Cenchrus biflorus called Karangiya in the Hausa language is an annual pastoral grass which is a valuable herbaceous fodder in dry land region in the context of climate change. However, little is known about the plant water use under the effects of cut in West Africa Sahel like Niger where the plant is a multipurpose grass species. Therefore, this study investigated the impact of grazing (simulated by shoot cuts) on biomass production. Cenchrus biflorus Roxb was grown on field plots and in pots and subjected to shoot cuts at different levels (3 cm and 5 cm from soil surface). The effect of shoot cuts on drought tolerance was evaluated by assessing the response of transpiration to the diurnal variation of vapor pressure deficit (VPD). Results showed that the biomass production varied in response to shoot cuts depending on the culture system, and the level or frequency of cuts. The mean biomass production increased significantly especially in field plots for 5 cm cuts compared to those at 3 cm and the control treatment. In addition, transpiration was highly increased in response to the VPD increase. Shoot cuts significantly reduced transpiration, whatever the level, largely because they reduced leaf surface. We concluded that moderate grazing (cuts to 5cm) can improve biomass production and allow better adaptation to water deficit as they significantly reduced water loss through transpiration. The study recommends the cropping of the Cenchrus biflorus as climate solution as it performs better under water deficit for improving grazing resilience in Niger.

Infection with the defective hepatitis D virus (HDV) co-infects or superinfects a patient carrying hepatitis B virus (HBV). Delta virus infection is relatively common. The seriousness of infection with the hepatitis D virus (HDV), a virus defective in a patient carrying the hepatitis B virus (HBV), lies in its aggravating effect on the associated viral hepatitis B. Our aim was to study the serological profile of HDV in HBsAg-positive patients. As this study was fragmentary, we initiated this study with the aim of investigating the serological profile of HDV in HBsAg-positive patients. This was a descriptive and analytical cross-sectional study that ran from March 2019 to February 2020, a 12-month period. It focused on the population of patients seen and screened positive for HBsAg. We obtained a frequency of 10%. The mean age of our patients was 41.8 ± 13.09 years. The sex ratio was 4.5. VHD RNA was detected in 50% of patients. In 100% of cases, the presence of VHD RNA was associated with advanced fibrosis according to the APRI score, but without any statistically significant link. All HBV-infected patients should be screened for anti-HDV Ac. HDV infection leads to rapid progression to complications of liver disease.

Analysis of Maternal Mortality at the Maradi Maternal and Child Health Center/Niger: About 379 Cases

Introduction: Pregnancy, childbirth, and their consequences continue to be the leading cause of death, illness, and disability among women of reproductive age in developing countries. In Niger, maternal deaths account for about 39% of all deaths of women aged 15-49. Our aim was to identify the factors linked to maternal mortality in order to contribute to its reduction. Method: this is a descriptive retrospective study of 379 cases of maternal deaths collected at the Maradi CSME from January 1, 2018, to December 31, ***: Our study’s maternal mortality ratio is 2645.72 per 100,000 live births. The mean age of our patients was 27.46 years, with extremes ranging from 15 to 47 years. The 20-24 age group was the most represented, with 88 cases (23.2%). Housewives were 361 (95.3%), and 334 patients (88.1%) were uneducated. There were 261 of them from rural areas, i.e. 69%. The mean parity in our study was 3.46, with extremes ranging from 0 to 15. Multiparous patients 117 cases (30.9%) represented the most significant proportion. Only 42 patients (11.08%) had performed four or more antenatal consultations. Direct obstetric causes accounted for 56.73% of the causes of maternal death. Anemia was the leading cause of maternal death (31.66%). Nearly half of the patients had died before 24 hours after admission, i.e. 49.86%. Conclusion: Maternal mortality remains a significant challenge for developing countries, given its magnitude and timid decline. The fight to reduce maternal mortality must be the priority of our countries’ health policies.

Acute Leukemia in Niger: Epidemiological, Diagnostic and Therapeutic Aspects

Objective: Improve the care of patients followed for acute leukemia in the Oncohematology department of the National Hospital of Niamey. Methods: This was a prospective study, over a period of 2 years from January 1, 2018 to December 31, 2019, in patients with acute leukemia in the Oncohematology department of the National Hospital of Niamey (HNN), whose diagnosis was made on a blood smear associated with a myelogram and immunophenotyping and who were consenting. Results: We collected 25 cases of acute leukemia confirmed by myelogram and immunophenotyping. The mean age of the patients was 31.32 years, with a predominance of women, a sex ratio of 0.92. Pupils and students were in the majority with 40% and most came from the Niamey region, i.e. 68%. Anemic syndrome was the most common clinical sign in 96%. ALL predominated in 64% of cases. On the blood count, the hyperleukocytosis was more marked in AML (mean white count: 197256.6 elts/mm3) than in ALL (137891.6 elts/mm3), it was the same for thrombocytopenia which is more marked in AML (75588.89/mm3) than in ALL (52156.25/mm3). Therapeutically, 52% of patients received chemotherapy. The mean overall survival was 16.223 ± 3.191 months, including a mean survival for AML of 6.853 ± 1200 months compared to 21.720 ± 5.920 months for ALL. Conclusion: Acute leukemia still remains a major problem in our context, due to the precariousness of limited financial, diagnostic and therapeutic resources. Thus reflecting in our results, the increasing number of cases, the diagnostic delay and the guarded prognosis. This is the reality in several other countries in the sub-region and even in certain developed countries.

Wasteland and Empty Plots Trivialization in the Maradi City (Niger): Between Environmental Degradation and Consequent Nuisance, Quality of life and Sustainability of Biodiversity

The trivialization of empty spaces has long been practiced in the city of Maradi (Niger), particularly in the Zaria 2 (Commune II) and Ali Dan Sofo (Commune III) districts. With stalls in the streets and illegal dumping grounds, empty plots and the surrounding areas are fairly privileged sources of various wastes. This study aims at analyzing the impact of these empty plots on the local population, through several aspects, especially in terms of the modes of occupation, internal and surrounding practices, physical environment and biodiversity, sanitation and elimination of waste. To do so, two methodological approaches were used: a survey on local population and in situ observation of these spaces. The results not only reveal a difficult proximity situation to the unbuilt but also showed that empty plots constitute not only a form of vegetation conservation for the future as well as clean surfaces which would be called upon to characterize the potential recharge of groundwater. The most potentially useful organic waste would be that of Kadro, especially feathers from poultry that could be used as compost in the fields. In addition to the degradation of urban environments (63% of respondents), the unbuilt area also contributes to poverty reduction and increases food security (69% of respondents). To reduce the negative impacts, the rewarding strategies would be awareness-raising and monitoring measures coupled with a policy of protection and environmental remedies.

Non-Malformative Emergency Abdominal Surgery in Children Aged 0 - 5 Years

Childhood abdominal surgery emergencies are a major challenge and problematic for the surgeon. The objective is to assess the pattern of non-malformative emergency abdominal surgery in children under 5 years old and to evaluate their management. Patients and Methods: A retrospective study of children under five years of age operated between January 2015 and December 2019 who presented with non-malformative abdominal surgical emergency at the Niamey National Hospital. Results: We collected 327 patients aged 0 to 5 years who underwent surgery for abdominal emergency. The average age was 2.8 ± 0.7 years. Boys accounted for 70.64% (n = 231) of the cases. Non-traumatic emergencies accounted for 97% (n = 317) of the cases. They were dominated by peritonitis in 44.6% (n = 146) and strangulated hernia in 43.7% (n = 143). Peritonitis was attributed to ileal typhoid perforation in 85.61% (n = 125) cases. The strangulated hernias were umbilical at 87.41% (n = 125) and inguinal at 12.59% (n = 18). Abdominal trauma accounted for 3% (n = 10);including 6 cases of abdominal contusion and 4 cases of penetrating wound. Intestinal resection with or without stoma was performed in 28.44% (n = 93). Postoperative complications were observed in 8.5% (n = 28) of the cases and mortality was 5.5% (n = 18). Conclusion: Non-malformative emergency abdominal surgery for children under 5 years was dominated by peritonitis. The morbidity and mortality are high. Fight against disease due to dirty hands and fecal peril like such as typhoid fever will reduce their frequency, but also improve the prognosis.

Forest and landscape restoration (FLR) practices have been reported to improve soil organic carbon stocks (SOCs) and contributing to climate change mitigation. This study aims to evaluate the impact of combined FLR practices, mainly developed in semiarid regions, on SOCs. The SOCs, soil texture, bulk density (ρ), pH, CO2 emissions, and herbaceous biomass were determined at a 0 - 30 cm depth. The experimental design comprised degraded land without FLR practices and three sets of combined FLR practices. These practices included “zaï” + stone bunds + organic manure + assisted natural regeneration (ANR) used to convert degraded land into forest (GF) and cropland (PARL);“zaï” + stone bunds + crop rotation + crop/fallow successions + ANR used to convert degraded land into cropland (OARL) and “zaï”+ stone bunds + organic manure used to convert degraded land into cropland (KARL). SOCs were highest (20.02 t C ha−1) under OARL compared with the other combinations of FLR practices. SOCs increased by 99% (+0.2 t C ha−1⋅yr−1), 58% (+0.3 t C ha−1⋅yr−1) and 13% (+0.2 t C ha−1⋅yr−1) under GF, OARL and KARL, respectively, and decreased by 15% (−0.1 t C ha−1⋅yr−1) under PARL. This study provides additional information explaining SOC variation in restored degraded land through the implementation of a combination of FLR practices. This is useful for recommending the combination “zaï” + stone bunds + crop rotation + crop/fallow successions + ANR to improve SOCs in the semiarid agroecosystem.

Background: An Enterocutaneous fistula (ECF) is an abnormal connection between the intestinal tracts or stomach and the skin. They are a major catastrophe to the patient and surgeons and still have a high incidence of morbidity and mortality which varies between 6% - 33%;their management remains a big challenge. These patients frequently face complications, and a well-organized multidisciplinary approach must be implemented in their management to improve outcomes. Objectives: Our study aims to assess the prevalence, management and outcomes of enterocutaneous fistulas in Buea regional hospital and Laquintinie hospital of Douala over the past 5 years. Methods: This was a hospital-based retrospective study in Buea regional hospital and Laquintinie hospital of Douala. Records of patients who had enterocutaneous fistulas within the period of 1st January 2017 to 31st December 2020 in the surgical departments. Data included demographics, pre-operative diagnosis, comorbidities, type of fistula, management modality and means, the indication of operative treatment, length of stay in the hospital and outcomes. Data was analysed using SPSSv26. Results: The study constituted 1343 medical records of which 83 medical records of patients with enterocutaneous fistulas, giving a prevalence of 6.2%, female predominance at 59% (n = 49), 42.2% (n = 35) were referred cases from the periphery for better management. A vast majority (96.4%) occurred as post-operative complications with appendectomy the most common indication (18.8%). High output fistulas were predominant (43.4%). 59% (n = 47) were managed medically, 6% (n = 5) received both conservative and surgical modalities while 35% (n = 5) were managed surgically. 64.1% (n = 50) were placed on enteral nutrition while 35.9% (n = 28) were placed on parenteral nutrition. Background: Anastatica hierochuntica L. (Brassicaceae) has been claimed to treat various conditions, including complicated childbirth. The principal objective of this research was to provide scientific evidence on the contractile effect of the aqueous and hydroethanolic extracts of the whole plant from A. hierochuntica during labor. Method: Qualitative and quantitative methods helped identify and quantify the phytoconstituents in both extracts. The antioxidant activity was studied using four standard methods (ABTS, DPPH, FRAP, and LPO). Results: The inhibitory power of the extracts on pro-inflammatory enzymes such as 15-lipoxygenase and phospholipase A2 was determined. The contractile effects of the extracts were evaluated using isolated pregnant mice uteri. Steroids, triterpenoids, flavonoids, tannins, coumarins, and reduced compounds were identified in both extracts. The hydroethanolic extract exhibited higher content of hydrolyzable tannins (15.07 ± 0.24 mg tannic acid equivalent/g) than the aqueous decoction (11.8 ± 0.69 mg TAE/g). The extracts demonstrated the ability to scavenge DPPH and ABTS radicals, reduce ferric ions, and inhibit especially lipid peroxidation. No significant difference was noted in the phospholipase inhibition between the extracts and betamethasone. The hydroethanolic extract displayed the most significant anti-lipoxygenase activity with an IC50 value of 55.82 ± 1.15 μg/mL. The lethal dose (LD50) of extracts was estimated to be superior to 2000 mg/kg body weight (bw). The ability of the samples to produce one cm amplitude of contraction at 50% effective concentrations (EC50) was 5.33 × 10−8 ± 0.32 mg/mL for oxytocin, 1.51 ± 0.08 mg/mL for aqueous extract and 3.57 ± 0.61 mg/mL for hydroethanolic extract. Conclusion: These findings could provide evidence for using Anastatica hierochuntica to facilitate childbirth.
