AIM To investigate the accuracy of fungal dysbiosis inmucosa and stool for predicting the diagnosis of Crohn's disease(CD). METHODS Children were prospectively enrolled in two medical centers: one university hospital and one private gastroenterology clinic in the city of Riyadh, Kingdom of Saudi Arabia. The children with confirmed diagnosis of CD by standard guidelines were considered cases, and the others were considered non-inflammatory bowel disease controls. Mucosal and stool samples were sequenced utilizing Illumina MiSeq chemistry following the manufacturer's protocols, and abundance and diversity of fungal taxa in mucosa and stool were analyzed. Sparse logistic regression was used to predict the diagnosis of CD. The accuracy of the classifier was tested by computing the receiver operating characteristic curves with 5-fold stratified cross-validation under 100 permutations of the training data partition and the mean area under the curve(AUC) was calculated. RESULTS All the children were Saudi nationals. There were 15 children with CD and 20 controls. The mean age was 13.9(range: 6.7-17.8) years for CD children and 13.9(3.25-18.6) years for controls, and 10/15(67%) of the CD and 13/20(65%) of the control subjects were boys. CD locations at diagnosis were ileal(L1) in 4 and colonic(L3) in 11 children, while CD behavior was non-stricturing and non-penetrating(B1) in 12 and stricturing(B2) in 3 children. The mean AUC for the fungal dysbiosis classifier was significantly higher in stools(AUC = 0.85 ± 0.057) than in mucosa(AUC = 0.71 ± 0.067)(P < 0.001). Most fungal species were significantly more depleted in stools than mucosal samples, except for Saccharomyces cerevisiae and S. bayanus, which were significantly more abundant. Diversity was significantly more reduced in stools than in mucosa. CONCLUSION We found high AUC of fungal dysbiosis in fecal samples of children with CD, suggesting high accuracy in predicting diagnosis of CD.

AIM: To assess the prevalence of nutritional disorders in children with inflammatory bowel disease(IBD) in Saudi ***: The data from a national cohort of children newly diagnosed with IBD between 2003 and 2012 were analyzed. The diagnosis of IBD and the differentiation between Crohn's disease(CD) and ulcerative colitis(UC) were confirmed by gastroenterologists according to the standard criteria. The body mass index(BMI) of each child [weight(kg)/height^2(m)] was calculated at the time of diagnosis. The World Health Organization standards and references were used and the BMI for age > +1 and ely. Age stratification analysis was performed to investigate any age-related variation in the prevalence of nutritional status between children ely, with a significantly higher prevalence of thinness in children with CD than in children with UC(P = 0.037) only in the age group of 10-17 years(P = 0.030). The prevalence of overweight was 16 %, 15% and 20 % in the children with IBD, CD and UC, respectively, indicating a higher prevalence in UC that was statistically significant only in the age group of 10-17 years(P = 0.020). CONCLUSION: A high proportion of children with IBD presented with overweight instead of the classical underweight. Awareness of this finding is important for patient care.

Phosphoric Acid Pretreatment and Saccharification of Paper Sludge as a Renewable Material for Cellulosic Fibers

Recycling of paper sludge waste is crucial for establishing a sustainable green industry. This waste contains valuable sugars that can be converted into important chemicals such as ethanol, poly hydroxybutyrate, and lactic acid. However, the main challenge in obtaining sugars in high yield from paper sludge is the high crystallinity of cellulose, which hinders hydrolysis. To address this, pretreatment using phosphoric acid was optimized using response surface methodology to facilitate cellulose hydrolysis with minimal energy and chemicals. The created prediction model using the response surface method considered factors such as acid concentration (ranging from 60% to 85%), consistency (ranging from 4% to 10%), temperature (ranging from 25°C to 80°C), and time (ranging from 0.5 to 4 h). The results revealed that the model’s significant factors affecting the yield were acid concentration, reaction time, temperature, and the product of acid concentration and temperature, while the model’s significant factors affecting the crystallinity were the consistency, the temperature, and their product. The results showed that the optimum conditions for pretreatment were using an acid concentration of 64%, temperature of 25°C, consistency of 10%, and time of 30 min. The hydrolysis of the conditionally pretreated paper sludge resulted in a weight loss of 42%, compared to only 18% weight loss in non-pretreated paper sludge. Furthermore, the optimized conditions led to low levels of furfurals and acetic acid, which are undesirable by-products that can interfere with sugar fermentation. The total sugar obtained under the optimized conditions was 0.43 g glucose/g sample (10.46 g/L), while the contents of methyl furfural, furfural, and acetic acid were 21.65, 235.7, and 4.57 mg/L, respectively. With the high level of proliferation of connected mobile devices,the risk of intrusion becomes *** Intelligence(AI)and Machine Learning(ML)algorithms started to feature in protection software and showed effective *** algorithms are nonetheless hindered by the lack of rich datasets and compounded by the appearance of new categories of malware such that the race between attackers’malware,especially with the assistance of Artificial Intelligence tools and protection solutions makes these systems and frameworks lose effectiveness *** this article,we present a framework for mobile malware detection based on a new dataset containing new categories of mobile *** focus on categories of malware that were not tested before by Machine Learning algorithms proven effective in malware *** carefully select an optimal number of features,do necessary preprocessing,and then apply Machine Learning algorithms to discover malicious code *** our experiments,we have found that the Random Forest algorithm is the best-performing algorithm with such mobile malware with detection rates of around 99%.We compared our results from this work and found that they are aligned well with our previous *** also compared our work with State-of-the-Art works of others and found that the results are very close and competitive.

Impact of auditory stimulation nursing protocol on auditory response in cerebrovascular stroke patients

Objectives:The objective of the study was to explore the impact of auditory stimulation protocol on auditory response in cerebrovascular stroke(CVS)*** and Methods:A pre-posttest nonequivalent control group quasi-experimental study was conducted at Al Mainal Hospital at Al Kars Einy Hospital,Cairo University,among 60 CVS patients,and the sample was randomly assigned through a coin to allocate subjects to study or control group(30 for each group).Auditory stimulation techniques were applied three times per week for the study group for two consecutive *** in the control group received only routine hospital care during the study *** Glasgow Coma Scale(GCS)score for assessing the neurological function and level of arousal and the auditory subscale score of Western Neuro Sensory Stimulation Profile(WNSSP)for assessing auditory response were recorded and compared between the two groups before and after the ***:There were statistically significant differences between the study group and the control group regarding third reading of GCS score(11.5±0.3 vs.10.6±0.9,t=4.676,P=0.000).The scores of all auditory subscales of WNSSP in the study groups increased after implementing the auditory stimulation protocol(P<0.05).Conclusion:Utilizing the auditory stimulation protocol by nurse proves to be an effective method for enhancing hearing responses in individuals with CVS.
