Disseminated cysticercosis is an uncommon presentation of a common *** disseminated cysticercosis is rarely reported in ***,we are reporting a case of asymptomatic disseminated cysticercosis incidentally diagnosed in a patient of low backache. Magnetic resonance imaging of lumbosacral spine and neuroimaging done subsequently during the course of evaluation revealed diffuse cysticercosis involving abdominal,paraspinal,pelvic and gluteal muscles along with *** a disseminated cysticercosis was diagnosed incidentally in this patient of low backache with right sciatica and radiculopathy at L5-S1 prolapsed intervertebral disc and was subsequently managed by L5-S1 interlaminar fenestration and discectomy.

Cognitive diagnosis models（CDMs）has gained a growing interest in educational or psychological *** of considering an overall ability,a detailed latent attribute profile is considered to obtain a finer-grained evaluation and *** the popularity of CDMs,rare researches were conducted in *** open up the analysis and mind,CDMs are employed in this article as a very first ***,the main goal of this study is to investigate on why it gets more difficult to learn Mathematics for Afghans students,and to evaluate and determine the strengths and weaknesses of the Afghanistan students in attributes and *** research aims at assessing the mathematics skills of 8 grade students in *** response data is from the questionnaires of Trends in International Mathematics and Science Study（TIMSS）*** commonly used CDMs were employed to fit the response data,including the deterministic,inputs,noisy,“and”gate（DINA）model and the deterministic,inputs,noisy,“or”gate（DINO）*** the assistance of CDMs,we could obtain not only the item parameters,but also the skill profile for each *** show that the examinees do best in Number domain while do worst at Data and *** in terms of model selection,DINA model fitted the data better than DINO model.

Multiple pometacarpal （CMC） joints volar dislocations of car- are uncommon and have been reported rarely. A 25 years old male presented with injury to his left hand 6 days following a road traffic accident. Clini- cal examination revealed gross swelling of the hand and diffuse tenderness over the carpometacarpal area. His ra- diographs of the hand showed volar dislocation of the second, third and fourth CMC joints in association with anextra-articular fracture of the base of thumb metacarpal. He was treated by open reduction and percutaneous fixation using Kirschner wires. The functional results were excellent at one year follow-up.

This study was conducted in Juniperus excels a stands on the southern slopes of Iran’s Alborz Mountains,to determine the effects of fire and seasonal variations on soil physicochemical properties and enzyme activities.A total of 64 composite soil s amples were randomly collected in the spring and fall from two burned and unburned sites at depths of 0-10 and 10-20 *** results of a two-way ANOVA analysis indicate that fire increased the organic carbon(OC),total nitrogen(TN),and available phosphorus(P_(ava)) contents of the soil by 16%,59%,and 53%,***,when burned sites were compared to unburned sites,the activities of acid phosphatase(ACP) and urease enzymes increased by 73% and 12%,***,fire did not affect soil texture,bulk density(BD),pH,electric conductivity(EC),exchangeable potassium(Kexc),or the activities of alkaline phosphatase(ALP) and *** to two-way ANOVA results,OC,TN,P_(ava),K_(exc),and EC values were significantly higher in the fall,whereas pH and ALP values were significantly higher in the ***,phosphorous and dehydrogenase activity were significantly different in selected soil factors at 0-10 and10-20 cm *** interaction of fire,season,and soil depth were significant for phosphorous and ***,the activity of ALP correlated well with pH(r=0.68),P(r=-0.74),OC(r=-0.53),and TN(r=-0.37),whereas the activity of ACP correlated significantly with OC(r=0.64) and TN(r=0.71).Two years after the fire,soil properties in *** a stands had either improved or remained unaffected,returning to pre-fire ***,soil chemical properties varied significantly across sampling seasons,which should be considered when comparing and interpreting soil data in future research.

Deposition behavior and tribological properties of diamond-like carbon coatings on stainless steels via chemical vapor deposition

A systematic investigation was carried out to observe the deposition rate of a diamond-like carbon(DLC) coating on two stainless steel substrates by chemical vapor deposition(CVD). The objective of this research is to study the deposition behavior of the DLC coating and its tribological properties in different combinations of methane(CH_4) and nitrogen, which were used as precursor gases. The results reveal that the deposition rate increases with increasing CH_4 content up to 50 vol%. The hardness of the DLC-deposited layer also increases while the friction coefficient decreases with increasing CH_4 gas content up to 50% in the precursor gas mixture.

BACKGROUND: Prognostic value of cortisol and thyroid function tests(TFTs) has previously been evaluated in medical ICUs. We aimed to evaluate prognostic efficacy of cortisol and TFTs in critically ill poisoned patients admitted to toxicology intensive care unit(ICU).METHODS: In a prospective study of consecutively enrolled subjects admitted to the toxicology ICU, lab analyses included TFTs(total T3 and T4 as well as TSH) and cortisol levels drawn between 8 am–10 am during period of the first 24 hours post-ingestion/exposure. Simplified Acute Physiology Score Ⅱ(SAPS Ⅱ) and Acute Physiology and Chronic Health Evaluation Ⅱ(APACHE Ⅱ) were recorded. All scores were compared to detect the best prognostic factor. Type of poisoning was also ***: In 200 patients evaluated, 129 were male and mean age was 31 years. In general, SAPS Ⅱ, T4, and cortisol could prognosticate death. After regression analysis, only cortisol had such efficacy(P=0.04; OR=1.06; 95%CI=1.05–1.08; cut-off=42 μg/d L; sensitivity=70%; specificity=82%). Between aluminium phosphide(ALP)-and non ALP-poisoned patients, level of consciousness, mean arterial pressure, and cortisol level could prognosticate death in ALP poisoning(all Ps<0.001 in both uni and multivariate analyses). Median(interquartile range; IQR) GCS was 7(6, 10) and 15(8, 15) in non-ALP and ALP-poisoned patients(P<0.003). SAPS Ⅱ and APACHE Ⅱ could not prognosticate death at ***: Cortisol best prognosticated outcomes for subjects admitted to the toxicology ICU. Its level is higher in ALP-poisoned patients probably due to the higher stress while they remain conscious till the final stages of toxicity and are aware of deterioration of their clinical condition or may be due to their significantly lower blood pressures.

Errors during Paediatric Cardiac Anaesthesia: Reporting and Learning

Incident reporting is a reliable quality assurance tool, frequently used in anaesthesia to identify errors. It was introduced in anaesthesia by Cooper in 1978 and since then several institutions have adopted this system to find adverse events and near misses. We think that the incident reporting would be more beneficial for prolonged and technically complex procedures like paediatric cardiac surgery. Methods: All paediatric CHD patients scheduled for cardiac surgery were included in this audit. Thoracic and general surgery patients were excluded. Any event in preoperative area, induction room, operating room and during transfer to cardiac ICU was documented in a predesigned proforma by resident/consultant. This proforma included information regarding demographics, the type and severity and responsible factors for the event. Results: 134 patients were included in this two and half years audit. 88 patients were male (65.7%) and 46 (34.3%) were female. The age of the patients ranged from one day to 15 years. Total 105 incidents were noticed in 61 patients. 46 incidents were declared as major events which were potentially serious while 59 events were of minor nature. Cuffed endotracheal tube was used in 73% patients. The majority of events occurred in the pre-bypass period. Most of the incidents were related to cardiovascular system (73%), followed by pharmacological incidents. Human factors (74%) were mainly responsible for the incidents. Conclusion: Incident reporting is a reliable and feasible method of improving quality care in developing countries. It helps in identifying areas which need improvement and helps in developing guidelines to improve safety.
