The intercostobrachial nerve (ICBN) is responsible for the sensory innervation of a part of the inner side of the arm. Injury of the intercostobrachial nerve is a complication of axillary dissection during lymph node dissection. Objective: This study aimed to determine the effect of preservation of the intercostobrachial nerve on postoperative sensory disturbances. Methods: This is a prospective, single-center study which was carried out in 90 patients followed in the oncology department of the Aristide Le Dantec hospital in Dakar, suffering from breast cancer and having undergone breast surgery associated with axillary dissection, for a period of 6 months. The patients were divided into two groups depending on whether the intercostobrachial nerve (ICBN) was preserved or not. The two groups were compared in terms of the incidence of sensory disturbances. Results: Sixty patients without nerve preservation and 30 patients with nerve preservation were included in the study, i.e., 90 patients in total.

Although high density lipoprotein(HDL)-mediated reverse cholesterol transport is crucial to the prevention and reversal of atheroma, a recent meta-analysis makes evident that current pharmaceutical strategies for modulating HDL cholesterol levels lower cardiovascular risk only to the extent that they concurrently decrease low density lipoprotein(LDL) cholesterol. This corresponds well with findings of a recent Mendelian randomization analysis, in which genetic polymorphisms associated with HDL cholesterol but no other known cardiovascular risk factors failed to predict risk for myocardial infarction. Although it is still seems appropriate to search for therapies that could improve the efficiency with which HDL particles induce reverse cholesterol transport, targeting HDL cholesterol levels per se with current measures appears to be futile. Itmay therefore be more promising to promote reverse cholesterol transport with agents that directly target foam cells. Macrophage expression of the cholesterol transport proteins adenosine triphosphate binding cassette transporter A1, adenosine triphosphate binding cassette transporter G1, and scavenger receptor class B member 1 is transcriptionally up-regulated by activated liver X receptors(LXR), whereas nuclear factor(NF)-kappa B antagonizes their expression. Taurine, which inhibits atherogenesis in rodent studies, has just been discovered to act as a weak agonist for LXRalpha. Conversely, it may be possible to oppose NF-kappa B activation in macrophages with a range of measures. Induction of heme oxygenase-1, which can be attained with phase 2 inducer phytochemicals such as lipoic acid and green tea catechins, promotes reverse cholesterol transport in macrophages and inhibits atherogenesis in rodents, likely due to, in large part, NF-kappa B antagonism. 本文主要研究了树莓花色苷微胶囊及其相互作用模式。采用冷冻干燥和超声技术分别制备了以阿拉伯胶(gum arabic,AG)、大豆分离蛋白(soy protein isolate,SPI)及二者组合为壁材的花色苷微胶囊。对于花色苷微胶囊的研究,主要分析了花色苷微胶囊的特性、包埋率(encapsulation efficiency,EE)、清除DPPH自由基的抗氧化能力,傅立叶变换红外光谱、色泽稳定性、热稳定性、储藏稳定性、形状及体外模拟消化特性。结果表明,所有微胶囊的包埋率为93.05%～98.87%。通过扫描电镜(scanning electron microscopy,SEM)观察到的微胶囊形状为破碎的玻璃形状,微胶囊的平均尺寸为21.07±2.71μm～48.19±2.33μm。ζ电位(zeta potential,ZP)与FTIR光谱之间有良好的相关性,表明壁材和芯材之间存在化学交联作用。制备的微胶囊均增加了花色苷在80～114?C下的热稳定性。此外,花色苷在37℃下储存60天的过程中保留率高达48%,表明SPI和AG结合的H处理(SPI+AG+0.05%花色苷)最佳。此外,与未包埋的花色苷相比,以SPI和AG共同作为壁材的花色苷微胶囊在模拟胃肠道条件下具有良好的释放特性。以上结果表明,大豆蛋白作为壁材使花色苷有较好的稳定性,因此,本研究还进一步通过光谱学和稳定性研究探讨了纳米壁材大豆蛋白纳米凝胶与花色苷之间的相互作用。相互作用结果表明,花色苷主要通过静态猝灭方式猝灭大豆蛋白的荧光强度。蛋白质与花色苷的结合位点数(n)接近1.0,在298 K时的结合常数(ka)为1.572*10 L/M,表明花色苷和大豆蛋白纳米凝胶之间有很强的结合力。FTIR,UV-vis和圆二色谱的结果表明,花色苷改变了大豆蛋白纳米凝胶的二级和三级结构。此外,随着花色苷浓度的增加,大豆蛋白纳米凝胶的能带强度降低。这些结果表明,微胶囊是一种提高树莓花色苷稳定性、改善花色苷模拟胃肠消化特性的良好方法。这些结果有助于更好地理解花色苷与大豆蛋白纳米凝胶之间的相互作用,促进大豆蛋白纳米凝胶在食品花色苷上的应用。因此,微胶囊可以作为一种提高树莓花苷稳定性的有效方法,而花色苷和壁材的相互作用增强了花色苷的理化特性,有助于花色苷在食品上的应用。

Outcomes of External-Beam Radiation Therapy Boost with Conventional Fractionation in Cervical Cancer: A Retrospective Analysis about 133 Cases

Introduction: External-beam radiation therapy boost is a treatment option in cervical cancer when brachytherapy is not feasible. Though less effective than brachytherapy, some encouraging results have been reported from some institutions experiences. We conducted this study to assess outcomes of EBRT boost for our patients at National Institute of Oncology in Rabat. Patients and Methods: We collected data from patients treated for cervical cancer between January 2012 and December 2015. Patients, tumor and treatment characteristics were collected. Overall survival (OS), disease-free survival (DFS) and prognostic factors influencing DFS were assessed. Results: One hundred and thirty-three patients were enrolled. Median age was 52 years. Patient haemoglobin level ranged from 3.9 to 15.5 g/dl [mean: 11.2 g/dl]. Most tumors were classified stage III/IVA (63.2%) according to the FIGO classification. Regional lymph node metastases (pelvic and or para-aortic) were observed in 45.1%. Median total dose to tumor was 69.6 Gy (ranging from 66 to 70 Gy). Overall treatment time was protracted, with a median of 60 days. Most of patients received concurrent chemotherapy (94.7%) and the number of cycle ranged from 2 to 7 (median = 5). The follow-up median was 31.3 months, ranging from 6.2 to 96.8 months. At the first visit, most patients achieved complete response (80.5%). Five years OS and DFS were 47% and 44% respectively. Although lanthanide double-decker complexes with hetero-macrocyclic ligands as functional luminescent and magnetic materials have promising properties, their inferior water solubility has negated their biomedical applications.[1] Herein, four water-soluble homoleptic lanthanide（Ln = Gd, Er, Yb and La） sandwiches with diethylene-glycol-disubstituted porphyrins（DD） are reported, with their structures proven by both quantum chemical calculations and scanning tunneling microscopy. Our findings demonstrate that the near-infrared emission intensity and singlet oxygen（1 O2） quantum yields of YbDD and GdDD in water are higher than those of the reported capped lanthanide monoporphyrinato analogues, YbN and GdN;the brightness and luminescence lifetime in water of YbDD are greater than those of YbN. This work provides a new dimension for the future design and development of molecular theranostics-based water-soluble double-decker lanthanide bisporphyrinates.[2]
