Evaluation of Different Ultrasonographic Modalities in the Diagnosis of Morbidly Adherent Placenta: A Cross-Sectional Study

Objective: To compare the accuracy of different ultrasonographic modalities;two-dimensional ultrasound (2D-US), color Doppler and three-dimensional power Doppler (3D-PD) in the antenatal diagnosis of the morbidly adherent placenta. Setting: Obstetrics and Gynecology Department, Faculty of Medicine, Assiut University, Assiut, Egypt. Study Design: A cross-sectional study. Methods: All patients fulfill the inclusions criteria: gestational age > 28 weeks, previous one or more cesarean delivery, previous uterine surgery, placenta previa, vitally stable patient and women accepted to participate in the study were included. All patients were evaluated using 2D-US, color Doppler and 3D-PD before delivery. The final diagnosis was established by laparotomy and by histopathology of hysterectomy sample if hysterectomy would be done. Results: One-hundred fifty patients were enrolled in the study. 2D-US has higher sensitivity (86.96%) than 2D color Doppler (84.06%) and 3D-PD (79.71%) in the diagnosis of placenta accreta. On the other hand, 3D-PD has slightly higher specificity (83.95%) than color Doppler (82.72%) and 2D-US (77.78%) in the diagnosis of placenta accreta. The most sensitive parameter in 2D-Us was the loss of retroplacental sonolucent zone (86.96%). As regards color Doppler, the most sensitive parameter was the hypervascularity of the uterine-bladder interface (84.06%). Tortuous vascularity with chaotic branching was the most sensitive parameter in 3D-PD with a sensitivity of 82.61%. Conclusions: The use of 3D power Doppler with both 2D-US and color Doppler as complementary techniques could improve the antenatal diagnosis or exclusion of morbidly adherent placenta.

埃及汉语学习者的文化、社会生活、风俗习惯等存在巨大差异,所以其母语语言系统中的语法、语音、词汇等都会影响对外汉语教学,因此开展“国别化”的研究是必不可少的。本文运用问卷调查手段,调查了埃及人描述自己国家的食物、服装、地名等的常用词汇,统计出了最常用的埃及国别化词汇72个,并翻译成相应的汉语词汇,并结合HSK汉语水平考试词汇大纲和在埃及常用的汉语教材提出了自己关于补充课堂教学内容,有针对性地进行国别化教材编写的建议。

目的：评估SCHWIND AMARIS准分子激光在LASEK术中矫治远视手术中的有效性,安全性和可预测性。方法：回顾性研究LASEK术联合使用丝裂霉素C矫治远视患者33例66眼,平均年龄35.42±1.12岁（范围18~56a）。每位患者予SCHWIND AMARIS准分子激光行LASEK术。术后对其可预测性、安全性、有效性及主观验光情况进行评估,并分析客观验光、裸眼视力、最佳矫正视力及不良反应。结果：平均屈光度为3.2±1.6D（0~7D）,术前及术后平均等效球镜分别为2.34±1.76D（-1.25~7D）及0.30±0.84D（-0.2~0.8D）（P=0.001）。术后6~12mo,平均远视为0.63±0.84D（-1.75~2.76D）。平均散光度为0.68±0.43D（0~2D）,61眼（78.2%）和31眼（39.7%）散光度分别在±1D和±0.50D范围内。安全指数和有效性指数分别为1.08和1.6。结论：应用SCHWIND AMARIS准分子激光联合丝裂霉素C行LASEK术矫治远视具有良好的视觉和屈光结果,而且无严重并发症。

Hepatic GCK is a key enzyme in glucose homeostasis and, as such, is a potential target for treatment strategies of diabetes. We investigated the effect of Persian shallot (Allium hirtifolium Boiss) hydroalchoholic extract on blood glucose level, plasma insulin level, GCK activity and its gene expression. Thirty two male rats were divided into 4 groups of 8, diabetic groups received 100 and 200 mg/kg Persian shallot extract, diabetic control and normal control received 0.9% saline for 30 days. Investigations of gene expression by Real-Time PCR showed that Persian shallot had led to gently increased GCK gene expression in diabetic rats. GCK activity increased significantly in Persian shallot treated group in dose dependent manner (P < 0.05). These results indicated that Persian shallot exhibited a significant potential as a hypoglycemic agent perhaps via its ability to enhance insulin secretion, GCK gene expression and its activity.

Background: Complications linked to postpartum & pregnancy are one of the major causes of female death. 植物病原菌不断危害农业生产和粮食安全。因此，病害发展早期的动态表征对病变监测和显症前诊断至关重要。高光谱成像（HSI）在跟踪病害初始侵染部位的动态进程以进行显症前诊断方面具有巨大潜力。然而，目前尚无相关文献提取出早期感染阶段活体叶片病变组织的指纹光谱特征（FSS），也没有探究HSI的检测机制。其中FSS是指能够表征特定植物病害的独特、有代表性的光谱特征。在本研究中，基于时序HSI数据分析，提取了接种Bipolaris sorokiniana的大麦叶片的FSS，以表征叶斑病症状发展，实现显症前诊断。还研究了叶斑病早期发展阶段叶片的光谱和生化响应。本文所提取的全波段FSS能够捕捉病变发展过程中褪绿组织和坏死组织的独特特征，从而原位可视化植物-病原菌像素级的早期互作动态进程。进一步，实现了接种后24 h叶斑病的显症前诊断，比传统的聚合酶链反应（PCR）测定或生化测定提前了12 h。为了揭示HSI显症前诊断的机制，还建立了叶片的平均光谱响应与其生化指标（叶绿素、类胡萝卜素、丙二醛、抗坏血酸和还原型谷胱甘肽）之间的定量关系，回归模型在预测集上的Rp2均高于0.84。总体结果表明，HSI反映了活体植物特性的变化，所提取的FSS可成功跟踪叶斑病发生发展的时空动态进程，实现显症前诊断。在其他植物病害上的试验表明，该方法在植物病害早期控制方面具有较大的推广潜力。

Reliable Analytic Strategy to Correlate the Morphological and Cytological Parameters on Lupinus termis L, against Fusarium oxysporum Infection

Many analytic strategies have emerged to estimate plant responses to Fusarium wilt. The demand for fast and reliable method （diagnosis, prediction） to determine isolate strength accurately is not established yet. Early determination of pathogen strength helps in plant medication. The aim of this study was to develop a faster strategy and method for early determination of fungal isolates strength in correlation to plant response. Till now, the scientists have no consensus on the most correlated parameters that could express wilt precisely. In this study, 30 isolates of Fusarium oxysporum isolated from Lupinus termis L. were used to provide an explicit image about the real strength of Fusarium isolates and its impact on the plant. Wilting percentage ranged from 26.67% to 93.33% of the infected plants depending on isolate virulence. Some of cellular, morphological and physical measurements were conducted on 8 out of 30 isolates, including root （length, fresh weight （FW） and dry weight （DW））, nodules （water content （WC）, FW, DW）, stem （height, WC, FW, DW）, total leaves/plant （WC, FW, DW） and the fourth leaf （WC, FW, DW, leaf area, epidermal cell area, epidermal cell number, succulence）. Hierarchical clustering was used to determine the variance between the isolates. Detrended correspondence analysis （DCA） and canonical correspondence analysis （CCA） were used to determine the most important growth parameters that could express wilting accurately. The CCA results showed that most of the measured parameters on the fourth leaf, except for leaf epidermal cell number, were highly and positively correlated to wilt. That makes these specific parameters valuable and sensitive for any changes in isolates strength. Accordingly, a mathematical model was created to be helpful in the quick determination of isolate strength and precise medication.

The Isolation of Vibrio cholera and Other Enteric Bacteria with Molecular Characterization of Vibrio cholera during the Outbreak of Baghdad/Iraq in 2015

Vibrio cholera, causing acute watery diarrhea known as cholera disease, affects all ages and both genders. Cholera infection outbreaks in Iraq have been reported for several years. The recent cholera outbreak, emerged throughout 2015, was investigated using bacteriological laboratory tests, singleplex and multiplex PCR technique for the detection of V. cholera from stool samples. Furthermore the toxigenic potential coupled with the antibiotic susceptibility test for cholera and other bacteria were also investigated. The stool samples were collected from 5698 patients admitted to Al-Yarmouk Teaching hospital and health care centers in Baghdad/Al-Karkh, Iraq, from the 1st of August to the 30th of December 2015. The V. cholera was isolated from 194 cases (3.4% of the cases age between 21 - 50 years). In addition, other enteric infections: Salmonellosis and Shigellosis 7 and 21 respectively, protozoan parasite Giardia lamblia and Entamoeba histolytica 2 and 43 cases respectively were also reported. High percentage of V. cholera infection was detected in October (122 cases, 62.8%), compared with other enteric infections that show high percentage of diarrheal disease in September and November. The results have confirmed that the cholera outbreak was caused by V. cholera O1, biotype el Tor, and serotype Inaba. Seven virulence genes were identified ctxA, toxR, zot, ace, rfbO1, tcpA and ompW. Moreover, the cholera isolated strains were found sensitive to most antibiotic but resistant to nalidixic acid.

Establishing Standardized Nursing Quality Sensitive Indicators

This is a pilot research project was prepared to assess the impact of implementing essential standardized nursing quality sensitive indicators in monitoring, evaluating, and controlling the nursing services and activities which implemented in daily nursing care works with the patients in a health institute, aiming for emphasizing the concept of “nursing quality sensitive indicators” which is valuable to elaborate nursing care performance, and for continuous upgrading of daily nursing care, which hopefully will result in more beneficial outcomes, and lesser negative or undesirable outcomes in all aspects of nursing care, and consequently the health care process, particularly regarding patient safety, patient satisfaction and nursing performance improvement. This pilot research project for the essential standardized nursing quality sensitive indicators was applied in intensive care unit (ICU) department in Qurayyat general hospital, and resulted in excellent improvement of all outcomes correlated with the essential standardized nursing quality sensitive indicators. These essential standardized nursing quality sensitive indicators are hopefully be accepted and accredited by the higher health authorities as standardized nursing quality sensitive indicators in health institutes.
