Autoimmune thrombocytopenia in response to splenectomy in cirrhotic patients with accompanying hepatitis C

AIM： To estimate the contribution of autoimmune thrombocytopenia to hepatitis C virus-related liver cirrhosis （type C cirrhosis）, we evaluated the influence of splenectomy upon platelet-associated immunoglobulin G （PAIgG） levels and platelet numbers. METHODS： PAIgG titers and immune markers were determined in 24 type C cirrhotic patients with an intact spleen, 17 type C cirrhotic patients submitted to splenectomy, and 21 non-C cirrhosis with an intact spleen. RESULTS： Thrombocytopenia （PLT〈15×10^4/μL） in type C cirrhosis was diagnosed in all patients with an intact spleen, 8 patients submitted to splenectomy, and in 19 non-C cirrhosis with intact spleen. Elevated titers of PAIgG at more than 25.0 ng/107cells were detected in all cirrhotic patients except for one splenectomized patient. PAIgG titers （ng/10^7cells） were significantly higher in the type C cirrhosis with an intact spleen （247.9 ± 197.0） compared with the splenectomized patients （125.6±87.8） or non-C cirrhosis （152.4± 127.4）. PAIgG titers were negatively correlated with platelet counts in type C cirrhotic patients with an intact spleen. In comparison with the type C cirrhosis with an intact spleen, the splenectomized patients had a reduced CD4/CD8 ratio and serum neopterin levels. The spleen index （cm^2） was negatively correlated with platelet counts in the non-C cirrhosis, but not in the type C cirrhosis. CONCLUSION： Our data indicate that the autoimmune mechanism plays an important role in thrombocytosis complicated by HCV-positive cirrhosis. In addition, splenectomy may impair T cells function through, at least in part, a reduction of CD4/CD8 ratio, consequently suppressing PAIgG production.

There have been lots of papers on the uniqueness theory of entire functions concerning shared-sets in the whole complex plane. However, it seems that the uniqueness theory in an angular domain is not widely investigated. In this paper, we study the uniqueness of entire functions concerning shared-sets in an angular domain instead of the whole complex plane, and we supply examples to show that Theorem 1 is sharp.

Rotation of a pair of wings was driven by the vertical harmonic motion of a pin inserted into the center hole of the *** elucidate the mechanism by which the rotational motion of the wings was excited,the relationship between the wings and the pin was examined by tracking their motions using both displacement measurements and high-speed *** motion modes occurred in this study were categorized into five types:slipping,rolling,jumping(without eccentricity),jumping(with eccentricity),and *** the case that the hole of the wings was located at a distance from the center of the wings,referred to as″with eccentricity,″the slipping,jumping(with eccentricity),and non-rotation modes *** experimental results showed that the mechanism of the jumping(with eccentricity)was different from that of the other modes(slipping,rolling,jumping(without eccentricity)),which are well known to be driven by the periodical reaction of the wings against the vertical vibration of the *** was found that the jumping(with eccentricity)was driven by the non-periodical force with the collision between the wing hole and the pin.

Efficacy of balloon-occluded retrograde transvenous obliteration, percutaneous transhepatic obliteration and combined techniques for the management of gastric fundal varices

AIM： To evaluate the effect of three interventional treatments involving transvenous obliteration for the treatment of gastric varices, and to compare the efficacy and adverse effects of these methods, METHODS： From 1995 to 2004, 93 patients with gastric fundal varices underwent interventional radiologic embolotherapy at our hospital. Of the 93 patients, 75 were treated with the balloon-occluded retrograde transvenous obliteration （BRTO） procedure; 8 were with the percutaneous transhepatic obliteration （PTO） procedure; and 10 were with the combined BRTO and PTO therapy. A follow-up evaluation examined the rates of survival, recurrence and rebleeding of the gastric varices, worsening of esophageal varices and complications in each group. RESULTS： The BRTO, PTO, and combined therapy were technically successful in 81% （75/93）, 44% （8/18）, and 100% （10/10） patients, respectively. Recurrence of gastric varices was found in 3 patients in the BRTO group and in 3 patients in the PTO group. Rebleeding was observed in 1 patient in the BRTO group and in 1 patient in the PTO group. The 1- and 3-year survival rates were 98% and 87% in the patients without hepatocellular carcinoma （HCC） in the BRTO group, 100% and 100% in the PTO group, and 90% and 75% in the combined therapy group, respectively. CONCLUSION： Combined BRTO and PTO therapy may rescue cases with uncontrollable gastric fundal varices that remained even after treatment with BRTO and/or PTO, though there were limitations of our study, including retrospective nature and discrepancy in sample size between the BRTO, PTO and combined therapy groups.

Reduction of virus burden-induced splenectomy in patients with liver cirrhosis related to hepatitis C virus infection

AIM： To examine the hepatitis C virus （HCV） levels and immunological markers in cirrhotic patients after splenectomy. METHODS： HCV RNA titers as well as cellular and humoral immune markers were determined in 20 cirrhotic patients after splenectomy and in 32 cirrhotic controls with an intact spleen. RESULTS： Serum HCV RNA titers were lower in the splenectomized patients than in the controls （186 ± 225 × 10^3 copies/mL vs 541 ± 417×10^3 copies/mL, P〈0.01）. HCV RNA was judged to have been spontaneously eradicated in 4 splenectomized patients, but in none of the controls. Natural killer cell activity was higher in the splenectomized patients than in the controls （41.2 ± 19.3% vs 24.7 ± 15.3%, P〈 0.01）, and natural killer cell activity was negatively correlated to HCV RNA titers in the splenectomized patients except in those with serotype 2-related infection. The CEH/CD8 ratio was significantly lower in the splenectomized patients than in the controls. CONCLUSION： The findings suggest that splenectomy may diminish virus burden in cirrhotic patients with HCV infection at least in part, through augmentation of natural killer cell activity.

Beneficial effects of fucoidan in patients with chronic hepatitis C virus infection

AIM:To evaluate the effects of fucoidan,a complex sulfated polysaccharide extract from marine seaweed,on hepatitis C virus(HCV) RNA load both in vitro and in ***:HCV-1b replicon-expressing cells were cultured in the presence of fucoidan obtained from Cladosiphon okamuranus Tokida cultivated in Okinawa,Japan,and quantified the level of HCV *** an open-label uncontrolled study,15 patients with chronic hepatitis C,and HCV-related cirrhosis and hepatocellular carcinoma were treated with fucoidan(0.83 g/d) for 12 *** clinical symptoms,biochemical tests,and HCV RNA levels were assessed before,during,and after ***:Fucoidan dose-dependently inhibited the expression of HCV *** 8-10 mo of treatment with fucoidan,HCV RNA levels were significantly lower relative to the *** same treatment also tended to lower serum alanine aminotransferase levels,and the latter correlated with HCV RNA ***,the improved laboratory tests did not translate into significant clinical *** had no serious adverse ***:Our findings suggest that fucoidan is safe and useful in the treatment of patients with HCVrelated chronic liver *** controlled clinical trials are needed to confirm the present findings.

Moxibustion Promotes Formation of Granulation in Wound Healing Process through Induction of Transforming Growth Factor-β in Rats

Photoluminescence and radioluminescence properties of (Gd8Ca2)(SiO4)6O2 crystals with different concentrations of Ce

(GdCa)(SiO)Oapatite crystals doped with different concentrations of Ce were synthesized by the Floating Zone(FZ) method, and we have evaluated the photoluminescence(PL) and radio luminescence(RL) properties. In PL, an emission band appeared around 400 nm under excitation at 300 nm due to the5 d-4 f transitions of Ce3+. The PL decay curves were approximated by a second-order exponential decay function, and the derived decay time constants ranged around 20-25 ns and 40-480 ns. Similar emission was observed in RL, but the decay time constants were much longer than those in PL. The RL afterglow was the lowest in the 1.0% Ce-doped sample. The 1.0% and 2.0% Ce-doped samples showed a full-energy peak in the pulse height spectrum measured underAm 5.5 MeV α-ray irradiation, and the absolute scintillation light yields were estimated to be around 55 and 36 ph/5.5 MeV-a, respectively.

RECENT ADVANCES IN HYDRATE-BASED TECHNOLOGIES FOR NATURAL GAS STORAGE——A REVIEW

Interest in the possibility of storing and transporting natural gas in the form of clathrate hydrates has been increasing in recent years, particularly in some gas-importing and exporting *** technologies necessary for realizing this possibility may be classified into those relevant to the four serial processes (a) the formation of a hydrate, (b) the processing (dewatering, pelletizing, etc. ) of the formed hydrate, (c) the storage and transportation of the processed hydrate, and (d) the regasification (dissociation) of the hydrate. The technological development of any of these processes is still at an early stage. For hydrate formation, for example, various rival operations have been proposed. However,many of them have never been subjected to actual tests for practical use. More efforts are required for examining the different hydrate-formation technologies and for rating them by comparison. The general design of the processing of the formed hydrate inevitably depends on both the hydrate-formation process and the storage/transportation process, hence it has a wide variability. The major uncertainty in the storage-process design lies in the as-yet unclarified utility of the "self-preservation" effect of the naturalgas hydrates. The process design as well as the relevant cost evaluation should strongly depend on whether the hydrates are well preserved at atmospheric pressure in large-scale storage facilities. The regasification process has been studied less extensively than the former processes. The state of the art of the technological development in each of the serial processes is reviewed, placing emphasis on the hydrate formation process.
