This paper discusses a search problem for a Helix target motion in which any information of the target position is not available to the searchers. There exist three searchers start searching for the target from the origin. The purpose of this paper is to formulate a search model and finds the conditions under which the expected value of the first meeting time between one of the searchers and the target is finite. Also, the existence of the optimal search plan that minimizes the expected value of the first meeting time is shown. Furthermore,this optimal search plan is found. The effectiveness of this method is illustrated by using an example with numerical results.

In this paper,we present a complex cooperative search technique for finding the Random Walking microorganism cells on one of n-intersect real lines at the *** have 2n unit speed searchers starting together from the ***,proving the existence of a finite search plan,we are discussing the existence of optimality for this search plan which minimizes the expected value of the first meeting time between one of the searchers and the microorganism cells.

Objective:To evaluate three flowers of Hibiscus rosa-sinensis(***-sinensis),Quisqualis indica(***) and Senna surattensis(***) for their antioxidant activity by different methods in addition to total phenolic,flavonoid and pigment ***:Antioxidant activity of water,ethanol and absolute ethanol extracts of three flowers; ***-sinensis,*** and *** was *** antioxidant activity was assessed by 1,1-diphenyl-2-picrylhydrazyl free radical scavenging activity,ferrous chelating activity,reducing power,nitric oxide scavenging activity,hydroxyl radical scavenging activity as well as total antioxidant *** flavonoids,total phenols and total pigments including chlorophylls and carotenoids were measured for the three ***:The results showed that the highest total antioxidant capacity at concentration of 500 mg/L was found in *** as 0.479 ± *** activity of ***,*** and *** flower extracts against 1,1-diphenyl-2-picrylhydrazyl radical showed the highest activity of(90.20 ± 0.29)% with 500 mg/*** screening of the three flowers extracts were carried out for alkaloids,flavonoids,saponins,tannins,steroids,glycosides,terpenoids,amino acid and ***-sinensis showed the total phenolic in water extract of(235.77 ± 14.31) mg/100 g,the other two flowers *** and *** had the total phenolic in ethanol extracts of(937.70 ± 25.06) and(850.30 ± 13.81) mg/100 g,*** the other hand total flavonoids were identified in absolute ethanol extracts in the three flowers[(32.83 ± 1.34),(49.24 ± 4.87) and(2.79 ± 0.23) mg/100 g,respectively].Conclusions:The extracts in the constituents of the three flowers could be used as additives as supplement fractions in foods.

Background:Liver fibrosis is a hallmark determinant of morbidity in biliary atresia(BA)even in successfully operated *** factors for this rapid progression of fibrosis are not completely *** expression of the transcription factor SOX9 and hepatic progenitor cells(HPCs)proliferation have roles in fibrogenesis in cholestatic ***,they were not investigated sufficiently in *** aimed to delineate the relation of SOX9 and HPCs to fibrosis and its progression in ***:Forty-eight patients with BA who underwent an initial diagnostic liver biopsy(LB)and consequent intraoperative LB were recruited and compared to 28 cases with non-BA cholestasis that had an LB in their diagnostic *** fibrosis,tissue SOX9 and HPC expressions were studied in both BA and non-BA-cholestasis *** fibrosis,SOX9,and HPCs’dynamic changes in BA cases were *** of fibrosis and its progression to SOX9 and HPCs in BA was ***:SOX9 and HPCs in ductular reaction(DR)form were expressed in 100%of BA and their grades increased significantly in the second *** rapidly progressive fibrosis in BA,represented by fibrosis grade of the intraoperative LB,correlated significantly to SOX9-DR and HPC-DR at the diagnostic(r=0.420,P=0.003 and r=0.405,P=0.004,respectively)and the intraoperative(r=0.460,P=0.001 and r=0.467,P=0.001,respectively)*** the other hand,fibrosis,SOX9-DR,and HPC-DR were significantly lower in non-BA cases at a comparable age(Pely).Conclusions:Fibrosis in BA is rapidly progressive within a short time and is significantly correlated to SOX9 and *** of targeting SOX9 and HPCs on fibrosis progression is warranted.

Right Ventricular Dysfunction in Patients with Acute STEMI, Radionuclide-Based Assessment

Background: An acute myocardial infarction, particularly one that is large and transmural, can produce alterations in the topography of both the infarcted and noninfarcted regions of the ventricle. Cardiac remodeling after acute myocardial infarction (AMI) is characterized by molecular and cellular mechanisms involving both left and right ventricles. Objective: To study the right ventricular affection in patients with acute myocardial infarction of the left ventricle and treated with primary PCI. Methods: The study was done in the critical care department, Cairo University (in which 2,500 primary percutaneous coronary interventions (PCIs) are done annually). Forty patients with acute ST-elevation myocardial infarction (STEMI) were subjected to primary (PCI), we excluded all patients with RV infarction and patients with pulmonary hypertension. RV function was assessed by first pass radionuclide angiocardiography within 48 hours of admission. Results: Regarding the incidence of right ventricular dysfunction in patients with acute left ventricular STEMI who treated with 1ry PCI, there were 21 patients (52.5%) with RV EF% patients (47.5%) with RV time to peak filling rate (TPFR) > 180 msec. There was a significant relationship between right ventricular dysfunction and duration of ICU stay, impairment of LV systolic function, failure of complete resolution of ST segment elevation, failure of early peaking of cardiac enzymes, occurrence of in-hospital complications and one year mortality. Conclusion: The incidence of right ventricular dysfunction in patients with acute left ventricular STEMI is higher than expected and has a negative impact on their outcome.

AIM To evaluate the accuracy of the elastography score combined to the strain ratio in the diagnosis of solid pancreatic lesions(SPL). METHODS A total of 172 patients with SPL identified by endoscopic ultrasound were enrolled in the study to evaluate the efficacy of elastography and strain ratio in differentiating malignant from benign lesions. The semi quantitative score of elastography was represented by the strain ratio method. Two areas were selected, area(A) representing the region of interest and area(B) representing the normal area. Area(B) was then divided by area(A). Sensitivity, specificity, positive predictive value(PPV), negative predictive value(NPV), and accuracy were calculated by comparing diagnoses made by elastography, strain ratio and final *** SPL were shown to be benign in 49 patients and malignant in 123 patients. elastography alone had a sensitivity of 99%, a specificity of 63%, and an accuracy of 88%, a PPV of 87% and an NPV of 96%. The best cut-off level of strain ratio to obtain the maximal area under the curve was 7.8 with a sensitivity of 92%, specificity of 77%, PPV of 91%, NPV of 80% and an accuracy of 88%. Another estimated cut off strain ratio level of 3.8 had a higher sensitivity of 99% and NPV of 96%, but with less specificity, PPV and accuracy 53%, 84% and 86%, respectively. Adding both elastography to strain ratio resulted in a sensitivity of 98%, specificity of 77%, PPV of 91%, NPV of 95% and accuracy of 92% for the diagnosis of SPL. CONCLUSION Combining elastography to strain ratio increases the accuracy of the differentiation of benign from malignant SPL.

Psychiatric Co-Morbidity and Quality of Life in Egyptian Type 2 Diabetic Patients

Background: Diabetes is a risk factor for depression, but little is known about anxiety and other psychiatric disorders and quality of life. The aim of this study was to assess the prevalence of depression, anxiety in diabetic patients in our locality and to assess the quality of life in type 2 DM. Subjects & Methods: This study was a cross-sectional study and was carried out in outpatient clinics of specialized medical hospital, Mansoura university for a period of one year. From 217 diabetes mellitus subjects, only 202 patients were matched with 247 healthy people as a control group. All subjects were examined by using socioeconomic data, clinical data, and anthropometric examinations to assess body mass index and waist circumference. All patients were interviewed by using the Mini-International Neuropsychiatric Interview (MINI) version 5, MINI, Hospital Anxiety and Depression scale (HAD) and health-related quality of life (HRQOL) scales. Laboratory investigation in the form of fasting and two-hour postprandial blood sugar (FBS & 2hpp) and HbA1C levels were done. Results: 18.3% were found to be major depressive disorder;and 2.5% panic disorder, 1% other phobia. Generalized anxiety disorder and obsessive-compulsive disorder were found in one patient, no patients were found to be diagnosed as Bipolar disorder, schizophrenia, or substance abuse. Although there was no statistically significant difference between subjects and control groups regarding height, there was statistically significant difference between weights, BMI, with more scores among DM group. Moreover our study showed that HbA1c, fasting blood sugar, two hours post prandial blood sugar were more among DM patients and control groups. Anxiety, depression, and poorer quality of life were found to be more prevalent among DM patients than control groups. Conclusion: DM is associated with depression anxiety disorder with poorer quality of life.

AIM To investigate the clinical utility of serum annexin A2(ANXA2) as a diagnostic marker for early hepatocellular carcinoma(HCC).METHODS This study was performed in HCC Clinic of Ain Shams University Hospitals, Cairo, Egypt and included: Group 1: Fifty patients with early stage HCC(Barcelona Clinic Liver Cancer stage A); Group 2: Twenty five patients with chronic liver disease; and Control Group: Fifteen healthy, age-and sex-matched subjects who were seronegative for viral hepatitis markers. The followinglaboratory investigations were done: Viral hepatitis markers [hepatitis B surface antigen and hepatitis C virus(HCV) antibodies], HCV RNA in HCV antibody-positive patients, serum alpha fetoprotein(AFP), and serum ANXA2 *** In this study, 88% of HCC patients(n = 44) were HCVpositive, while HBV infection represented only 8% of all HCC patients(n = 4); and two patients were negative for both viral markers. A highly significant difference was found between patients with HCC and chronic liver disease as well as controls with regard to serum ANXA2 levels(130, IQR 15-240; 15, IQR 15-17; and 17, IQR 15-30 ng/m L, respectively). The area under the curve of ANXA2 was 0.865; the cut-off value was established to be 18 ng/mL with a diagnostic sensitivity of 74% and a specificity of 88%, while the sensitivity and specificity of AFP at the cut-off value of 200 ng/dL were 20% and 100%, *** Serum ANXA2 may serve as a biomarker for the early detection of HCC.
