Solitary Osteogenic Exostosis of the Femur Revealed by Recurring Acute Distal Thigh Pain: About a Case

Introduction: Solitary exostoses are the most common benign tumors of the fertile metaphyses of the long bones of children. Their radiological diagnosis of metaphyseal bone growth must be confirmed on pathological examination. These tumors can remain asymptomatic for a long time and reveal themselves during a particularly vascular complication. The objective of this study was to describe the epidemiological, diagnostic, therapeutic and evolutionary aspects of these tumors. Patient and Observation: We report the case of a 15-year-old adolescent girl, with no particular pathological history, received in the pediatric surgery department of the Donka National Hospital (HND) of the Conakry University Hospital for recurrent acute painful swelling of the lower third of the left thigh in an afebrile context accompanied by lameness and stopping school for a few days (2 - 3 days). The symptoms appear to have evolved over the past 3 years and after physical activities. It regresses with rest, analgesics and non-steroidal anti-inflammatory drugs. The notion of trauma and sickle cell disease was not reported in the patient's clinical history. It is the persistence of the symptomatology which motivates the said consultation. On palpation, a small hard mass is noted at the expense of the internal metaphysis of the left distal femur. Deep palpation of this area causes a tingling sensation and during rapid mobilization of the knee. The remainder of the orthopedic examination was unremarkable. Standard x-ray of the femur shows a bony growth with a pointed tip from the distal metaphysis of the left femur. On surgical exploration, we noted a wedge-shaped exostosis oriented towards the vastus medialis muscle. Histological examination of the surgical specimen confirms osteogenic exostosis. There is no recurrence after 2 years. Conclusion: The distal femoral metaphysis is the most common location of solitary osteochondromas in children. Blood transfusion saves lives and reduces morbidity and mortality for a large number of diseases and clinical conditions, but it is not without danger. The aim of this study was to determine the seroprevalence of HIV and hepatitis B in blood donors received at the regional Blood Transfusion Centre of N’Zérékoré (Guinea). This was a 5-year retrospective analytical study. We included records of blood donors aged 18 to 60 years admitted to the N’Zérékoré Regional Blood Transfusion Centre for blood donation from January 2016 to December 2020. We performed a descriptive analysis followed by Chi-2 or Fish-er-exact tests and the Student or Wilcoxon test, followed by multivariate logistic regression. In this study, donor age ranged from 18 - 60 years, with a pre-dominance of donors aged 25 - 34 (44.2%). Male donors were the most represented in our study (79.0% versus 21.0% female). More than half of the donors were blood group O (55.6%). We observed a seroprevalence of 3.6% for HIV, 13.4% for HBsAg and 0.2% for co-infection. In our series, age 25 - 34 (OR = 1.89 and P = 0.001) and 35 - 44 for HIV (OR = 2.01 and P = 0.001), HBsAgserostatus (OR = 3.04 and P = 0.001) and blood donation history (OR of 3.04 and P = 0.001) were factors associated with HIV positivity (P < 0.05). In our study, HIV serostatus (OR = 3.04 and P = 0.001) and blood donation history (OR = 0.01 and P = 0.001) were factors associated with HBsAgseropositivity. We reported a high prevalence of HIV and HBsAg. Sex, serological status and blood donation history were associated factors.

Cenchrus biflorus called Karangiya in the Hausa language is an annual pastoral grass which is a valuable herbaceous fodder in dry land region in the context of climate change. However, little is known about the plant water use under the effects of cut in West Africa Sahel like Niger where the plant is a multipurpose grass species. Therefore, this study investigated the impact of grazing (simulated by shoot cuts) on biomass production. Cenchrus biflorus Roxb was grown on field plots and in pots and subjected to shoot cuts at different levels (3 cm and 5 cm from soil surface). The effect of shoot cuts on drought tolerance was evaluated by assessing the response of transpiration to the diurnal variation of vapor pressure deficit (VPD). Results showed that the biomass production varied in response to shoot cuts depending on the culture system, and the level or frequency of cuts. The mean biomass production increased significantly especially in field plots for 5 cm cuts compared to those at 3 cm and the control treatment. In addition, transpiration was highly increased in response to the VPD increase. Shoot cuts significantly reduced transpiration, whatever the level, largely because they reduced leaf surface. We concluded that moderate grazing (cuts to 5cm) can improve biomass production and allow better adaptation to water deficit as they significantly reduced water loss through transpiration. The study recommends the cropping of the Cenchrus biflorus as climate solution as it performs better under water deficit for improving grazing resilience in Niger.

Measuring Cognitive and Psychosocial Accessibility to Modern Contraception: A Comprehensive Framework

Nowadays, more than ever, the improvement of access to family planning (FP) has become an international goal. What constitutes access to FP? Current human rights-based contraceptive guidelines indicate that access begins as soon as women or couples express a desire to avoid pregnancy and their risk of unintended pregnancy is established. However, few studies have sought to define and measure cognitive and psychosocial access to contraception. To propose a comprehensive framework for the cognitive and psychosocial accessibility of contraception, we critically analyzed the literature on attitudes toward FP. The main dimensions that emerged were knowledge about FP, fear of side effects, approval of contraception, and contraceptive agency. We then identified and adjusted some questions that can capture these dimensions more comprehensively. As a result, we developed a questionnaire module comprising 15 questions, which was integrated into the 6th round of the PMA2020 survey in Burkina Faso in 2019. This research highlighted that previous studies have collected separate dimensions of contraceptive access, and the psychosocial dimension tended to be neglected. Our results demonstrate that it is possible to collect comprehensive data on cognitive and psychosocial dimensions of access to family planning.

This work consists of evaluating the quality of the mechanical parameters of large-diameter steels, i.e. 20, 25, 28 and 32, through a process of recycling scrap metal that fills garages, rubbish dumps, gutters and other abandoned sites, as well as imported concrete reinforcing steel sold in the Republic of Guinea. To carry out this important work, a number of mechanical tensile and bending tests and a microscopic analysis combining two devices, an electron microscope and a photographic camera, were carried out. The samples were taken from sampling areas in the major communes of Conakry, namely: Casse Sonfonia, Matoto and Kagbélen. The tensile strength values of the large dimensions 20, 25, 28 and 32 are given in the tables.

The behavior of beams with variable stiffness subjected to the action of variable loadings (impulse or harmonic) is analyzed in this paper using the successive approximation method. This successive approximation method is a technique for numerical integration of partial differential equations involving both the space and time, with well-known initial conditions on time and boundary conditions on the space. This technique, although having been applied to beams with constant stiffness, is new for the case of beams with variable stiffness, and it aims to use a quadratic parabola (in time) to approximate the solutions of the differential equations of dynamics. The spatial part is studied using the successive approximation method of the partial differential equations obtained, in order to transform them into a system of time-dependent ordinary differential equations. Thus, the integration algorithm using this technique is established and applied to examples of beams with variable stiffness, under variable loading, and with the different cases of supports chosen in the literature. We have thus calculated the cases of beams with constant or variable rigidity with articulated or embedded supports, subjected to the action of an instantaneous impulse and harmonic loads distributed over its entire length. In order to justify the robustness of the successive approximation method considered in this work, an example of an articulated beam with constant stiffness subjected to a distributed harmonic load was calculated analytically, and the results obtained compared to those found numerically for various steps (spatial h and temporal τ ¯ ) of calculus, and the difference between the values obtained by the two methods was small. For example for ( h=1/8 , τ ¯ =1/ 64 ), the difference between these values is 17%.

Mapping Climate Services for Disaster Risk Management: A Systematic Review and Research Gaps from a Policy Process Perspective

Climate services (CS) are crucial for mitigating and managing the impacts and risks associated with climate-induced disasters. While evidence over the past decade underscores their effectiveness across various domains, particularly agriculture, to maximize their potential, it is crucial to identify emerging priority areas and existing research gaps for future research agendas. As a contribution to this effort, this paper employs the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) methodology to review the state-of-the-art in the field of climate services for disaster risk management. A comprehensive search across five literature databases combined with a snowball search method using ResearchRabbit was conducted and yielded 242 peer-reviewed articles, book sections, and reports over 2013-2023 after the screening process. The analysis revealed flood, drought, and food insecurity as major climate-related disasters addressed in the reviewed literature. Major climate services addressed included early warning systems, (sub)seasonal forecasts and impact-based warnings. Grounded in the policy processes’ theoretical perspective, the main focus identified and discussed three prevailing policy-oriented priority areas: 1) development of climate services, 2) use-adoption-uptake, and 3) evaluation of climate services. In response to the limitations of the prevalent supply-driven and top-down approach to climate services promotion, co-production emerges as a cross-cutting critical aspect of the identified priority areas. Despite the extensive research in the field, more attention is needed, particularly pronounced in the science-policy interface perspective, which in practice bridges scientific knowledge and policy decisions for effective policy processes. Trends of Land Use and Land Cover Change in the Savannah Ecological of the Protected Area Reserve Partielle de Dosso, Niger

Information on the dynamics of savannah is important to a country's plan to overcome the problems of uncontrolled development and environmental hazards. Taking the reserve partielle de Dosso, Niger as the case study area, this paper analyzed the long-term land use land cover change from 2002 to 2022. Satellite images were processed by using Google Earth Engine (GEE). Therefore, four major land cover classes were identified based on spectral characteristics of Land sat, namely, built-up, vegetation, cropland, bare land and water. The result revealed that barren and built-up areas increased at the expense of vegetation and water. From the four major land use land cover the large area is covered by vegetation which comprises about 192963.5 hectares followed by cropland and water consisting of 32506.43 and 1596.4 hectares respectively. The built-up area gained substantial area (most) during the study period. The reduction in some of the land cover/uses underlines the dangerous trend of the pressure poised by population growth and the changing functionality. Land cover change is influenced by a variety of societal factors operating on several spatial and temporal levels. The area estimates and spatial distributions of the LULC classes produced from the current study will assist local authorities, managers, and other stakeholders in decision-making and planning regarding forest land cover and uses.

Characteristics of Gold and Its Mineralization Style in the Boulon Djounga Eastern Perimeter of Liptako Mining Company (Central Southwestern Niger)

The Boulon Djounga eastern perimeter is part of the Tiawa operating permit of the Société des Mines du Liptako (SML), located in the central southwestern part of Liptako (Niger). In this study, we used field data, Reverse Circulation (RC) surveys and chemical analyzes of gold to determine the characteristics of gold and its mineralization style. The eastern perimeter of Boulon Djounga is represented by a succession of metabasalts and metasediments both intersected by intrusions of quartz and dolerite dykes, and covered by sandstone and clayey rocks. Gold is present in low contents (0.00 - 0.30 ppm) in the sedimentary cover and in medium (0.30 - 1.00 ppm) or high contents (1.00 - 4.534 ppm) in the metasediments, and in the gray quartz veins and locally in the volcanics. It exists in a disseminated state or in a concentrated state in the surrounding areas in the form of discrete grains associated with sulphurous minerals (pyrite: FeS2, chalcopyrite: CuFeS2 or arsenopyrite: FeAsS). The presence of gold in the quartz veins, and the NE-SW and NW-SE orientations of the ore bodies suggest that the eastern Boulon Djounga gold mineralization would be established during a late magmatic extensive phase.

The monitoring of our watercourses for their preservation must be an imperative concern of the whole *** aim of this study is to describe the main causes of degradation and destruction of the natural resources of the Mamouwol River in the Mamou *** were collected using a survey method(questionnaire,interviews and observations).The study revealed that extensive agro-pastoral activities are the main socio-economic activity of the population,accounting for 78.5%-90%and 20.30%*** follows(1.2%).Related activities include handicrafts and petty trade;Other human activities:hunting(11%),charcoal burning(58%),brick making and firing(45.6%).Riverbanks and water resources in the vicinity of dwellings are damaged and polluted by socio-economic activities and the use of agricultural *** absence of industrial units on the path for the data collection.
