Pros and cons of live kidney donation in prediabetics: A critical review and way forward

There is shortage of organs,including kidneys,*** with deceased kidney transplantation,there is a significant rise in live kidney *** prevalence of prediabetes(PD),including impaired fasting glucose and impaired glucose tolerance,is on the rise across the *** teams frequently come across prediabetic kidney donors for *** are at risk of diabetes,chronic kidney disease,cardiovascular events,stroke,neuropathy,retinopathy,dementia,depression and nonalcoholic liver disease along with increased risk of all-cause ***,most of the studies done in prediabetic kidney donors are retrospective in nature and have a short follow up *** is lack of prospective long-term studies to know about the real risk of complications after ***,there are variations in recommendations from various guidelines across the globe for donations in prediabetics,leading to more confusion among *** increases the responsibility of transplant teams to take appropriate decisions in the best interest of both donors and *** review focuses on pathophysiological changes of PD in kidneys,potential complications of PD,other risk factors for development of type 2 diabetes,a review of guidelines for kidney donation,the potential role of diabetes risk score and calculator in kidney donors and the way forward for the evaluation and selection of prediabetic kidney donors.

Organic dye pollutants present in wastewater pose a significant global challenge. Among pollutants, the synthetic dye Rhodamine B(RB) stands out due to its non-biodegradable nature and associated neurotoxic, carcinogenic,and respiratory irritant properties. Extensive research has been conducted on the efficacy of adsorption and photodegradation techniques for the removal of RB from wastewater. While adsorption and advanced oxidation processes(AOPs) have gained considerable attention for their effectiveness in recent years, the underlying behaviors and mechanisms of these technologies remain incompletely understood. Therefore, a comprehensive of recent research progress in this domain is imperative to clarify the basics and present the up-to-date *** review provides an in-depth exploration of the fundamentals, advancements, and future trajectories of RB wastewater treatment technologies, mainly encompassing adsorption and photodegradation. This work starts with a general introduction of outlining the sources, toxicity, and diverse applicable removal strategies. Subsequently, it thoroughly examines crucial techniques within non-photochemical, photochemical, and adsorption technologies, such as UV light assisted AOP, catalyst assisted AOP, ozonation, Fenton system, electrochemical AOP, and adsorption technology. The primary objective is to furnish a broad overview of these techniques,elucidating their effectiveness, limitations, and applicability. Following this, the review encapsulates state-of-theart computational simulations pertaining to RB adsorption and interactions with clays and other ***, it delves into column adsorption of RB dye, and elucidates various influencing factors, including bed height, feed concentration, pollutant(RB) feeding or flow rate, and column regeneration. Determining the adsorption of shale gas on complex surfaces remains a challenge in molecular simulation *** essentially stem from the need to create a realistic shale structure model in terms of mineral heterogeneityand ***,precise characterization of the competitive adsorption of hydrogen andmethane in shale generally requires the experimental determination of the related adsorptive *** thisstudy,the adsorption of adsorbates,methane(Ch_(4)),and hydrogen(h_(2))on heterogeneous shale surface modelsof Kaolinite,Orthoclase,Muscovite,Mica,C_(60),and Butane has been simulated in the frame of a moleculardynamic’s numerical *** results show that these behaviors are influenced by pressure and *** increasing the pressure from 500 to 2000 psi,the sorption effect for Ch_(4)significantly increasesbut shows a decline at a certain stage(if compared to h_(2)).The research findings also indicate that raw shalehas a higher capacity to adsorb Ch_(4)compared to ***,in shale,this difference is negligible.

The Impact of Blood Transfusion on the Efficiency of Stem Cell Transplants

Background: While blood product transfusion is essential for managing hematologic deficits in Allogenic hematopoietic stem cell transplant (AhSCT) recipients, it has risks including infectious disease transmission, alloimmunization, and transfusion reactions. These risks have sparked an ongoing debate regarding the overall impact of transfusions on patient outcomes. Thus, this study aimed to evaluate the impact of Red Blood Cells (RBCs) and/or platelet transfusion on the infection incidence and overall survival in AhSCT patients. Methods: We performed a retrospective analysis of clinical and laboratory data of sixty adult patients with primary malignant hematological disorder who had undergone AhSCT. Participants’ data were categorized into two groups;Group 1 (low transfusion group) consisted of patients receiving 10 units. Quantitative data were expressed as mean ± SD. The t-test of significance and Chi-square (χ2) test were used, with p ≤ 0.05 considered significant. Result: A total of 60 patients’ data was included. In Group 1, out of 30 patients, 13 (43.33%) developed infections. In contrast, Group 2 had 21 (70%) out of 30 patients develop infections. Group 1 had a higher survival rate (57.8%) than Group 2 (transfusion > 10 units) (46.2%) with a chi-square value = 23.56, and p-value Conclusion: The volume of blood product transfusions has a considerable impact on patient outcomes, particularly infection and survival rates. Additional long-term prospective studies and larger randomized controlled trials are needed to strengthen the evidence for determining transfusion protocols for these patients.

Use of Topical Complementary and Alternative Medicines (CAM) in Diabetic Septic Foot Disease in Makkah Region, Western of Saudi Arabia, an Exploratory Study

Diabetes is a chronic condition that is significantly impacted daily by environment. At least one in five Saudis, suffer from diabetes. “Diabetes is a pandemic of unprecedented magnitude spiraling out of control” (Boulton). Saudi Arabia ranks the second highest in the Middle East, and is the seventh in the world for the prevalence of diabetes (WhO). Diabetes mellitus (DM) has been found to be related to high mortality, morbidity accompanied by poor general health and lower quality of life. Current Saudi diabetes patients’ behaviors, practices, and beliefs regarding foot health problems such as ulcers, are not well investigated. Diabetic patients frequently utilize natural remedies and herbs for self-medication as a part of complementary and alternative Medicine. Aim of study: To explore the widespread use of the alternative remedies in the local communities of the Western Saudi Arabia, the drives behind using and to focus on the potential adverse effects following their usage. Method: A descriptive, cross-sectional study of 386 diabetic patients with feet problems in Makkah Region, West of Saudi Arabia during the period April - June 2023. Results: Out of the 531 participants, only 386 participants reported diabetic foot disorder (DFD). Most of the participant were above 65 years of age group (24.61 %). Male (52.08%) more than female (47.92%) in the research group. All of the participants had diabetic foot disorders at least once. The prevalence of complementary and alternative medicines (CAM) use among them was 67.88%, as solitary or in combination with hospital medicines. honey headed the list followed by myrrh and black seeds. Liver transplantation(LT)provides a life-saving option for cirrhotic patients with complications and hepatocellular *** the increasing number of liver transplants performed each year,the number of LT candidates on the waitlist remains unchanged due to an imbalance between donor organ supply and the demand which increases the waitlist time and *** donor liver transplant had a great role in increasing the donor pool and shortened waitlist time for LT ***,further strategies can be implemented to increase the pool of potential donors in deceased donor LT,such as reducing the rate of organ *** hepatitis C virus(hCV)seropositive liver grafts is one of the expanded donor organ criteria.A yearly increase of hundreds of transplants is anticipated as a result of maximizing the utilization of hCV-positive organs for hCV-negative ***-acting antiviral therapy's efficacy has revolutionized the treatment of hCV infection and the use of hCV-seropositive donors in *** American Society of Transplantation advises against performing transplants from hCV-infected liver donors(D+)into hCV-negative recipient(R-)unless under Institutional Review Board-approved study rules and with full informed consent of the knowledge gaps associated with such *** selection of patients to be transplanted with hCV-infected grafts and confirming their access to direct-acting antivirals if needed is *** and international consensuses are needed to regulate this process to ensure the maximum benefit and the least adverse events.
