Detection of respiratory viruses and bacteria in children using a twenty-two target reverse-transcription real-time PCR(RT-qPCR)panel

Background:Rapid detection of the wide range of viruses and bacteria that cause respiratory infection in children is important for patient care and antibiotic *** therefore designed and evaluated a readyto-use 22 target respiratory infection reverse-transcription real-time polymerase chain reaction(RT-qPCR)panel to determine if this would improve detection of these agents at our pediatric ***:RT-qPCR assays for twenty-two target organisms were dried-down in individual wells of 96 well plates and saved at room *** included 18 respiratory viruses and 4 *** automated nucleic acid extraction of nasopharyngeal aspirate(NPA)samples,rapid qPCR was ***-qPCR results were compared with those obtained by the testing methods used at our hospital ***:One hundred fi fty-nine pediatric NPA samples were tested with the RT-qPCR *** or more respiratory pathogens were detected in 132/159(83%)*** was significantly higher than the detection rate of standard methods(94/159,59%)(Ps difference was mainly due to improved RT-qPCR detection of rhinoviruses,parainfl uenza viruses,bocavirus,and *** panel internal control assay performance remained stable at room temperature storage over a two-month testing ***:The RT-qPCR panel was able to identify pathogens in a high proportion of respiratory *** panel detected more positive specimens than the methods in use at our *** pre-made panel format was easy to use and rapid,with results available in approximately 90 *** now plan to determine if use of this panel improves patient care and antibiotic stewardship.

Assessment of hepatic fibrosis in pediatric cases with hepatitis C virus in Egypt

AIM: To assess hepatic fibrosis and factors associated with its progression in children with HCV infection. METHODs: At the Hepatology Unit,Cairo University Children's Hospital,a single liver biopsy was performed to 43 children with HCV infection after an informed consent between 1998-2004. Their mean age at liver biopsy was 8.67 ± 4.3 years. REsULTs: Among the 43 patients' biopsies,12 (27.9%) were having no fibrosis,20 (46.5%) mild fibrosis and 11 (25.6%) moderate to severe fibrosis. The median time for development of fibrosis was estimated to be 5.5 years. Developing fibrosis was significantly associated with shorter duration from first detected ALT elevation to biopsy (12 mo vs 1.2 mo,P = 0.015) and having higher levels of direct serum bilirubin (0.3 mg/dL vs 0.5 mg/dL,P = 0.048). No association was found between fibrosis stage and the presence of co-morbid conditions (P = 0.33). CONCLUsION: Hepatic fibrosis was present in 72.1% of children with HCV infection. The development of fibrosis was associated with higher levels of direct serum bilirubin. There was no significant association between fibrosis and age,duration of infection,risk factors,co-morbid conditions and most biochemical parameters.

White blood cells (WBC) or leukocytes are a vital component ofthe blood which forms the immune system, which is accountable to fightforeign elements. The WBC images can be exposed to different data analysisapproaches which categorize different kinds of WBC. Conventionally, laboratorytests are carried out to determine the kind of WBC which is erroneousand time consuming. Recently, deep learning (DL) models can be employedfor automated investigation of WBC images in short duration. Therefore,this paper introduces an Aquila Optimizer with Transfer Learning basedAutomated White Blood Cells Classification (AOTL-WBCC) technique. Thepresented AOTL-WBCC model executes data normalization and data augmentationprocess (rotation and zooming) at the initial stage. In addition,the residual network (ResNet) approach was used for feature extraction inwhich the initial hyperparameter values of the ResNet model are tuned by theuse of AO algorithm. Finally, Bayesian neural network (BNN) classificationtechnique has been implied for the identification of WBC images into distinctclasses. The experimental validation of the AOTL-WBCC methodology isperformed with the help of Kaggle dataset. The experimental results foundthat the AOTL-WBCC model has outperformed other techniques which arebased on image processing and manual feature engineering approaches underdifferent dimensions.

Adolescents’perceived stress of COVID-19 and self-compassion in saudi Arabia:A cross-sectional study

Objectives Worldwide,the COVID-19 pandemic has significantly impacted mental health challenges for adolescent *** current study aims to examine the levels of perceived stress of COVID-19 and self-compassion and their relationship among adolescents in saudi *** This study employed a cross-sectional online survey of adolescents from secondary schools in Asir,saudi *** that we distributed online included the modified Perceived stress scale(Pss-10)and the self-Compassion scale(sCs),in addition to demographic and health-related questions.A total of 500 adolescents completed the *** Adolescents in the study reported an average moderate perceived stress level of 18.6(sD=6.67)and an average moderate self-compassion level of 3.22(sD=0.53).A significant correlation is also found between the two variables(r=−0.460,Ps negative correlation indicates that lower levels of perceived stress are significantly correlated with higher levels of *** The study results show that the perceived stress of COVID-19 is inversely correlated with self-compassion in saudi *** research is needed to explore how to enhance adolescents’*** nurses’role should be given full play in this area.

Burden and outcomes for complex perianal fistulas in Crohn's disease:systematic review

AIM To systematically review the literature on epidemiology,disease burden, and treatment outcomes for Crohn's disease(CD) patients with complex perianal *** PubMed, Embase, and Cochrane were searched for relevant articles(published 2000-November 2016) and congress abstracts(published 2011-November 2016).REsULTs Of 535 records reviewed, 62 relevant sources were identified(mostly small observational studies). The cumulative incidence of complex perianal fistulas in CD from two referral-centre studies was 12%-14%(follow-up time, 12 years in one study; not reported in the second study). Complex perianal fistulas result in greatly diminished quality of life; up to 59% of patients are at risk of faecal incontinence. Treatments include combinations of medical and surgical interventions and expanded allogeneic adipose-derived stem cells. High proportions of patients experience lack of or inadequate response to treatment(failure and relapse rates,respectively: medical, 12%-73% and 0%-41%; surgical:0%-100% and 11%.20%; combined medical/surgical:0%-80% and 0%-50%; stem cells: 29%-47% and not reported). Few studies(1 of infliximab; 3 of surgical interventions)have been conducted in treatment-refractory patients, a population with high unmet needs. Limited data exist on the clinical value of anti-tumour necrosis factor-α dose escalation in patients with complex perianal fistulas in *** Complex perianal fistulas in CD pose substantial clinical and humanistic burden. There is a need for effective treatments, especially for patients refractory to antitumour necrosis factor-α agents, as evidenced by high failure and relapse rates.

Assessing decontamination practices at a medical microbiology research laboratory

To our knowledge,this is the first study to conduct an objective assessment of the routine decontamination practices at a medical microbiology research laboratory(MRL)a year after a biosafety training was provided to all laboratory *** March 28th and June 28th,2021,unobtrusive observations were carried out to identify-three high-touch surfaces at the MRL during different working *** was used to evaluate the effectiveness of the disinfectant used in the *** three high-touch surfaces were sampled before and after decontamination with 200 ppm of 5%sodium hypochlorite(household bleach)to quantify the microbial load and identify the types of organisms residing on the laboratory surfaces.A higher concentration(500 ppm)of 5%sodium hypochlorite was employed after refresher training was provided to housekeeping staff,and resampling of the three surfaces was carried out during a 4-week follow-up period using the same *** three high-touch surfaces identified were the two sides of the workbench(22%–24%)and the front surface of one incubator(14%).Anthracoid bacilli and staphylococcus aureus were the most commonly found organisms on laboratory surfaces preintervention(100%and 89%,respectively)and post-intervention(56%and 44%,respectively).Other microorganisms detected included salmonella spp.(27.7%),Proteus spp.(5.6%),Escherichia coli(5.6%),and Klebsiella spp.(33.3%).Employing a higher concentration(500 ppm)of sodium hypochlorite significantly(ps study demonstrated suboptimal decontamination practices at the MRL and the need to apply a higher concentration(500 ppm)of sodium hypochlorite to reduce the overall microbial *** also demonstrated the importance of quantitative assessment to monitor decontamination practices and ensure staff *** studies are needed to identify bacterial communities within the laboratory,which will

Corneal sensitivity,ocular surface health and tear film stability after punctal plug therapy of aqueous deficient dry eye

AIM： To evaluate the effect of punctal occlusion using thermosensitive（smart plug） versus silicone plug for management of aqueous deficient dry eye on cornea sensitivity, ocular surface health and tear film ***： A comparative prospective interventiona case study included 45 patients with bilateral severe form of aqueous deficient dry eye. In each patient, the smar plug was inserted in the lower punctum of the right eye which was considered as study group 1 and silicone plug was inserted in the lower punctum of the left eye o the same patient which was considered as study group2. All patients were subjected to careful history taking and questionnaire for subjective assessment of severity of symptoms. Corneal sensitivity, corneal fluorescein rose bengal staining, schirmer＇s I test, tear film break up time and conjunctival impression cytology were performed pre and 1, 3 and 6mo post plug ***： A statistically significant improvement in subjective and objective manifestations occurred following treatment with both types of plugs（P 〈0.01）The thermosensitive plug caused significant overal improvement, decrease in frequency of application o tear substitutes and improvement of conjunctiva impression cytology parameters in the inserted side（P 〈0.01）. Canaliculitis was reported in two eyes（4.4% following punctal occlusion using thermosensitive plug（study group 1）. spontaneous plug loss occurred in 21eyes（46.6%） in the silicone plug group（study group 2）.CONCLUsION： Improvement of subjective and objective manifestations of aqueous deficient dry eye occurs following punctal plug occlusion. Thermosensitive plug has good patient＇s compliance with fewer complications and lower rates of loss compared to the silicone plug.
