Evaluation of.Dif.erent Ultrasonographic Modalities in the Diagnosis of.Morbidly Adherent Placenta: A Cross-Sectional Study

Objective: To compare the accuracy of.dif.erent ultrasonographic modalities;two-dimensional ultrasound (2D-US), color Doppler and three-dimensional power Doppler (3D-PD) in the antenatal diagnosis of.the morbidly adherent placenta. Setting: Obstetrics and Gynecology Department, f.culty of.Medicine, Assiut University, Assiut, Egypt. Study Design: A cross-sectional study. Methods: All patients f.lf.ll the inclusions criteria: gestational age > 28 weeks, previous one or more cesarean delivery, previous uterine surgery, placenta previa, vitally stable patient and women accepted to participate in the study were included. All patients were evaluated using 2D-US, color Doppler and 3D-PD bef.re delivery. The f.nal diagnosis was established by laparotomy and by histopathology of.hysterectomy sample if.hysterectomy would be done. Results: One-hundred f.f.y patients were enrolled in the study. 2D-US has higher sensitivity (86.96%) than 2D color Doppler (84.06%) and 3D-PD (79.71%) in the diagnosis of.placenta accreta. On the other hand, 3D-PD has slightly higher specif.city (83.95%) than color Doppler (82.72%) and 2D-US (77.78%) in the diagnosis of.placenta accreta. The most sensitive parameter in 2D-Us was the loss of.retroplacental sonolucent zone (86.96%). As regards color Doppler, the most sensitive parameter was the hypervascularity of.the uterine-bladder interf.ce (84.06%). Tortuous vascularity with chaotic branching was the most sensitive parameter in 3D-PD with a sensitivity of.82.61%. Conclusions: The use of.3D power Doppler with both 2D-US and color Doppler as complementary techniques could improve the antenatal diagnosis or exclusion of.morbidly adherent placenta.

Objective:Rheumatoid arthritis(RA)requires comprehensive *** nursing protocols may enhance outcomes,but evidence is *** study evaluated the ef.ect of.a structured nursing protocol on RA *** and Methods:In this one-group pre-post study,30 Egyptian RA patients completed assessments bef.re and af.er a 12-week nursing protocol comprising education,psychosocial support,and self.management *** included clinical evaluation of.joint counts,erythrocyte sedimentation rate(ESR),and C-reactive protein(CRP)and patient-reported Arthritis Self.Ef.icacy Scale(ASES),Health Assessment Questionnaire(HAQ),Visual Analog Scale(VAS)f.r pain,and Hospital Anxiety and Depression Scale(HADS).Results:The study demonstrated signif.cant improvements in both clinical-and patient-reported *** count decreased f.om 18.4±4.2 to 14.2±3.8(Pf.om 30.1±6.8 mm/h to 25.5±6.8 mm/h(Pf.om 15.2±3.6 mg/L to 11.8±2.9 mg/L(Pf.om 140±25 to 170±30(Pf.om 1.6±0.4 to 1.3±0.3(Pf.om 7.8±1.7 to 6.2±1.2(Pf.om 11±3 to 8±2(Pf.cantly improved clinical disease activity,physical f.nctioning,pain,self.ef.icacy,and emotional well-being in RA patients.A multif.ceted nursing intervention appears benef.cial f.r optimizing RA outcomes.

Background Resuscitation can sometimes be f.tile and making a do-not-resuscitate(DNR)decision is in the best interest of.the *** electronic poor outcome screening(ePOS)score was developed to predict 6-month poor outcomes of.critically ill *** explored the diagnostic accuracy of.the ePOS score in predicting DNR decisions in the intensive care unit(ICU).Methods This study was conducted at the ICU of.a tertiary ref.rral hospital in Saudi Arabia between March and May ***,we calculated ePOS scores f.r all eligible consecutive admissions af.er 48 h in the ICU and recorded the DNR *** ability of.the score to predict DNR was explored using logistic ***'s ideal cut-of. value was calculated using the DeLong method,and dif.erent diagnostic accuracy measures were generated with corresponding 95%conf.dence intervals(CIs).Results We enrolled 857 patients,125 received a DNR order and 732 did *** average ePOS score of.DNR and non-DNR patients was 28.2±10.7 and 15.2±9.7,*** score,as a predictor of.DNR order,had an area under receiver operator characteristic(AUROC)curve of.81.8%(95%CI:79.0 to 84.3,Pf. value>17 was associated with a sensitivity of.87.2(95%CI:80.0 to 92.5,Pf.city of.63.9(95%CI:60.3 to 67.4,Pf.29.2(95%CI:24.6 to 33.8,Pf.96.7(95%CI:95.1 to 98.3,Pf.rmed well as a diagnostic test f.r patients who will be labeled as DNR during their ICU stay.A cut-of. score>17 may help guide clinical decisions to withhold or commence resuscitative measures.

A lightweight f.exible thermally stable composite is f.bricated by com-bining silica nanof.ber membranes(SNM)with MXene@c-MWCNT hybrid *** f.exible SNM with outstanding thermal insulation are prepared f.om tetraethyl orthosilicate hydrolysis and condensation by electrospinning and high-temperature calcination;the MXene@c-MWCNT_(x:y)f.lms are prepared by vacuum f.ltration *** particular,the SNM and MXene@c-MWCNT_(6:4)as one unit layer(SMC_(1))are bonded together with 5 wt%polyvinyl alcohol(PVA)solution,which exhibits low thermal conductivity(0.066 W m^(-1)K^(-1))and good electromagnetic interf.rence(EMI)shielding perf.rmance(average EMI SE_(T),37.8 dB).With the increase in f.nc-tional unit layer,the overall thermal insulation perf.rmance of.the whole composite f.lm(SMC_(x))remains stable,and EMI shielding perf.rmance is greatly improved,especially f.r SMC_(3)with three unit layers,the average EMI SET is as high as 55.4 *** addition,the organic combination of.rigid SNM and tough MXene@c-MWCNT_(6:4)makes SMC_(x)exhibit good mechanical tensile ***,SMC_(x)exhibit stable EMI shielding and excellent thermal insulation even in extreme heat and cold ***,this work provides a novel design idea and important ref.rence value f.r EMI shielding and thermal insulation components used in extreme environmental protection equipment in the f.ture.

With the massive success of.deep networks,there have been signi-cant ef.orts to analyze cancer diseases,especially skin *** this purpose,this work investigates the capability of.deep networks in diagnosing a variety of.dermoscopic lesion *** paper aims to develop and ne-tune a deep learning architecture to diagnose dif.erent skin cancer grades based on dermatoscopic ***-tuning is a powerf.l method to obtain enhanced classication results by the customized pre-trained ***,batch normalization,and hyperparameter optimization are perf.rmed f.r ne-tuning the proposed deep *** proposed ne-tuned ResNet50 model successf.lly classied 7-respective classes of.dermoscopic lesions using the publicly available HAM10000 *** developed deep model was compared against two powerf.l models,i.e.,InceptionV3 and VGG16,using the Dice similarity coef.ient(DSC)and the area under the curve(AUC).The evaluation results show that the proposed model achieved higher results than some recent and robust models.

Objective: The great saphenous vein (GSV) is commonly used as a conduit f.r graf.ing during CABG surgery, and open GSV harvesting (OVH), commonly used with long incision to expose the vein. However, endoscopic vein harvesting (EVH) is an alternative approach, utilizing specialized instruments and small incisions to harvest the vein. Methods: A retrospective analysis was conducted on a cohort of.patients who underwent Coronary artery bypass graf. (CABG) requiring great saphenous vein (GSV) harvesting which was done by EVH or OVH procedures. Demographic variables, including age and gender, were assessed f.r both groups. Intraoperative variables such as the number of.graf.s, cardiopulmonary bypass time, X clamp time, and type of.procedure were analyzed. Postoperative variables, including inf.ction and bleeding rates, were also evaluated. Results: The study included 30 patients each undergoing Coronary artery bypass graf. (CABG) with need of.great saphenous vein harvesting which was done by EVH and OVH. Demographic variables were well-matched between the two groups in terms of.age, while a signif.cant dif.erence in gender distribution was observed. Obesity and smoking were more prevalent in the OVH group, and EVH was associated with a higher mean number of.graf.s compared to OVH. Conversion to an open technique occurred in a portion of.the EVH cases, and inf.ction rates did not signif.cantly dif.er between the EVH and OVH groups. However, the incidence of.postoperative bleeding was signif.cantly higher in the EVH group. Conclusion: This study provides valuable insights into the demographic, intraoperative, and postoperative variables associated with EVH and OVH techniques. EVH demonstrated advantages in terms of.reduced inf.ction rates compared to OVH. However, the higher incidence of.postoperative bleeding associated with EVH raises concerns about potential risks.

Sum Rate Maximization-based f.ir Power Allocation in Downlink NOMA Networks

Non-orthogonal multiple access(NOMA)has been seen as a promising technology f.r 5G *** perf.rmance optimization of.NOMA systems depends on both power allocation(PA)and user pairing(UP).Most existing researches provide sub-optimal solutions with high computational complexity f.r PA problem and mainly f.cuses onmaximizing the sum rate(capacity)without considering the f.irness ***,the joint optimization of.PA and UP needs an exhaustive *** main contribution of.this paper is the proposing of.a novel capacity maximizationbased f.ir power allocation(CMf.A)with low-complexity in downlink *** investigation and analysis of.the joint impact of.signal to noise ratio(SNR)per subcarrier and the channel gains of.the paired users on the perf.rmance of.NOMA in terms of.the capacity and the user f.irness is ***,a closed-f.rm equation f.r the power allocation coef.icient of.CMf.A as a f.nction of.SNR,and the channel gains of.the paired users is *** addition,to jointly optimizeUP and PAinNOMA systems an ef.icient low-complexity UP(ELCUP)method is proposed to be incorporated with the proposed CMf.A to compromise the proposed joint resource allocation(JRA).Simulation results demonstrate that the proposed CMf.A can improve the capacity and f.irness perf.rmance of.existing UP methods,such as conventional UP,and random UP ***,the simulation results show that the proposed JRA signif.cantly outperf.rms the existing schemes and gives a near-optimal perf.rmance.

Non-orthogonal multiple access(NOMA)is one of.the leading technologies f.r 5G *** pairing(UP)and power allocation(PA)are the key controlling mechanisms f.r the optimization of.the perf.rmance of.NOMA *** paper presents a novel UP and PA(UPPA)technique f.r capacity and f.irness maximization in NOMA called(Cf.-UPPA).The impact of.the power allocation coef.icient and the ratio between the channel gains of.the paired users on the sum-rate capacity and the f.irness in NOMA is f.rstly ***,based on this investigation,the PA and UP algorithms of.the Cf.-UPPA technique are *** power allocation coef.icient of.the proposed PA is f.rmulated as an exponentially decaying f.nction of.the ratio between the channel gains of.the paired users to maximize the capacity and the f.irness,and its maximum value is adjusted to guarantee the successive interf.rence cancellation(SIC)*** proposed UP is based on selecting the user that has the highest channel gain per subcarrier as the strong user to maximize the capacity and selecting the user that has the closest lower channel gain to the strong user’s channel gain as the weak user to improve the f.irness and *** perf.rmance evaluation of.the proposed Cf.-UPPA technique in terms of.capacity,f.irness,and outage probability demonstrates that its perf.rmance signif.cantly outperf.rms that of.the orthogonal multiple access(OMA)system and that of.the NOMA system with random ***,the simulation results demonstrate the ef.iciency of.the proposed PA in improving the perf.rmance of.other UP algorithms,such as the random UP algorithm.
