Objective:To compare effect of Hypericum perforatum(***) vaginal gel with metronidazole on bacterial vaginosis(BV) in terms of initial response to treatment and preventing recurrence(primary outcomes) and also patient complaints(secondary outcomes).Methods:In this double-blind,double dummy trial,married women aged 18-49 with BV were randomized into two groups and administered 5 g of 3%*** and placebo of metronidazole(n=82),or 5 g of 0.75%metronidazole and placebo of ***(n=80)vaginally for 5 *** criteria were used for diagnosis and assessing cure and recurrence of *** comparisons was done using Chi-square,Fisher's exact and logistic ***:At 10-12 *** rate was 82%in the *** and 85%in metronidazole group(risk ratio 0.9.95%confidence interval 0.6 to 1.3).Among the cured women,recurrence rate was 9%in the *** and 13%in the metronidazole group at the 30-35 d visit(risk ratio 0.8,95%confidence interval 0.4 to 1.3),There was no statistically significant difference between the groups regarding any patient complaints,except itching which was less in *** group(5%vs.16%,P=0.018 at the first and 13%vs.43%,P<0.001 at the second follow-up).No significant adverse event was reported at any ***:*** could be a good option for treatment of ***,further studies arc needed for its public use.

In this paper,the performance of a solar thermal system with a focus on space heating was investigated.A 70 m^(2) detached house was considered in the weather conditions of the city of Tehran,Iran.A thermosyphon solar water heater with a flat plate collector combined with an auxiliary electrical heater supplies the heating demand of the *** proposed system was modeled and analyzed using TRNSYS *** this regard,the TRNBuild module was employed for the building load *** model has been simulated for one year of *** effects of the solar collector’s surface area and storage volume were *** results show that for a solar collector with a 15 m^(2) surface area,the solar fraction is 0.29 in January,during which the solar radiation is the *** solar collectors of10 m^(2) and 5 m^(2) surface areas,the solar fraction falls to 0.23 and 0.14,respectively in ***,two cases of 150 L and 300 L storage tanks are taken into ***,it is found that using a 15 m^(2) solar collector and a 150 L storage tank can appropriately provide the building’ s heating demand taking the thermal performance and economic aspects into consideration.

Simultaneous zinc and vitamin A deficiency are common health problems in developing countries. The objective of this study was to assess effect of supplementation of high zinc or vitamin A on immune function. After three months of feeding with a zinc and vitamin A deficient diet, mice were assigned into four groups which, for additional two months, received a normal or high zinc along with vitamin A deficient diet and a normal or high vitamin A along with zinc deficient diet. Serum and intestinal mucosa immunoglobulin A (IgA) were determined and supernatants of splenocytes were used to assess interlukin (IL)-2, IL-5, IFN-γ. Mice maintained on zinc deficient diet with normal or high vitamin A resulted in significantly lower production of IFN-γ. Also, supplementation of high dose vitamin A augmented production of the cytokine as compared to normal intake of the vitamin. Supplementation of either normal or high zinc along with low vitamin A diet significantly led to higher production of IFN-γ as compared to those receiving zinc limited but adequate vitamin A. High intake of zinc along with vitamin A deficient diet significantly enhanced secretion of IL-2. Levels of serum and mucosal IgA and IL-5 were not be significantly modulated. Moreover, animals fed with high doses of zinc showed increased IL-2 production than those that had normal intake of zinc. Results indicated that zinc and vitamin A supplementation up-regulates production of T-cell cytokines, IFN-γ and IL-2.

Climate change can impact hydrological cycle through changes in precipitation pattern and amount,temperature,evapotranspiration,and run *** changes in climatic parameters can subsequently impact agricultural crop *** study has been conducted with the aim to find relationships between crop yield and evapotranspiration with historical data（1984-2005） and predict crop yield under expected climate changes in the upcoming years in the northeast of Iran（kashafrood basin）.Four major crops in this region were selected such as sugar beet,cotton,been,and *** future precipitation and temperature data are simulated by downscaling outputs of global climate model HadCM3（A2 scenario） with ASD（Automated Statistical Downscaling） *** temperature under A2 Scenario showed increasing trend for all three time *** annual precipitation increased by 4.64%,5.41%,and 2.22% for（2010-2039）,（2040-2069） and（2070-2099）,*** calculate the reference crop evapotranspiration,the Hargreaves-Samani（1985） equation was applied and validated for this *** results showed a nonlinear relationship between crop yields and the seasonal evapotranspiration for all selected *** on our results,seasonal evapotranspiration would increase under climate change scenarios for selected crops in study *** the other hand,crop yield would increase for all crops except sugar *** 2070-2099 period,sugar beet yield would decrease in to base line period（1985-2005）.

In the present study,the performance of reinforced concrete tunnel(RCT)under internal water pressure is evaluated by using nonlinear finite element analysis and surrogate *** parameters,including the compressive and tensile strength of concrete,the size of the longitudinal reinforcement bar,the transverse bar diameter,and the internal water pre ssure,are considered as the input *** on the levels of variables,36 mix designs are selected by the Taguchi method,and 12 mix designs are proposed in this *** fiber reinforced concrete(CFRC)or glass fiber reinforced concrete(GFRC)is considered for simulating these 12 *** component regression(PCR),Multi Ln equation regression(MLnER),and gene expression programming(GEP)are employed for predicting the percentage of damaged surfaces(PDS)of the RCT,the effective tensile plastic strain(ETPS),the maximum deflection of the RCT,and the deflection of crown of *** error terms and statistical parameters,including the maximum positive and negative errors,mean absolute percentage error(MAPE),root mean square error(RMSE),coefficient of determination,and normalized square error(NMSE),are utili zed to evaluate the accuracy of the *** on the results,GEP performs better than other models in predicting the *** results sh ow that the internal water pressure and the mechanical properties of concrete have the most effect on the damag e and deflection of the RCT.

Biochemical response to lamivudine treatment in HBeAg negative chronic hepatitis B patients in Iran

AIM： To study the effect of a one-year lamivudine regimen in patients with chronic hepatitis B. METHODS： Medical records of HBeAg negative hepatitis B patients who attended a hepatitis clinic in Tehran between March 2002-March 2004 were evaluated. The patients received 100 mg lamivudine tablets once daily for at least 12 mo. Liver enzymes and complete blood count were checked at baseline and the end of treatment （12th mo） and 6 mo after discontinuation of treatment. RESULTS： Of all patients, 24 were excluded. Of 71 patients left, 58 （81.7%） were men. Mean age of the patients was 38 ± 14 years. Mean level of ALT in serum was 1437 ± 205 nkat/L at baseline with a significant reduction at the end of treatment to a mean level of 723 ± 92 nkat/L （P = 0.002）. In 38 patients （53.5%）, the ALT level was normal after one-year treatment. Five patients （7.3%） relapsed （biochemically） within 6 mo after discontinuing lamivudine therapy （the patients with good end of treatment response）. Mean level of AST in serum was 1060 ± 105 nkat/L at baseline which decreased significantly to 652 ± 75 nkat/L at the end of treatment （P = 0.002）. CONCLUSION： Over half （53.5%） of chronic hepatitis B patients with HBeAg negative have normal liver enzyme level at 12-mo lamivudine therapy.

AIM: To investigate whether single-nucleotide polymor- phisms in the promoter regions of endotoxin-responsive genes CD14 C (-159) T is associated with chronic hepatitis B. METHODS: We obtained genomic DNA from 80 patients with established diagnosis of chronic hepatitis B and 126 healthy subjects served as a control population. The CD 14 C (-159) T polymorphism was investigated using an allele specific PCR method. RESULTS: Twenty seven percent of chronic hepatitis B patients and 75% of controls were heterozygous for CT genotype. The difference between the chronic hepatitis B and control groups was statistically significant [P < 0.0001; Odds ratio (OR) = 2.887; 95% CI: 1.609-5.178]. Twenty four point six percent of chronic hepatitis B and patients 12.3% of the control group were heterozygous for TT genotype. The difference between groups was not statistically significant (P = 0.256; OR = 0.658; 95% CI: 0.319-1.358). Forty eight point four percent of chronic hepatitis B patients and 12.7% of control were homozy- gote for CC genotype (P < 0.004; OR = 0.416; 95% CI: 0.229-0.755). The frequency of allele C was 61.9% and allele T was 38.1% in hepatitis B patients group. The frequency of allele C was 55.2% and allele T was 44.8% for the control group (P = 0.179; OR = 1.319; 95% CI: 0.881-1.977). CONCLUSION: The TT heterozygous genotype was not a risk factor for chronic hepatitis B. CC homozygote genotype is protective for hepatitis B. Lack of heterozy- gosis of genotype CT is a risk factor for chronic hepatitis B. Alleles C or T were not risk factors for chronic hepatitis B. These findings show the role of a single-nucleotide polymorphism at CD14/-159 on the development ofchronic hepatitis B. Endotoxin susceptibility may play a role in the pathogenesis of chronic hepatitis B.

The aim of this study was to formulate and develop a low calorie and low glycemic index (GI) of soft ice cream by using mixture of sucrose and Stevia. Five different formulations of ice cream were produced by using different proportions of sucrose and Stevia. Physicochemical characteristics, hedonic sensory evaluations and glycemic index determination of products were carried out by following conventional methods. Replacement of sucrose with Stevia resulted in a significantly lower viscosity and brix with a higher overrun and melting rate in a dose dependent manner. Total replacing of sucrose with Stevia resulted in significant reduction in caloric value from 143.03 to 105.25 Kcal and GI from 79.06 ± 4.0 to 72.18 ± 5.27 as compared to those of sucrose based formulation (p 0.05) indicating a 37.78% and 6.88% reduction, respectively. TB had the best sensory acceptance among all the treatments. We concluded that substitution of sucrose with Stevia may be a choice to produce low caloric and GI ice creams. However, using mixture of the two sweeteners improves sensory acceptance of the formulations.

Polymorphisms of interleukin-1R receptor antagonist genes in patients with chronic hepatitis B in Iran

AIM： To investigate the relationships between polymorphisms of interleukin-1R receptor antagonist genes and susceptibility to chronic hepatitis B in Iran population. METHODS： Genomic DNA was extracted from the peripheral blood of 80 patients with chronic hepatitis B （57 males, 23 females） aged 12-77 years （mean 36.1 ± 13.8 years） and 147 normal controls （96 males, 51 females） aged 6-75 years （mean 41 ± 18.7 years） who referred to a liver clinic of Tehran and then subjected to polymerase chain reaction （PCR） amplification. PCR products were resolved on a 3% agarose gel and stained with ethidium bromide. RESULTS： Only three of the five kinds of polymorphism （2/2, 2/4, and 4/4） were found in this study. The frequencies of 2/2, 2/4, and 4/4 were 12.5%, 17.5%, 70% respectively in chronic hepatitis B patients and 6.8%, 24.5%, and 68.7% respectively in controls. IL-1 R allele 2 was detected in 30% of chronic hepatitis B patients and in 31.3% of controls, while IL-1 R allele 4 was detected in 87.5% of chronic hepatitis B patients and in 93.2% of controls. The frequency of IL-1R alleles 2 and 4 was detected in 21.25% and 78.75% of the patients and 19.04% and 80.96% of the controls, respectively. CONCLUSION： Our results suggest that the carriage of IL-1R receptor antagonist alleles 2, 4, 6 may not play any role in the development of HBV infection. Large population-based studies are needed to investigate the role of IL-1 polymorphisms in the pathogenesis of developing chronic hepatitis B.
