Penile fracture and its treatment： Is retrograde urethrograghy necessary for management of penile fracture？

[Abstract] Objective： Penile fracture, being defined as rupture of the tunica albuginea of the corpus cavernosum, is uncommon. Here, we analyze findings on our patients during a 10-year period and evaluate the role of retrograde urethrography. Methods： From February 2002 to April 2012, 116 patients were admitted with penile fracture at Ghaem Medical Center. Patient history and physical examination were taken at their admittance to detect probable urethral injury. Before surgery, retrograde urethrography was performed in all patients. The size and site of the tunical rupture were recorded. Then the rupture of tunica albuginea was sutured with nonabsorbable （3-0 nylon） sutures and the ties were placed on the internal surface （continuous method）. All patients were followed up for 12 months. Results： Patients＇ mean age was （32.78±10.61） years and ranged （16-62） years. The mechanism of trauma was sexual intercourse in 103 patients （89%） and masturbation in 13 patients （11%）. The most common site of injury found after exploration was right （55%） and lateral （74%） of the corpus cavernosum. The size of the tunical rupture was from 0.5 to 3.0 cm （mean 1.88±0.72）. Three of the patients had Marphan＇s syndrome. Urethral injury was detected by retrograde urethrography in 4 patients （3%） who had macroscopic hematuria and urethrorrhagia. During 12 months follow-up, no complication was seen. Conclusion： There is no need to perform retrograde urethrography unless the patients have gross hematuria or urethrorrhagia. The key to success in treatment of penile fracture is to achieve a rapid diagnosis based on history and a physical examination, avoid unnecessary imaging tests and perform immediate surgery to reconstruct the site of injury.

【Research Objectives】 Short term extreme temperature(ET) has been predicted to fluctuate in rice yield and quality under future climate. The objectives of this study were to explore the mechanism by which the extreme temperature impacts on rice yield and quality during the diverse growth stages through morpho-physiology, physiochemical and particular gene expressions observation.【Methods】From the verdict of last decade climate, extreme high temperature(EHT) and extreme low temperature(ELT) were exposed(from 7-0 days before heading(DBH);1-7 days after heading(DAH);8-14 DAH and 15-21 DAH) to Wanxiangyouhuazhan(V1) and Rongyouhuazhan(V2) cultivars in controlled climatic chamber.【Results】ET significantly decline yield, seed setting rate, filled grain number, chlorophyll fluorescence, chlorophyll index and photosynthetic components across both cultivars. Averaged across both the cultivars, grain yield was reduced by12%-17%. Conversely, EHT and ELT significantly reduced the amylose content, peak, trough and final viscosity, breakdown, peak time, brown rice(%), milled rice(%), head rice(%) and grain length-width ratio across cultivars and stages. Inversely, a significant enhanced response in case of protein content, gelatinization temperature, broken rice(%), chalkiness number(%), chalkiness area(%) and transparency with extreme temperature. Interestingly, 8-14 DAH stage showed the most prominent impact among almost all the parameters under EHT exposure and we focused to examine the gene expression at 4 th and 7 th days of treatment. EHT significantly increased the expression of Os AAP6 gene and decrease expression of Waxy gene, which results consistently with protein and amylose content in both rice cultivars, respectively.【Conclusion】Interestingly, EHT exhibited prominent strain than ELT at initial grain filling and will severely distress global food quality, nutrition and security. ADVENTURE on Windows is a collection of finite element modules including ADVENTUREolid ported to Microsoft Windows for use in a single CPU. It supports the iterative domain decomposition method with a preconditioner for the solution of linear equations. This paper analyzed 3 dimensional linear elastic stress problems and measured the performance of the finite element software ADVENTURE on Windows. A complete finite element analysis from CAD design to visualization was performed in this research successfully. Two simple models have been considered for the linear elastic problems. Computational results show the computation time, required memory, convergence criteria and some limitations of the software.

＜正＞Cell planning is a crucial part of designing a mobile communication ***,there are many proposed algorithms for this purpose,the problem has been remained challenging in non-uniform traffic *** this paper,we propose a new cell planning method based on active contour *** structure of the proposed method lets several parameters be applied in cellular mobile communication network design *** of these parameters are the coverage area of each base station,the overlap between neighbor base transceiver stations and the location of each base station in the test *** proposed technique is noticeably fast and accurate comparing with other competing *** addition,the dimension of areas can be much larger than the test areas in other *** novelty of this paper is to utilize active contour theory for solving complex geometry and traffic *** our method, each active contour represents a base station coverage *** iterative optimization algorithm is proposed to optimize the parameters of the set of active contours. The optimization criteria include the maximum overlap coverage area as well as minimum number of cells and cell *** results shows 99.92%and 97.23%overall coverage in uniform and non-uniform traffic situation respectively,revealing a great improvement with respect to genetic algorithm.

One of the new methods for powering low-power electronic devices at sea is a wave energy harvesting system. In this method, piezoelectric material is employed to convert the mechanical energy of sea waves into electrical energy. The advantage of this method is based on avoiding a battery charging system. Studies have been done on energy harvesting from sea waves, however, considering energy harvesting with random JONSWAP wave theory, then determining the optimum values of energy harvested is new. This paper does that by implementing the JONSWAP wave model, calculating produced power, and realistically showing that output power is decreased in comparison with the more simple Airy wave model. In addition, parameters of the energy harvester system are optimized using a simulated annealing algorithm, yielding increased produced power.

Background: To describe the mental health status of 78 child and adolescent survivors of post-war landmine ***: Child and adolescent survivors of landmine explosions who were younger than 18 years old at the time of the study were identified and enrolled in this study. The mental health status of the participants was assessed by general health assessment and psychiatric examinations. Psychiatric assessment and diagnosis were undertaken using the Diagnostic and Statistical Manual for mental disorders(DSM-IV) criteria. A psychiatrist visited and interviewed each survivor and identified psychiatric ***: Seventy-eight child and adolescent survivors with a mean age of 16.11±2 years old were identified and agreed to participate in the study. The mean age of the victims at the time of injury was 8.2±3.12 years old(range 2–15). Thirty-seven(47.4%) of the adolescent survivors suffered from at least one psychiatric disorder. Twenty-nine survivors(37.1%) were newly diagnosed and needed to start medication and psychiatric treatment. The most common findings were anxiety disorders(34.6%), including posttraumatic stress disorder(PTSD) in 20(25.6%), and generalized anxiety disorder(GAD) in 7(9%) subjects. Mild-Moderate depression was found in 5(6.4%) subjects. No personality disorders were observed, and two patients suffered from mental retardation. The study results revealed a significant association between age of casualty, duration of injury and limb amputation, and types of psychological ***: Child and adolescent survivors of landmine explosions had a high prevalence of psychiatric disorders.

The present study investigated the potential probiotic effect of Enterococcus faecalis against a pathogenic microorganism,Vibrio harveyi occurring in the giant freshwater prawn(Macrobrachium rosenbergii).*** was isolated from the intestine of *** through biochemical tests and PCR-based molecular *** in-vitro challenge was conducted by mixing isolated probiotic(5.12 log CFU/g)and ***(3.09 log CFU/g).Feeds incorporated with ***(9.02 log CFU/g)were applied to the treatment group during in vivo test in the aquarium and earthen pond(70 m^(2),1 m depth).Pathogenic bacteria were found to be reduced after 8 h(from 5.02 to 3.62 log CFU/g)and 60 days(from 5.44 to 2.15 log CFU/g)of probiotic administration in the in vitro and in vivo test,*** *** mixed feed also showed higher weight gain(WG%,481.22%±17.71%;SGR%,2.93%±0.18%)and digestive enzymes activities(amylase,1.26±0.08 unit/mg;protease,2.78±0.02 unit/mg)compared to control group(WG%,371.31%±13.35%;SGR%,2.30%±0.32%;amylase,0.7±0.03 unit/mg;protease,1.82±0.02 unit/mg).Furthermore,the probiotic also improved the immune response by augmenting NGH(from 73.33%±0.23%to 74.63%±0.11%)and SGH(from 22.24%±0.06%to 23.13%±0.08%)in the treatment ***,*** could be recommended to use against bacterial infections of the ***.
