Recovery after Ordeal: Respiratory Alterations Linked to Quitting Smoking

Background: Smoking cessation has been long considered a sole precautionary mode of respiratory disability in both developed and developing countries. Aim of Work: our study aims to assess the impact of smoking cessation on clinical features as well as pulmonary functional measurement and exhaled CO level in ex-smokers. Patients and Methods: This observational cross sectional study was conducted on 306 subjects who were attending to outpatient pulmonology clinic in Center of medical services in Taibah University at Al-Medina Al-Munwara, Kingdom of Saudi. Subjects included in this study were selected as being ex-smoker for more than 6 months. They were classified according to the duration of smoking cessation into a) recent smoking quitter (stopped smoking ≤2 years) and;b) remote smoking quitter (stopped smoking >2 years). They were subjected to clinical evaluation, spirometry and exhaled CO level measurement in exhaled air. Results: All clinical features including cough, dyspnea, chest pain, hemoptysis, wheezes, hospitalization and relapse in both recent quitter and remote quitter subjects were statistically comparable but without statistical significant difference. There was statistical positive correlation between duration of smoking cessation and actual FEV1, actual FVC, actual PEFR, actual TLC, functional defect FEV1, functional defect of FVC, functional defect of PEFR and functional defect of TLC while statistical negative correlation was with CO exhaled level. Conclusion: Smoking cessation demonstrated an appreciated role in recovering patient clinical as well as pulmonary functional and gaseous characters towards healthy side.

A LS-SVM （Least Squares Support Vector Machines） Approach for Predicting Critical Flashover Voltage of Polluted Insulators

LS-SVM （least squares support vector machines） are a class of kemel machines emphasizing on primal-dual aspects in a constrained optimization framework. LS-SVMs aim at extending methodologies typical of classical support vector machines for problems beyond classification and regression. This paper describes a methodology that was developed for the prediction of the critical flashover voltage of polluted insulators by using a LS-SVM. The methodology uses as input variables characteristics of the insulator such as diameter, height, creepage distance, form factor and equivalent salt deposit density. The estimation offlashover performance of polluted insulators is based on field experience and laboratory tests are invaluable as they significantly reduce the time and labour involved in insulators design and selection. The majority of the variables to be predicted are dependent upon several independent variables. The results from this work are useful to predict the contamination severity, critical flashover voltage as a function of contamination severity, arc length, and especially to predict the flashover voltage. The validity of the approach was examined by testing several insulators with different geometries. Moreover, the performance of the proposed approach with other intelligence method based on ANN （artificial neural networks） is compared. It can be concluded that the LS-SVM approach has better generalization ability that assist the measurement and monitoring of contamination severity, flashover voltage and leakage current.

Expression levels of pro-inflammatory interleukin-8 and certain antimicrobial peptides in concurrent with bacterial conjunctivitis

AIM:To detect the quantitative expression levels of the pro-inflammatory interleukin-8(IL8),antimicrobial peptides human beta defense-2(HBD2),and human beta defense-3(HBD3)genes in bacterial ***:The human conjunctival epithelial cells were obtained using the impression cytology technique from healthy controls and *** genes expression levels were determined utilizing a reverse transcription quantitative polymerase chain reaction(RT-q PCR).The contribution of causative agent type,the number of isolates and severity of clinical features,in the increase of genes expression was also ***:The RT-q PCR showed that IL8,HBD2,and HBD3 expression increased in bacterial conjunctivitis as compared to healthy control(P<0.001).In gram-negative bacterial conjunctivitis,HBD2 was highly up-regulated(P<0.001)compared to other types of bacterial *** mixed bacterial conjunctivitis,a direct correlation between HBD2 up-regulation and HBD3 up-regulation was observed(P<0.05).The severity of clinical features was related to the up-regulation of IL8 and HBD2(P<0.05).CONCLUSION:IL8,HBD2,and HBD3 are immuneeffectors in infectious ***2 is active during different bacterial conjunctivitis but is more released with gram-negative bacteria compared to gram-positive ***3 is an obvious defender in different bacterial conjunctivitis.

Results of the Sauvé-Kapandji Procedure for Posttraumatic Disorders of the Distal Radioulnar Joint in Young Patients

The authors report their experience with the Sauvé-Kapandji procedure for the management of posttraumatic disorders of the distal radioulnar joint in 9 younger and active patients. The mean age of our series was 37 years. We operated patients for whom pain in the distal radioulnar joint was more relieved by usual analgesic medical treatment, and those with a limitation of wrist movements. Until the latest follow-up, patients have benefited from antero-posterior and profile radiographs of the wrist. Functional assessment was based on the Modified Mayo Wrist Score whose average value was around 77 points. Postoperatively all patients experienced relief of pain. Rotation of the forearm increased to near normal values. On a professional level, eight of nine employed patients had returned to work. There were no major complications.

The Total Hip Arthroplasty in Ankylosing Spondylitis

Objectives: The assessment of the radio clinical results of sixty one total hip arthroplasties was performed among young and active patients, with follow-up for ankylosing spondylitis, in order to evaluate the effect of surgery on the quality of life of these patients. Methods: For the functional improvement of the patients, we relied on the scores of Devane, Harris and that of Postel Merle d’Aubeigné. The radiological analysis was based on the coxometry. Results: The study confirms the notable functional gain and consequently the improvement of the quality of life of the patients. However, the longevity of the implants remains difficult to envisage due to our limited and insufficient hindsight. Conclusion: The total hip arthroplasty transforms the life of the young patients’ carriers of the disabling coxitis on the stiffening spondylarthritis, by getting them the laziness and the mobility wished for.

Bilateral Subtalar Dislocation in Gymnast: A Case Report

Subtalar dislocation is a rare injury in elite athletes. We report an unusual case of simultaneous bilateral subtalar dislocation in an international female athlete competing in gymnastics. These injuries require prompt reduction and immediate immobilisation in a plaster cast for 8 weeks or wire stabilisation after open reduction if an abrupt end to a promising career is to be prevented.

Background: toxic gases and fumes prompted from petrol and gas stations imply their disabling and deadly risks on human lungs. Aim of Work: The aim of this work was to assess the pulmonary function derangements in petrol pump workers at Al-Medina Al-Munawara city. Patients and Methods: This cross sectional study was conducted on 189 cases out of them in control group (group I) with age and sex matched healthy adult males gathered from nonsmokers students and employees in Taibah University. The study group (group II) comprised of 89 males who were working in different petrol pump stations. History, clinical examination and pulmonary functional assessment were carried out by using portable spirometry, Co detector and puls oximeter. Results: The study’s results showoed that the lung function parameters (FEV1, FVC, PEFR, FEFR25% - 75%) were significantly reduced in petrol pump workers (Group II) as compared to the values for these parameters in subjects belonging to Group II (Control group), Similar findings were observed in previous studies on pulmonary functions in petrol-pump workers. This finding specified the restrictive nature of pulmonary involvement in the study group. Measurement of the exhaled CO was slightly higher in Group II than Group I. Conclusion: Fuel vapors and fumes in petrol stations showed direct incapacitating impact on workers pulmonary functions regarding spirometric measurements showing predominantly restrictive pattern and to less extent obstructive. Exhaled CO level was higher in addition to hypoxemia identified in these cases.

This paper presents the results of petrographic study of sedimentary iron ore from surface strata of the shendi-Atbara Basin,River Nile State,*** aims of this study are to investigate the geological behavior and geological conditions affecting precipitation of sedimentary iron *** methodologies have been used to realize the objectives of this study include field work,office work and laboratory work including thin sections and polished sections *** to field observation sedimentary iron ore can broadly be considered as occurring in three major classes:Ferribands iron,ferricrete iron and oolitic iron *** modes of occurrence of iron ore were described at the outcrops and vertical sedimentary profiles revealed that the iron occurred in the study area at different types in stratigraphic sequence such as cap,bedded and interbedded conformable with shendi *** study of iron ore in collected samples using polarized microscope and ore microscope includes study of the textures and structures of ores to obtain ore *** main types of textures and structures in studied samples are oolitic,granular,lamellar and *** to these results the origin of iron ore is formed by chemical precipitation during chemical weathering of surrounding areas in continental lacustrine *** iron ore in study area is potential for future mining works and steel industry.
