Botulinum toxin augmented bilateral lateral rectus recession versus three muscles surgery in large-angle intermittent exotropia

AIm: To report the surgical outcomes of correcting large angle intermittent exotropia in adult patients by bilateral lateral rectus muscle recession with intraoperative use of botulinum toxin and to compare the results with those of bilateral lateral rectus muscle recession and unilateral medial rectus resection. mETHODS: The medical records of patients who underwent surgical correction of large angle intermittent exotropia [exotropia >50 prism dioptre(PD)] were retrospectively reviewed. Two groups of patients were identified;Group Ⅰ(21 patients) had bilateral lateral recti recession augmented with intraoperative botulinum toxin A(BTA) injection into the recessed muscles and group Ⅱ(30 patients) were treated by bilateral lateral recti recession with unilateral medial rectus muscle resection. Preoperative data were extracted for age, gender, refraction, type of exotropia, angle of stereopsis and angle of deviation. The main outcome measures were the postoperative angle of deviation and stereoacuity angle by Titmus test measured at the end of one year of postoperative follow up. RESULTS: By the end of the first postoperative year, 10 patients in group Ⅰ(47.6%) and 20 patients in group Ⅱ(66.7%) achieved esotropia/esophoria mproved stereopsis;this difference in the sensory outcome was not statistically significant(P=0.8). In the BTA augmented surgery group, good stereoacuity and smaller preoperative angle of deviation were associated with significantly higher surgical success rate(P=0.004, 0.01 respectively). New chelates derived from the novel ligand, (Z)-3-amino-N-(3-amino pyrazine-2-carbonyl)pyrazine-2-carbohydrazonic acid (APA, L), with Cu2+, Co2+, Pt4+ and Pd2+ salts were investigated. The results suggest that APA acts as mononegative tridentate in the case of Cu2+, binegative tetradentate in the case of Co2+ and as mononegative bidentate towards Pt4+ and Pd2+ chelates. The results of the corrected μeff. and spectral suggest the structures of the isolated chelates. The results of the corrected μeff. and spectral suggest the geometries of the isolated chelates. molecular modeling is deduced and chemical reactivity, energy components for chelates and also mEP for APA is illustrated. In Vitro, the SOD and radical scavengers like activity of the synthesized compounds Hep G2 liver cancer cells and cytotoxic activity were checked. metal chelates show potent anti-oxidative activity. The results of cytotoxic activity assay against hepatocellular carcinoma cell line Hep G2 confirmed that Pt4+ complex has the highest value, while APA, Cu2+, Co2+ and Pd2+ chelates have no significant cytotoxic activity.

Nowadays, nanomaterials have become an emerging field that has shown great promise in the development of novel diagnostic, imaging and therapeutic agents for a variety of diseases, including cancer, due to their nanoscale size effects and increased surface area. In comparison to their larger counterparts, nanomaterials have unique physicochemical and biological properties including size, shape, chemical composition, surface structure and charge, aggregation and agglomeration, and solubility which can affect their interactions with biomolecules and cells. Nanoparticles (NPs) with size-tunable light emission have demonstrated an impressive potential as high-efficiency delivery transporters for biomolecules into cells, being used to produce exceptional images of tumor sites. moreover, NPs delivery system has been widely applied in pharmaceutical field to enhance absorption of bioactive compounds since they can interact with several phytochemicals by hydrogen bonds and hydrophobic interactions to encapsulate these phytochemicals in NPs and thus enhance aqueous solubility of the chemicals. moreover, NPs also can prevent against oxidation/degradation of the phytochemicals encapsulated in the gastrointestinal tract and can be taken directly up by epithelial cells in the small intestine resulting in the increase of absorption and bioavailability of phytochemicals. In general, there are two specific fields of utilization of intrinsically active NPs as pharmacologic agents including oxidative-related pathologies and cancer. On the other hand, Redox active NPs have been shown to ameliorate many clinically relevant pathological disorders that implicate oxidative stress, reducing the oxidative burden and alleviating many important symptoms. Angiogenesis is an essential process for organ growth and repair. Thus, an imbalance in this process can lead to several diseases including malignancy. Angiogenesis is a critical step in vascular remodeling, tissue damage and wound healing besides being required for invasive tumor growth and metastasis. Because angiogenesis sets an important point in the control of tumor progression, its inhibition is considered a valuable therapeutic approach for tumor treatment. Chronic liver disease including hepatitis C virus(HCV) infection is one of the main cause for the development of hepatic angiogenesis and thereby plays a critical role in the modulation of hepatic angiogenesis that finally leads to hepatocellular carcinoma progression and invasion. Thus, understanding of the molecular mechanisms of HCV-mediated hepatic angiogenesis will help design a therapeutic protocol for the intervention of HCV-mediated angiogenesis and subsequently its outcome. In this review, we will focus on the molecular mechanisms of HCV-mediated hepatic angiogenesis and the related signaling pathways that can be target for current and under development therapeutic approaches.

AIm:To evaluate the relationship between vascular endothelial growth factor(VEGF),p53,and the H-ras oncogene and different clinicopathological parameters in Egyptian patients with Schistosoma-associated transitional cell carcinoma of the ***:The study included 50 patients with transitional cell carcinoma for whom radical cystectomy and urinary diversions were carried *** and p53 protein expressions were evaluated with an immunohistochemical staining method,and H-ras oncogene mutations were analyzed with a polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP) ***:High grade tumors revealed higher p53 immunostaining than low grade tumors(P = 0.016).p53 and VEGF protein expressions,as well as H-ras oncogene mutations,had an insignificant impact on patient outcomes(P = 0.962,P = 0.791,and P = 967,respectively).Cancer extension to regional lymph nodes was associated with poor outcomes(P = 0.008).CONCLUSION:VEGF,p53 and the H-ras oncogene have no relation to patient survival and outcome in Schistosoma-associated transitional cell carcinoma.

Background: Dobutamine stress echocardiography (DSE) is a well-established method for detecting myocardial ischemia. The classic echocardiographic findings of ischemia are new or worsening abnormalities of radial wall thickening. However, interpretation of these findings is subjective and dependent on experience. Speckle tracking strain imaging (STI) has been introduced as a novel method to overcome these limitations. Objective: The aim of the study is to evaluate the value of speckle tracking imaging (STI) by estimating the mean global longitudinal systolic (GLS) and the mean global circumferential systolic (GCS) strain imaging at recovery period of dobutamine stress echocardiography for prediction of significant coronary artery disease (CAD) in patients with chest discomfort. methods: Fifty three patients presented for evaluation of chest discomfort underwent dobutamine stress echocardiography and coronary angiography at Cardiology Department,menoufia University Hospital. The mean global left ventricular longitudinal systolic strain (GLS) at apical views—3 chambers, 4 chambers & 2 chambers apical views and global left ventricular circumferential systolic strain (GCS) at short axis view at level of papillary muscle were measured at rest and during recovery period of dobutamine stress echocardiography using automated functional imaging (AFI). Coronary angiography was done for all patients. Significant coronary artery lesion was defined as having a ≥ 70% diameter stenosis on coronary angiography. Patients were divided into two groups based on the presence or absence of significant coronary artery lesion into CAD positive (+ve) group vs. CAD negative (-ve) group. Results: In both groups,there were no statistically significant differences in the clinical characteristics and baseline conventional transthoracic echocardiography. Purpose: There is a need for a continued effort to develop rapid immunodiagnostic assays for tuberculosis (TB) infection with greater sensitivity and specificity that can be used in the field and in the laboratory and that can be formatted for use with multiple species. This would help to obtain definitive early diagnosis of TB. The present study was developed to determine the role of using early secreted antigenic target-6 (ESAT-6) in immunodiagnosis of mycobacterium tuberculosis. methods: Serum samples were obtained from TB infected patients and normal healthy controls. Two rapid immunodiagnostic assays (Enzyme-linked immunosorbent assay (ELISA) and Immunoblotting) were performed. Results: The sensitivity of immunoblotting assay was 100%;however, ESAT-6 antigen was not able to discriminate between patients and normal controls. Application of direct ELISA using ESAT-6 antigen yielded 97.6% sensitivity and 75% specificity for the diagnosis of TB infection. Conclusion: In conclusion, the detection of antibodies against ESAT-6 antigen in the sera of TB patients by direct ELISA could be used as a preliminary assay for diagnosis of human m. tuberculosis infection. A combination of the ELISA with either radiological or microscopic examination is required to overcome the low specificity of the assay for negative results.
