Risk Factors Associated with Anthracycline Induced Cardiac Dysfunction in Pediatric Patients

Anthracyclines (i.e., doxorubicin, daunorubicin) have significant impact on outcome in many pediatric chemotherapy protocols and therefore remain the mainstay of treatment. The objective of this study was to identify the risk factors for anthracycline induced cardiac dysfunction in pediatric patients. Multiple logistic regression model was applied to assess the risk factors for development of cardiac dysfunction. 110 pediatric oncology patients were available for final analysis. 75 (66%) children were males and mean age was 74 ± 44 months. ALL (n = 70, 64%) was the most common primary diagnosis followed by lymphoma (n = 19;17%) and AML (n = 12, 11%). Daunorubicin alone or in combination with doxorubicin was used in (n = 94, 85%) patients and cumulative dose n = 95;86%) children. 24 (22%) children received radiation therapy as per protocol and sepsis were observed in 47 (43%) cases. Post anthracycline, 15 (14%) children had cardiac dysfunction within a month;out of them 10/15 (67%) had isolated diastolic dysfunction, while 28 (25%) developed dysfunction within a year. 19 (17%) had pericardial effusion. 11 expired and out of them, 7 had significant cardiac dysfunction. Cumulative dose > 300 mg/m2 (p p = 0.009;AOR 3.5) and sepsis (p = 0.002;AOR 2.6) were found to be independent risk factors associated anthracycline induced cardiac dysfunction. At univariant level use of daunorubicin alone or in combination therapy (p p 0.048, OR 9.7) were also found statistically significant. In conclusion anthracycline induced cardiac dysfunction is mostly related to cumulative dose > 300 mg/m2, use of Daunorubicin alone or in combination with doxorubicin, mode of delivery, radiation therapy and sepsis. Regular long term follow-up with cardiologist is the key point for early diagnosis and therapy for a long term survival.

Validation of Reference Genes for mRNA Quantification in Adjuvant Arthritis

Real time quantitative PCR (RT-qPCR) requires a method to normalize the expression of target genes against an en- dogenous reference gene. It is known that commonly used housekeeping genes (HKGs) vary tremendously in inflam- matory conditions;however information about the stability and expression of HKGs in chronic inflammatory joint dis- ease such as rheumatoid arthritis (RA) is scarce. The expressional stability of 10 commonly used HKGs was analyzed in the neuronal (spinal cord, dorsal root ganglia) and in the musculoskeletal tissues (tendon, muscle, epiphysis, capsule, periosteum and ankle joint) using RT-qPCR in the rat model of RA. In individual tissues, suitable HKGs were selected by | △Ct| (│Ct control-Ct arthritis│) and further analyzed by using software programs;geNorm and normfinder. We found hypoxanthine-guanine phosphoribosyl tranferase (HPRT) as the most stable gene except ankle joint while glyce-raldehyde-3-phosphate dehydrogenase (GAPDH) was found as the least stable gene in musculoskeletal tissues. In in-flamed ankle joint where no reference gene was found to be stably expressed, an inflammatory cell marker CD3 was used to normalize peptidylprolyl isomerase B (PPIB), the most homogenous HKG identified among the 10 HKGs. The normalized PPIB was then used to analyze the gene expression of neurokinin 1 (NK1), receptor of substance P, a potent pro-inflammatory mediator. We observed a 3.5 fold increase (p = 0.009) in NK1 expression in inflamed ankle joint compared to control. Our results indicate that reference genes stability should be evaluated before using them as refer- ence during inflammatory conditions. In tissues with intense inflammatory cell infiltration, an inflammatory cell marker should be used to normalize the selected reference gene to avoid erroneous results.

Seroprevalence of Toxoplasma gondii infection in domestic animals of Mohmand agency,Pakistan

Objective:To determine the prevalence of toxoplasmosis among domestic animals of Mohmand Agency,***:Indirect haemagglutination test was used to detect Toxoplasma gondii(***)antibodies in the ***:Out of 384 domestic animals,124(32.29%)were seropositive for antibodies of *** antibodies were detected in 56 out of 104 examined goats(53.84%).The seropositivity in male and female goats were 20(38.46%)and 36(69.23%)*** seropositivity of *** in sheep was 36%(n=100)while the prevalence in female sheep was higher(41.66%)as compared to male sheep(30.76%).A total of 8 out of 48 male cows were detected seropositive(16.66%)and 8 out of 32 female cows were detected seropositive(25%).Lowest prevalence was detected in buffaloes which was 16%(n=100).Out of 52 male buffaloes 4 were detected seropositive for *** antibodies(7.69%)while in female buffaloes was 25%(n=48).The seroprevalence of *** antibodies was higher in goats and lower in ***:The findings of the present study indicate that *** infection is high in domestic animals in Mohmand agency,Pakistan,which has implications for public health in this *** control strategies and suitable measures should be carried out rationally in this region,in order to lower the risk of exposure to ***.

Heat Dissipation Modeling of <i>In-Situ</i>Conversion Process of Oil Shale

In-situ conversion of process of oil shale has been technically proven as a pilot field project. Gradually heating the reservoir by using subsurface electric heaters converts the oil shale reservoir kerogen into oil, gas and other producible components. This process also enhances the internal energy of the porous media as well as the subsurface fluid. Heat is transmitted in the reservoir within each fluid by different processes i.e. , due to the flow of fluid called advective process, and due to molecular diffusion where dispersive and diffusive processes take place. Heat transfer through conduction and convection mechanisms in the porous media are modeled mathematically and numerically incorporating the advective, dispersive and diffusive processes in the reservoir. The results show the production of oil and gas as a result of conversion of kerogen due to modeled heat dissipation.

Water Soluble Ionic Species in the Atmospheric Fine Particulate Matters (PM2.5) in a Southeast Asian Mega City (Dhaka, Bangladesh)

Atmospheric fine particulate matters (PM2.5) were collected with an Envirotech Instrument (Model APM 550) at the roof of Khundkur Mukarram Hussain Science Building, University of Dhaka, Bangladesh between January and February, 2013. PM2.5 samples were collected on Quartz fiber filters during day and night time. Water soluble ions (sulfate, nitrate, phosphate, chloride, bromide, sodium, potassium and calcium) were analyzed with Ion Chromatography (Model 881, Metrohm Ltd., Switzerland) and Flame photometer (Model PFP7, Jenway, UK). Average PM2.5 mass was 136.1 μg·mǅ during day time and 246.8 μg·mǅ during night time with a total average of 191.4 μg·mǅ. Nighttime PM2.5 concentration was about double compared than that of daytime presumable due to the low ambient temperatures with high emissions from heavy duty vehicles. The 24-hour average PM2.5 mass (average of day and night) was about eight times higher than WHO (25.0 μg·mǅ) and about three times higher than DoE, Bangladesh (65.0 μg·mǅ) limit values. The total average concentrations of sulfate, nitrate, phosphate, bromide, chloride, sodium, potassium and calcium were 5.30, 7.75, 0.62, 0.16, 1.19, 1.30, 8.11, and 3.09 μg·mǅ, respectively. The concentrations of the water soluble ions were much higher during nighttime than daytime except nitrate, bromide and potassium. Excellent correlations were observed between sulfate and nitrate, sodium and chloride, bromide and phosphate indicating joint sources of origin. Potassium, sulfate, nitrate and calcium are the most dominant species in PM2.5. Water soluble ionic components in Dhaka contributed about 15% mass of the PM2.5. Ratio analysis showed that sodium and chloride were from mainly sea salt. Potassium has varieties of sources other than biomass burning. Sulfate and nitrate are mainly from fossil fuel origin. In this paper, synthesis and anticancer activities of morpholine hydrazones scaffold（1-17） thoroughly studied. Small series of morpholine hydrazones synthesized by treating 5-morpholinothiophene-2-carbaldehyde with different aryl hydrazides to form morpholine hydrazones scaffold（1-17）. The in vitro anticancer potential of all these compounds was checked against human cancer cell lines like Hep G2（human hepatocellular liver carcinoma） and MCF-7（human breast adenocarcinoma）. Analogs 13 had similar substantial cytotoxic effects towards Hep G2 with IC_（50） value 6.31±1.03μmmol/L when compared with the standard Doxorubicin（IC_（50）value 6.00±0.80μmmol/L）; while compounds 5,8 and 9 showed potent cytotoxicity against MCF-7 with IC_（50） value 7.08±0.42μmmol/L, 1.26±0.34μmmol/L and11.22±0.22μmmol/L respectively when compared with the standard Tamoxifen（IC_（50）= 11.00±0.40μmol/L）. Molecular docking studies also performed to understand the binding interaction.

This study was planned to assess genetic variation in Jatropha curcas as affected by colchichine treatments. During the experiment, seeds of Jatropha curcas were collected from mature and healthy Jatropha plants and were soaked in different concentrations of colchicines. The second treatment was that of time of soaking (seeds were soaked in different concentrations for different time durations). The results clearly revealed that by increasing the concentration of colchicine treatment, parameters like stomatal density, and size of the guard cells were reduced.

Cutaneous Abnormalities in Chronic Kidney Disease Patients with and without Dialysis

Background: Chronic kidney disease (CKD) is increasingly prevalent worldwide. CKD may present with different cutaneous manifestations. Objectives: To evaluate the dermatological manifestations and compare these manifestations between patients with maintenance hemodialysis (MHD) and non-dialysis groups. Methods: It was a cross sectional study conducted in the Department of Nephrology, Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka, Bangladesh from April 2013 to March 2014. A total of 150 hospital admitted CKD patients were evaluated for dermatological manifestations. Age, gender, haemoglobin level, 24-hours urinary total protein (UTP), serum creatinine, serum fasting lipid profile, estimated glomerular filtration rate (e-GFR) and dermatological manifestations of the study subjects were recorded accordingly. Inter-group comparisons were made between patients with and those without cutaneous abnormality. Results: Out of 150 study subjects, 99 (66%) were male and 51 (34%) were female, 69 (46%) patients were on MHD and 81 (54%) patients were without dialysis, their mean (±SD) age was 44.6 ± 12.3 year. A Total of 126 (84%) patients had cutaneous abnormalities;among them 69 (54.76%) were pre-dialytic and 57 (45.24%) were on MHD group. The mean (±SD) serum creatinine was relatively higher but haemoglobin level was significantly lower (p = 0.021), while UTP was significantly higher (p = 0.038) among patients with cutaneous abnormality. There was no relationship between lipid profile with cutaneous abnormality. Among 126 (84%) patients with cutaneous abnormality;pallor was the most common cutaneous abnormality (72%) followed by xerosis (68.66%), pruritus (65.33%), half and half nails (38.66%), pigmentation (33.33%), purpura/ecchymosis (16.66%), fungal infection (16%), ulcerative stomatitis (10.66%) and bacterial infection (10%).
