Diagnostic Utility of Indigenous Technique of Pleuroscopy in Undiagnosed Cases of Exudative Pleural Effusions

Background and Aims： Since our rural institution has liamted resources we always choose econoamcal options. Recently, indigenous technique of pleuroscopy is gaining popularity due to various advantages. We developed indigenous technique of medical thoracoscopy has been developed and done by using set of patented conduits and fiberoptic bronchoscope （FOB）.Therefore, we have used this technique for undiagnosed pleural effusions. Methods： An observational study was conducted in 79 undiagnosed cases of exudative pleural effusion from June 2016 to Jmmary 2017. Indigenous technique consists of use of fiber optic bronchoscope through various metallic conduits to be used in specified order for medical thoracoscopy. Procedure is done under conscious sedation and conduits are passed one after another in a specified order, through the chest stoma. The visualization of pleural cavity and various procedures were done with FOB （fiberoptic bronchoscope）. Results： Out of the 79 cases, the appearance of pleura showed, inflamed/reddened pleura in 16 （20.2%） cases, thin transparent adhesions in 18 （22.7%）, thin transparent loculations in 16 （20.2%） cases, thick loculations in 6 （7.6%） cases, hard pleural surface in 5 （6.3%）, large nodule/masses in 6 （7.6%）, small amlliary seedlings or sago grain appeoxmlce in 6 （7.6%）, scattered masses or nodules in 5 （6.3%） and, broncho-pleural fistula was observed in 1 （1.2%） case. Histopathological analysis showed chronic inflammation in 34.1% （27）, tubercular lesions in 24.05% （19） of patients. Primary aspergillosis and mesothelioma each has 1 case, and the rest 29.11% （23） were pleural metastasis. Thus, diagnostic yield of pleuroscopy pleural biopsy was 89.9% （71）. Conclusion： Indigenous technique appears to be an efficient and relatively safe procedure with good diagnostic yield in undiagnosed pleural effusions.

The objective of the present study was to evaluate the pre-clinical efficacy and toxicity of polyherbal cough syrup Linkus. Method: Animals (healthy Wistar albino rats;(150 - 250 g) of either sex) were housed under standard environmental conditions;i.e. 25°C ± 1°C and 12 h dark/light cycle. Food and water were available at libitum. The rats were treated orally with the recommended doses of the test drug (Linkus). After 15 minutes, they were individually placed in a closed Plexiglas chamber (20 × 10 × 10 cm) and exposed to citric acid (0.1 g/ml) inhalation for 7 minutes. The cough reflexes were produced and counted for the last 5 minutes and compared with those of the control animals. The following studies were conducted to evaluate the toxicity of the test drug in healthy Wistar albino rats: lethal dose50 (LD50);rats of either sex (n = 10/sex) were treated orally with doses (1 or 5 g/kg) of the test drug. Mortality and behavioral changes were observed for 1 week. Repeated dose toxicity on the healthy Wistar albino rats of both sexes (n = 5/dose/sex) was treated orally with doses of 20 mg/kg (adult human dose = ~1400 mg), 500 mg/kg (adult human dose = ~35,000 mg) and 1000 mg/kg (adult human dose = ~70,000 mg) of test drug (Linkus) for 14 days. Additionally, the control animals were treated orally with water for 14 days. Results: In female rats, the test drug (Linkus) at the dose of 300 mg/kg caused significant (p < 0.01) reduction in the cough reflexes as compared to the control. However, in male rats, a significant reduction was observed at the tested dose of 200 mg/kg (p < 0.05) and 300 mg/kg (p < 0.01). The test product did not cause mortality in rats at the given doses of 1 or 5 g/kg. Other signs of toxicity like hair loss and weight reduction were not observed. In female and male rats, the test drug (Linkus) at different doses did not show any abnormal effects on complete blood count profile of rats. The United Nations 2030 Agenda for Sustainable Development provides an important framework for economic,social,and environmental action.A comprehensive indicator system to aid in the systematic implementation and monitoring of progress toward the Sustainable Development Goals(SDGs)is unfortunately limited in many countries due to lack of *** availability of a growing amount of multi-source data and rapid advancements in big data methods and infrastructure provide unique opportunities to mitigate these data shortages and develop innovative methodologies for comparatively monitoring *** Earth Data,a special class of big data with spatial attributes,holds tremendous potential to facilitate science,technology,and innovation toward implementing SDGs around the *** programs and initiatives in China have invested in Big Earth Data infrastructure and capabilities,and have successfully carried out case studies to demonstrate their utility in sustainability *** paper presents implementations of Big Earth Data in evaluating SDG indicators,including the development of new algorithms,indicator expansion(for SDG 11.4.1)and indicator extension(for SDG 11.3.1),introduction of a biodiversity risk index as a more effective analysis method for SDG 15.5.1,and several new high-quality data products,such as global net ecosystem productivity,high-resolution global mountain green cover index,and endangered species *** innovations are used to present a comprehensive analysis of SDGs 2,6,11,13,14,and 15 from 2010 to 2020 in China utilizing Big Earth Data,concluding that all six SDGs are on schedule to be achieved by 2030.

Transition Metal Oxides have drawn significant attention due to their reversible chemical redox reaction and long-life *** agglomeration and shrinkage/expansion of transition metal oxides in the nanosize regime have a noticeable effect on their electrochemical *** in this work,mesoporous nanowires(NWs)with a typical composition of iron-nickel-cobalt ternary oxide(FNCO)are synthesized using a simple,facile and cost-effective hydrothermal process followed by furnace *** NWs are then extensively investigated as an electrode material for supercapacitor *** compare the electrochemical properties,nanowires of nickel-cobalt oxide(NCO),iron-cobalt oxide(FCO)and cobalt oxide(CO)were also produced by following the same *** FNCO NWs are found to overcome the shortcomings in the electrochemical energy storage devices by exhibiting higher values of specific capacitance(2197 Fg^(-1))and energy density(109 Whkg^(-1))at 1 Ag^(-1) current ***,the FNCO NWs also showed a cyclic charge/discharge stability of 96%even up to 20,000 ***,a FNCO//graphene asymmetric device,fabricated with FNCO NWs and graphene as positive and negative electrodes,respectively,which exhibit high energy density(47 Whkg^(-1)),power density(375 Wkg^(-1))and excellent capacitance retention(86%)after 15,000 cycles.

背景:血管紧张素转换酶作为肾素-血管紧张素系统的关键酶,通过影响P物质降解等机制参与阿尔茨海默病的发生与发展。目的:分析血管紧张素转换酶基因多态性与阿尔茨海默病的关系,探讨性别、高血压病对两者关系的影响。方法:纳入96例阿尔茨海默病患者,102例年龄、性别等具有可比性的正常人作为对照组。采集血液,抗凝后进行DNA抽提,用PCR反应进行扩增,20 g/L琼脂糖凝胶电泳,紫外光下观察结果,分别计算II、DD型纯合子及ID杂合子基因型及基因型频率。在此基础上,综合分析收集的相关临床资料,进行性别及血压分层分析。结果与结论:阿尔茨海默病组与对照组血管紧张素转换酶基因型和等位基因频率差异有显著性意义,伴有高血压的阿尔茨海默病患者与对照组基因型和等位基因频率差异有显著性意义;不同性别分层及血压正常的阿尔茨海默病组与对照组基因型和等位基因频率差异无显著性意义。提示血管紧张素转换酶基因多态性与阿尔茨海默病的发病有关,其中II基因型可能是阿尔茨海默病发病的危险因素,血管紧张素转换酶基因型可能是阿尔茨海默病伴高血压病的危险因素。

甲烷(CH_(4))是仅次于二氧化碳(CO_(2))的重要温室气体,对全球气候变化有重要的反馈作用。亚热带森林土壤是全球陆地生态系统重要的CH_(4)汇,但因在亚热带的观测较少,致使模型估算得出的结果具有不确定性,因此需对土壤CH_(4)通量进行现场观测和实验,以便准确估计亚热带森林土壤CH_(4)通量及其对该生态系统碳汇能力的贡献。采用大型多点自动开闭箱式自动连续测定法在哀牢山亚热带常绿阔叶林开展土壤CH_(4)通量、气温(t_(a))、地表温度(t_(0))、土壤5 cm温度(t_(5))、降雨量(P)和土壤含水量(C_(sw))进行连续1年定位观测。结果表明,(1)云南哀牢山亚热带常绿阔叶林土壤为CH_(4)的汇,年通量为(−11.79±0.001)kg·hm^(-2)·a^(-1),年均速率为(−0.13±0.05)mg·m^(-2)·h^(-1),呈现出显著的季节动态,表现为干季土壤CH_(4)的吸收通量[(−0.17±0.06)mg·m^(-2)·h^(-1)]显著高于雨季[(−0.10±0.03)mg·m^(-2)·h^(-1)](P<0.05)。(2)土壤CH_(4)通量与地表温度(r^(2)=0.2125,P<0.001)、土壤5 cm温度(r^(2)=0.1948,P<0.001)和气温(r^(2)=0.0983,P<0.001)呈显著相关关系,但土壤CH_(4)通量与土壤5 cm温度的相关关系在12.35℃由正相关转为负相关。(3)土壤CH_(4)通量对土壤含水量的变化响应较温度敏感,土壤含水量可解释其90.36%的相关性(P<0.001),单因素和双因子关系模型进一步证明土壤含水量是调节哀牢山亚热带常绿阔叶林土壤CH_(4)通量的主导因子。(4)在20年和100年时间尺度上,CH_(4)通量的全球增温潜势(global warming potential,GWP)分别可以抵消土壤CO_(2)排放产生增温潜势的2.2%和0.7%,可以增加该生态系统碳汇的10.6%和3.5%。为此,在未来降水格局变化的情景下,土壤含水量可用于预测哀牢山亚热带常绿阔叶林土壤CH_(4)通量的变化和生态系统CH_(4)汇的强度,进而反映其对该森林生态系统碳汇能力的贡献。
