Predictors of Vascular Calcification in Hemodialysis Patients

Background: The prevalence of both cardiovascular mortality and vascular calcification is much higher in patients with chronic kidney disease (CKD) than in the general population so early detection and intervention of VC may prevent or delay the progression and achieve improved patient outcomes. Objectives: To detect different predictors of vascular calcification in haemodialysis patients. Methods: This was a cross sectional observational study that included 85 patients with end stage renal disease (ESRD) on regular dialysis 47 males and 38 females ranged between 18 and 80 years old selected from Nephrology Unit, Internal Medicine Department, Menoufia University Hospital from April 2019 to May 2020. Serum calcium, phosphorus, alkhaline phosphatase, intact PTH, serum Matrix GLA protein, vitamin D and non-contrast CT for calcium scoring of femoral arteries were performed. Results: There was a significant correlation between age of the patient by years (p value 0.0001), serum calcium (p value 0.0001), phosphate (0.0001), calcium phosphate products (p value 0.0001) and alkhaline phosphatase (0.0001), and vascular calcification score detected by non-contrast CT on femoral arteries and negative correlation between serum Matrix GLA protein p value 0.0001) and the detected calcification score;the lower the MGP level the higher calcification score while there was no correlation between body mass index (BMI) (p value 0.021) intact PTH (p value 0.117), serum vitamin D level (p value 0.643), serum albumin (p value 0.643), serum haemoglobin (p value 0.257) and duration of dialysis (p value 0.260) and the detected score. Serum phosphate and calcium phosphorus product are independent risk factors for vascular calcification severity in haemodialysis patients. Jellyfish-inspired alginate composite hydrogel filter prepared by macro-micro double bionic strategy for efficient water purification

Recently,research on hydrogel materials with a porous structure and superior water absorption capabilities significantly ***,the hydrogel under gravity-driven separation conditions often exhibit an unstable pore structure,poor mechanical properties,and limited *** this end,this work presents a novel approach that combines a macro-micro double bionic strategy with a triple crosslinking method to develop a multifunctional alginate composite hydrogel filter(2%-SA-κCG-PVA-Ca^(2+),2%-SKP-Ca^(2+)for short)with a stable pore structure and superior mechanical properties,which possessed an umbrella-shaped structure resembling that of *** 2%-SKPCa^(2+)filter was synthesized using polyvinyl alcohol(PVA)as a stable structure-directing agent,and sodium alginate(SA)andκ-carrageenan(κ-CG)as polymer *** distinctive umbrellashaped hydrogel of 2%-SKP-Ca^(2+)filter,formed through the triple crosslinking method,overcomes the limitations of unstable pore structure and poor durability seen in hydrogels prepared by traditional crosslinking ***,the utilization of the 2%-SKP-Ca^(2+)filter in water treatment demonstrates its good selective permeability,excellent resistance to fouling,and extended longevity,which enables it to simultaneously achieve the multifunctional water purification and the coating of multi-substrate anti-fouling ***,not only does this research provide an efficient,multifunctional,highly pollution-resistant preparation method for designing a new filter,but it also confirms the application prospect of the macro-micro dual bionic strategy developed in this study in complex water treatment.

Chlorine has been widely used in different advanced oxidation processes(AOPs)for micropollutants *** this study,different chlorine-based AOPs,namely medium pressure(MP)UV/chlorine,low pressure(LP)UV/chlorine,and in-situ chlorination,were compared for carbamazepine(CBZ)removal efficiency,energy consumption,and disinfection by-products(DBPs)*** three processes could achieve nearly 100%CBZ removal,while the reaction time needed by in-situ chlorination was double the time required by UV/chlorine *** energy consumed per magnitude of CBZ removed(EE/O)of MP UV/chlorine was 13 times higher than that of LP UV/chlorine,and relative to that of in-situ chlorination ***,MP and LP UV/chlorine processes generated one to two orders of magnitude more hydroxyl radicals(^(·)OH)and reactive chlorine species(RCS)than in-situ ***,RCS were the dominant reactive species,contributing to 78.3%,75.6%,and 71.6% of CBZ removal in MP,LP UV/chlorine,and in-situ chlorination,*** to the Gibbs free energy barriers between CBZ and RCS/^(·)OH calculated based on density functional theory(DFT),RCS had more reaction routes with CBZ and showed lower energy barrier in the main CBZ degradation pathways like epoxidation and formation of *** applied to secondary wastewater effluent,UV/chlorine and in-situ chlorination produced overall DBPs ranging from 104.77 to 135.41μg/***,the production of chlorate during UV/chlorine processes was 15 times higher than that during in-situ chlorination.

Sources and Ecological Risks of Heavy Metals in Soils Under Different Land Uses in Bangladesh

Soil heavy metal pollution, influenced by both natural and anthropogenic factors, significantly reduces environmental quality. In this study, Cr, Ni, Cu, As, Cd, and Pb in eight different land-use soils from Patuakhali District in Bangladesh were assessed. Concentrations of Cr, Ni, Cu, As, Cd, and Pb in soils were 1-87, 5-271, 4-181, 0-80, 0.2-24.0, and 5-276 mg kg-1, respectively, measured using an inductively coupled plasma-mass spectrometer. The enrichment factor, pollution load index (PLI), and contamination factor (CJ) of metal i were used to assess the ecological risk posed by metals in soils. The PLI ranged from 0.78 to 2.66, indicating baseline levels to progressive deterioration of soil due to metal contamination. However, C;of Cd ranged from 1.8 to 12.0, which showed that the studied soils were strongly impacted by Cd. Considering the severity of the potential ecological risk of a single metal, the descending order was Cd > As > Pb > Cu > Ni > Cr. Soils under all land uses showed moderate to very high potential ecological risk.

Safety and effi cacy of hepatitis A vaccine in children with chronic liver diseas

AIM： To study the safety and efficacy of hepatitis A vaccine （HAV） in children with chronic liver disease of various etiologies. METHODS： Eleven children with chronic liver disease and thirteen age- and sex-matched controls negative for HAV antibodies were vaccinated against hepatitis A after they gave their informed consent. Children with uncontrolled coagulopathy or signs of hepatic decompensation were excluded. The vaccine （Havrix： 720 ELISA units in 0.5 mL, from GlaxoSmithKline Biologicals） was given intramuscularly in the deltoid in 2 doses 6 mo apart. Children were tested for HAV antibodies one and six months after the ist dose and one month after the 2^nd dose. Total serum bilirubin, alanine aminotransferase （alT）, and aspartate aminotransferase （AST） were determined immediately before and after one month of the 1st dose of the vaccine. RESULTS： Only 7 out of the 11 patients were positive for HAV antibodies after the 1^st dose of the vaccine, as compared to 100% of the controls. One month after the 2nd dose, all patients tested were positive for HAV antibodies. No deterioration in liver functions of patients was noted after vaccination. No adverse events, immediate or late, were reported by the mothers after each dose of the vaccine. CONCLUSION： Hepatitis A vaccine is both safe and effective in this small studied group of children with chronic liver disease. Given the high seroconversion rate, post-vaccination testing for HAV antibodies is not needed.

The role and potential of attapulgite in catalytic pyrolysis of refinery waste activated sludge

Localization of sensor nodes in the internet of underwater things(IoUT)is of considerable significance due to its various applications,such as navigation,data tagging,and detection of underwater ***,in this paper,we propose a hybrid Bayesian multidimensional scaling(BMDS)based localization technique that can work on a fully hybrid IoUT network where the nodes can communicate using either optical,magnetic induction,and acoustic *** communication technologies are already used for communication in the underwater environment;however,lacking localization *** and magnetic induction communication achieves higher data rates for short *** the contrary,acoustic waves provide a low data rate for long-range underwater *** proposed method collectively uses optical,magnetic induction,and acoustic communication-based ranging to estimate the underwater sensor nodes’final ***,we also analyze the proposed scheme by deriving the hybrid Cramer-Rao lower bound(H-CRLB).Simulation results provide a complete comparative analysis of the proposed method with the literature.

Mental Health Care in Saudi Arabia: Past, Present and Future

We review the past, present and future state of mental health care in the Kingdom of Saudi Arabia (KSA). The past is reviewed prior to the modern era, discussing early explanations and treatments for mental health illness up through the establishment of the first mental hospital in the 1950s, tracking advances in mental health care over the past 60 years. The present is explored in terms of the current need for mental health care based on the prevalence of mental health problems in KSA. We also discuss the role of the family in caring for the needs of the mentally ill today. Finally, we look forward into the future, discuss the current education system that will produce the next generation of mental health professionals, examine areas of mental health care that need improvement, and provide a research agenda to guide the continued development of the mental health care system in KSA. Our goal is to present a blue print for the development of a state-of-the-art mental health that may serve as a model for other countries in the Middle East, while taking into account the political, cultural and religious factors that are unique to this region of the world.

Using immune cell-based bioactivity assays to compare the inflammatory activities of oil sands process-affected waters from a pilot scale demonstration pit lake

In this study,we provide evidence that oil sands process-affected waters(OSPW)contain factors that activate the antimicrobial and proinflammatory responses of immune ***,using the murine macrophage RAW 264.7 cell line,we establish the bioactivity of two different OSPW samples and their isolated ***,we directly compared the bioactivity of two pilot scale demonstration pit lake(DPL)water samples,which included expressed water from treated tailings(termed the before water capping sample;BWC)as well as an after water capping(AWC)sample consisting of a mixture of expressed water,precipitation,upland runoff,coagulated OSPW and added *** inflammatory(*** activating)bioactivity was associated with the AWC sample and its organic fraction(OF),whereas the BWC sample had reduced bioactivity that was primarily associated with its inorganic fraction(IF).Overall,these results indicate that at non-toxic exposure doses,the RAW 264.7 cell line serves as an acute,sensitive and reliable biosensor for the screening of inflammatory constituents within and among discrete OSPW samples.
