Unraveling the Impact of Direct-Acting Antivirals on Hepatitis-Linked Cirrhosis: A Comprehensive Analysis of Fibrosis, Child Score, and Disease Progression

The treatment of hepatitis C has undergone a significant boom since the advent of direct acting antivirals (DAA). Indeed, the interferon-ribavirin combination that has been used to treat hepatitis C has a virological response in only 45% of cases with significant side effects. The advent of direct-acting antivirals has changed the prognosis of cirrhotic patients with hepatitis C. DAAs have ensured a sustained viral response in the majority of patients. Our work aims to see the evolution of hepatitis C patients at the cirrhosis stage under DAA. We conducted a retrospective study over 15 years (January 2009, January 2024) including all patients with post-viral cirrhosis C, whom we divided into two groups: group A, cirrhotic patients who received ribavirin and interferon, and group B, patients on DAA. From January 2009 to January 2024, we conducted a study of 182 patients with viral hepatitis C, including 102 cirrhotic patients. The mean age was 55 years. 66% of patients were initially treated with the ribavirin interferon combination, while 34% received direct-acting antivirals (DAAs). Since the introduction of DAAs, the most commonly used regimens have been sofosbuvir/daclatasvir with or without ribavirin and sofosbuvir/ledipasvir with or without ribavirin. Group A achieved sustained virological response (SVR) in 60% of cases, with notable side effects. In Group B, SVR was 98.18%, with improved tolerability and fewer side effects than previous treatments. Fifteen patients developed hepatocellular carcinoma (HCC), with a significantly lower mortality rate in those treated with DAAs compared with pegylated dual therapy (p: 0.001).

Hypoxic hepatitis, also known as ischemic hepatitis, is characterized by acute hepatocellular injury due to inadequate oxygen delivery to the liver. Celiac trunk stenosis can lead to hepatic ischemia and subsequent liver damage. We present the case of an 81-year-old patient with a history of hypertension, ischemic heart disease, hypothyroidism, and biliary lithiasis, who developed hypoxic hepatitis secondary to Dunbar syndrome and a stenosis of the superior mesenteric artery. The patient improved symptoms and liver function tests with conservative management, including intravenous fluids and supportive care. Long-term management involved continued antiplatelet therapy and statins, with consideration of further interventions for celiac trunk stenosis. This case underscores the importance of recognizing Dunbar syndrome as well as superior mesentery trunk stenosis as a potential cause of hypoxic hepatitis. It highlights the need for multidisciplinary management in such cases.

Optimizing wind farm layout for enhanced electricity extraction using a new hybrid PSO-ANN method

With the growing need for renewable energy,wind farms are playing an important role in generating clean power from wind *** best wind turbine architecture in a wind farm has a major influence on the energy extraction *** paper describes a unique strategy for optimizing wind turbine locations on a wind farm that combines the capabilities of particle swarm optimization(PSO)and artificial neural networks(ANNs).The PSO method was used to explore the solution space and develop preliminary turbine layouts,and the ANN model was used to fine-tune the placements based on the predicted energy *** proposed hybrid technique seeks to increase energy output while considering site-specific wind patterns and topographical *** efficacy and superiority of the hybrid PSO-ANN methodology are proved through comprehensive simulations and comparisons with existing approaches,giving exciting prospects for developing more efficient and sustainable wind *** integration of ANNs and PSO in our methodology is of paramount importance because it leverages the complementary strengths of both ***,this novel methodology harnesses historical data through ANNs to identify optimal turbine positions that align with the wind speed and direction and enhance energy extraction efficiency.A notable increase in power generation is observed across various *** percentage increase in the power generation ranged from approximately 7.7%to 11.1%.Owing to its versatility and adaptability to site-specific conditions,the hybrid model offers promising prospects for advancing the field of wind farm layout optimization and contributing to a greener and more sustainable energy future.

Kostmann’s Neutropenia: A Case Report

Kostmann syndrome, or severe congenital neutropenia, is a rare condition in children marked by a neutrophil count of less than 500/mm3. This congenital agranulocytosis, an autosomal recessive genetic disorder, is often first identified by a neonatal infectious syndrome. The deficiency in neutrophils increases susceptibility to bacterial and fungal infections. Prior to the availability of hematopoietic growth factors, the disease was associated with significant morbidity and early mortality. We present the case of a 17-month-old boy who was admitted to the pediatric emergency department at Hassan II University Hospital in Fes with skin abscesses.

Characterisation of sediments polluted by acid mine drainage in the northeast of Algeria

The present study investigates the composition and the mineralogy of sediments in the Essouk River, located in the Northeast of Algeria in Sidi Kamber mining zone at approximately 2km upstream Guenitra *** mine is abandoned since 1976,but acidic drainage water is still a source of river *** X-Ray diffraction(XRD),scanning electron microscopy(SEM) with energy dispersive analyses(EDX),X -Fluorescence analyses and infrared spectroscopy analyses were used to characterize surface sediments sampled at various sites in the area near the mining effluents. The obtained results show that the composition of the sediments is mainly dominated by iron and *** the vicinity of the mine,jarosite and schwertmannite are the essential precipitates. Downstream,sediments are formed by various phases of iron oxyhydroxydes with the presence of adsorbed sulphate.

Paracetamol is a non-steroidal, anti-inflammatory drug widely used in pharmaceutical applications for its sturdy, antipyretic and analgesic action. However, an overdose of paracetamol can cause fulminant hepatic necrosis and other toxic effects. Thus, the development of advantageous analytical tools to detect and determine paracetamol is required. Due to simplicity, higher sensitivity and selectivity as well as costefficiency, electrochemical sensors were fully investigated in last decades. This review describes the advancements made in the development of electrochemical sensors for the paracetamol detection and quantification in pharmaceutical and biological samples. The progress made in electrochemical sensors for the selective detection of paracetamol in the last 10 years was examined, with a special focus on highly innovative features introduced by nanotechnology. As the literature is rather extensive, we tried to simplify this work by summarizing and grouping electrochemical sensors according to the by which manner their substrates were chemically modified and the analytical performances obtained.

Genome-wide association studies （GWAS） have successfully identified a substantial number of risk variants for prostate cancer [1-10]. In total, more than 20 single nucleotide polymorphisms （SNPs） that predispose to prostate malignancy have been found scattered throughout the genome in coding and non-coding regions. The 8q24 chromosomal region is an intriguing locus as it harbors five independent susceptibility variants （rs10086908; rs16901979; rs620861; rs6983267; rs1447295） that span a 440-kb genomic interval devoid of genes or protein-coding transcripts.

Chronic Viral Hepatitis C: Before and after Direct Acting Antivirals (DAA) in Morocco

For a long time, a combination of interferon and ribavirin has been used to treat viral hepatitis C, but the sustained virological response was only achieved in 45% of cases and side effects were serious [1]. Direct acting antivirals (DAA) have provided a cure for almost everyone with hepatitis C, with few side effects. The Purpose of Our Work is to compare the results of treatment for viral hepatitis C before and after DAA. Patients and Methods: This is a retrospective study, bringing together all patients with chronic viral hepatitis C treated between January 2009 and March 2020 at the University Hospital Hassan II in Fez, Morocco. The epidemiological, clinical, biological, virological characteristics of the included patients were collected from the two groups: A, treated with interferon and ribavirin or by triple therapy and B, treated with DAA. Results: 162 patients were included, the average age was 55 y/o, with 90 women and 72 men. 88 patients (54.3%) were already cirrhotic, of which 61 were compensated and 27 were decompensated. Genotype 1 was dominant with a frequency of 71.6%, 107 patients (66%) initially treated with old HCV treatments and 55 (34%) treated with DAA. Sustained viral response was obtained in 59 cases (55.14%) in group A versus 54 cases (98.18%) in group B with a very significant difference (p < 0.0001). Treatment failure was observed in 14 patients (13.1%) in group A and only one patient, i.e. 2% in group B (p = 0.019). 14 patients relapsed in group A (13.1%) versus 0 patient in group B (p = 0.003). The tolerance of the treatment was excellent in group B as a whole with only five patients (9%) reported side effects which were minor, not leading to the discontinuation of treatment while the side effects were major in 49 patients (45.7%) in group A with led to the permanent discontinuation of treatment in 6 patients.
