Intelligent traffic control requires accurate estimation of the road states and incorporation of adaptive or dynamically adjusted intelligent algorithms for making the *** this article,these issues are handled by proposing a novel framework for traffic control using vehicular communications and Internet of Things *** framework integrates Kalman filtering and *** smoothing Kalman filtering,our data fusion Kalman filter incorporates a process-aware model which makes it superior in terms of the prediction *** traditional Q-learning,our Q-learning algorithm enables adaptive state quantization by changing the threshold of separating low traffic from high traffic on the road according to the maximum number of vehicles in the junction *** evaluation,the model has been simulated on a single intersection consisting of four roads:east,west,north,and south.A comparison of the developed adaptive quantized Q-learning(AQQL)framework with state-of-the-art and greedy approaches shows the superiority of AQQL with an improvement percentage in terms of the released number of vehicles of AQQL is 5%over the greedy approach and 340%over the state-of-the-art ***,AQQL provides an effective traffic control that can be applied in today’s intelligent traffic system.

Diabetes is a chronic condition that is significantly impacted daily by environment. At least one in five Saudis, suffer from diabetes. “Diabetes is a pandemic of unprecedented magnitude spiraling out of control” (Boulton). Saudi Arabia ranks the second highest in the Middle East, and is the seventh in the world for the prevalence of diabetes (WHO). Diabetes mellitus (DM) has been found to be related to high mortality, morbidity accompanied by poor general health and lower quality of life. Current Saudi diabetes patients’ behaviors, practices, and beliefs regarding foot health problems such as ulcers, are not well investigated. Diabetic patients frequently utilize natural remedies and Herbs for self-medication as a part of complementary and alternative Medicine. Aim of study: To explore the widespread use of the alternative remedies in the local communities of the Western Saudi Arabia, the drives behind using and to focus on the potential adverse effects following their usage. Method: A descriptive, cross-sectional study of 386 diabetic patients with feet problems in Makkah Region, West of Saudi Arabia during the period April - June 2023. Results: Out of the 531 participants, only 386 participants reported diabetic foot disorder (DFD). Most of the participant were above 65 years of age group (24.61 %). Male (52.08%) more than female (47.92%) in the research group. All of the participants had diabetic foot disorders at least once. The prevalence of complementary and alternative medicines (CAM) use among them was 67.88%, as solitary or in combination with hospital medicines. Honey headed the list followed by myrrh and black seeds. Acquaintances advice (59.54%) was the most reason for using the alternative medicine. 60.30% of Diabetic foot patient complained of adverse effects during CAM remedies use. Hypoxic hepatitis, also known as ischemic hepatitis, is characterized by acute hepatocellular injury due to inadequate oxygen delivery to the liver. Celiac trunk stenosis can lead to hepatic ischemia and subsequent liver damage. We present the case of an 81-year-old patient with a history of hypertension, ischemic heart disease, hypothyroidism, and biliary lithiasis, who developed hypoxic hepatitis secondary to Dunbar syndrome and a stenosis of the superior mesenteric artery. The patient improved symptoms and liver function tests with conservative management, including intravenous fluids and supportive care. Long-term management involved continued antiplatelet therapy and statins, with consideration of further interventions for celiac trunk stenosis. This case underscores the importance of recognizing Dunbar syndrome as well as superior mesentery trunk stenosis as a potential cause of hypoxic hepatitis. It highlights the need for multidisciplinary management in such cases.

Objective: The great saphenous vein (GSV) is commonly used as a conduit for grafting during CABG surgery, and open GSV harvesting (OVH), commonly used with long incision to expose the vein. However, endoscopic vein harvesting (EVH) is an alternative approach, utilizing specialized instruments and small incisions to harvest the vein. Methods: A retrospective analysis was conducted on a cohort of patients who underwent Coronary artery bypass graft (CABG) requiring great saphenous vein (GSV) harvesting which was done by EVH or OVH procedures. Demographic variables, including age and gender, were assessed for both groups. Intraoperative variables such as the number of grafts, cardiopulmonary bypass time, X clamp time, and type of procedure were analyzed. Postoperative variables, including infection and bleeding rates, were also evaluated. Results: The study included 30 patients each undergoing Coronary artery bypass graft (CABG) with need of great saphenous vein harvesting which was done by EVH and OVH. Demographic variables were well-matched between the two groups in terms of age, while a significant difference in gender distribution was observed. Obesity and smoking were more prevalent in the OVH group, and EVH was associated with a higher mean number of grafts compared to OVH. Conversion to an open technique occurred in a portion of the EVH cases, and infection rates did not significantly differ between the EVH and OVH groups. However, the incidence of postoperative bleeding was significantly higher in the EVH group. Conclusion: This study provides valuable insights into the demographic, intraoperative, and postoperative variables associated with EVH and OVH techniques. EVH demonstrated advantages in terms of reduced infection rates compared to OVH. However, the higher incidence of postoperative bleeding associated with EVH raises concerns about potential risks.

Publishing big data and making it accessible to researchers is important for knowledge building as it helps in applying highly efficient methods to plan,conduct,and assess scientific ***,publishing and processing big data poses a privacy concern related to protecting individuals’sensitive information while maintaining the usability of the published *** anonymization methods,such as slicing and merging,have been designed as solutions to the privacy concerns for publishing big ***,the major drawback of merging and slicing is the random permutation procedure,which does not always guarantee complete protection against attribute or membership ***,merging procedures may generatemany fake tuples,leading to a loss of data utility and subsequent erroneous knowledge *** study therefore proposes a slicingbased enhanced method for privacy-preserving big data publishing while maintaining the data *** particular,the proposed method distributes the data into horizontal and vertical *** lower and upper protection levels are then used to identify the unique and identical attributes’*** unique and identical attributes are swapped to ensure the published big data is protected from disclosure *** outcome of the experiments demonstrates that the proposed method could maintain data utility and provide stronger privacy preservation.
