Introduction: Congenital chylothorax, defined by the presence of chyle in the pleural cavity, is a rare congenital disorder of varying severity. The objective of this study was to report a case of idiopathic congenital chylothorax treated with povidone iodine effectively without side effects after failure of conservative treatment by chest drainage. Case presentation: A term female Newborn was admitted in the neonatal intensive care unit at 16 days of life, for respiratory distress. The Clinical examination objectified a Sliverman score of 3/10 with a pleural effusion syndrome. The chest radiography confirmed the pleural effusion and the analysis of the pleural fluid after aspiration confirmed the chylothorax, with a protein rate of 35 g/l, Triglycerids 37 mg/L, and Leukocytes count was 1140/ml with 90% lymphocytes. A chest drainage was performed on the seventh day but followed by the recurrence of the effusion. The use of 5 ml of 4% povidone iodine in the pleural space as a chemical pleurodesis was performed. The clinical and radiological evolution was favorable after 24 hours without relapse or side effects such as allergic reaction or worsening of the respiratory condition. Thoracic ultrasound was performed after 15 days, having objectified minimal right side effusion measuring 5 mm maximum thickness. Conclusion: Chemical pleurodesis by the polyvidone iodine in the chylothorax appears to be affordable and effective, and needs to be more considerate in the conservative treatment of congenital chylothorax.

In this work, the authors consider the applications of sliding mode controllers to the longitudinal flight dynamics of the benchmark system of scaled Yak-54 Unmanned Aerial Vehicle for executing maneuvers over full flight *** of unmanned aerial vehicle involves the problem of incomplete measurements, modeling uncertainties and external disturbances. Sliding mode control provides good precision and robustness to the aforementioned factors. First order sliding mode controller are designed but they provide chattering at the control input thus a non linear second order super twisting controller is used to provide chattering free control with improved performance for the robust stabilization of pitch angle and control of its altitude. The simulation results indicate good tracking and regulation control of the vehicle.

Internal carotid artery (ICA) dissection with associated stroke is rare event in children. The usual mechanism is either direct trauma or sudden neck movements. We describe the case of a 3-year-old patient admitted with right hemiplegia following a stroke. The diagnosis was confirmed by ANGIO-mRI. During treatment with anticoagulants and antiplatelet agents, the patient showed significant improvement in the right-sided hemiplegia. minor head trauma is a possible pathological mechanism for ICA dissection in children. However, the scenario is extremely rare.

Background: Congenital cystic adenomatoid malformation (CCAm) is a congenital anomaly of lung development, accounting for approximately 25% of congenital lung lesions. Respiratory distress often occurs during the neonatal period, and in 80% to 85% of cases, the diagnosis is made before the age of 2 years following respiratory infections. Case Report: We report a case of mAKC diagnosed in the neonatal period. The diagnosis was based on clinical, radiological and histological elements. Our patient underwent surgical resection. Histological examination confirmed the diagnosis of mAKC without any sign of malignancy. The postoperative evolution was good. Conclusion: Clinicians and pathologists should recognize the early discovery of mAKC in neonatal age. The clinical diagnosis strongly guided by the radiological approach is confirmed by the pathological anatomy insofar as the therapeutic sanction is surgical in the majority of the cases.

This study aims to compare the performance of two hydrological models, conceptual (HEC-HmS) and spatial (ATHYS) on the Aguibat Ezziar watershed. The comparative analysis is based on the performances of simulation in terms of Nash-Sutcliffe and RSR. The study requires the collection of a series of data as inputs models namely rainfall data, water quantity, soil occupation, DTm and requires also a calibration in order to evaluate these models in validation phase. The simulation results were obtained from the validation phase aiming to replicate the operation of watershed Aguibat Ezziar, and present a suitable adjustment perspective of the observed hydrograph. These results show that the objective is achieved and a model distributed like ATHYS plays an effective role to improve the efficiency and present a high advantage in anticipation of runoff volume comparing with other models.

BACKGROUND Endoscopic ultrasonography(EUS)has become an established method in diagnostic and therapeutic procedures in gastroenterology;however,it has recently gained a growing role in *** To evaluate the role of EUS features,strain elastography(SE),and EUS-tissue acquisition in diagnosing hepatic focal lesions(HFLs)that could affect further *** This cross-sectional study included 215 patients with pancreatic,biliary,or gastrointestinal malignancies referred for EUS *** were identified in 43 patients(20%),and EUSguided tissue acquisition was performed from these *** EUS features were highly sensitive(100%)but much less specific(57%)in diagnosing HFLs;the overall accuracy was 94%.Real-time elastography was also very sensitive(97%)but less specific(67%)in diagnosing HFLs;however,the overall accuracy was 92%.EUS tissue acquisition was extremely sensitive(100%)and specific(100%),with a 100%overall diagnostic *** The diagnostic utility of EUS-guided tissue acquisition was extremely accurate in diagnosing *** characteristics and real-time SE accurately predicted the histological diagnosis of both benign and malignant HFLs.

There is a growing body of evidence showing a close correlation between left ventricular mass with cardiovascular morbidity and overall mortality. Therefore, identifying the determinants of left ventricular hypertrophy can be of great importance for cardiovascular prevention, for prognosis and therapeutic intervention. Objective: To assess the prevalence and identify the independent determinants of echocardiographic left ventricular hypertrophy in The mA-Ghreb and Sub-Saharan Africa Left-Ventricul ArGEometry Study (mAG-SalVAGES) participants. methods: The mAG-SalVAGES is a community based study in which 100 asymptomatic Black Sub-Saharan African (BSSA) and 189 white skin maghreb within the age of 18 to 55 years underwent a resting echocardiography. multivariate logistic regression analysis was utilized to identify the independent determinants of LVH left ventricular hypertrophy. Results: men represented the majority of the enrolled participants: 173 (59.9%). Echocardiographic left ventricular hypertrophy was seen in 10 (3.5%) participants. Age ≥40 years, female gender, overall obesity, abdominal obesity, hypertension status and less fruit consumption were significantly associated with echocardiography left ventricular hypertrophy. After adjusting for confounding factors, age ≥40 years, female gender, abdominal obesity and less fruit consumption were independently and significantly associated with echocardiographic left ventricular hypertrophy, as illustrated in the following equation: Y = 0.36 + 0.162 age >40 years + 2.69 female gender + 2.52 abdominal obesity + 1.31 less fruit consumption. Conclusion: Lifestyle changes for the prevention of abdominal obesity and encouraging fruit consumption may be beneficial in preventing left ventricular hypertrophy.

The Influence of Ethnicity in the Relationship between Sedentary Screen Time and Left Ventricular mass: Insights from the mAG-SalVAGES

Background: The influence of race/ethnicity on the relationship between sedentary screen time and left ventricular mass has been recently suggested, but remains a subject of debate, and has never been explored in Africa. Purpose: To determine whether there is a racial/ethnic influence on the relationship between sedentary screen time and left ventricular mass in mAGhreb and Sub Saharan Africa Left-Ventricul ArGEometry Study (mAG-SalVAGES) participants. methods: 100 blacks sub-Saharan African and 187 white maghreb aged 18 - 55 years underwent an interview on their behavioral measures, physical activity and eating habits. Their left ventricular mass has also been measured by a resting transthoracic echography according to the American Society of Echography. Generalized linear models evaluated a test-for-trend across higher levels of sedentary screen time in progressive models with left ventricular measurements as dependent variables. The study population was stratified into quartiles of sedentary screen time (separately for whites and blacks) and examined the joint association of sedentary screen time and LVm within quartiles of physical activity. Results: Among White maghreb, higher screen time was associated with smaller left ventricular mass (P Conclusions: Sedentary screen time is associated with smaller left ventricular mass in White maghreb, not in black sub-Saharan African. The lack of association in blacks supports a potential qualitative difference in the cardiovascular consequences of sedentary screen based behavior.

Choledochal cyst is defined as a cystic dilatation of the distal common bile duct protruding into the duo- denum. It is considered as the rarest congenital cyst of the biliary tract by 1.4%. We report a 46 years old woman who presented with recurrent jaundice associated with abdominal pain of 07 years duration. The liver function tests showed cholestasis. The abdominal scan and Biliary mRI revealed a dilatation of intrahepatic, pancreatic ducts and a dilatation of the common bile duct with a stenosis in its the lower part. The diagnosis of a common bile duct cholangiocarinoma was mentioned. The gastroscopy revealed a stenosed duodenal bulb not allowing us to perform an echo endoscopy and ERCP. An intraoperative cholangiography illustrating a cystic dilatation of the papillary region in which exist a separate protrusion of the choledochal and wirsung ducts. Therefore, we didn’t accomplish the cephalic duodeno-pancreatectomy and we decided to realize a partial resection of the papilla. The histolological examination proved the absence of any tumoral lesion and the presence of biliary mucosa layered the internal surface of the cyst. The patient is still asymptomatic after one year of the surgery.
