogonadism. His parents were first degree relatives. He was also 95% blind and had hearing loss. When we looked up these findings with a clinical diagnosis we thought about Alstrom Syndrome. It is a rare disease and difficult to make differential diagnosis with other similar syndromes, therefore this case will be a good example of Alstrom Syndrome for the literature.

目的:评估一种由睫状体前和脉络膜上腔引流房水的新型植入装置Suprajet的有效性和安全性。方法:该项研究使用5只兔子。每只兔子的一眼植入Suprajet。引流器通过位于上方的透明角膜切口,经由前房置入脉络膜上腔。引流器近端位于睫状体根部,远端位于脉络膜上腔。兔子饲养、观察4wk。使用Tonopen AVIA测量术前和术后的眼压。最后一次随访观察时,兔子被牺牲处死,眼球摘除,进行大体上和组织学的观察评估。结果:术前眼压为18.6±6.1mmH g。术后1wk眼压为8.4±1.1mmH g。术后2wk 1只兔子死亡。因此,仅有4只兔子进行了后续观察。术后2wk眼压为11.0±2.8mmH g,术后3wk为9.50±3.1mmH g,术后4wk为11.3±3.3mmH g。与术前平均眼压相比,仅第1周的平均眼压显著降低(P=0.042),术后2wk、3wk和术后4wk的平均眼压无明显变化(P=0.66,P=0.66,P=0.102)。术中并发症包括3眼少量出血。术后2d出血已经完全吸收。对摘除眼球的大体观察发现,1眼中引流器的远端位于玻璃体内,而不是位于脉络膜上腔;其它3眼引流器的远端位于脉络膜上腔。所有眼中,近端都位于前房角。对摘除眼的组织学检查发现:不规则的胶原蛋白束和纤维沉积,包括引流器周围大量的成纤维细胞和组织细胞。结论:这一项初始动物研究显示,青光眼中Suprajet植入是一项有前景的术式。需要进行更进一步的研究以评估其有效性和安全性。

Recently, NK1R （Neurokinin-1 receptors） take attention as new and promising target in anticancer drug development area. It has been proved that non-peptide NK1R antagonists L-733,060, aprepitant and L-732,138 inhibited tumor growth in several cancer cell lines. For the development of novel NK1R antagonists as antitumor agents, heterocyclic compounds which were previously synthesized by our team, tested for their cytotoxic activities in several cancer cell lines in this study. Among the tested compounds, a benzothiazole derivative BSN-009 inhibited colon cancer cell lines growth by 57.53% by comparing the activity to the control drug aprepitant. Molecular modeling studies such as molecular docking and pharmacophore generation were performed with known NK1R antagonists and BSN-009 by using Discovery Studio 3.5 in order to explain their binding modes to NK1R. BSN-009 may be a good anticancer drug candidate as a possible NK1R antagonist and is worthy to carry on the anticancer studies.
