Case Report: Dual Antiplatelet in Capsular Warning Syndrome

Background: Capsular warning syndrome (CWS) is defined as a recurrent stereotype transient lacunar syndrome without cortical symptoms. However, to date, there is limited literature proposed on the definitive treatment options for the prevention of persistent stroke in cases of capsular warning syndrome. Aim: The aim of this case report is to address dual antiplatelet therapy as a treatment choice in CWS to prevent progressive stroke. Case presentation: A 56-year-old man with several comorbidities experienced recurrent periods of limb weakness and dysarthria lasting 5 - 20 minutes with a complete resolution of symptoms in between. The fluctuating episodes lasted for 72 hours followed by a persistent minor stroke. A gadolinium-enhanced MRI scan of the brain revealed a right basal ganglia infarct. He was treated with dual antiplatelet, and his functional outcome was favourable at 90 days post stroke. Conclusion: Dual antiplatelet may be a beneficial therapeutic option in CWS in improving functional outcomes in the early neurological deficit and decreasing the clinical fluctuation.

Studies have indicated that lack of regular physical exercise and not adhering to a health-promot- ing lifestyle among student nurses are barriers to their encouraging patients to exercise. On the other hand, nurses who personally embrace physical activity are more likely to encourage their patients to improve their health through physical activity or diet. As a result, promoting an active and healthy lifestyle among student nurses is important because this has a crucial role in their subsequently encouraging healthy lifestyles among their patients. The aims of this study were to determine the physical activity levels and health-promoting lifestyles among student nurses in Malaysia, and to investigate whether there was a correlation between the physical activity levels of student nurses and their lifestyle habits. Further investigation was also carried out to correlate lifestyle habits with physical activity during weekdays and at the weekends. This study involved 189 student nurses who were pursuing Diplomas and Bachelor Degrees in local government universities. The Yamax Digi-Walker pedometer was used to measure physical activity, while the Health-Promoting Lifestyle Profile (HPLP) II questionnaire was used to assess the health-pro- moting lifestyles of the participants. The HPLP II consisted of six sub-scales which included health responsibility, physical activity, nutrition, spiritual growth, interpersonal relations, and stress management. According to the results on physical activity, the student nurses had a mean of 8275.97 ± 2210.174 steps per day and were thus categorised as somewhat active. Additionally, the student nurses were found to be more active during weekends (M = 8389.75, SD = 2689.12) as compared to weekdays (M = 8230.46, SD = 2353.97). According to previous studies, both step counts are classified in the “somewhat active” category. Some geological studies were performed in the Rafflesia Trail, near Kampung Jedip, in Lojing Highlands, Kelantan. This area is famous for its Rafflesia and has become one of the tourism attractions in Kelantan. However, the recent development of agricultural activities has significant effects on the area. The objective of these studies is to give substantial geological information including geomorphology, lithology, structural features, geohazard potentials, and water quality analysis in the study area. Desk study was conducted by reviewing some literatures related to the topic and the study area. Field work was organized during a scientific expedition in January 2014, to collect data, samples, and photographs. Geomorphologically, the study area is a mountainous area which mostly consists of mountain ridges and mountain valleys. Some fluvial features occur in the study area, such as waterfalls, cascades, rapids, runs, pools, potholes, lateral bars, and point bars. This area is composed of granitic rocks, mostly granite porphyry, and covered by superficial deposits of Quaternary age. The geological structure that is commonly found in the study area is joints, where they divide the rock body into large, roughly angular blocks, which is called as brecciation. The potential geohazard in the study area is landslides, where some of them are composed of soil only and others are mixtures of rock and soil. The water quality analysis has managed to identify that the stream water in this area generally has the quality of Class III (according to INWQS for Malaysia). These studies recommend that this area should be supported as a sustainable tourism site in Lojing Highlands.

Introduction: A psychoactive substance has become an overwhelming public health burden globally. It causes social problems for the user and surrounding people which may affect work or study and cause negative economic impact. Objective: This study aims to translate and culturally adapt the Alcohol, Smoking and Substance Involvement Screening Test (ASSIST) into Malay and to assess its reliability and validity. Methods: The Malay version of the ASSIST v 3.1 was developed after the translation and back-translation, which included the stages recommended by Beaton. The ASSIST v 3.1 was administered to 125 respondents. The Malay ASSIST v 3.1 was completed twice by each respondent 7 to 14-day intervals to assess test-retest reliability based on the intra-rater and interrater correlation coefficient. Results: Majority of the respondents were male, Malay and currently employed. The intra-rater reliability is 0.84 and the test-retest reliability (ICC = 0.97) were excellent. Conclusion: Malay ASSIST v3.1 was a valid and reliable tool to screen substances abuse at varying degree. Nonetheless, further studies are needed to assess its responsiveness.

The purpose of this study is to analyze the robust performance of equity-based sukuk in Malaysia, that is, Musharakah and Mudharabah contract. Accounting and Auditing Organization for Islamic Financial Institutions (AAOIFI), as an international standard-setting organization for Islamic finance, raised an issue that 85% issuances of Musharakah and Mudharabah sukuk have breached the shariah rulings and do not comply with Islamic principles (AAOIFI, 2002). Consequently, the performance of equity-based sukuk in Malaysia was affected. This study used a secondary data for size of issuances which are gathered from Bank Negara Malaysia (BNM)"s Bond Info website and Securities Commission (SC) website. The performance of equity-based sukuk, either Musharakah or Mudarabah, is robust in the study year period of 2000-2012 as regards their sizes of issuances.

Trichoderma species have shown efficiency on biocontrol of *** commercial application,it must be propagated in mass scale using a cost-effective *** an alternative way to effectively deliver biocontrol fungi inoculum to the field;seven agro-based wastes including rice bran,biochar,empty fruit bunches,coconut fibre,compost,topsoil and mixed soil were used in this study for evaluating mass multiplication of Trichoderma *** on the evaluation of colony-forming units(cfu)among the agro-based waste media used,coconut fibre is the most suitable in promoting the sporulation of Trichoderma asperellum and *** asperellum C1667 showed the higher micropropagule count through incubation period compared to *** *** 120 days on the agro-based waste media,*** C1667 and *** C1675 produced the highest(7.717×10^(5)cfu/g and 6.836±13.79×10^(5)cfu/g)coconut fibres,***,the mixed soil appeared with the lowest *** fibres were shown as a great biocomposting medium for both Trichoderma *** of the present study are valuable for disease management using agro-based wastes as a cost-effective medium for biocontrol agents like Trichoderma species.

A dichotomous key and descriptions of the species of pitcher plants recorded from bris forest at Jambu Bongkok is *** species of pitcher plants were recognized from the study *** are Nepenthes rafflesiana,Nepenthes gracilis and Nepenthes *** species can easily be identified by their morphological *** gracilis differed from the other two species in having triangular stem,sessile leaves,winged leaf base,under lid surface of the pitcher sparsely covered with honey glands,inner pitcher cavity wall covered exposed digestive glands and very narrow peristome with the thickness of≤1 *** the other hand,Nepenthes rafflesiana and Nepenthes mirabilis have cylindrical stem,stalked leaves,semi-amplexicaul leaf base,under lid surface densely covered with honey glands,inner pitcher cavity wall covered with overarched digestive glands and broad peristome with a thickness of≥1 *** rafflesiana can be differentiated from Nepenthes mirabilis by its infundibulate upper pitcher,ellipsoidal lower pitcher,inner pitcher wall cavity wholly covered with digestive glands whereas Nepenthes mirabilis have tublose-ventricose upper and lower pitchers,inner pitcher cavity wall of both pitchers partly covered with digestive glands.
