Evaluating the influence of a structured nursing protocol on targeted outcomes in rheumatoid arthritis patients

Objective:Rheumatoid arthritis(RA)requires comprehensive management.Structured nursing protocols may enhance outcomes,but evidence is limited.This study evaluated the effect of a structured nursing protocol on RA outcomes.Materials and Methods:In this one-group pre-post study,30 Egyptian RA patients completed assessments before and after a 12-week nursing protocol comprising education,psychosocial support,and self-management promotion.Assessments included clinical evaluation of joint counts,erythrocyte sedimentation rate(ESR),and C-reactive protein(CRP)and patient-reported Arthritis Self-Efficacy Scale(ASES),Health Assessment Questionnaire(HAQ),Visual Analog Scale(VAS)for pain,and Hospital Anxiety and Depression Scale(HADS).Results:The study demonstrated significant improvements in both clinical-and patient-reported outcomes.Joint count decreased from 18.4±4.2 to 14.2±3.8(P<0.001),ESR from 30.1±6.8 mm/h to 25.5±6.8 mm/h(P<0.01),and CRP levels from 15.2±3.6 mg/L to 11.8±2.9 mg/L(P<0.01)postintervention.Patient-reported outcomes showed a marked increase in ASES score from 140±25 to 170±30(P<0.001)and reductions in HAQ from 1.6±0.4 to 1.3±0.3(P<0.01),VAS pain score from 7.8±1.7 to 6.2±1.2(P<0.001),and HADS anxiety and depression scores from 11±3 to 8±2(P<0.05)and 10±2 to 7±1(P<0.05),respectively.Conclusion:A structured nursing protocol significantly improved clinical disease activity,physical functioning,pain,self-efficacy,and emotional well-being in RA patients.A multifaceted nursing intervention appears beneficial for optimizing RA outcomes.

Background:Fever of unknown origin(FUO)in developing countries is an important dilemma and further research is needed to elucidate the infectious causes of FUO.Methods:A multi-center study for infectious causes of FUO in lower middle-income countries(LMIC)and lowincome countries(LIC)was conducted between January 1,2018 and January 1,2023.In total,15 participating centers from seven different countries provided the data,which were collected through the Infectious DiseasesInternational Research Initiative platform.Only adult patients with confirmed infection as the cause of FUO were included in the study.The severity parameters were quick Sequential Organ Failure Assessment(qSOFA)≥2,intensive care unit(ICU)admission,vasopressor use,and invasive mechanical ventilation(IMV).Results:A total of 160 patients with infectious FUO were included in the study.Overall,148(92.5%)patients had community-acquired infections and 12(7.5%)had hospital-acquired infections.The most common infectious syndromes were tuberculosis(TB)(n=27,16.9%),infective endocarditis(n=25,15.6%),malaria(n=21,13.1%),brucellosis(n=15,9.4%),and typhoid fever(n=9,5.6%).Plasmodium falciparum,Mycobacterium tuberculosis,Brucellae,Staphylococcus aureus,Salmonella typhi,and Rickettsiae were the leading infectious agents in this study.A total of 56(35.0%)cases had invasive procedures for diagnosis.The mean qSOFA score was 0.76±0.94{median(interquartile range[IQR]):0(0–1)}.ICU admission(n=26,16.2%),vasopressor use(n=14,8.8%),and IMV(n=10,6.3%)were not rare.Overall,38(23.8%)patients had at least one of the severity parameters.The mortality rate was 15(9.4%),and the mortality was attributable to the infection causing FUO in 12(7.5%)patients.Conclusions:In LMIC and LIC,tuberculosis and cardiac infections were the most severe and the leading infections causing FUO.

Objective:The current study aimed to assess the knowledge and attitudes of nurses toward the current COVID-19 pandemic and Omicron variant.Materials and Methods:A cross-sectional online survey was used.Totally 277 Egyptian nurses completed an electronic questionnaire comprising demographics with 5 items,nurses’knowledge on Omicron with 18 questions,and nurses’attitudes on Omicron with 5 questions.The data collected were expressed using numbers,percentages,and mean and standard deviation.Results:The mean age of nurses was 27±5 years,nearly 50%of the nurses were bachelor degrees.The total score of correct answers related to knowledge was 75%,showing the different degrees of knowledge about Omicron.The majority of nurses showed positive attitudes toward controlling and managing the COVID-19 pandemic.Conclusions:Egyptian nurses have average knowledge about Omicron and a positive attitude toward controlling the COVID-19 pandemic.There should be scope for further assessment to enhance understanding about COVID-19.Moreover,health education programs should be developed for nursing staff to improve information,beliefs,behaviors,and attitudes towards the Omicron variant in particular and the COVID-19 pandemic in general.

Grid-connected reactive-load compensation and harmonic control are becoming a central topic as photovoltaic(PV)grid-connected systems diversified.This research aims to produce a high-performance inverter with a fast dynamic response for accurate reference tracking and a low total har-monic distortion(THD)even under nonlinear load applications by improving its control scheme.The proposed system is expected to operate in both stand-alone mode and grid-connected mode.In stand-alone mode,the proposed controller supplies power to critical loads,alternatively during grid-connected mode provide excess energy to the utility.A modified variable step incremental conductance(VS-InCond)algorithm is designed to extract maximum power from PV.Whereas the proposed inverter controller is achieved by using a modified PQ theory with double-band hysteresis current controller(PQ-DBHCC)to produce a reference current based on a decomposition of a single-phase load current.The nonlinear rectifier loads often create significant distortion in the output voltage of single-phase inverters,due to excessive current harmonics in the grid.Therefore,the proposed method generates a close-loop reference current for the switching scheme,hence,minimizing the inverter voltage distortion caused by the excessive grid current harmonics.The simulation findings suggest the proposed control technique can effectively yield more than 97%of power conversion efficiency while suppressing the grid current THD by less than 2%and maintaining the unity power factor at the grid side.The efficacy of the proposed controller is simulated using MATLAB/Simulink.

Objective:The objective of this study was to assess the prevalence and level of social media addiction among nursing students in the Faculty of Nursing,Cairo University.Materials and Methods:A descriptive cross‑sectional research design was conducted on samples of 340 students at the Faculty of Nursing,Cairo University.Data were collected through demographic background information sheet and Social Networking Addiction Scale.Results:All the students were addicted as 6.76%were severely addicted and 60.59%and 32.65%were moderately and mildly addicted,respectively.Significant relations were found between social media addiction and students’age(χ^(2)=11.331,P=0.003),educational level(χ^(2)=20.239,P=0.003),and grade point average(χ^(2)=19.378,P=0.013).Conclusion:Internet addiction was prevalent among all students but at different levels,so early screening of students for Internet addiction using the Internet Addiction Scale is important to provide early treatment and prevent hazards to health.

The rapid expansion of both the global economy and the human population has led to a shortage of water resources suitable for direct human consumption.As a result,water remediation will inexorably become the primary focus on a global scale.Microalgae can be grown in various types of wastewaters(WW).They have a high potential to remove contaminants from the effluents of industries and urban areas.This review focuses on recent advances on WW remediation through microalgae cultivation.Attention has already been paid to microalgae-based wastewater treatment(WWT)due to its low energy requirements,the strong ability of microalgae to thrive under diverse environmental conditions,and the potential to transform WW nutrients into high-value compounds.It turned out that microalgae-based WWT is an economical and sustainable solution.Moreover,different types of toxins are removed by microalgae through biosorption,bioaccumulation,and biodegradation processes.Examples are toxins from agricultural runoffs and textile and pharmaceutical industrial effluents.Microalgae have the potential to mitigate carbon dioxide and make use of the micronutrients that are present in the effluents.This review paper highlights the application of microalgae in WW remediation and the remediation of diverse types of pollutants commonly present in WW through different mechanisms,simultaneous resource recovery,and efficient microalgae-based co-culturing systems along with bottlenecks and prospects.

This paper examines the effects of three distinct numerical schemes(Adomian Decomposition,quintic&septic Spline methods)to investigate semi-analytical and approximate solutions on Wu-Zhang(ZW)system.It describes the(1+1)-dimensional dispersive long wave in two horizontal directions on shallow waters.The ZW model is one of the fractional nonlinear partial differential equations.Conformable derivatives properties are employed to convert the nonlinear fractional partial differential equation into an ordinary differential equation with integer order so as to obtain the approximate solutions for this model.The solutions obtained for each technique were compared to reveal their relationship to their characteristics illustrated under the suitable choice of the parameters values.The obtained solutions showed the power,easiness,and effectiveness of these methods on nonlinear partial differential equations.

Influence of Plastic Deformation and Aging Process on Microstructure and Tensile Properties of Cast Ti-6Al-2Sn-2Zr-2Mo-1.5Cr-2Nb-0.1Si Alloy

In the present work, titanium alloy with a composition of Ti-6.5Al-3Mo-1.9Nb-2.2Sn-2.2Zr-1.5Cr (TC21) was subjected to plastic deformation and aging processes. A Plastic deformation at room temperature with 2%, 3% and 4% stroke strain was applied on the studied samples. Then, the samples aged at 575°C for 4 hr. By applying different plastic deformation ratios, the structure revealed an elongated and thin β-phase embedded in an α-phase. Secondary α-platelets were precipitated in the residual β-phase. Maximum hardness (HV440) was obtained for 4% deformed + aged samples. Minimum hardness (HV320) was recorded for the as-cast samples without deformation. The highest ultimate tensile strength of 1311 MPa was obtained for 4% deformed + aged samples due to presence of high amount of dislocation density as well as precipitation of secondary α-platelets in the residual β-phase. The lowest ultimate tensile strength of 1020 MPa was reported for as-cast samples. Maximum elongation of 14% was registered for 4% deformed + aged samples and minimum one of 3% was obtained for as-cast samples. Hence, strain hardening + aging can enhance considerably the elongation of TC21 Ti-alloy up to 366% and 133% in case of applying 4% deformation + aged compared to as-cast and aged samples without applying plastic deformation, respectively.

Objective: This study investigated cytotoxicity and induction of apoptosis in human cervical cancer cells(HELA) and prostate cancer cells(PC-3) using the most active fraction of Moringa peregrina seed extract.Methods: Dried and powdered seeds were extracted using 95% ethanol. The total ethanolic extract was further dissolved in distilled water and separated into petroleum ether, chloroform, ethyl acetate and aqueous extracts. Based on the results of in vitro anticancer studies of all extracts, the most highly active extract was selected for evaluation of apoptosis induction and cell cycle analysis on HELA and PC-3 cells at its half maximal inhibitory concentration using flow cytometry;DNA fragmentation by agarose gel electrophoresis and the expression of protein were measured by Western blot.Results: The chloroform fraction from the ethanolic extract of M. peregrina(CFEE) was the most active antitumor fraction. The selectivity index, determined using the normal Vero cell line, indicated that CFEE had a high degree of selectivity against HELA and PC-3 cells. CFEE induced apoptosis, confirmed by cell cycle arrest at sub-G0 phase and DNA fragmentation. CFEE induced an increase in mRNA expression of caspase-3, a decrease in Bcl-2 mRNA expression, and decreased ATP levels. CFEE increased protein expression of caspase-3 and decreased protein expression of poly-ADP-ribose polymerase-1(PARP-1).Flow cytometric analysis showed an appreciable increase in the number of cells in the early apoptotic stage in CFEE-treated HELA and PC-3 cells. CFEE treatment significantly increased lipid peroxidation(malondialdehyde level) in HELA and PC-3 cells.Conclusion: Seed extract of M. peregrina displayed a significant antitumor effect through apoptosis induction in HELA and PC-3 cells.
