Tailored pancreatic reconstruction after pancreaticoduodenectomy: a single-center experience of 892 cases

BACKGROUND: Pancreatic reconstruction following pancreaticoduodenectomy(PD) is still debatable even for pancreatic surgeons. Ideally, pancreatic reconstruction after PD should reduce the risk of postoperative pancreatic fistula(POPF) and its severity if developed with preservation of both exocrine and endocrine pancreatic functions. It must be tailored to control the morbidity linked to the type of *** study was to show the best type of pancreatic reconstruction according to the characters of pancreatic stump. METHODS: We studied all patients who underwent PD in our center from January 1993 to December 2015. Patients were categorized into three groups depending on the presence of risk factors of postoperative complications: low-risk group(absent risk factor), moderate-risk group(presence of one risk factor) and high-risk group(presence of two or more risk factors). RESULTS: A total of 892 patients underwent PD for resection of periampullary tumor. BMI >25 kg/m~2, cirrhotic liver, soft pancreas, pancreatic duct diameter <3 mm, and pancreatic duct location from posterior edge <3 mm are risk variables for development of postoperative complications. POPF developed in 128(14.3%) patients. Delayed gastric emptying occurred in 164(18.4%) patients, biliary leakage developed in 65(7.3%) and pancreatitis presented in 20(2.2%). POPF in low-, moderate-and high-risk groups were 26(8.3%), 65(15.7%) and 37(22.7%) patients, respectively. Postoperative morbidity and mortality were significantly lower with pancreaticogastrostomy(PG) in high-risk group, while pancreaticojejunostomy(PJ) decreases incidence of postoperative steatorrhea in all groups. CONCLUSIONS: Selection of proper pancreatic reconstruction according to the risk factors of patients may reduce POPF and postoperative complications and mortality. PG is superior to PJ as regards short-term outcomes in high-risk group but PJ provides better pancreatic function in all groups and therefore, PJ is superior in l

Autunite-Group Minerals and Their Paragenesis from the Sheared Granite of Gabal El Sela, South Eastern Desert, Egypt

G. El Sela is located in the Southern Eastern Desert of Egypt cropping as two parts, occupied by monzogranites that were categorized as biotite granite, muscovite granite and two mica granites. The northern part is more significant according its high concentrations of uranium that controlled by complicated structure regime;faulting, infrastructures and shearing are the most common structural criteria of this part. The Egyptian Nuclear Materials Authority (NMA) mined this part to produce the uranium ore. The previous mineralogical studies indicated that this granite was dominated by primary uranium minerals (pitchblende and coffinite) and secondary minerals belong to the autunite group (autunite, metautunite, phurcalite) in addition to uranophane. In the present work, petrographic and mineralogical studies are applied for the granites using the polarized and stereo microscopes and followed by electron microscope and XRD. The result of the microscopic examinations revealed the tectonic regime controlling the radioactivity and recognized the sodic autunite (meta-natroautunite) beside the pre-mentioned autunite group minerals completing the paragenetic sequrnce of these minerals. In this study, it is concluded that the sheared biotite granite is monzogranite originated during the episode of the continental plate collision (syncollision). The study finished to presence of two main types of the alteration corresponding to the two high levels of radioactivity (moderate and anomalous). The first is the thermal alteration (saussiritization, sericitization, kaolinization, silicification and hematization) and the second is the chemical transformation (oxidation, dehydration, ion substitutions and confusion) responsible for formation of the secondary uranium minerals. The temperature needed for the thermal alteration is sourced by the hydrothermal solutions, while the temperature needed for the uranium minerals transformation may be generated during the episode of the continent

Petrology, geochemistry, radioactivity, and M–W type rare earth element tetrads of El Sela altered granites, south eastern desert,Egypt

The southern part of the Eastern Desert of Egypt—about 30 km southwest of abu-Ramad city—is mainly covered by ophiolitic rocks(Sul Hamed), island arc assemblage, younger granites(muscovite granites of Qash Amer and two mica granites of El Sela), and various acidic and basic dikes. Field, petrological, and geochemical studies indicate that the El Sela shear zone has been subject to hydrothermal and supergene alteration such as kaolinization, albitization, sericitization, and hematitization. It is invaded by quartz ENE–WSW veins associated with hydrothermal alteration accompanied by radioactive mineralization. The investigated younger granitic rocks had very low contents of compatible elements, such as Cr, Ni,and Co; and high contents of incompatible elements, such as Zr, and large ion lithophiles, such as Sr, especially in the El Sela shear zone. Major oxide and trace element analyses revealed calc-alkaline affinity and peraluminous *** highly differentiated granitic rocks' lower Zr/Hf and higher Y/Ho than the normal ratio are consistent with hydrothermal alteration. Most samples had rare earth element(REE)-patterns with an M-type tetrad effect in the first and fourth segments and a W-type tetrad in the third segment. The average ∑REE in the studied granites was lower than the world granite average; the ratio of light to heavy REEs greater. The main radioactive, uranium-bearing, and uraniferous Fe and Mn minerals are uranothorite,autunite, uranophane and autunite as compounds, kasolite,columbite, xenotime, uranophane-bearing zircon and jarosite, silver-bearing pyrite, hematite, and autunite-bearing pyrolusite.

Geochemistry, mineralogy, and radioactivity of the abu Furad Area, Central Eastern Desert, Egypt

abu Furad is in the Central Eastern Desert,Egypt. It comprises metasediments, metavolcanics,metagabbros, syn-to late-orogenic granites, and post-orogenic granites, in addition to numerous dykes and veins of different shapes and composition invading all the older rocks cropping out in the study area. Field, petrographic,mineralogical, and chemical investigations led to the classification of abu Furad granites as quartz diorite, tonalite,granodiorite, and *** oxide and trace element data revealed that the syn-to late-orogenic granites and post-orogenic granites are peraluminous. Syn-to lateorogenic granites originated from calc-alkaline volcanic arc–related magma; while the post-orogenic granites(syenogranite) are highly fractionated, calc-alkaline granite from a within plate regime. Radiometrically, the studied quartz diorite, tonalite, and granodiorite had lower uranium and thorium contents and higher eU/eTh ratios than the syenogranites. This may indicate that the syn-to late-orogenic rocks originated from sources depleted in these elements. The average e Th/eU ratio of syenogranites was higher than that of average continental crust, suggesting that the syenogranites are relatively depleted in U. The studied altered syenogranites were strongly enriched in U and Th compared to the Earth's crust. On the other hand, the average of e U in pegmatites is lower than the global average for uraniferous pegmatites. The most recorded accessory minerals in the altered syenogranites were thorite, fergusonite, samarskite, columbite, zircon, monazite, xenotime,apatite, fluorite, sphene, atacamite, and malachite, in addition to chromium and nickel inclusions.

Seroprevalence of celiac disease among United Arab Emirates healthy adult nationals: A gender disparity

AIM:To determine celiac disease(CD)prevalence and associated manifestations or risk factors in healthy adult ***:It is a cross-sectional prospective study,recruiting 1197(573 women and 624 men)healthy Emiratis between September 2007 and April 2008among those who went to Al Ain Hospital to undertake the prenuptial *** for anti-tissue transglutaminase(tTG)IgA antibodies was used for CD *** with positive results in the anti tTG antibodies assay were also tested for anti-endomysial(EMA)IgA antibodies.A structured interview was used to collect basic demographic and clinical recall data including:information on name,contact address,age,gender,education status,previous diagnosis of CD,diagnosis of CD in 1st degree relatives and history of"chronic diarrhea,anemia,headache,hepatitis,diabetes,tumor,and thyroid disorder".RESULTS:Fourteen blood samples(1.17%;14/1197)were seropositive for *** latent CD seropositive patients were 13 women and 1 man and therefore the seroprevalence of CD was 1:86(14/1197)for adult Emiratis:1:44(13/573)for women and 1:624 for *** logistic regression revealed that history of chronic anemia(crude OR=7.09;95%CI:2.32-21.61;P=0.003)and being a woman(OR=14.46;95%CI:1.89-110.91;P=0.001)were associated with CD ***,the thyroid disorder showed a positive association with CD seropositivity that approach statistical significance(OR=11.30;95%CI:1.32-96.95;P=0.09)and therefore was included in the multiple logistic regression analysis,which showed that CD seropositivity is independently associated only with history of chronic anemia(OR=4.58;95%CI:1.45-14.48;P=0.01)and being a woman person(OR=10.47;95%CI:1.33-82.14;P=0.026).CONCLUSION:Compared to men the CD seroprevalence among women was remarkably *** CD association with women and chronic anemia is of importance from a public health perspective.

Trends and outcomes of pancreaticoduodenectomy for periampullary tumors: A 25-year single-center study of 1000 consecutive cases

AIM To evaluate the evolution, trends in surgical approaches a n d r e c o n s t r u c t i o n t e c h n i q u e s, a n d i m p o r t a n t lessons learned from performing 1000 consecutive pancreaticoduodenectomies(PDs) for periampullary *** This is a retrospective review of the data of all patients who underwent PD for periampullary tumor during the period from January 1993 to April 2017. The data were categorized into three periods, including early period(1993-2002), middle period(2003-2012), and late period(2013-2017).RESULTS The frequency showed PD was increasingly performed after the year 2000. With time, elderly, cirrhotic and obese patients, as well as patients with uncinate process carcinoma and borderline tumor were increasingly selected for PD. The median operative time and postoperative hospital stay decreased significantly over the periods. Hospital mortality declined significantly, from 6.6% to 3.1%. Postoperative complications significantly decreased, from 40% to 27.9%. There was significant decrease in postoperative pancreatic fistula in the second 10 years, from 15% to 12.7%. There was a significant improvement in median survival and overall survival among the *** Surgical results of PD significantly improved, with mortality rate nearly reaching 3%. Pancreatic reconstruction following PD is still debatable. The survival rate was also improved but the rate of recurrence is still high, at 36.9%.

Evaluation of the Expansive Esna Shale and Its Role in the Deterioration of Heritage Buildings at West Bank of Luxor

The heritage buildings at the west bank of Luxor were affected by expansive Esna shale differentially from site to others. The role of expansive Esna shale was evaluated by using X-ray diffraction spectra (particle size, strain), X-ray fluorescence, basic soil properties test, swelling test, soil classification, and unconfined compression test. Three different samples were examined to determine the expansive role in the deterioration of heritage sites. The expansive is high, according to: (a) Soil classification based on plasticity index, (b) Classification chart based on the liquid limit and plastic index, (c) Classification chart based on clay content and activity, (d) Soil classification, and (e) Swelling pressure which determines the safety factor. In addition, expansive classification assesses that Esna shale Formation is one of environmental hazards characterized by damage role near Nile flood and irrigation area and characterized by deterioration role in heritage sites far from Nile flood and irrigation area. This paper refers to evaluation as a main process of integrated conservation approach.
