Sonographic gallbladder wall thickness measurement and the prediction of esophageal varices among cirrhotics

Acute variceal bleeding in patients with liver cirrhosis and portal hypertension(PHT) is the most serious emergency complication among those patients and could have catastrophic outcomes if not timely managed. Early screening by esophago-gastro-duodenoscopy(EGD) for the presence of esophageal varices(EVs) is currently recommended by the practice guidelines for all cirrhotic patients. Meanwhile, EGD is not readily accepted or preferred by many patients. The literature is rich in studies to investigate and validate non-invasive markers of EVs prediction aiming at reducing the unneeded endoscopic procedures. Gallbladder(GB) wall thickness(GBWT) measurement has been found promising in many published research articles. We aim to highlight the validity of sonographic GBWT measurement in the prediction of EVs based on the available evidence. We searched databases including Cochrane library, Pub Med, Web of Science and many others for relevant articles. GBWT is associated with the presence of EVs in cirrhotic patients with PHT of different etiologies. The cut-off of GBWT that can predict the presence of EVs varied in the literature and ranges from 3.1 mm to 4.35 mm with variable sensitivities of 46%-90.9% and lower cutoffs in viral cirrhosis compared to non-viral, however GBWT > 4 mm in many studies is associated with acceptable sensitivity up to 90%. Furthermore, a relation was also noticed with the degree of varices and portal hypertensive *** cirrhotics, GBWT > 3.5 mm predicts the presence of advanced(grade Ⅲ-Ⅳ) EVs with a sensitivity of 45%, the sensitivity increased to 92% when a cut-off ≥ 3.95 mm was used in another cohort. Analysis of these results should carefully be revised in the context of ascites, hypoalbuminemia and other intrinsic GB diseases among cirrhotic patients. The sensitivity for prediction of EVs improved upon combining GBWT measurement with other non-invasive predictors, e.g., platelets/GBWT.

A Transfer Learning Approach Based on Ultrasound Images for Liver Cancer Detection

Facilitators and barriers to colorectal cancer screening in an outpatient setting

Seismic Imaging and Reservoir Architecture of Sub-Marine Channel Systems Offshore West Nile Delta of Egypt

Offshore Nile Delta gas reservoirs are dominated by slope-channel systems of Plio-Pleistocene age. High-quality, three-dimensional seismic imaging has significantly helped in defining the geomorphology and architectures of these channels. Integrating seismic, logs and core data from four wells resulted in understanding of different stages of channel development and reservoir quality. The studied reservoirs that are largely controlled by episodes of transgressive-regressive events resulted in deposition of fine grained sediment and shale. Sienna channel complex consists of unconfined channel system with clearly defined development stages. The stages include amalgamated or stacked channels followed by channel abandonment phases and local flooding events. The depositional pattern continued through the Late Pliocene-Pleistocene. SimSat-P1 and SimSat-P2 reservoirs are characterized by isolated sand bodies, most probably relics of fan depositional setting. The depositional scenario that is largely controlled by successive transgression and flooding events resulted in deposition of interbedded, sheet-like, fine grained sediment and shale.

粗糙集是一种处理含糊和不确定性信息的新型数学工具,其主要思想是,在保持信息系统分类能力不变的前提下,通过知识约简,导出问题的决策或分类规则。但是,数据的不完备或者对数据直接分类,往往会对做出的决策产生影响。当人们对决策问题缺乏数据时,常常采用层次分析法来进行量化,其基本原理是,将复杂问题进行等价分解,形成阶梯层次结构,通过两两比较,确定各因素的相对重要性,然后综合决策者的判断,确定决策方案相对重要性的总排序。本文针对AHP层次分析法评价模型中评价因子主观确定的不足,运用改进的加权平均属性重要度启发式约简算法,将粗糙集和层次分析法相结合,提出了一种全新的组合预测方法。该方法先用层次分析法进行特征化,把结果运用到基于粗糙集的组合预测方法中,构造出决策表。用组合预测方法计算属性的权系数,将权系数的确定转化为属性重要度问题,经简单的定量计算,得到各个指标的重要度,弥补层次分析法的主观性带来的偏差,使筛选更加客观可信。然后用组合预测方法进行相应处理,从而更加客观地做出最优决策,提高计算效率。此外,这个方法由计算机完成评价因子自动选取,建立合理的评价模型。同时也会产生一些易于理解的规则,从而更方便地应用于知识库中。

Evaluating the Performance of Fault-Tolerant S2A vs. In-Loop Controller Models for Ethernet-Based NCS

Two different controller-level fault-tolerant models for Ethernet-based Networked Control Systems (NCSs) are presented in this paper. These models are studied using unmodified Fast and Gigabit Ethernet. The first is an in-loop controller model while the second is a direct Sensor to Actuator (S2A) model. OMNeT++ simulations showed the success of both models in meeting system end-to-end delay and strict zero packet loss (with no over-delayed packets) requirements. It was shown in the literature that the S2A model has a lower end-to-end delay than the in-loop controller model. It will be shown here that, on the one hand, the in-loop fault-tolerant model performs better in terms of less total end-to-end delay than the S2A model in the fault-free situation. While, on the other hand, in the scenario with the failed controller(s), the S2A model was shown to have less total end-to-end delay.

目前的研究把数据库系统作为计算机科学的重要组成部分。选择注册过程作为应用研究是因为处理注册过程的信息系统是非常需要的,它将减少时间和学生及注册操作者的工作量。同时为学生的信息提供了安全保证。本文的研究选择苏丹的Nile Valley大学贸易经济管理学院作为一个案例。使用VB6作为系统平台、CR6作为报告处理过程工具。本文采用描述和分析的方法来解释大学数据库和注册过程。论文包括下面部分: 首先是数据管理系统的简单回顾,然后是分析学生注册系统,包括:关系数据管理系统中的实体关系数据模型,学生注册系统的关系表,用Microsoft Access创建数据表。现在我们已经准备好注册系统的设计、执行及性能分析,包括系统环境的选择、用Visual Studio6设计系统的界面、通过ADO访问数据库中的数据。数据安全防止未被授权的用户查看或更新数据库。为了实现学生的注册系统,研究包括下面的两个过程: 1、在执行文件里获得在线注册系统:首先出现登陆页面要求输入用户ID和密码,点击OK按钮后,系统触发主页面。 2、性能测试包括测试系统环境和测试结果。目前的研究提供了有力的结论验证了注册系统的过程。将来的研究应该更注意改进注册系统的方法的提高,包括关系数据库、系统界面应用和在线注册系统。
