A new structural optimization method of coupled extended finite element method and bound constrained quadratic optimization method(XFEM-BCQO)is adopted to quantify the optimum values of four design parameters for a circular tunnel lining when it is subjected to *** parameters are:tunnel lining thickness,tunnel diameter,tunnel lining concrete modulus of elasticity and tunnel lining concrete ***-Carlo sampling method is dedicated to construct the meta models so that to be used for the BCQO method using matlab *** simulations of the tensile damage in the tunnel lining due to a real earthquake in the literature are created for three design *** approach is used to show the cracks for the mentioned design *** results of the BCQO method for the maximum design case for the tunnel tensile damage was matching the results obtained from XFEM approach to a fair *** new coupled approach manifested a significant capability to predict the cracks and spalling of the tunnel lining concrete under the effects of dynamic earthquakes.

In this study,we conducted experimental tests on two specimens of reinforced concrete beams using a three-point bending test to optimize the flexure and stiffness *** first specimen is a reinforced concrete beam with an ordinary reinforcement,and the second specimen has an invented reinforcement system that consists of an ordinary reinforcement in addition to three additional bracings using steel bars and steel *** results of the flexure test were collected and analyzed,and the flexural strength,the rate of damage during bending,and the stiffness were *** element modeling was applied for both specimens using the LS-DYNA program,and the simulation results of the flexure test for the same outputs were *** results of the experimental tests showed that the flexural strength of the invented reinforcement system was significantly enhanced by 15.5%compared to the ordinary ***,the flexural cracks decreased to a significant extent,manifesting extremely small and narrow cracks in the flexure spread along the bottom face of the *** addition,the maximum deflection for the invented reinforced concrete beam decreased to 1/3 compared to that of an ordinary reinforced concrete *** results were verified through numerical simulations,which demonstrated excellent similarities between the flexural failure and the stiffness of the *** invented reinforcement system exhibited a high capability in boosting the flexure design and stiffness.

There are many certain and uncertain design factors which have unrevealed rational effects on the generation of tensile damage and the stability of the circular tunnels during seismic *** this research paper,we have dedicated three certain and four uncertain design factors to quantify their rational effects using numerical simulations and the Sobol's sensitivity *** effects and interaction effects between the design factors have been determined supporting on variance-based global sensitivity *** results detected that the concrete modulus of elasticity for the tunnel lining has the greatest effect on the tensile damage generation in the tunnel lining during the seismic *** the other direction,the interactions between the concrete density and both of concrete modulus of elasticity and tunnel diameter have appreciable effects on the tensile ***,the tunnel diameter has the deciding effect on the stability of the tunnel *** the interaction between the tunnel diameter and concrete density has appreciable effect on the stability *** is worthy to mention that SoboFs sensitivity indices manifested strong efficiency in detecting the roles of each design factor in cooperation with the numerical simulations explaining the responses of the circular tunnel during seismic actions.

The exact risk association of coronavirus disease 2019(COVID-19)for surgeons is not quantified which may be affected by their risk of exposure and individual *** objective of this review is to quantify the risk of COVID-19 among surgeons,and explore whether facemask can minimise the risk of COVID-19 among surgeons.A systematised review was carried out by searching MEDLINE to locate items on severe acute respiratory syndrome coronavirus 2 or COVID-19 in relation to health care workers(HCWs)especially those work in surgical specialities including surgical nurses and ***,systematic reviews that assessed the effectiveness of facemask against viral respiratory infections,including COVID-19,among HCWs were *** from identified articles were abstracted,synthesised and *** primary studies that provided data on severe acute respiratory syndrome coronavirus 2 infection or experience among surgeons and 11 systematic reviews that provided evidence of the effectiveness of facemask(and other personal protective equipment)were *** the risk of COVID-19 could not be quantified precisely among surgeons,about 14%of HCWs including surgeons had COVID-19,there could be variations depending on *** was found to be somewhat protective against COVID-19,but the HCWs’compliance was highly variable ranging from zero to 100%.Echoing surgical societies’guidelines we continue to recommend facemask use among surgeons to prevent COVID-19.

背景2019年冠状病毒病(COVID-19)在全世界流行期间,紧急接种新冠疫苗成为了增强人群抗感染有效的方法,但未接种疫苗的人仍存在感染或者感染后发展成重症的风险。目的本研究的主要目的是评估COVID-19疫苗接种对改善约旦马安野战医院新冠患者感染后病情转归的影响。方法收集约旦的马安野战医院86名COVID-19住院患者的基本信息和COVID-19疫苗接种信息,将住院前接种过与同期住院的未接种过新冠疫苗的两组患者进行对比分析。结果本研究共招募了86名COVID-19患者,以接受至少一剂COVID-19疫苗满7天即纳入COVID-19疫苗接种患者组。在接种疫苗和未接种疫苗的患者之间比较主要临床结果(以临床异常和需要转移到ICU为主要指标)进行logistic回归分析以确定与临床结果相关的因素。接种COVID-19疫苗组和未接种COVID-19疫苗组分别有42名和44名患者,两组在年龄、性别、吸烟或有任何合并症等样本特征方面均无显著差异(P>0.05)。但因感染新冠进入ICU的患者在接种疫苗患者组中的比例为27.8%(5/18),而在未接种疫苗患者组中进入ICU的比例达到72.2%(13/18),未接种疫苗的患者发展为重症或危重症的临床表现显著高于接种疫苗的患者(Pearson卡方(df)=4.041(1),P=0.039)。未接种疫苗的患者入院时肌酐和尿素水平显著高于接种疫苗组(P=0.045,P=0.044);同样,因新冠感染转入ICU的患者中,未接种疫苗组的C反应蛋白高于接种疫苗组(P=0.031),尿素和血钠含量显著低于接种疫苗组(P=0.038,P=0.049),表明接种疫苗组的临床结果比未接种疫苗组的更温和。结论在约旦的马安野战医院,有强有力的证据表明疫苗接种改善了COVID-19感染的医疗结果,特别是对于高危人群。我们的研究结果支持不断努力提高疫苗在人群中的覆盖率,保护易感人群的生命安全与健康。图5幅,表9个,参考文献51篇。

Background: To reduce patients’ unnecessary suffering, Opioids Induced Constipation (OIC) should be identified and treated as early as possible with the most current treatment. Aims: to investigate the effectiveness of prophylactic laxatives in reducing the severity of OIC and its impact on cancer patients’ quality of life. Methods: A Randomized Clinical Trial design was used. 57 cancer patients from one oncology clinic at a governmental hospital participated in the study. Patients in the intervention groups were given an oral colonic stimulant laxative (i.e. Bisacodyl, Dose = 3 tab/day) or an oral colonic osmotic laxative (i.e. Lactulose, Dose = 15 ml three times per day), while patients in the control continue receiving their routine care as usual. Results: A significant decrease in the severity of constipation symptoms among the intervention group at four weeks post-intervention was noted (p Conclusions: It can be concluded that the use prophylactic of first line laxatives concurrently with opioids decreased the severity of OIC and improved the QOL for cancer patients.

Ongoing improvements in Computational Biology research have generated massive amounts of Protein-Protein Interactions （PPIs） dataset. In this regard, the availability of PPI data for several organisms provoke the discovery of computational methods for measurements, analysis, modeling, comparisons, clustering and alignments of biological data networks. Nevertheless, fixed network comparison is computationally stubborn and as a result several methods have been used instead. We illustrate a prohabilistic approach among proteins nodes that are part of various networks by using Chapman-Kolmogorov （CK） formula. We have compared CK formula with semi-Markov random method, SMETANA. We significantly noticed that CK outperforms the SMETANA in all respects such as efficiency, speed, space and complexity. We have modified the SMETANA source codes available in MATLAB in the light of CK formula. Discriminant-Expectation Maximization （D-EM） accesses the parameters of a protein network datasets and determines a linear transformation to simplify the assumption of probabilistic format of data distributions and find good features dynamically. Our implementation finds that D-EM has a satisfactory performance in protein network alignment applications.

Photothermal deflection spectroscopy is a method used indirectly to measure optical absorption of a sample. Different techniques can be employed to measure the amount of deflection, hence evaluate optical absorption of sample. This work investigates an alternative method both in principle and technique to measure sample’s optical absorption. The new method employed for the first time, relies on the simple idea of light beam deflection from the medium under investigation as a result of change in the index of refraction in its vicinity. The amount of deviation executed by the deviated beam is estimated using new technique that is used for the first time in deflection spectroscopy. As the deviated beam is allowed to pass through a single slit, the value of beam deflection is estimated from the resulting diffraction pattern, i.e. indicating the value of changes taking place in the sample and or measure sample’s optical absorption. The new detection technique used in the estimation of probe beam deflection was also applied in photothermal spectroscopy. Results from both methods were compared and revealed the ease of use of the new method, in addition it cuts cost and experimental efforts although its sensitivity is less than the conventional photothermal method.
