Vitamin D deficiency: Correlation to interleukin-17, interleukin-23 and PⅢNP in hepatitis C virus genotype 4

AIM: To assess vitamin D (Vit D) abnormalities in hepatitis C infected patients and their relationship with interleukin (IL)-17, IL-23 and N-terminal propeptide of type Ⅲ pro-collagen (PⅢNP) as immune response mediators. METHODS: The study was conducted on 50 Egyptian patients (36 male, 14 female) with hepatitis C virus (HCV) infection, who visited the Hepatology Outpatient Clinic in the Endemic Disease Hospital at Cairo University. Patients were compared with 25 ageand sexmatched healthy individuals. Inclusion criteria were based on a history of liver disease with HCV genotype 4 (HCV-4) infection (as new patients or under followup). Based on ultrasonography, patients were classified into four subgroups; 14 with bright hepatomegaly; 11 with perihepatic fibrosis; 11 with hepatic cirrhosis; and 14 with cirrhosis and hepatocellular carcinoma (HCC).Total Vit D (i.e., 25-OH-Vit D) and active Vit D [i.e., 1,25-(OH) 2 -Vit D] assays were carried out using commercial kits. IL-17, IL-23 and PⅢNP levels were assayed using enzyme linked immunosorbent assay kits, while HCV virus was measured by quantitative and qualitative polymerase chain reaction. RESULTS: Levels of Vit D and its active form were significantly lower in advanced liver disease (hepatic cirrhosis and/or carcinoma) patients, compared to those with bright hepatomegaly and perihepatic fibrosis. IL-17, IL-23 and PⅢNP levels were markedly increased in HCV patients and correlated with the progression of hepatic damage. The decrease in Vit D and active Vit D was concomitant with an increase in viral load, as well as levels of IL-17, IL-23 and PⅢNP among all subgroups of HCV-infected patients, compared to normal healthy controls. A significant negative correlation was evident between active Vit D and each of IL-17, IL-23 and PⅢNP (r = -0.679, -0.801 and -0.920 at P fected men and women showed no differences with respect to Vit D levels. Octreotide ameliorates gastric lesions in chronically mild stressed rats

AIM:To evaluate the effect of chronic mild stress(CMS) on the emergence of gastric ulcers and possible modulation by octreotide,a synthetic somatostatin analogue. METHODS:Adult male Wistar rats were subjected to nine different unpredictable random stress procedures for 21 d,a multifactorial interactional animal model for *** was administered daily for 21 d at two dose levels(50 and 90μg/kg)before exposure to stress ***-and microscopical assessments were made,in addition to quantification of plasma corticosterone and gastric mucosal inflammatory,oxidative stress, and apoptotic biomarkers. RESULTS:Exposure to CMS elevated plasma corticosterone(28.3±0.6μg/dL,P=0.002),an event that was accompanied by gastric lesions(6.4±0.16 mm,P=0.01) and confirmed ***,the insult elevated gastric mucosal lipid peroxides(13±0.5 nmol/g tissue,P=0.001),tumor necrosis factor-α(3008.6±78.18 pg/g tissue,Pfurthermore,in animals exposed to CMS,octreotide restored plasma corticosterone(61%and 71%from CMS,P=0.002)at both dose *** beneficial effects were associated with a remarkable suppression of gastric lesions(38%and 9%from CMS,P=0.01)and reversal of derangements in gastric mucosa. CONCLUSION:The current investigation provides evidence that exposure to CMS induces gastric ulceration, which was alleviated by administration of octreotide possibly possessing antioxidant,anti-inflammatory,and anti-apoptotic actions.

Effectiveness of Telmisartan as an Adjunct to Metformin in Treating Type II Diabetes Mellitus in Rats

The monitoring reports of most patients with type II diabetes mellitus (T2DM) revealed that monotherapy with metformin could not achieve long-term glycemic control. Thus, we designed this study aiming to investigate the effect of telmisartan, a unique AT1 receptor antagonist and a partial agonist of peroxisome-proliferator activated receptor gamma (PPARγ), individually and as an adjunct to metformin, on a rat model that simulates the metabolic characteristics of human T2DM. Adult male Wistar rats were fed high-fat, high-fructose diet (HffD) for 8 weeks followed by a single low dose of streptozotocin (STZ) (35 mg/kg/day, i.p.) rendering them diabetic and insulin resistant. The effectiveness of both drugs and their combination was tested by assessing the changes in the levels of serum glucose, insulin, homeostasis model assessment of insulin resistance (HOMA-IR) index, lipid profile and adiponectin. In addition, the level of reduced glutathione (GSH) in the liver, was investigated. Results showed that the addition of telmisartan to metformin successfully restored serum glucose back to normal levels and corrected the altered serum total cholesterol (TC), triglycerides (TG) and adiponectin, emphasizing the potential role of telmisartan as an adjunct to metformin.

Phytotherapy in diabetes: Review on potential mechanistic perspectives

Diabetes mellitus(DM) is a widely spread epidemic dis ease that results from the absence of insulin, decreased secretion and/or impaired function. Since DM is a multi factorial disease, the available pharmaceuticals, despite their sensible treatment, target mostly one pathway to control hyperglycemia and encounter several side effects. Therefore, new therapeutic paradigms aim to hit several pathways using only one agent. Tradition ally, antidiabetic plants and/or their active constituents may fulfill this need. More than 200 species of plants possess antidiabetic properties which were evaluated mostly by screening tests without digging far for the exact mode of action. Searching among the differen literature resources and various database and in view o the above aspects, the present article provides a com prehensive review on the available antidiabetic plants that have been approved by pharmacological and clini cal evaluations, and which their mechanism(s) of ac tion is assured. These plants are categorized according to their proved mode of action and are classified into those that act by inhibiting glucose absorption from in testine, increasing insulin secretion from the pancreasinhibiting glucose production from hepatocytes, or enhancing glucose uptake by adipose and muscle tissues. The current review also highlights those that mimic in their action the new peptide analogs, such as exenatide, liraglutide and dipeptidylpeptidase-4 inhibitors that increase glucagon-like peptide-1 serum concentration and slow down the gastric emptying.

Evaluation of the nutritional value of Artemia nauplii for European seabass(Dicentrarchus labrax L.)larvae

Adapted European seabass(Dicentrarchus labrax L.)larvae at 25 days post-hatching(dph)with a primary weight and length of 23 mg and 9.92 mm,start the weaning up to 46 dph using the weaning micro-diet(54%crude protein from fish meal,powder milk,and poultry egg)concurrently with Artemia enriched with fortification emulsions(0.6 g/L)of DHA selco®as a control group(DHAS)or fish oil+20%Vitamin C(fOVC),or fish oil+20%Vitamin E(fOVE),or fish oil+10%Vit.C+10%Vit.E(fOCE),or fish oil only(fO).At 46 dph,groups of larvae fed enriched Artemia with DHAS and fOVE exhibited the highest final body weight,weight gain,average daily gain,specific growth rate,feed intake,protein efficiency ratio,and survival%as well as the lowest feed conversion ***,larvae in fOVC displayed the lowest final body weight,total length,weight gain,average daily gain,specific growth rate,feed intake,protein efficiency ratio,and survival%as well as the highest feed conversion *** histological assessment exhibited no pathological alteration as well as an improvement in the structure with DHAS,fOVE,fOVC,and fOCE co-additives compared to *** peroxidase enzyme(GPx)recorded the highest rates(Pfed Artemia supplemented with DHAS and fOVE followed by fOCE and *** the lowest record for GPx activity was noted in the fO *** conclusion,Using augmented Artemia nauplii with fish oil+vitamin E in single or in companion with vitamin C as cheap antioxidants support *** larval growth,survival,and antioxidant efficacy during the critical weaning period.

A New Evaluation Criterion for Optimizing the Mechanical Properties of Toughened Polypropylene/Silica Nanocomposites

This study aims to experiment with the mechanical properties of polypropylene(PP)/thermoplastic elastomer/nano-silica/compatibilizer nanocomposite using the melt mixing *** addition of polyolefin elastomers has proved to be an approachable solution for low impact strength of PP,while it would also reduce the Young's modulus and tensile *** is why reinforcement would be applied to this combination to enhance the elastic *** mechanical properties of the prepared composites were devised to train an artificial neural network to predict these properties of the system in 6256 unknown ***,the sensitivity analysis was performed and the share of each input parameter on the respective output values was ***,a novel parameter called nanocomposite evaluation criterion(NEC)is introduced to analyze the suitability of the nanocomposites considering the mechanical ***,the formulation with optimal mechanical properties of toughness,elongation at break,tensile strength,Young's modulus,and impact strength was obtained.

Alzheimer’s disease(AD)is a chronic and common form of dementia that mainly affects elderly *** disease is dangerous because it causes damage to brain cells and tissues before the symptoms appear,and there is nomedicinal or surgical treatment available yet *** loss of memory and functionality control in multiple degrees according to AD’s progression ***,early diagnosis of AD can hinder its *** imaging tools such as magnetic resonance imaging(MRI),computed tomography(CT)scans,positron emission tomography(PET),*** help in medical diagnosis of ***,computer-aided diagnosis(CAD)such as deep learning applied to brain images obtained with these tools,has been an established strategic methodology that is widely used for clinical assistance in prognosis of *** this study,we proposed an intelligent methodology for building a convolutional neural network(CNN)from scratch to detect AD stages from the brain MRI images dataset and to improve patient *** is worth mentioning that training a deep-learning model requires a large amount of data to produce accurate results and prevent the model from overfitting ***,for better understanding of classifiers and to overcome the model overfitting problem,we applied data augmentation to the minority classes in order to increase the number of MRI images in the *** experiments were conducted using Alzheimer’s MRI dataset consisting of brain MRI scanned *** performance of the proposed model determines detection of the four stages of *** results show high performance of the proposed model in that the model achieved a 99.38%accuracy rate,which is the highest so ***,the proposed model performance in terms of accuracy,precision,sensitivity,specificity,and f-measures is promising when compared to the very recent state-of-the-art domain-specific models existing in the literature.

Liver fat Scores for Noninvasive Diagnosis and Monitoring of Nonalcoholic fatty Liver Disease in Epidemiological and Clinical Studies

Nonalcoholic fatty liver disease(NAfLD)is strongly associated with the metabolic syndrome and type 2 diabetes and independently contributes to long-term *** often asymptomatic but reversible,it would require population-wide screening,but direct diagnostics are either too invasive(liver biopsy),costly(MRI)or depending on the examiner’s expertise(ultrasonography).Hepatosteatosis is usually accommodated by features of the metabolic syndrome(***,disturbances in triglyceride and glucose metabolism),and signs of hepatocellular damage,all of which are reflected by biomarkers,which poorly predict NAfLD as single item,but provide a cheap diagnostic alternative when integrated into composite liver fat *** liver index,NAfLD LfS,and hepatic steatosis index are common and accurate indices for NAfLD prediction,but show limited accuracy for liver fat *** indices are rarely *** fibrosis scores are commonly used in clinical practice,but their mandatory reflection of fibrotic reorganization,hepatic injury or systemic sequelae reduces sensitivity for the diagnosis of simple ***-induced liver fat changes are poorly reflected by liver fat indices,depending on the intervention and its specific impact of weight loss on *** limited validity in longitudinal settings stimulates research for new ***,hepatokines,markers of cellular integrity,genetic variants but also simple and inexpensive routine parameters might be potential ***,liver fat indices lack precision for NAfLD prediction or monitoring in individual patients,but in large cohorts they may substitute nonexistent imaging data and serve as a compound biomarker of metabolic syndrome and its cardiometabolic sequelae.
